Abstract

Background: In the 2022 mpox (monkeypox) outbreak, 79,000 global cases have been reported. Yet, limited dermatologic data have been published regarding lesion morphology and progression. Objective: The objective of this study was to characterize skin lesion morphology, symptomatology, and outcomes of mpox infection over time. Methods: The American Academy of Dermatology/International League of Dermatological Societies Dermatology COVID-19, Mpox, and Emerging Infections Registry captured deidentified patient cases of mpox entered by health care professionals. Results: From August 4 to November 13, 2022, 101 cases from 13 countries were entered, primarily by dermatologists (92%). Thirty-nine percent had fewer than 5 lesions. In 54% of cases, skin lesions were the first sign of infection. In the first 1-5 days of infection, papules (36%), vesicles (17%), and pustules (20%) predominated. By days 6-10, pustules (36%) were most common, followed by erosions/ulcers (27%) and crusts/scabs (24%). Crusts/scabs were the predominant morphology after day 11. Ten cases of morbilliform rash were reported. Scarring occurred in 13% of the cases. Limitations: Registry-reported data cannot address incidence. There is a potential reporting bias from the predilection to report cases with greater clinical severity. Discussion: These findings highlight differences in skin findings compared to historical outbreaks, notably the presence of skin lesions prior to systemic symptoms and low overall lesion counts. Scarring emerged as a major possible sequela.

Edward Hébert School of Medicine, Uniformed Services University, Bethesda, MD, 2Henry M Jackson Foundation for the Advancement of Military Medicine, Bethesda, MD INTRODUCTION Sickle Cell Trait (SCT) is a carrier status for the Hemoglobin gene mutation that causes Sickle Cell Disease. In general, SCT is a benign condition, but has been associated with exertion-related events (ERE), which may become serious medical emergencies to Service Members worldwide. Understanding this association is central to a case-controlled study for the genomics of ERE associated with SCT. This report aims to study strenuous exercise, specifically how frequency and duration contribute to ERE in SCT. METHODS RESULTS DISCUSSION & CONCLUSION ACKNOWLEDGEMENTS Overall, we observed significant differences between cases and controls for strenuous activity frequency (p=0.00022) and duration (p=0.01), with cases tending to perform the strenuous activities daily and for longer periods of time (>30 min) than the cases. When accounting for gender, male cases were significantly different from controls in strenuous activity frequency (p=0.0049) and duration (p=0.044), whereas female cases and controls did not differ significantly. Due to a lack of military controls (n=1) in our cohort, the male military cases (n=13) were compared to male civilian controls (n=10) and we found significant differences in strenuous activity duration (p=0.0042) and frequency (p=0.043). In contrast, male civilian cases (n=6) were not significantly different from the male civilian controls. However, a comparison of male military cases with male ci, RITM0017363, Sickle Cell Trait (SCT) is a carrier status for the Hemoglobin gene mutation that causes Sickle Cell Disease. 20. Algorithms and architectures for image processing

Abstract

Novel algorithms for detecting, classifying and tracking vehicles in infra red images are presented and their implementation on an array of transputers is discussed. Two classification techniques have been developed; the first, Segmentation Analysis, partitions each image into rectangular subimages from which several feature parameters are extracted. It is hypothesised that those subimages which have features differing significantly from the mean feature values will contain objects of interest which can be classified from the values of the features. In the second technique, ACF Classification, simple parameters are derived from the shape of the autocorrelation functions from a segmented image. The parameters are then be used to assign the segmented regions to the appropriate class of target or background. The ACF classification algorithm has been tested against a standard data base and has given a performance comparable with a human observer. A computationally efficient tracking algorithm has been designed which matches histograms from successive frames with a histogram from a reference image giving significant advantages in execution time over conventional algorithms. Further improvement in execution time can be obtained by combining histogram matching with novel search patterns. The algorithms were initially implemented on a microprocessor based image processor to demonstrate feasibility. Following a detailed emulation of the Inmos transputer, the algorithms were implemented on a multi-transputer network to demonstrate real-time image processing.

Abstract

Background: There are two distinctive acral manifestations of COVID‐19 embodying disparate clinical phenotypes. One is perniosis occurring in mildly symptomatic patients, typically children and young adults; the second is the thrombotic retiform purpura of critically ill adults with COVID‐19. Objectives: To compare the clinical and pathological profiles of these two different cutaneous manifestations of COVID‐19. Methods: We compared the light microscopic, phenotypic, cytokine and SARS‐CoV‐2 protein and RNA profiles of COVID‐19‐associated perniosis with that of thrombotic retiform purpura in critical patients with COVID‐19. Results: Biopsies of COVID‐19‐associated perniosis exhibited vasocentric and eccrinotropic T‐cell‐ and monocyte‐derived CD11c+, CD14+ and CD123+ dendritic cell infiltrates. Both COVID‐associated and idiopathic perniosis showed striking expression of the type I interferon‐inducible myxovirus resistance protein A (MXA), an established marker for type I interferon signalling in tissue. SARS‐CoV‐2 RNA, interleukin‐6 and caspase 3 were minimally expressed and confined to mononuclear inflammatory cells. The biopsies from livedo/retiform purpura showed pauci‐inflammatory vascular thrombosis without any MXA decoration. Blood vessels exhibited extensive complement deposition with endothelial cell localization of SARS‐CoV‐2 protein, interleukin‐6 and caspase 3; SARS‐CoV‐2 RNA was not seen. Conclusions: COVID‐19‐associated perniosis represents a virally triggered exaggerated immune reaction with significant type I interferon signaling. This is important to SARS‐CoV‐2 eradication and has implications in regards to a more generalized highly inflammatory response. We hypothesize that in the thrombotic retiform purpura of critically ill patients with COVID‐19, the vascular thrombosis in the skin and other organ systems is associated with a minimal interferon response. This allows excessive viral replication with release of viral proteins that localize to extrapulmonary endothelium and trigger extensive complement activation. What is already known about this topic? Acral perniotic‐like lesions are seen in mildly symptomatic patients with COVID‐19 – typically children – while acral retiform purpura associated with pauci‐inflammatory thrombosis is a manifestation of patients who are critically ill with COVID‐19. What does this study add? This study emphasizes that the disparate clinical and pathological manifestations of two distinct forms of acral disease in the setting of COVID‐19 are reflective of differences in interferon signalling.In the mild perniotic presentation, interferon signalling is robust, inflammation is striking and viral load in tissue samples is expectedly minimal.In contrast, in retiform purpura of severe COVID‐19, interferon signalling is absent, viral protein localization to endothelium is extensive and complement deposition with vascular thrombosis is impressive.
