Box 9317, Stamford, CT, 06902, USA, Email SRose@stamhealth.orgAbstract: Stamford Hospital (SH) is a 305 bed Level II Trauma center located in Fairfield County, the epicenter of the coronavirus disease 2019 (COVID-19) pandemic in Connecticut. The COVID-19 pandemic was a fast paced, rapidly evolving crisis, presenting our leadership team with unique challenges related to resource availability, patient care, and staff safety. The existing Hospital Incident Command System (HICS) was activated in March 2020 to coordinate our operational emergency management planning, response, and recovery capability for this unplanned event. Although HICS lays the groundwork for hospital preparations and protocol implementation, it is not designed to withstand prolonged crisis circumstances. Given the conditions that the COVID-19 pandemic presented, the possibility for future waves became an impending reality, prompting the need for a long-term solution. To establish guidelines that promoted balance between necessary preparations in the case of additional surges of the pandemic and concurrent resumption then maintenance of routine hospital operations, the SH COVID-19 Playbook was created. The Playbook, presented here, is arranged in accordance with the evidence-based 4 S’s (Space, Staff, Systems, and Stuff) strategic critical care planning framework, to address surge capacity management within our hospital’s four main patient care areas and additional supportive services. Through feedback from frontline caregivers and leaders within SH, the Playbook captures our experience, best practices, and insight acquired during the first wave of the pandemic. Established with the intentions of equipping leadership and staff globally, guidelines are presented to aid in the navigation of future pandemic surges and successfully care for COVID-19 patients, ensure staff safety, allow for normal services to operate, and provide optimal communication and support for the community, patients, and staff.Keywords: COVID-19, Hospital Incident Command System, surge capacity, emergency preparedness, healthcare leadership, critical care guidance framework

Abstract

Lorentz symmetry is a foundational property of modern physics, underlying the standard model of particles and general relativity. It is anticipated that these two theories are low energy approximations of a single theory that is unified and consistent at the Planck scale. Many unifying proposals allow Lorentz symmetry to be broken, with observable effects appearing at Planck-suppressed levels; thus precision tests of Lorentz invariance are needed to assess and guide theoretical efforts. Here, we use ultra-stable oscillator frequency sources to perform a modern Michelson-Morley experiment and make the most precise direct terrestrial test to date of Lorentz symmetry for the photon, constraining Lorentz violating orientation-dependent relative frequency changes $\Delta\nu$/$\nu$ to 9.2$\pm$10.7$\times10^{-19}$ (95$\%$ confidence interval). This order of magnitude improvement over previous Michelson-Morley experiments allows us to set comprehensive simultaneous bounds on nine boost and rotation anisotropies of the speed of light, finding no significant violations of Lorentz symmetry., Comment: 20 pages, 13 figures

Abstract

We have carried out full imaging simulation studies to explore the impact of frequency standards in millimeter and sub-millimeter Very Long Baseline Interferometry (VLBI), focusing on the coherence time and sensitivity. In particular, we compare the performance of the H-maser, traditionally used in VLBI, to that of ultra-stable cryocooled sapphire oscillators over a range of observing frequencies, weather conditions and analysis strategies. Our simulations show that at the highest frequencies, the losses induced by H-maser instabilities are comparable to those from high quality tropospheric conditions. We find significant benefits in replacing H-masers with cryocooled sapphire oscillator based frequency references in VLBI observations at frequencies above 175 GHz in sites which have the best weather conditions; at 350 GHz we estimate a 20-40% increase in sensitivity, over that obtained when the sites have H-masers, for coherence losses of 20-10%, respectively. Maximum benefits are to be expected by using colocated Water Vapour Radiometers for atmospheric correction. In this case, we estimate a 60-120% increase in sensitivity over the H-maser at 350 GHz.

Abstract

We report on the evaluation of microwave frequency synthesis using two cryogenic sapphire oscillators developed at the University of Western Australia. A down converter is used to make comparisons between microwave clocks at different frequencies, where the synthesized signal has a stability not significantly different from the reference oscillator. By combining the CSO with a H-maser, a reference source of arbitrary frequency at X-band can be synthesized with a fractional frequency stability of sub-$4 \times 10^{-15}$ for integration times between 1 s and 10,000 s., Comment: 11 pages, 8 figures, presented at the Joint conference of EFTF-IFCS, Besancon, France, 2009

Abstract

Whispering Gallery modes in bulk cylindrical Gallium Arsenide and Gallium Phosphide samples have been examined both in darkness and under white light at cryogenics temperatures < 50 K. In both cases persistent photoconductivity was observed after initially exposing semiconductors to white light from a halogen lamp. Photoconductance decay time constants for GaP and GaAs were determined to be 0.900 +/- 0.081 ns and 1.098 +/- 0.063 ns, respectively, using this method., Comment: 13 pages, 5 figures

Abstract

Whispering Gallery modes in bulk cylindrical Gallium Arsenide and Gallium Phosphide samples have been examined both in darkness and under white light at 50 K. In both samples we observed change in permittivity under light and dark conditions. This results from a change in the polarization state of the semiconductor, which is consistent with a free electron-hole creation/recombination process. The permittivity of the semiconductor is modified by free photocarriers in the surface layers of the sample which is the region sampled by Whispering Gallery modes., Comment: 8 pages, 3 figures

Abstract

Cryogenic Sapphire Oscillators (CSO) developed at UWA have now been in operation around the world continuously for many years. Such oscillators, due to their excellent spectral purity are essential for interrogating atomic frequency standards at the limit of quantum projection noise; otherwise aliasing effects will dominate the frequency stability due to the periodic sampling between successive interrogations of the atomic transition. For this reason, UWA oscillators are now operational at NMI (Sydney), LNE-SYRTE (Paris), the French Space Agency (CNES, Toulouse) and at UWA (Perth). Other applications, which have attracted attention in recent years, include tests on fundamental principles of physics, such as tests of Lorentz invariance. This paper reports on the long-term operation and performance of such oscillators. We compare the long-term drift of some different CSOs. The drift rates turn out to be linear over many years and in the same direction. However, the magnitude seems to vary by more than one order of magnitude between the oscillators, ranging from 10^14 per day to a few parts in 10^13 per day., Comment: Accepted for publication in IEEE Trans. We report on the development of a sapphire cryogenic microwave resonator oscillator long-term fractional frequency stability of 2x10^-17Sqrt[\tau] for integration times \tau>10^3 s and negative drift of about 2.2x10^-15/day. 10. Carmeli's accelerating universe is spatially flat without dark matter

Abstract

Carmeli's 5D brane cosmology has been applied to the expanding accelerating universe and it has been found that the distance redshift relation will fit the data of the high-z supernova teams without the need for dark matter. Abstract

The metric by Carmeli accurately produces the Tully-Fisher type relation in spiral galaxies, a relation showing the fourth power of the rotation speed proportional to the mass of the galaxy. And therefore it is claimed that it is also no longer necessary to invoke dark matter to explain the anomalous dynamics in the arms of spiral galaxies. 45. Cost analysis of Down syndrome screening in advanced maternal age

Abstract

Objective: To analyze the potential cost and efficacy of Down syndrome screening in the population with advanced maternal age. Methods: Three screening methods defining Down syndrome risk for women with advanced maternal age were analyzed: advanced maternal age; advanced maternal age and maternal serum triple screen; and advanced maternal age, maternal serum triple screen and genetic sonogram. Costs for all tests and procedures were estimated. Procedure-related loss for amniocentesis was assumed to be 1 : 200. Efficacy was defined as: number of amniocenteses performed, number of Down syndrome cases detected, procedure-related losses, Down syndrome cases detected per fetal loss, cost per Down syndrome case detected and total cost of screening. Results: In 1999 in the USA, there were 530 610 women with advanced maternal age at 16 weeks' gestation carrying an estimated 4043 fetuses with Down syndrome. Screening by maternal age alone would result in the 100% detection of Down syndrome cases, but would require over 530 000 amniocenteses and result in 2653 procedure-related losses. Combining age with serum screen and genetic sonogram would detect 97.6% of Down syndrome cases, but would require only 119 791 amniocenteses and result in 599 procedure-related losses. The projected cost per Down syndrome case detected using age screening is $219 109 versus $155 992 using serum screen and genetic sonogram. Conclusions: The combination of advanced maternal age, maternal serum screen and genetic sonogram would result in the fewest procedure-related losses and lowest cost per Down syndrome case detected.

48. Natural Convection From Horizontal Wires to Viscoelastic Fluids

Abstract

Natural convection heat transfer from horizontal wires of three different diameters (0.0254, 0.0508, and 0.0826 cm) to a pool of viscoelastic liquid was studied. Aqueous solutions of Natrosol and Polyox constituted the viscoelastic test fluids. The experimental Nusselt numbers were found to agree with the correlations recommended by Fand and Brucker for Newtonian fluids if the zero shear rate viscosity is used in the Rayleigh and Prandtl numbers.
