4. The impact of information on illness representations and coping in early dementia from the perspective of the person with dementia : a phenomenological approach

Abstract

The onset of chronic illness often leads to threats to perceptions of self identity and challenges to prior ways of living. The self-regulation model of illness behaviour of Leventhal, Nerenz and Steele (1984) has provided a framework for understanding how people may manage these threats to self. To date this has been applied predominantly to physical illnesses and a few chronic neurological illnesses including Motor Neurone Disease. Whilst there have been several studies which have looked at the experience of people with early dementia, none has used the self-regulation model as a framework. A semi-structured interview methodology was used to explore the following questions. What representations do people with early-stage dementia have following diagnosis How do people's representations relate to the experience of early-stage dementia and how they manage it What are the implications for theory from what people tell us about their experience Interpretative Phenomenological Analysis (IPA) was used to analyse the data from the interview transcripts. This approach attempts to understand people's beliefs about a given topic through the assumption that there is an indirect link between what people report and underlying cognitions. IPA is a phenomenological approach in that it is concerned with individual subjective accounts and beliefs about objects or events and interpretive in that these accounts are elicited through a dynamic process in which the researcher's own beliefs play a part as they interpret data. Two overarching themes emerged from the analysis. These themes are 'Understanding Dementia: It Will Get Worse', which represents the individual's knowledge and representations of dementia, and 'I Want To Be Me', which represents the lived experience of adapting to dementia. The first of these themes is constructed from a number of sub-themes. 'I Want To Be Me' has two components of 'Personal Dilemmas: Where Do I Stand' and 'Interpersonal Dilemmas: The Line Is Crossed', each of which is also constructed of further sub-themes. The results were discussed in relation to each of the research questions in turn. It was then considered how the results add to the understanding of the experience of early dementia. A tentative model of how illness representations contribute to the lived experience is suggested. Areas for future research in the field that emerged from the study were discussed including the potential of exploring and comparing the representations of different groups of people. Possible methodological limitations of the study were explored in the context of the implications these may have had for the wider applicability of the results and how the researcher's assumptions and experience may have influenced the outcome of the study. Finally, implications for clinical practice arising from the study were discussed. These included the need to recognise the importance of directly involving people with dementia in the diagnosis sharing process, if they so choose and how people with early stage-dementia may be helped in maintaining their sense-of-self whilst adjusting to dementia related changes.

9. International PhD Students in Australian Universities: Financial Support, Course Experience and Career Plans

Abstract

Using data from a social survey of PhD students in two major Australian universities supplemented by student interviews, this article reports on the financial support, course experience and career plans of international PhD students. While most international PhD students hold scholarships which include stipends, a minority of students experience financial problems and lack adequate research support. Overall international PhD students express a high degree of satisfaction with their courses, although there are concerns about the quality and effectiveness of supervision, working space available to research students and help provided in designing research projects. Language problems sometimes adversely affect student progress while some international students find difficulty adjusting to a less deferential working arrangement with their supervisors and less structure in research direction. International PhD students are optimistic about their career prospects and certainly more confident about their careers than Australian PhD students. High proportions of international PhD students expect to follow research careers and say that the PhD degree will enhance their career prospects.

44. Cloning and high-level production of a chitinase from Chromobacterium sp. and the role of conserved or nonconserved residues on its catalytic activity

Abstract

A gene encoding an alkaline (pI of 8.67) chitinase was cloned and sequenced from Chromobacterium sp. strain C-61. The gene was composed of 1,611 nucleotides and encoded a signal sequence of 26N-terminal amino acids and a mature protein of 510 amino acids. Two chitinases of 54 and 52kDa from both recombinant Escherichia coli and C-61 were detected on SDS-PAGE. Maximum chitinase activity was obtained in the culture supernatant of recombinant E. coli when cultivated in TB medium for 6days at 37°C and was about fourfold higher than that from C-61. Chi54 from the culture supernatants could be purified by a single step based on isoelectric point. The purified Chi54 had about twofold higher binding affinity to chitin than to cellulose. The chi54 encoded a protein that included a type 3 chitin-binding domain belonging to group A and a family 18 catalytic domain belonging to subfamily A. In the catalytic domain, mutation of perfectly conserved residues and highly conserved residues resulted in loss of nearly all activity, while mutation of nonconserved residues resulted in enzymes that retained activity. In this process, a mutant (T218S) was obtained that had about 133% of the activity of the wild type, based on comparison of K cat values

47. Course Accreditation in Australian Colleges of Advanced Education: The Past, Present Practices and the Future.

Abstract

A description of course accreditation in Australian colleges of advanced education considers the development of the present system, procedures followed at state and national levels, strengths and weaknesses of the present system, and possible future directions. A discussion of what accreditation is and what purposes it might achieve and a brief outline of two overseas accreditation systems is presented. Accreditation, as one means of quality control and improvement, concentrates on courses rather than institutions. Both government and professional associations run their own accreditation systems. Two overseas models used by the Council for National Academic Awards in the United Kingdom and the National Commission on Accrediting in the United States did not fit Australian needs. The organization and responsibilities of the Australian Council on Awards in Advanced Education are outlined. Steps in applying for course accreditation are described. It is suggested that the present system be continued and improved and that a comprehensive review of the system be conducted at the national, state, and institutional levels by an independent authority. Improvements could be made regarding the composition and tenure of assessment committees and in the briefing provided by agencies to committees. (SW)

The politics of education is a new field of research specialization both in education and in political science. As with any new field, the politics of education faces a number of problems. On the other hand, it shows promise of being able to make important contributions both in increasing our understanding of educational and political systems and in providing substantial help in tackling current pressing educational problems. The aim of this paper is to describe briefly the development of the politics of education to date, to note some of the main literature that has been produced, particularly in the United States and in Australia, and to discuss some of the problems the politics of education faces and some of the possibilities it offers.
