Context • Use of dextrose prolotherapy. Prolotherapy is defined as injection that causes growth of normal cells or tissue. Objective • Determine the i and 3 year efficacy of dextrose injection prolotherapy on anterior cruciate ligament (ACL) laxity. After year 1, determine patient tolerance of a stronger dextrose concentration (25% versus 10%). Design • Prospective consecutive patient trial. Setting • Outpatient physical medicine clinic. Patients or other participants • Eighteen patients with 6 months or more of knee pain plus ACL knee laxity. This laxity was defined by a KT1000 anterior displacement difference (ADD) of 2 mm or more. Intervention • Intraarticular injection of 6-9 cc of 10% dextrose at months 0, 2, 4, 6, and 10. Injection with 6 cc of 25% dextrose at 12 months. Then, depending on patient preference, injection of either 10% or 25% dextrose every 2-4 months (based on patient preference) through 36 months. Main Outcome Measures • Visual analogue scale (VAS) for pain at rest, pain on level surfaces, pain on stairs, and swelling. Goniometric flexion range of motion, and KT1000-measured ADD were also measured. All measurements were obtained at 0,6,12 and 36 months. Results • Two patients did not reach 6 month data collection, 1 of whom was diagnosed with disseminated cancer. The second was wheelchair-bound and found long-distance travel to the clinic problematic. Sixteen subjects were available for data analysis. KT1000 ADD. measurement indicated that 6 knees measured as normal (not loose) after 6 months, 9 measured as normal after 1 year (6 injections), and 10 measured as normal at 3 years. At the 3 year follow-up, pain at rest, pain with walking, and pain with stair use had improved by 45%, 43%, and 35% respectively, Individual paired t tests indicated subjective swelling improved 63% (P = .017), flexion range of motion improved by 10.5 degrees (P = .002), and KT1000 ADD improved by 71% (P = .002). Abstract. Correlation of first postnatal year infections among 251 babies was made with the infant feeding mode, socioeconomic status of family, maternal educational level, age, and maternal behavioral factors including smoking habit and number of siblings in the family. Initially, 127 mothers intended to breast-feed and 124 intended to formula-feed their infants. In the correlation matrix maternal education was the highest variable inversely correlated with infection; it was the first to enter in stepwise regression analysis and was highly significant (P < .0001). Maternal weight gain during pregnancy and infant feeding mode were the next two variables, respectively, to be added during the stepwise regression but they were not significant in the presence of maternal education. The statistical significance of increased maternal education in inverse relation to infection persisted in the presence of the other two factors (P < .0064). The t test showed no significant difference in the number of infections among the breast-fed and formula-fed babies within any of the three maternal education groups. It may be concluded that in the population studied, the infant feeding mode itself was not significantly related to the number of infections during the infant's first postnatal year when the education of the mother was taken into account. The semantic differential, a means of measuring attitudes, was administered to 178 fourth grade students to compare attitudes toward health. One school was composed almost entirely of black children from the inner city, the other contained white children from tipper middle class homes. When the children were divided into the two groups by sex, differences between the groups were not statistically significant but there were highly significant differences between the races. Generally, white children held more positive attitudes toward health personnel and health institutions than black children, while on the average black children were less concerned about sickness than white children. Since recent reports suggest structural brain abnormalities in autistic patients, we analysed magnetic resonance imaging (MRI) scans of autistic children. Planimetric measurements were done on midsagittal MRI scans, produced with a 0.5 T superconducting magnet. Scans of 13 'high-level' autistic subjects were compared with 35 control MRI scans, read as anatomically normal by a neuroradiologist. Corpus callosal, fourth ventricular, cerebellar, cerebral, and cranial areas were measured. The fourth ventricle was found to be significantly larger in the autistic group. No other areas in the midsagittal scans differed statistically between groups. Results suggest that structures defining the fourth ventricle are anatomically altered in autistic patients.
