A case of systemic nodular panniculitis is described in which the myocardium was found at necropsy to be extensively involved with focal interstitial carditis, identical histologically with nodules of panniculitis biopsied from the skin. This degree of myocardial involvement, which was not apparent during life and was not confined to pericardial or myocardial adipose tissue, has not previously been reported. Two cases of idiopathic oedema are reported and the literature is reviewed. The diagnosis of this disorder is made by exclusion of other known causes of oedema. We report the case of a 63-year-old Caucasian female, with reno-vascular hypertension, who developed a generalized skin eruption with lichenoid histological features, induced by nifedipine. A repeat challenge with nifedipine confirmed that it was the causative agent. [ABSTRACT FROM AUTHOR]

We report fifteen cases showing focal acral hyperkeratosis. They were clinically identical to acrokeratoelastoidosis (AKE) but lacked the elastorrhexis which is a feature of AKE. Previous studies on hair in kwashiorkor have been equivocal regarding the normal or abnormal incorporation of amino acids into the hair shaft. In this study we have applied a sensitive electron histochemical technique to transverse sections of scalp hair from children with severe kwashiorkor in order to study its ultrastructure and the incorporation of sulphur-containing amino acids (cystine). We have found no evidence to support the idea that a protein deficient diet in the weaning or post-weaning period directly affects the distribution and relative amounts of sulphur-containing amino acids in the hair shaft in this disease. Thirty-seven patients with psoriatic nail dystrophy were treated with intradermal nail-fold injections of either triamcinolone acetonide or triamcinolone hexacetonide, using the Port-O-Jet needleless injector. Mild nail-matrix changes showed a very good response to treatment, whilst nail-bed and/or hyponychial changes causing onycholysis and the more severe types of dystrophy usually showed little improvement. Although in occasional cases this may be a useful form of treatment, we would not recommend its general use in patients with psoriatic nail dystrophy. [ABSTRACT FROM AUTHOR]

A patient with acute necrotizing vasculitis is described in whom tests for circulating immune complexes were negative. The patient's serum injected intradermally produced lesions which closely resembled those occurring spontaneously. When the serum was fractionated, the skin-reactive fraction was found to be associated with IgG, but was of a higher molecular weight than normal IgG. Immunofluorescent studies showed that lesions induced by this active fraction contained IgG and complement in the epidermal basement membrane zone and within the small dermal vessels. We conclude that an immune complex of relatively low molecular weight is present in the active fractions, and capable of initiating the lesions of acute necrotizing vasculitis. The legs of patients with venous ulcers have been examined for fungi, and the factors affecting the growth and spread of the organisms found have been studied. Candida parapsilosis, Fusarium oxysporum and F. solani were the most common species. Their headquarters was the moist skin surrounding the ulcers rather than the ulcers themselves, but such skin was regularly invaded by fungi only in patients from certain clinics: in these clinics there was evidence of cross-infection between patients during treatment. C. parapsilosis was found to be less dependent on moisture than the Fusarium spp. and was associated with the less severe and drier ulcers as well as with the severe ones. The Fusaria were found only in legs with severe, moist ulcers. Experiments and observations to determine the origin of the fungi and the effect on them of various environmental conditions are reported, and the clinical and epidemiological significance of the results discussed. Economic pressures have brought about a change in the Conductors of electrical power cables; aluminium is now widely used in place of copper. The development of a jointing flux containing aminoethylethanolamine made this possible, and this has been very extensively used by the Electricity Supply Industry in this country. This amine has given rise to a high incidence of dermatitis amongst cable jointers, and is here shown to be a powerful sensitizer. Surveys have shown an incidence of dermatitis of 10% to more than 30%, in all grades of severity. A satisfactory alternative flux has been developed and is now in field use. Two cases of infection of the toenails with the fungus Pseudeurotium oralis are described for the first time. Both were transitory, One was associated with a T. rubrum infection and one with leukonychia trichophytica caused by Cephalosporium sp. The cases are used to illustrate common difficulties in the logical diagnosis of onychomycosis, Dermatophytes may be very difficult to isolate from nails -T. rubrum was grown from one out of two hundred inocula in the case reported-and it is impossible to find standardized criteria to determine time pathogenic status of a non -dermatophyte. The clinical, histological and mycological features of subcutaneous phycomycosis seen in Nigeria are described. Basidiobolus ranarum has been grown from three Nigerian cases. Treatment with potassium iodide by mouth is dramatic and surgical removal is not indicated. The disease has now been described in Indonesia, India, the Sudan, Uganda, Ghana, Senegal, Ivory Coast, Cameroons as well as Nigeria and England. [ABSTRACT FROM AUTHOR]

The article examines the case of two patients who acquired lymphangiomata following surgery and radiotherapy for breast carcinoma. One of the patient was diagnosed as having carcinoma of the left breast in July 1985 and treated by lumpectomy and post-operative radiotherapy. However, there were no clear signs to suggest that the patient had acquired lymphangioma. It is opined that acquired lymphangioma can be indistinguishable clinically and histologically from lymphangioma circumscriptum. It is a rare but well-recognized complication of chronic lymphoedema.

Cutaneous larva migrans is becoming more common in the U.K. with the popularity of tropical countries as holiday destinations. We describe the increasing use of a new benzimidazole derivative, albendazole, which is very effective in the treatment of cutaneous larva migrans. Staining of the nails due to hydroquinone-containing creams is not a well-recognized condition. We report two cases in older women in whom brown discoloration developed after the use of cosmetic skin-lightening creams for actinic lentigines of the hands. The cause of the discoloration and its relation to light exposure is discussed. Nail discoloration after the use of hydroquinone does not appear to have been reported in the British literature and a relationship to light exposure has not been previously noted. The article presents a case study of primary biliary cirrhosis with unusual vascular skin changes. The patient was female, aged 51 years. Twelve years ago the patient noticed a symptomless black spot near the right ankle. Gradually similar lesions have appeared higher up the legs together with red spots on the trunk, and new lesions continue to erupt. Ten years ago she developed xanthelasma around the eyes. Three years ago she suffered from recurrent urticaria for a period. She had eczema as a child, and a rash on the neck 20 years ago, but the diagnosis is unknown.

The article presents a case study of leukocytoclastic vasculitis with chronic fibrosing panniculitis. The patient was a male, aged 64 years. His illness began in January 1972. Initially he had 6 weeks off work with a flu-like illness with generalized weakness, weak wrists and pains in the fingers and arms. Later he developed a rash on the trunk, spreading to the arms and legs, which was sore and itchy. He also had transient joint swelling. In 1972 he was obese and unwell with very gross dental sepsis. In 1974 he developed progressive softening and thinning of the skin with loss of elasticity and subcutaneous tissue.

The article presents the case report of a female suffering from incontinentia pigmenti. This girl was born at Hull, with a grossly abnormal skin showing redness, blisters and later nodules. She had eczema of the hands and face in 1970. She had a right mastoidectomy following bilateral chronic middle ear disease. Her eyes and visual acuity are normal but the sclerae are slightly blue. The tongue is radiographically within normal limits, but the teeth are small and peg shaped, with normal enamel.
