2. 619. Pharmacokinetic-Pharmacodynamic (PK-PD) Target Attainment Analyses to Support Epetraborole Dose Selection for the Treatment of Patients with Mycobacterium avium Complex (MAC) Lung Disease

Background Epetraborole (EBO) is an orally available, bacterial leucyl transfer RNA synthetase inhibitor that concentrates in alveolar macrophages and exhibits potent activity against nontuberculous mycobacteria. PK-PD target attainment (PTA) analyses using plasma exposures were performed to support the selection of an EBO oral (PO) dosing regimen for clinical development in MAC lung disease. Methods Hill-type models were used to characterize the PK-PD relationships between EBO free-drug plasma AUC:MIC ratio and change in log10 CFU from baseline for 5 MAC isolates (EBO MIC range 2-8 mg/L) in a murine chronic MAC lung infection model. Change in log10 CFU was calculated by comparing lung CFUs after 56 days of EBO dosing (100 to 300 mg/kg/day) to CFUs of untreated controls prior to dosing. Data from PK studies were used to determine AUC by EBO dose. EBO PK parameters were calculated for 10,000 simulated MAC lung disease patients using an EBO population PK model with inflated PK variance to generate total-drug plasma concentration-time profiles at steady-state after administration of EBO 250 mg and 500 mg PO q24h for 21 days in a fasted state. Free-drug plasma AUC values were calculated using a human protein binding estimate of 0%. Percent probabilities of PTA were assessed using median, randomly assigned, and the highest of the free-drug plasma AUC:MIC ratio targets from the Hill models developed using the above-described in vivo efficacy study data. Results Table 1 summarizes the free-drug plasma AUC:MIC ratio targets for each MAC isolate. For EBO 250 mg q24h, percent probabilities of PTA were ≥ 90% for a 1-log10 CFU reduction (Figure 1) at an MIC of 4 mg/L (all PK-PD targets) or 8 mg/L (median target only) and for a 2-log10 CFU reduction (Figure 2) at an MIC of 2 mg/L (all PK-PD targets) or 4 mg/L (median target only). For EBO 500 mg q24h, percent probabilities of PTA were ≥ 90% for a 1-log10 CFU reduction at an MIC of 8 mg/L (all PK-PD targets) or 16 mg/L (median target only) and for a 2-log10 CFU reduction at an MIC of 4 mg/L (median and randomly assigned PK-PD targets) or 8 mg/L (median target only). Conclusion The high PTA associated with plasma exposures after oral EBO 500 mg q24h support advancing this dosing regimen into clinical studies in patients with MAC lung disease. 3. 593. Population Pharmacokinetic Model Development for Epetraborole and Mycobacterium avium Complex (MAC) Lung Disease Patients Using Data from Phase 1 and 2 Studies

Background Epetraborole (EBO), an orally available bacterial leucyl transfer RNA synthetase inhibitor with potent activity against nontuberculous mycobacteria, is under clinical development for treatment of MAC lung disease. A population pharmacokinetic (PK) model describing the disposition of EBO after oral (PO) and intravenous (IV) administration was developed to support EBO PK-PD analyses and dose selection for patients with MAC lung disease. Methods Model development was conducted using NONMEM (v.7.4.3). Models were attempted for oral absorption, systemic compartments, and linear vs. non-linear elimination. Model evaluation involved goodness-of-fit plots and prediction-corrected visual predictive plots, which describe the ability of model-based simulations to capture the observed data. Included were data from 5 Phase 1 (3 IV and 2 PO) and 2 Phase 2 studies (IV only) (Table 1). Results The pooled dataset included 2637 EBO PK samples from 138 subjects/patients. A robust fit to observed data across studies was obtained using a three-compartment model with linear elimination (Table 2). The impact of body weight on PK was included using an allometric scaling approach to accommodate observed lower body weight in MAC lung disease patients. PO dosing was modeled using an absolute bioavailability term (F) and transit compartments with separate absorption rates for fed and fasted administration. Interindividual variability (IIV) in systemic clearance was low (7.9%), but IIV in F (32.7%) contributed to slightly higher variability in PK for PO vs. IV administration. Moderate shrinkage was observed for the IIV in the model parameters. This was considered acceptable given inclusion of patients with limited PK data in the model and the objective to facilitate simulations designed to inform dose selection. The prediction-corrected visual predictive check plots for the data obtained from a recently completed Phase 1b study evaluating 28-day oral dosing regimens (NCT04892641) are provided in Figure 1. Conclusion This model is useful for describing expected PK in MAC lung disease patients and was used to conduct simulations for the advancement of the oral EBO 500 mg q24h dosing regimen into clinical studies in patients with MAC lung disease. Background Tebipenem pivoxil hydrobromide (TBP-PI-HBr) is an oral prodrug that is converted to tebipenem (TBP), the active moiety. TBP is a carbapenem with activity against multidrug-resistant Gram-negative pathogens, including extended-spectrum-β-lactamase-producing Enterobacterales and is being developed for treating complicated urinary tract infections (cUTI) and acute pyelonephritis (AP). Data from three Phase 1 studies and one Phase 3 study in patients with cUTI/AP were used to develop a population pharmacokinetic (PPK) model for TBP and identify covariates that described the variability in TBP pharmacokinetics (PK). Methods The PPK model was developed using TBP plasma and urine concentration-time data from the above-described Phase 1 and 3 studies. TBPPI-HBr doses, which ranged from 100 to 900 mg, were administered as single or multiple doses every 8 hours. After development of the structural model, stepwise forward and backward selection procedures were used to identify significant covariate relationships. The robustness of the final PPK model was assessed using a prediction-corrected visual predictive check (PC-VPC). Results The final dataset included 3448 plasma concentrations from 99 Phase 1 subjects and 647 Phase 3 patients and urine concentrations from 128 Phase 1 subjects. A two-compartment model with linear, first-order elimination and transit compartments to describe the rate of drug absorption after oral administration of TBP-PI-HBr best described TBP PK. The most clinically significant covariate effect, which would warrant dose adjustment, was the relationship between apparent oral clearance and creatinine clearance. In contrast, age, body size, sex, and fed status each had a minimal impact on TBP exposure. The PC-VPC showed good agreement between median simulated plasma concentrations based on the final PPK model and the median observed plasma concentrations for the pooled dataset (Figure 1). Figure 1. The population PK model was considered appropriate for model-based simulations and the assessment of PK-PD relationships for TBP. Background Hydromethylthionine is a potent inhibitor of pathological aggregation of tau and TDP-43 proteins. Objective To compare hydromethylthionine treatment effects at two doses and to determine how drug exposure is related to treatment response in bvFTD. Methods We undertook a 52-week Phase III study in 220 bvFTD patients randomized to compare hydromethylthionine at 200 mg/day and 8 mg/day (intended as a control). The principal outcomes were change on the Addenbrookes Cognitive Examination - Revised (ACE-R), the Functional Activities Questionnaire (FAQ), and whole brain volume. Secondary outcomes included Modified Clinical Global Impression of Change (Modified-CGIC). A population pharmacokinetic exposure-response analysis was undertaken in 175 of the patients with available blood samples and outcome data using a discriminatory plasma assay for the parent drug. Results There were no significant differences between the two doses as randomized. There were steep concentration-response relationships for plasma levels in the range 0.3-0.6 ng/ml at the 8 mg/day dose on clinical and MRI outcomes. There were significant exposure-dependent differences at 8 mg/day for FAQ, Modified-CGIC, and whole brain atrophy comparing patients with plasma levels greater than 0.346 ng/ml with having minimal drug exposure. The exposure-response is biphasic with worse outcomes at the high concentrations produced by 200 mg/day. Conclusions Hydromethylthionine has a similar concentration-response profile for effects on clinical decline and brain atrophy at the 8 mg/day dose in bvFTD as recently reported in AD. Treatment responses in bvFTD are predicted to be maximal at doses in the range 20-60 mg/day. A confirmatory placebo-controlled trial is now planned.

Background Although hydromethylthionine is a potent tau aggregation inhibitor, no difference was found in either of two Phase III trials in mild to moderate Alzheimer's disease (AD) comparing doses in the range 150-250 mg/day with 8 mg/day intended as a control. Objective To determine how drug exposure is related to treatment response. Methods A sensitive plasma assay for the drug was used in a population pharmacokinetic analysis of samples from 1,162 of the 1,686 patients who participated in either of the Phase III trials with available samples and efficacy outcome data. Results There are steep concentration-response relationships for steady state plasma levels in the range 0.3-0.8 ng/ml at the 8 mg/day dose. Using a threshold based on the lower limit of quantitation of the assay on Day 1, there are highly significant differences in cognitive decline and brain atrophy in patients with above threshold plasma levels, both for monotherapy and add-on therapy, but with effect sizes reduced by half as add-on. Plasma concentrations in the range 4-21 ng/ml produced by the high doses are not associated with any additional benefit. Conclusions Hydromethylthionine has pharmacological activity on brain structure and function at the 8 mg/day dose as monotherapy or as add-on to symptomatic treatments. This combined with a plateau at higher doses is consistent with the lack of dose-response seen in the Phase III trials. Treatment benefit is predicted to be maximal at 16 mg/day as monotherapy. A placebo-controlled trial in mild/moderate AD is now ongoing to confirm efficacy at this dose.

Background RZF is a novel antifungal of the echinocandin class with distinctive pharmacokinetics that support weekly dosing intervals. RZF is being developed for the treatment of candidemia and invasive candidiasis (IC) and the prevention of invasive fungal infections. A previously developed population PK model based on Phase 1 intravenous (IV) data [AAC 2018; e02603–17] was refined using IV data from additional Phase 1 and Phase 2 (STRIVE) studies. Methods Data from the two Phase 1 studies used previously to develop the model were pooled with data from an additional Phase 1 study and the STRIVE trial in patients with candidemia and/or IC. The population PK model was refined using NONMEM Version 7.2. The ability of covariates such as body size, age, sex, albumin, markers of liver and renal function, and infection status to explain a portion of the interindividual variability on select PK parameters was explored using stepwise forward selection (α = 0.01) and backward elimination (α = 0.001). The final model was externally validated by comparing model-based predictions to observed data from STRIVE, which were not available during model development. Results The final population PK model was a linear, four-compartment model with zero order IV input. Albumin was the most important predictor of the interindividual variability in RZF PK as significant relationships were found between serum albumin concentration and clearance, volume of the central compartment, volume of peripheral compartment 1, and volume of peripheral compartment 2. Additional relationships were found between PK parameters and sex, body weight, and infection status. The model provided precise and unbiased fits to the observed data (Figure 1). Differences in predicted median AUC across a wide range of covariate values were modest (Figure 2). The final model was also able to predict the central tendency and variability in RZF concentration–time data from patients with candidemia and/or IC not included in the model development (Figure 3). Conclusion A population PK model describing RZF PK in healthy subjects and patients with candidemia and/or IC was successfully developed. Usually, music is generally classified on the basis of its genre which indicates its musical style or musical form based on some sort of shared history. On the contrary, this paper aims to classify a given track into a mood such as happy, sad, peaceful and angry rather than based on its genre because more often than not, the listener prefers to hear songs similar to each other both in terms of the mood it evokes in the listener and otherwise. This is done by using the valence and arousal values that came along with the 1000 song database that we are using, as well as extraction of several musical features such as tempo, energy, pitch, etc. Using a combination of all these features, we use 4 different classification algorithms namely — Support Vector Machine (SVM), Naive Bayes, Linear Discriminant Analysis and Decision Trees to classify the music into the aforementioned moods and the accuracy achieved with these classifiers is 59.8%, 75.4%,78.5% and 71.4% respectively.
