Secondarily healing surgical wounds can be treated with negative pressure dressings. Dressing changes can be painful due to the strong adherence of the polyurethane foam placed in the wound. After debridement and conditioning of the wound bed, secondary surgical wound closure with a surgical suture can be performed. Cutaneous negative pressure treatment is used preventively after primary surgical suturing. Descriptions for secondary wound closure without a surgical suture are not known to date. The preparation and handling of an innovative transparent dressing for the cutaneous application of negative pressure treatment is demonstrated. The dressing assembly consists of a transparent drainage film and a transparent occlusion film. Negative pressure is applied via a tubing connection using a negative pressure pump. A new method of secondary wound closure using the transparent negative pressure dressing is presented based on a case example. The treatment cycle with instructions for making the dressing is shown in a video. [ABSTRACT FROM AUTHOR]

In diesem Beitrag wird der coronabedingte Berührungsentzug und der dadurch entstandene skin hunger aus phänomenologischer Perspektive beleuchtet. Dabei wird zunächst die Leiblichkeit des Subjektes thematisiert und erläutert, dass zwischen Leiblichkeit und Berührung ein gegenseitiges Konstitutionsverhältnis besteht. Darauf aufbauend, wird die Haut als Medium der Berührbarkeit und zugleich ihre identitätsstiftende Bedeutung vorgestellt. Die Auseinandersetzung mit dem Leib und dem Haut- Ich macht deutlich, dass das Subjekt ständig mit sich selbst, der Welt und potentiell mit seinen Mitmenschen in Berührung steht. Die Zwischenleiblichkeit und die in ihr grundgelegte Möglichkeit zur Fremdberührung, welche stets durch die Haut vermittelt ist, eröffnen ein Verständnis für die Gewichtigkeit der Haut-Nähe und Haut-Berührung für die intersubjektive Kommunikation und für die menschliche Vulnerabilität. Schließlich werden zentrale Gesichtspunkte des permanenten Berührungsentzuges, welcher einen erheblichen Vulnerabilitätsfaktor darstellt, vermittelt.

16. Kutane T-Zell-Lymphome – Update 2021.

Kutane T-Zell-Lymphome (CTCL) machen den Großteil aller primären kutanen Lymphome (CL) aus. 80 % aller Fälle von CTCL wiederum entfallen auf Mycosis fungoides (MF) und CD30-positive lymphoproliferative Erkrankungen der Haut. Das klinische Bild der verschiedenen Formen von CTCL zeigt Überschneidungen. Darum ist die klinisch-pathologische Korrelation von großer Bedeutung für die finale Diagnosestellung. Die MF zeigt einen charakteristischen Verlauf mit makuläre Läsionen (Patches), infiltrierte Plaques und, bei einem Teil der Patienten (10–20 %), Tumoren. Die Behandlung erfolgt stadienorientiert mit auf die Haut gerichteten Therapien wie UV-Lichttherapien und Kortikosteroiden in frühen Stadien der Erkrankung (Patch- und begrenztes Plaque-Stadium) und systemischen Therapien (Retinoide bzw. Rexinoide, Interferon, Monochemotherapie, zielgerichtete Therapie) und/oder Strahlentherapie (lokal oder Ganzkörperbestrahlung mit Elektronen) in fortgeschrittenen Stadien. Neuartige Ansätze umfassen zielgerichtete Therapien wie Mogamulizumab (Antikörper gegen CCR4) oder Brentuximab Vedotin (Antikörper gegen CD30) sowie Histon-Deacetylase-Inhibitoren. Angesichts des Impacts von zielgerichteten Therapien sind Biomarker wie CD30 nicht nur für die Diagnosestellung und korrekte Klassifikation eines Lymphoms im Einzelfall von großer Bedeutung, sondern als potenzielle Wirkstoff-Zielmoleküle auch für die Therapie. In der kürzlich überarbeiteten WHO-Klassifikation von 2017 und der aktualisierten WHO-EORTC-Klassifikation der CL von 2018 ist erstmals das CD8-positive akrale T-Zell-Lymphom als eigene, noch provisorische Entität aufgeführt. Diese Form zeigt charakteristische klinische, histologische und phänotypische Merkmale und eine hervorragende Prognose. Zu den seltenen, aber aggressiven CTCL zählen das primär kutane aggressive epidermotrope CD8-positive T-Zell-Lymphom und das kutane Gamma/Delta-T-Zell-Lymphom, die durch rasches Auftreten nekrotischer oder ulzerierender Plaques und Tumoren gekennzeichnet sind. Da bei diesen Formen die Prognose ungünstig ist, umfasst die Behandlung Polychemotherapien und hämatopoetische Stammzelltransplantationen. [ABSTRACT FROM AUTHOR]

Impact of Anthropogenic Pressures on the Flora of the Haut-Sassandra Classified Forest (Central-West of Côte d'Ivoire). The classified forest of Haut-Sassandra located in the center-west of Ivory Coast is subjected to a strong anthropization since the politico-military conflicts of the year 2002. This study aims to quantify the degradation in the northern, central and southern zones of this domain. To achieve this, a spatiotemporal analysis of the dynamics of the three zones was made based on five Landsat images from 1990, 1997, 2002, 2006 and 2013. It was supplemented by a floristic inventory of each zone. The results show that forest cover conversions of the three zones occurred during the conflict period with regression rates of more than 50 %. At the floristic level, 622 plant species distributed among 410 genera and 104 families were recorded in all three zones. The North and the Center differ significantly from the South by the average specific richness. The inventoried flora generally differs from one zone to another depending on the degradation factors, with a greater diversity in cocoa plantations. However, this flora, although diversified, does not have any similarity with that of the forest relics still present in the classified forest of Haut-Sassandra.

22. Thermal analysis of new transdermal devices for power transfer to ventricular assist devices

Different engineered approaches have led the design of implants with controlled physical features to minimize adverse effects in biological tissues. Aiming to prevent infection, similar efforts have focused on optimizing the design features of drivelines used to transfer power to percutaneous ventricular assistance devices (VAD), omitting however a thorough look on the implant-skin interactions that govern local tissue reactions. Here, we utilized an integrated approach for the biophysical modification of transdermal implants and their evaluation by chronic sheep implantation in comparison to the standard of care VAD drivelines. We developed a novel method for the transfer of breath topographical features on thin wires with modular size. Moreover, we examined the impact of implant’s diameter, surface topography, and chemistry on macroscopic, histological, and physical markers of inflammation, fibrosis, and mechanical adhesion. All implants demonstrated infection-free performance. The fibrotic response was enhanced by the increasing diameter of implants but not influenced by their surface properties. The implants of 0.2 mm diameter promoted mild inflammatory responses with improved mechanical adhesion and restricted epidermal downgrowth, in both silicone and polyurethane coated transdermal wires. On the contrary, the VAD drivelines with larger diameter triggered severe inflammatory reactions with frequent epidermal downgrowth [1]. Furthermore, we performed COMSOL simulations to investigate the electrothermal implications of conductive wires with different sizes for the power transfer to VADs. In our model, we simulate the electrical properties of the prototype’s wires, to confirm that it does not produce a significant body temperature rise. The skin model (Fig. 1a) mimics the multiple skin layer’ properties of epidermis, dermis, fat and muscle [2]. 24. Toward Milder Personal Care Cleansing Products: Fast ex vivo Screening of Irritating Effects of Surfactants on Skin Using Raman Microscopy.

We report a novel Raman technique that allows fast and reliable ex vivo assessment of the irritability of personal care cleansing products to the skin in terms of the molecular-level effects such as retention of water by corneocytes, change in the packing order and content of intercellular lipids, and the structure of keratin. We test this technique for the single surfactants (dodecyl glucoside, sodium dodecyl sulfate, sodium cocoyl glycinate, lauramidopropyl betaine) that are typically used in personal care, as well as on three types of commercial soap bars ("superfat", "syndet", and "combar"). We find that soaking of the skin for prolonged time in pure water can cause unfolding of keratin, which is commonly considered as a signature of "harshness" when dealing with the surfactant formulations. Moreover, molecular-level signatures of irritability of the test surfactants and soaps at brief (10 min) exposure times do not follow the trend expected from their critical micelle concentrations (CMC) and collagen swelling. In particular, dodecyl glucoside has positive impact on the barrier properties of the stratum corneum (SC) and apparent detergency properties (solubilizes lipids without affecting their packing order). We also find that two qualitatively different soap bars ("superfat" and "syndet") are similarly mild under the conditions studied, while the "combar" soap has detergency properties. These results demonstrate that to improve methodology of predicting irritability of a surfactant-based formulation, we need to study more systematically the molecular-level responses of the SC to exposure. Im Fokus der vorliegenden kumulativ angefertigten Arbeit stehen vier wissenschaftliche Untersuchungen, deren Thematik die Vibrationssensorik des Menschen darstellt. Die aus Vibrationen gewonnenen Informationen führen zu wichtigen Regulationsmechanismen zum Beispiel zur Aufrechterhaltung des Gleichgewichtes und des Ganges. Die Ermittlung von sogenannten Vibrationswahrnehmungsschwellen stellt eine Möglichkeit zur Quantifizierung des Funktionszustandes des somatosensorischen Systems dar. In diesem Zusammenhang ist die Kontrolle und Standardisierung einflussnehmender Messbedingungen auf die Bewertung der Vibrationssensorik wichtig. Im ersten Beitrag der vorliegenden Arbeit rückt die Rolle der Auflagekraft, mit welcher der Stößel der Messgeräte die zu untersuchende Hautpartie belastet, in den Fokus (Studie I). In den sich anschließenden Beiträgen werden die Wirkmechanismen der mechanischen Hauteigenschaften auf die Wahrnehmung von Vibrationen aus evolutionsbiologischer (Studie II) und pathologischer (Studie III) Sichtweise betrachtet. Im letzten Beitrag wird die potentielle Wirkung von unterschwelligen Rauschstimulationen als mögliche bewegungstherapeutische Maßnahme zur Verbesserung der sensorischen Leistungsfähigkeit untersucht (Studie IV). Am Ende der Arbeit erfolgt die übergreifende Diskussion der einzelnen Beiträge. Mit Hilfe der gewonnen Erkenntnisse werden Handlungsempfehlungen zur Verbesserung der Quantifizierung der Vibrationssensorik in klinischen und wissenschaftlichen Bereichen abgeleitet. Das erweiterte Verständnis über die Wirkmechanismen verschiedener Einflussgrößen auf die Vibrationssensorik unterstützt die gezielte Erforschung von bewegungstherapeutischen Maßnahmen zur Steigerung der sensorischen Leistungsfähigkeit des Menschen. The focus of the present cumulative dissertation is on four scientific investigations that deal with the human vibration sensory system. The information obtained from vibrations leads to important regulatory mechanisms, for example to maintain balance and gait. Determining so-called vibration perception thresholds represents a possibility to quantify the functional condition of the somatosensory system. In this context, it is important to control and standardize the measurement conditions that influence the evaluation of the vibration sensory system. The first article of the present dissertation focuses on the role of the contact force with which the probe of a measuring device loads the skin area to be examined (Study I). In the subsequent contributions, the action mechanisms of mechanical skin properties regarding vibration perception are considered from an evolutionary (Study II) and pathological (Study III) point of view. In the last paper, the potential effect of subliminal noise stimulation is examined as a possible exercise therapy intervention to improve sensory performance (Study IV). A comprehensive discussion of the individual contributions is included at the end of the present dissertation. Using the gained knowledge, recommendations are derived to improve the quantification of vibration sensation in clinical and scientific areas. The broadened understanding of the mechanisms of action of different influencing variables regarding vibration sensation supports the targeted research of exercise therapy interventions to increase sensory performance in humans.

Impairments and disorders of the skin are often an (early) sign of a psychiatric disorder. In the sense of true psychosomatics, psychiatrists should also be aware of this relationship as far as possible. This review article focuses on the most important dermatological diagnoses in relation to the respective psychiatric comorbidities and presents the most important aspects of epidemiology, symptomatology, pathophysiology and treatment options. Der Alterungsprozess ist in den letzten Jahren in den Mittelpunkt des Forschungsinteresses gerückt, da die Menschen immer älter werden. Insbesondere die Haut ist ein erster Indikator für die Beurteilung des Alters einer Person, da sowohl die Umwelt als auch die biologische Uhr sie beeinflussen. Diese Bachelorarbeit befasst sich mit der Hautalterung, insbesondere der Alterung der Dermis. Anhand eines Mausmodells soll die Frage, welche noch unbeschriebenen Proteine in der Dermis sich wie nachweislich verändern, beantwortet und mit verschiedenen Methoden validiert werden. Dazu wurden zwei Gruppen von Mäusen, eine junge (8 Wochen) und eine alte (52 Wochen), untersucht und verglichen. Mit Hilfe der Einzelzell-RNA-Sequenzierungsmethode wurde das Transkriptom einzelner Zellen aus den Hautproben der Mäuse analysiert. Auf der Grundlage dieser ersten Analyse wurden Gene von Interesse ausgewählt und mittels Western Blot Analyse und Immunfluoreszenzfärbung weiter untersucht. Kollagen5a1, Elastin und Vimentin sind die wichtigsten Proteine und stehen daher im Mittelpunkt dieser Arbeit. Für alle drei Proteine wurde bei der Einzelzell-RNA-Sequenzierung eine Abnahme der Proteinexpression festgestellt. Kollagen5a1 und Elastin zeigten in Immunfärbungen und Western Blots kompatible Ergebnisse, mit einer Abnahme bei der älteren Generation. Vimentin zeigte ähnliche Ergebnisse bei der Immunfärbung, aber bei der Western Blot Analyse konnte keine signifikante Veränderung festgestellt werden. The aging process has become the focus of research interest in recent years, as the human population is getting older. The skin in particular is a first indicator for assessing a person's age, as both the environment and the biological clock influence it. This bachelor thesis is dealing with skin aging, in particular the dermis. Using a mouse model, the question, of which undescribed proteins in the dermis change demonstrably and how, will be answered and validated using different methods. Therefore, two groups of mice, a young one (8 weeks) and an aged one (52 weeks) have been investigated and compared. Using the single-cell RNA sequencing method, the transcriptome of single cells from the mouse skin samples has been analysed. Based on this initial analysis, genes of interest have been selected and further investigated using western blot analysis and immunofluorescence staining. Collagen5a1, elastin and vimentin are the main proteins and therefore the focus of this work. For all three proteins, a decrease in protein expression was found in the single cell RNA sequencing. Collagen5a1 and elastin showed compatible results in immunostaining and western blots, with a decrease in the older generation. Vimentin showed similar results in the immunostaining, but no significant change could be identified in the western blot analysis.

40. Wirkpotential von Wundgelen unter Verwendung eines ex vivo Wundmodells mit menschlicher Haut

Das Ziel dieser Bachelorarbeit ist der Vergleich von sechs kommerziellen Wundgelen mit Octenilin®, einem Octenidin Dihydrochlorid-basiertem Wundgel, mittels eines ex vivo humanen Wundmodells. Die Wirkung der Wundgele auf die Sekretion des pro- und anti-inflammatorischen Zytokins Interleukin (IL)-8, die Zellmorphologie und Hautstruktur sowie die Apoptose von Epidermalzellen wurden analysiert. Hierfür wurde auf humaner Haut ex vivo eine oberflächliche Wunde mittels „tape stripping“ gesetzt und anschließend mit oder ohne (Kontrolle) Wundgel behandelt und 48 Stunden kultiviert. Danach wurden die Kulturüberstände mittels eines enzyme-linked immunosorbent assay auf die IL-8 Sekretion analysiert. Die Ergebnisse haben frühere Daten bestätigt, die gezeigt haben, dass Octenilin® die IL-8 Sekretion signifikant inhibiert. Darüber hinaus konnten wir nachweisen, dass auch Microdacyn60® diesen Effekt aufweist. Die Hauptbiopsien wurden via Hämatoxylin und Eosin gefärbt und histologisch untersucht. Eine visuelle Analyse ergab, dass sowohl Octenilin® als auch Lavanid® weder eine Veränderung der Hautstruktur noch eine Veränderung der Epidermis Architektur hervorgerufen hat. Darüber hinaus wurde die Apoptose-Induktion der Wundgele mittels Caspase-3 Färbung untersucht. Mit Ausnahme von Octenilin® and Betadona®, erhöhten die meisten Wundgele die Zahl der Caspase-3+ Zellen signifikant, was darauf hindeutet, dass erstere Wundgele keine apoptotische Wirkung durch lokale Applikation aufweisen. Im Allgemeinen konnte diese Bachelorarbeit nicht nur vorherige Ergebnisse für die herausragende Wirkung von Octenilin® im Vergleich zu anderen Wundgelen bestätigen, sondern auch um neue Erkenntnisse über die Wirkungsweise diverser Wundgele erweitern. The aim of this bachelor thesis was to comparatively assess six commercially available wound gels with Octenilin®, an octenidine dihydrochloride-based hydrogel, using a superficially wounded ex vivo human skin model. The effects of the wound gels on interleukin (IL)-8 secretion in culture supernatants, cell morphology and skin structure, as well as apoptosis of epidermal cells were analysed. For this purpose, a mild superficial wound model was used by tape stripping (TS) of ex vivo human skin and subsequent topical application of wound gels and the TS untreated control and cultured for 48 hours. Culture supernatants were measured with an enzyme-linked immunosorbent assay for the IL-8 content. The results confirmed previous data stating that Octenilin® significantly inhibits IL-8 secretion and revealed that Microdacyn60® has a similar effect. Skin biopsies were histologically analysed via haematoxylin and eosin staining. Visual analysis revealed that similar to the untreated control, Octenilin® as well as Lavanid® visibly neither alter the skin structure nor induce changes in the epidermis, in contrast to other wound gels. Furthermore, the wound gels’ induction of apoptosis was examined via immunofluorescent staining of caspase-3+ cells. Most wound gels significantly increased the number of caspase-3+ cells except for Octenilin® and Betadona®, implying that these two wound gels do not have a visible apoptotic effect when applied locally. Overall, this study not only confirms the previous findings of Octenilin®s outstanding effects in comparison to other wound gels but also gives new insight into their mode of action.

Durante la fase de compresión Atlásica, la cuenca Triásica de Tizi n’Test ha sufrido un proceso de inversión tectónica responsable de la reactivación de las estructuras extensionales Triásicas. A diferencia de las teorías anteriores que hablaban de un régimen transpresivo, las fallas NE-SO a ONO-ESE han sido inversas, mientras que las de direcciones NO-SE a NNO-SSE han sido reactivadas en su mayoría como fallas de desgarre. El juego inverso de las fallas inicialmente normales provocó el levantamiento de la cuenca y su zócalo paleozoico mediante un estilo de “pop-up”. Esta inversión también generó un conjunto de sinclinales en los compartimientos inferiores y anticlinales en los compartimientos superiores, con dimensiones de onda variables y ligados al movimiento vertical de las fallas inversas de zócalo. El análisis microtectónico de las estrías utilizando el programa informático R4DT ha sido llevado a cabo en 45 estaciones para determinar el estado de esfuerzos. Los 25 tensores más fiables muestran que la dirección principal de compresión σ1 tiene direcciones comprendidas entre NO-SE a NNE-SSE. El análisis de la repartición espacial del eje σ1 muestra un máximo principal hacia N012 y dos máximos secundarios entre N123 y N147. A falta de una cronología absoluta o relativa de los movimientos, interpretamos estas direcciones como ligadas a dos fases diferentes cuyas edades serían, por comparación con otros tranbajos, anterior al Mioceno para la fase N012 y Mio-Plioceno para la fase NO-SE.

The welfare of male fattening pigs may be improved by refraining from castration, but may be compromised, in turn, by harmful social behaviour in groups of boars. In addition, boar taint may be problematic. This study aimed to evaluate these potential problems in boar fattening under commercial organic conditions. In total, 625 boars were compared with 433 barrows and 83 gilts regarding their social behaviour, lesions and lameness at 80 kg, before and after split marketing. The mixed-model analysis showed that significantly more short agonistic interactions, fights and mounting behaviours were observed in groups of boars. Agonistic interactions were reduced in spring/summer and when boars grew older. Fights and mounts were increased when boars had contact to female pigs in the neighbouring pen. No effect of split marketing, growth rate, homogeneity of groups, group size, feeding space and illumination hours could be detected. Increased interaction frequencies did not result in significantly more skin lesions, lameness, treatments or mortality. Increased space allowance reduced skin lesions. On 9.8% of the dissected boars’ penises, wounds were detected, they were absent on two farms with generous litter provision. Boar taint prevalence, as detected by human nose method, was 1.44%. Under the studied organic husbandry conditions, boar fattening appears to be practicable, although penile injuries should be monitored at slaughter.

The urological and andrological examination of male patients should include inspection of the genital skin in order to detect malignant neoplasms, such as erythroplasia of Queyrat and infectious diseases, such as genital warts (condylomata acuminata). Independent of the incidental finding of relevant dermatological alterations in the genital area, sexually transmitted diseases such as syphilis must again be considered more often as the incidence has greatly increased in Germany during the last few years. In addition, urologists should be able to adequately advise patients on all aspects of diseases caused by human papillomavirus. Background: Peristomal skin lesions are frequent complications of ostomy; however, there is no generally accepted nomenclature and classification system. Objective: An interdisciplinary German expert panel (GESS) composed of ten members, developed an innovative semiquantitative classification system for peristomal skin lesions for further stratification of ostomy therapy. This score is based on criteria which can be assessed by stomal therapists and treating physicians. Results: The new peristomal skin lesion score grades three categories: lesion (L), status of ostomy (S) and disease (D). The L category describes the integrity of the skin as normal (L0), lesion with sustained integrity of skin (L1), integrity destroyed (L2) and local infection (L3). The S category rates the complexity of ostomy therapy as normal (S0), increased (S1) and high but not sufficiently effective (S2). The additional letters for categorization O. R. P. H. E. US describe anatomical pathologies of the stoma itself: ostomy stenosis (O), retraction (R), prolapse (P), hernia (H), edema (E) and unfavorable site (US). A systemic disorder is either absent (D0), irrelevant (D1) or relevant (D2). The LSD score is the basis for a management algorithm. Conclusion: The LSD score is comprehensive, standardized and holistic. Its straightforward use by health professionals can improve the consistency of the description of skin lesions and enhance the quality of ostomy therapy. [ABSTRACT FROM AUTHOR]
