Several buildings in a Danish social housing estate exceeded indoor air guidance values for polychlorinated biphenyls (PCBs), leading to their demolition. Here, we conducted a biomonitoring study among the workers on-site (n = 24) to evaluate their exposure to all 209 PCBs across the two-year demolition. We compared their PCB serum concentrations and accumulation to those of demolition workers at other worksites (n = 21) and office workers (n = 17). Demolition workers were provided with personal protective equipment according to risk assessments for PCB-related work. Serum PCBs were measured from baseline and up to two annual follow-up visits using gas chromatography high-resolution mass spectrometry. Forty-six peaks representing 58 PCBs were detected in > 60 % of serum samples; eight congeners were found in every sample. PCB-153 was the most abundant congener (median = 22.1 ng/g lipid). After adjusting for age and smoking status, demolition workers after one year on the contaminated site experienced more than a four-fold increase in all lower chlorinated PCBs compared to office workers at baseline, with increases most prominent for tri- and tetra-CBs (10β = 6.2 and 9.2, p

OBJECTIVE: To test the hypothesis that the association between levels of mould in floor dust and prevalence of potentially building-related symptoms may depend on the type of ventilation. METHODS: This stratified cross-sectional study is part of the Danish moulds in buildings program (DAMIB), including 503 adolescent schoolboys aged 13–17 years at 15 schools. Besides assessing symptom prevalences in questionnaires, we measured numerous potential risk factors in the school buildings. RESULTS: Stratifying on type of ventilation (natural, exhaust only, or full mechanical ventilation system), the negative effect of high levels of mould in floor dust was more pronounced in rooms with mechanical ventilation system. With a variable combining high level of moulds in floor dust with type of ventilation in the classroom, a significantly increased risk was found for all symptoms in the mechanically ventilated classrooms with high mould concentrations. In multiple logistic regression models, significant odds ratios (OR) ranged from 3.9 [95% confidence interval (95% CI) 1.5–10.1] (nasal congestion) to 17.0 (95% CI 2.1–138) (dizziness). CONCLUSIONS: The combined effect of moulds in dust and ventilation form might be a result of the higher air flow keeping the dust in the breathing zone for a longer time, thereby increasing the exposure for the occupants of the classrooms. It is important in future indoor air research also to focus on the combination effects of risk factors, including the type of ventilation.

OBJECTIVE: This paper reports a rare case of an occupational hypersensitivity reaction to an antineoplastic agent. METHODS: This is a clinical case report of a 45-year-old nurse who developed throat irritation and chronic nasal congestion followed by sinusitis shortly after beginning work at an oncological out-patient clinic. The symptoms disappeared upon leaving the clinic two years later, but they returned when she resumed work at the oncology unit at Hillerød Hospital, Denmark, handling chemotherapy on a daily basis. We performed in vitro histamine release tests against nine suspected antineoplastic agents. RESULTS: The patient’s histamine release test against the antineoplastic agent etoposide was positive; the other test results were negative. The histamine release test against etoposide using passive sensitization was also negative. Upon leaving the oncology department, the symptoms of the nurse disappeared once again. She was given a diagnosis of rhinosinusitis. CONCLUSION: This case of a hypersensitivity reaction to etoposide was judged to be of occupational origin. It was not clear whether it was immunoglobulin E (IgE) or non-IgE mediated.
