Abstract

The Permanent magnet DC (PMDC) motors are commonly utilized in motion control systems for electrical or mechanical propulsion. The PMDC motor is described as a motor with a permanent magnet (PM) pole that is utilized when the required speed of a motor is lower than the base speed. Thus, these motors are not operated above the base speed. To maximize the efficiency of the motor, a high gain isolated DC-DC converter topology is suggested. The isolated converter topology consists of quasi Z-source (QZS) network which enhance the source voltage and also maintains the steadiness of the output voltage. The main advantage of QZS network is as it offers high level voltage (dv/dt) and current derivative (di/dt) protection to the circuit. In addition, a single phase high frequency inverter, isolation transformer and voltage doubler circuit are enclosed in the developed converter topology. The speed of the PMDC motor is regulated by developing an ANN controller. The ANN regulates the speed of a motor by comparing the actual speed with reference speed and thereby it generates the resultant speed signal. The ANN output is given to the PWM generator which generates gating pulse for 1¢ high frequency inverter. Consequently the speed of the motor has been adjusted by varying the PWM pulse generated by the PWM generator. The overall control performance of the PMDC motor with isolated DC-DC converter and ANN controller has been verified through MATLAB/Simulink simulation [ABSTRACT FROM AUTHOR]

Abstract

Tracheostomy is an artificial opening in the trachea in order to provide a clear airway and to remove secretions. Its general indications include upper airway obstruction, prolonged endotracheal intubation, impaired respiratory function, tracheobronchial toilet and to assist in weaning from ventilator. Several complications can be associated with the care of tracheostomy. There have been several case reports of foreign bodies in the tracheostomy tube. We hereby present a unique case of a broken culture swab stick acting as a foreign body inside a tracheostomy tube and hindering effective ventilation in a head injured patient. We also highlight how a change in the tracheostomy tube was required to remove the foreign body from the tracheostomy tube. [ABSTRACT FROM AUTHOR]

Abstract

Robotic surgery is the future of minimally invasive surgery. Anaesthesiologists have to be prepared for the various challenges posed by robotic surgeries in terms of steep trendelenberg position, prolonged pneumoperitoneum and fluid restriction. Rheumatoid arthritis (RA) is a debilitating autoimmune inflammatory arthritis affecting almost every joint of the body. The various deformities due to RA can pose a challenge for proper positioning. It can also cause difficult venous cannulation, difficult airway, impaired respiratory function, multi-system involvement and propensity to cause neurovascular impingement. A difficult airway scenario is every anaesthesiologist's nightmare. We present a unique case of difficult airway where a cryoprobe was used to clear the path for securing the airway in a cancer patient, where established difficult airway tools like fibreoptic bronchoscope and videolaryngoscope failed. We could pull the giant slippery polyp downwards and laterally with the help of cryoprobe, to facilitate tracheal intubation. Trans-oral robotic surgery, done for removing cancerous growths from difficult to reach areas of the oral cavity, has revolutionised the field of head and neck cancer surgery.
