3. Importance of fetal fluid imbalance in congenital cystic adenomatoid malformation of the lung

Abstract

Congenital cystic adenomatoid malformation (CCAM) is a regional overgrowth of bronchioles with suppression of alveolar development in fetal and newborn lung. Twelve patients with CCAM were treated. Six premature infants had acute respiratory distress. Prenatal ultrasound was performed only five patients and was abnormal in all five. Five premature neonates also had patent ductus arteriosus complicating their courses. Six older children presented with recurrent pneumonitis. Radiographs showed asymmetry of the chest and radiolucent masses in all 12 patients. Seven had type I lesions, two had type 11 lesions, and three had type Ill lesions. Hydramnios and hydrops were present in three, and hydrops alone was present in one of the six neonates. These four patients died. One other neonate died of respiratory failure and persistent fetal circulation. Seven patients survived for an extended period. Ultrasound makes the prenatal diagnosis of hydramnios and hydrops possible. It is in this group that fetal interventions can be considered. For the remaining patients, surgical intervention is indicated at the time of diagnosis.

15. RETHINKING HEALTH CARE: AN OPENING DISCUSSION.

Abstract

The article focuses on issues related to health care in the United States. As the expenses of health care and the development of valuable interventions increase, a more efficient and effective health care system needs to be designed. Issues related to health care in the last year of life are also of prime concern. The reality is that medical care in the last year of life understandably is driven by a natural bias toward hope even in the face of futility. Accidents rank fifth in the overall causes of death on both the state and national level. Unintentional injuries are most likely to be caused by poisoning (usually use of drugs in an illicit manner) in the 35 to 54 year-old age range. By using all powers of observation and expertise in preventive medicine, the citizens of the United States can free themselves from old paradigms of inaction and effect savings not only of health care dollars but also of lives and productive years.

16. Long-Segment Hirschsprung's Disease

Abstract

• We identified 21 children (14 boys and seven girls) with long-segment Hirschsprung's disease defined as aganglionosis extending proximal to the ileocecal valve. Long-segment Hirschsprung's disease is difficult to diagnose and treat; symptoms may be mild, and diagnosis delayed. Abdominal distention and constipation or delayed passage of meconium are the most common symptoms. Radiologic studies are unreliable in establishing the diagnosis. The morbidity rate is high because of the high transition zone and short gut. The long-term outcome of patients after the standard Duhamel procedure is satisfactory, except with extremely high transition zones. Pull-through procedures should not be performed in the small infant, but should be delayed until patients are old enough to be continent.(Arch Surg. 1992;127:1047-1051)

Abstract

The results of this study indicate that DMSO has a significant effect in preserving PCEC luminal ATPase activity and fine structure and survival in endotoxin-treated rats. Although the exact mechanisms remain speculative, data from this study and others suggest that the effect may involve the inhibition of the release of lysosomal enzymes and the scavenging of O2 metabolites, such as superoxide anion (.O2(-)), hydrogen peroxide (H2O2), and hydroxyl radicals (.OH) , produced by endotoxin-stimulated phagocytes. Most organisms have active enzymes for catabolism of H2O2 and .o2(-) (precursors of .OH). However, there are no direct enzymatic mechanisms for the clearance of the highly cytotoxic . OH.
