7. Induction of labour in nulliparous women at term: Factors influencing a high caesarean section rate.

Abstract

Background: Induction of labour (IOL) is a common obstetric intervention used to minimise the risks of maternal and neonatal adverse outcomes, particularly at a term gestation. Many randomised controlled trials indicate that IOL is not associated with an increased risk of caesarean section. Preliminary data from the ARRIVE trial comparing IOL at 39 weeks in low-risk nulliparous women to expectant management further strengthens the trend towards IOL in this population group, with no increased risk of caesarean birth reported. However, for nulliparous women undergoing IOL in three obstetric hospitals in Melbourne, a 40% caesarean section rate was observed. Objective(s): To identify factors associated with an increased risk of caesarean birth in nulliparous women having an IOL at term to better guide clinical decision-making to minimise the rate of primary caesarean section in this population. Method(s): Nulliparous women with a singleton pregnancy undergoing an IOL between 37 and 42 weeks' gestation between January 2012 and December 2015 at Monash Health were identified. Maternal and neonatal outcomes, as well as labour characteristics, were obtained from the Birth Outcome Summary, maternal and neonatal health record. Mode of induction was identified allowing comparison between mechanical and prostaglandin cervical ripening, as well as artificial membrane rupture (ARM) and oxytocin infusion in the setting of cervical favourability. Cervical dilation and effacement at the time of membrane rupture or commencement of the oxytocin infusion were documented. Statistical analysis of baseline demographics and birth outcomes was undertaken stratified for mode of induction. Multivariate logistic regression was performed to identify factors associated with an increased risk of caesarean birth.
