16. Appalachian Women's Perceptions of Their Community's Health Threats

Abstract

Context: Decades of behavioral research suggest that awareness of health threats is a necessary precursor to engage in health promotion and disease prevention, findings that can be extended to the community level. Purpose: We sought to better understand local perspectives on the main health concerns of rural Appalachian communities in order to identify the key health priorities. While Kentucky Appalachian communities are often described as suffering from substandard health, resource, and socioeconomic indicators, strong traditions of community mobilization make possible positive, home-grown change. Methods: To assess what women, the key health gatekeepers, perceive as the most significant health threats to their rural communities, 10 focus groups were held with 52 Appalachian women from diverse socioeconomic backgrounds. Tape-recorded narratives were content analyzed and a codebook was developed. Measures designed to increase data trustworthiness included member checks, negative case evidence, and multiple coding. Findings: The following rank-ordered conditions emerged as posing the greatest threat to the health of rural Appalachian communities: (1) drug abuse/medication dependence; (2) cancer; (3) heart disease and diabetes (tied); (4) smoking; (5) poor diet/overweight; (6) lack of exercise; and (7) communicable diseases. These health threats were described as specific to the local environment, deriving from broad ecological problems and were connected to one another. Conclusion: Drawing on participants' community-relevant suggestions, we suggest ways in which rural communities may begin to confront these health concerns. These suggestions range from modest, individual-level changes to broader structural-level recommendations.

19. Evaluating Social Support and T2D Risk Factors Among Members of Rural-Dwelling Grandparent-Headed Households

Abstract

Purpose: This study examines the associations of social support and type 2 diabetes (T2D) risk factors among members of rural-dwelling, grandparent-headed households (GHH). Methods: Prospective data were collected from rural-dwelling members of GHH with no known diagnosis of T2D. Data collected on family characteristics, T2D clinical risk factors, and social support were assessed. Results: Sixty-six grandparents and 72 grandchildren participated in the study. The average age and HbA1Cs were 59.4 years and 6.2% ± 1.4 for grandparents and 11.8 years and 4.9% ± 0.6 for grandchildren. Most grandparents were found to have prediabetes or undiagnosed diabetes. The number of people living in GHHs was associated with grandparents’ triglycerides, HDL, and BMI. Average social support scores among grandparents suggested moderately high perceived social support (79 ± 3.4). For grandchildren, social support from grandparents was associated with diastolic blood pressure and HbA1C, whereas support from teachers, classmates, and close friends was associated with HbA1C and BMI in grandchildren. Implications: This study shows that grandparent caregivers are at an increased risk for T2D. Perceived social support between grandparents and grandchildren influences T2D risk factors. However, social support provided by peers, teachers, and close friends is also associated with T2D risk factors in grandchildren. These findings support the use of family-based diabetes prevention programming, peer support, and school settings as mechanisms for interventions to reduce T2D in adolescents, particularly those within GHHs.

Abstract

Background: Black women suffer disproportionately from obesity-related chronic diseases, particularly at younger ages. In order to close the gap in these health disparities, efforts to implement culturally appropriate interventions using community-engaged and community-based approaches have increasingly popular in recent years. The hair salon, a community staple for Black women and highly accessible geographically in the U.S., provides increased opportunity for outreach and engagement of multi-level health promotion interventions. Black women regularly frequent the hair salon and oftentimes have long-established relationships with their stylist. This established relationship and routine interaction provides access and opportunity to deliver health messaging and interventions. Hair stylists are well-respected and well positioned to serve as lay health workers in addressing health disparities among Black women in the U.S. Objectives: This dissertation is comprised of three studies: a systematic review of the literature and two qualitative studies; semi structured interviews with hair stylists and focus group discussions with Black women who patron hair salons. The studies sought to address the following three aims to: 1) synthesize the literature assessing obesity-related chronic disease health promotion interventions for Black men and women delivered in U.S. barbershops and hair salons; 2) assess the physical, cultural, and social environment of the hair salon setting as a setting for health promotion among Black women in the U.S.; and 3) assess the sociocultural factors of stylist-client engagement and perceptions about stylist-delivered health promotion. The dissertation was guided by the settings approach theory to better understand the hair salon as a health promoting setting. Abstract

Introduction: Breast cancer mortality rates among African American (AA) women are at 29.2 deaths per 100,000 persons compared with 20.6 deaths per 100,000 persons among Caucasian women. Regular mammography screening may significantly reduce breast cancer mortality and narrow this disparity. This study guided by PEN-3 model aims to explore the relationships and expectations domain and identify perceptions, enablers, and nurturers of regular mammography among AA women. Method: As part of an intervention study, in-depth interviews were conducted with 39 AA women recruited from the emergency department of a public university hospital. Results: Women’s perceptions included fear and limited knowledge. Enablers identified were cost, socioeconomic, and race-related discrimination, and health care previous experiences. Nurturers identified included observation of family experiences and lack of health-related social support. Discussion: Findings underscore the need to develop culturally tailored interventions to address the issues salient to this population.

Abstract

Racial disparities in breast cancer screening, morbidity, and mortality persist for Black women. This study examines Black women's mammography beliefs and experiences with specific focus on barriers to mammography access in an urban city in the South East, United States. This retrospective, qualitative study used Penchansky and Thomas' conceptualization of health care access as the framework for the data analysis. In-depth, semistructured interviews were conducted with 39 Black women. Structural and personal factors continue to create barriers to mammography among Black women. Barriers to mammography were identified for each of the Penchansky and Thomas five dimensions of access to care: accessibility, affordability, availability, accommodation, and acceptability. Clinical practice strategies to increase mammography screening in Black women must be multifactorial, patient-centered, and culturally congruent. Policy development must address the structural barriers to mammography screening through expansion of health insurance coverage and increased accessibility to health care. [ABSTRACT FROM AUTHOR]
