Sitagliptin has been suggested as a treatment option for older adults with type 2 diabetes (T2D). However, no randomized controlled trial has been performed to evaluate the efficacy and safety of sitagliptin treatment in older Japanese patients with T2D. The STREAM study was a multicenter, open-label, randomized controlled trial. T2D outpatients aged 65–80 years with moderately controlled glycemic levels (HbA1c 7.4–10.4%) under lifestyle interventions without or with oral anti-diabetic drugs excluding DPP4 inhibitors or GLP-1 receptor agonists were recruited (n = 176). The participants were randomized into sitagliptin group (n = 88) who received sitagliptin as an initial or an additive anti-diabetic drug and control group (n = 88) who did not. The treatment goal was HbA1c level < 7.4%. Efficacy and safety during 12-month treatment period were investigated. The mean (± SD) ages were 70.6 ± 3.9 and 71.9 ± 4.4 years old in sitagliptin and control groups, respectively. According to a mixed-effects model analysis, average changes from baseline over the treatment period in fasting plasma glucose (FPG), HbA1c, and glycated albumin (GA) were − 27.2 mg/dL, − 0.61%, and − 2.39%, respectively, in sitagliptin group, and 0.50 mg/dL, − 0.29%, and − 0.93%, respectively, in control group. The reductions in FPG, HbA1c, and GA were significantly greater in sitagliptin group (P < 0.0001, P < 0.01, and P < 0.0001, respectively). There were no differences in the incidence of adverse effects, except for cystatin C elevation and platelet count reduction in sitagliptin group. Sitagliptin treatment effectively improved the glycemic profile without any serious adverse effects in older T2D patients. Trial registration number: UMIN000010376. [ABSTRACT FROM AUTHOR]

Source and Description. A partial genomic library (300 to 500 bp) was constructed using DNA obtained from Japanese flounder, Paralichthys olivaceus, digested with Sau3AI. The DNA was inserted in pBluescript SK (Toyobo, Japan) digested by BamHI and transformed into competent E. coli cells (strain DH5[Alpha]). The partial genomic library was screened with a [[Gamma].sup.32]P-labeled [(CA).sub.10] probe. About 800 positive clones were obtained, and part of that was sequenced on an ABI Prism 310 Genetic analyzer (Perkin Elmer Japan Applied Biosystems, Tokyo, Japan) using a BigDye Terminator cycle sequencing kit (Perkin Elmer). A total of 150 primer pairs flanking the dinucleotide repeats were designed. A marker, named Poli-RC35-TUF, was found to be highly polymorphic. Primer Sequences. Forward: 5'-CAC AAG CCT GCA CAA ATC AT; reverse: 5'-TAT TAC TCT GCC ATC CCC CA. The original sequence has been submitted to the GenBank database (accession no. AB032370). PCR conditions. PCR was performed in a 25-[micro]L solution containing 12.5 pmol of forward primer and .6 pmol of reverse primer end-labeled with [[Gamma].sup.33]P-ATP, .6 U of Taq Polymerase (Takara, Japan), 1x PCR buffer (50 mM KCl, 10 mM Tris-HCl pH 8.3, 1.5 mM Mg[Cl.sub.2]), 50 ng of genomic DNA, 200 [micro]M of each dNTP, and 1% BSA. Thermal cycling was performed in a GeneAmp PCR System 9600 (Perkin Elmer) and consisted of an initial denaturation at 95 [degrees] C for 2 min, followed by 35 cycles of 95 [degrees] C for 30 s, 62 [degrees] C for 1 min, 72 [degrees] C for 1 min, and a final extension of 3 min at 72 [degrees] C. The PCR products were separated in a 6% polyacrylamide gel. Allele sizes were determined using an M13 standard sequencing ladder. Mendelian Inheritance. Codominant Mendelian inheritance of alleles was observed on 44 individuals in a family of farmed flounder. Two genotypes were generated by the homozygous male parent and by the heterozygous female parent. The two different genotypes comprised 20 and 24 individuals, thus making a ratio close to the 1:1 segregation expectations. Polymorphism. Genomic DNA from 22 unrelated flounders caught in three different areas off the Japanese coast (north, southeast, and south) generated 19 alleles. The allele frequencies observed were .136 for the 131-bp allele, .092 for the 119-bp allele, .068 for alleles 115, 121, 123, 125, 129, and 143 bp, .046 for alleles 127, 133, 135, 147 and 153 bp, and .023 for alleles 117, 137, 139, 141, 149, and 163 bp. Repetitive discharges of a motor unit potential were observed during ulnar nerve conduction studies in a patient with pathologically verified carcinomatous sensory neuropathy. The repetitive discharges in this patient had a latency similar to that of the F-wave latency. The first discharge was followed by discharges identical in configuration every 3 to 4 msec. These discharges responded to all of the suprathreshold stimuli, had a constant configuration, and exhibited jitter in their latency. When paired stimuli were applied, the second stimulus collided with the first discharge, leaving the afterdischarges behind at the previous position. Based on these electromyographic findings, the origin of these repetitive discharges is believed to have been the proximal part of a motor axon or a neighboring demyelinating lesion, since degenerative changes have been demonstrated in ventral roots as well as dorsal root ganglia in this disease.

In contrast to the well-defined brainstem centers for saccades and eye reflexes, the brainstem centers mediating smooth pursuit eye movements are poorly understood. In monkeys, unilateral damage to the basal pons impairs horizontal smooth pursuit in the direction of the side of the lesion. The major source of afferents to the basal pons is the middle temporal area (MT). The dorsolateral pontine nucleus (DLPN) in the basal pons is considered to be the major gateway for smooth pursuit related signals to the contralateral flocculus. In humans, only one patient was found to have a unilateral lesion in the basal pons and selective impairment of smooth pursuit toward the side of the lesion. We report a 34-year-old man with a lesion in the basal pons identified by MRI. He had suffered from recurrent oral aphthosis, erythema nodosum and enterocolitis, and was hospitalized because of dysarthria. He was clinically diagnosed as neuro-Behçet disease. Saccades, smooth pursuit, vestibulo-ocular reflex (VOR), and VOR cancellation were measured. Ipsilateral smooth pursuit eye movements were predominantly impaired and interrupted by saccades, whereas VOR cancellation was impaired in both directions, in contrast with normal saccades and VOR. This observation of predominantly ipsilateral smooth pursuit impairment after a lesion in the human basal pons suggests that the concept of a middle temporal-ponto-floccular pathway for smooth pursuit, previously established in monkeys, can be extended to humans.(ABSTRACT TRUNCATED AT 250 WORDS)

We herein report a patient with the abnormal eye movement characterized by a conjugate slow-upward eye movement followed by a fast-downward movement to the primary position (slow-upward ocular bobbing). A 55-year-old man with a 12 years' history of striatonigral degeneration developed pneumonia and was admitted to our hospital. In addition to the parkinsonian features like akinesia and rigidity, examination revealed slow-upward ocular bobbing. There were no accompanying ocular abnormalities. This spontaneous eye movement was recognized throughout his stay in the hospital, irrespective of his consciousness level. Although other forms of ocular bobbing/dipping are usually associated with the loss of consciousness, all three reported patients with this slow-upward ocular bobbing were awake and responsive, therefore, suggesting a different kind of background pathophysiology in this unique eye sign.

In contrast to juvenile rigid form of Huntington's disease (HD) in which myoclonus is often seen, only 5 patients with myoclonus complicating adult HD have been reported. We herein described an adult HD patient who suffered from severe action myoclonus leading to physical disability. To our knowledge, this is the first case report in Japan. The patient, a 32-year-old female with a family history of chorea, developed choreiform movements and mental changes since the age of 24. Subsequently her motor disability has been aggravated by distinctively different involuntary movements characterized by sudden, violent, continuous muscular contractions of four extremities on any attempts at movement. Examination revealed moderate dementia and chorea complicated by frequent myoclonic jerks involving upper and lower extremities in posture or during movement. A head CT scan and MRI revealed caudate atrophy. The myoclonus, as recorded by surface electromyography over the right arm consisted of 40-60 msec-synchronous semirhythmic bursts. The cortical component of SEP was enlarged and C reflex was also observed. Clonazepam (4 mg a day) was instituted with a pronounced reduction in myoclonus and a return to her previous level of daily life activity. Although myoclonic jerks are often recognized in juvenile patients with rigid form of HD, they have been considered to exert a minor influence on physical disability. By contrast, our present observation and review of literature suggest that myoclonus may lead to severe motor impairment in adult HD.
