2. Gafchromic EBT2 film dosimetry in reflection mode with a novel plan-based calibration method

Abstract

Purpose: A dosimetric system formed by Gafchromic EBT2 radiochromic film and Epson Expression 10000XL flatbed scanner was commissioned for dosimetry. In this paper, several open questions concerning the commissioning of radiochromic films for dosimetry were addressed: a) is it possible to employ this dosimetric system in reflection mode; b) if so, can the methods used in transmission mode also be used in reflection mode; c) is it possible to obtain accurate absolute dose measurements with Gafchromic EBT2 films; d) which calibration method should be followed; e) which calibration models should be used; f) does three-color channel dosimetry offer a significant improvement over single channel dosimetry. The purpose of this paper is to help clarify these questions. Conclusions: Reflection mode with Gafchromic EBT2 radiochromic film was found to be a viable alternative to transmission mode. The same methods that are used in transmission mode can be followed in reflection mode. A novel plan-based method was developed for calibration and multichannel dosimetry. This novel method offers increased robustness against film response inhomogeneities and reduces considerably the time required for calibration.

Abstract

Background: Treatment of bile duct injuries (BDI) during cholecystectomy depends on the severity of injury and the timing of diagnosis. Standard of care for severe BDIs is hepaticojejunostomy. The aim of this retrospective multi-center study was to assess the optimal timing for repair of BDI with hepaticojejunostomy. Methods: Members of the European-African HepatoPancreatoBiliary Association were invited to report all consecutive patients with hepaticojejunostomy after BDI from January 2000 to June 2016. Patients were stratified according to the timing of biliary reconstruction with hepaticojejunostomy: early (day 0–7), intermediate (1–6 weeks) and late (6 weeks–6 months). Primary endpoint was re-intervention >90 days after the hepaticojejunostomy and secondary endpoints were severe 90-day complications and liver-related mortality. Results: In total 913 patients from 48 centers were included in the analysis. In 401 patients (44%) the bile duct injury was diagnosed intraoperatively, and 126 patients (14%) suffered from concomitant vascular injury. In multivariable analysis the timing of hepaticojejunostomy had no impact on postoperative complications, the need for re-intervention after 90 days nor liver-related mortality. The rate of re-intervention more than 90 days after the hepaticojejunostomy was significantly increased in male patients but decreased in older patients. Severe co-morbidity increased the risk for liver-related mortality (HR 3.439; CI 1.37–8.65; p = 0.009). Conclusion: After BDI occurring during cholecystectomy, the timing of biliary reconstruction with hepaticojejunostomy did not have any impact on severe postoperative complications, the need for re-intervention or liver-related mortality. Individualised treatment after iatrogenic bile duct injury is still advisable.

Item does not contain fulltext, Background: The Kidney Donor Risk Index (KDRI) is a quantitative evaluation of the quality of donor organs and is implemented in the US allocation system. This single-centre study investigates whether the implementation of the KDRI in our decision-making process to accept or decline an offered deceased donor kidney, increases our acceptance rate. Methods: From April 2015 until December 2016, we prospectively calculated the KDRI for all deceased donor kidney offers allocated by Eurotransplant to our centre. The number of the transplanted versus declined kidney offers during the study period were compared to a historical set of donor kidney offers. Results: After implementation of the KDRI, 26.1% (75/288) of all offered donor kidneys were transplanted, compared with 20.7% (136/657) in the previous period (P < 0.001). The median KDRI of all transplanted donor kidneys during the second period was 0.97 [Kidney Donor Profile Index (KDPI) 47%], a value significantly higher than the median KDRI of 0.85 (KDPI 34%) during the first period (P = 0.047). A total of 68% of patients for whom a first-offered donor kidney was declined during this period were transplanted after a median waiting time of 386 days, mostly with a lower KDRI donor kidney. Conclusions: Implementing the KDRI in our decision-making process increased the transplantation rate by 26%. The KDRI can be a supportive tool when considering whether to accept or decline a deceased donor kidney offer. More data are needed to validate this score in other European centres.

37. Psychosocial correlates of health care utilization for children and adolescents with type 1 diabetes mellitus.

Abstract

Our objective was to examine the relation of self-reported depressive symptomatology to health care utilization (HCU) in families with a child with Type 1 diabetes mellitus (DM1). Data were obtained from a community sample (N = 88) being followed in 2 private outpatient diabetes clinics. Children and adolescents completed measures of depressive symptomatology. Mothers provided demographic information and completed measures of HCU and depressive symptomatology. Physicians provided measures of glycosylated hemoglobin (HbA1c) and illness severity. Hierarchical regression analyses indicated that higher levels of child depressive symptomatology were associated with increased HCU for children with DM1 above and beyond demographic and illness variables. However, no significant relationship was found between either adolescent or maternal depressive symptomatology and HCU. These findings suggest self-reported depressive symptomatology in children with DM1 is a salient correlate of HCU even after statistically controlling for the influence of demographics and illness parameters. The possibility that children with DM1 with symptoms of depression may have higher utilization of health services supports the need to target psychosocial variables for intervention. Abstract

Examined psychological adjustment in a college sample of older adolescents and young adults (n = 49) with histories of childhood asthma. A substantial number of subjects evidenced clinically significant levels of overall distress. In addition, greater perceived asthma uncertainty and increased stable attributions for negative events were significantly associated with poorer psychological adjustment after controlling for demographic and disease variables. Further analyses revealed a moderating influence of uncertainty on attribution-adjustment relationships. These findings provide initial support for a cognitive diathesis-stress view of adjustment in long-standing asthma. Results also support a growing body of evidence suggesting that the focus of efforts to enhance adjustment to asthma need to be expanded beyond childhood and early adolescence. 41. Notes on Group Psychotherapy with Pedophiles*

Abstract

Here reported are some of the highlights of a research project completed at the Forensic Clinic, Toronto Psychiatric Hospital. It was aimed at exploring the dynamics of pedophilia and the testing treatment techniques in a context of long-term psychotherapy with chronic pedophiles. Analytically oriented group psychotherapy is seen as the treatment method of choice for unmotivated pedophiles. The group psychotherapeutic sessions are seen as providing the therapeutic attention and interaction necessary for better reality testing, acceptance of responsibility, insight, integrated knowledge about self and the lessening and disappearance of pedophilic urges. In conducting the group, emphasis is placed on the need for flexibility in the therapist. In the integrative phase the therapist must be active, involved, reality-oriented and supportive, to foster ego and the development necessary for the abandonment of compromise devices. Later the therapist's activities are mainly focused on exploring resistances, and there is a gradual change to free group association, to the underlying unconsciously motivated common group themes and to the emphasis on the group as a whole.The pedophilic act per se is seen as a defensive measure by which the ego establishes homeostasis in time of stress. For pedophiles almost any stress caused by disturbance in object relationships can produce a state of panic. The ego of the pedophile faces tremendous regressive urges toward the recreation of the infantile symbiosis with the mother. Simultaneously there are intense fears of disintegration caused by sadistic rage towards the mother-figure. The unbearable tensions created by these are alleviated by the pedophilic act in those cases where a complex mother-child identification takes place.The powerful therapeutic effect of the group sessions is seen also as being further strengthened by the symbolic meaning that the group as a whole becomes, viz, assurance of the group members against object loss.The writer's gratitude to the group members of the Forensic Clinic's group psychotherapy seminar, for their valuable discussions, is herewith acknowledged.
