Abstract Background Schizophrenia and bipolar disorder are characterized by a high disease burden. Antipsychotic medication is an essential part of the treatment. However, non-adherence is a major problem. Our aim was to examine potential determinants of non-adherence for patients with severe mental disorders. Methods Baseline data of the study “Post stationary telemedical care of patients with severe psychiatric disorders” (Tecla) were used. Medication adherence was assessed with the Medication Adherence Report Scale German version (MARS-D). A logistic regression was calculated with age, sex, education, employment status, level of global functioning, social support and intake of typical and atypical antipsychotics as predictors. Results N = 127 participants were included in the analysis (n = 73 men, mean age 42 years). The mean MARS-D Score was 23.4 (SD 2.5). 4. Inter-individual Differences in Heart Rate Variability Are Associated with Inter-individual Differences in Empathy and Alexithymia

In the present study, we investigated whether inter-individual differences in vagally mediated heart rate variability (vmHRV) would be associated with inter-individual differences in empathy and alexithymia. To this end, we determined resting state HF-HRV in 90 individuals that also completed questionnaires assessing inter-individual differences in empathy and alexithymia. Our categorical and dimensional analyses revealed that inter-individual differences in HF-HRV were differently associated with inter-individual differences in empathy and alexithymia. We found that individuals with high HF-HRV reported more empathy and less alexithymia than individuals with low HF-HRV. Moreover, we even found that an increase in HF-HRV was associated with an increase in empathy and a decrease in alexithymia across all participants. Taken together, these findings indicate that individuals with high HF-HRV are more empathetic and less alexithymic than individuals with low HF-HRV. These differences in empathy and alexithymia may explain why individuals with high HF-HRV are more successful in sharing and understanding the mental and emotional states of others than individuals with low HF-HRV.

Social impairments are a core feature of autism-spectrum disorders. However, there is a considerable variability in these impairments. Most autistic individuals show large impairments in social functioning but some autistic individuals show small impairments in social functioning. The variability of these impairments has been attributed to the presence or absence of alexithymia. To address this issue, we capitalized on the fact that alexithymic and autistic traits are broadly distributed in the population. This allowed us to investigate how alexithymic and autistic traits affect social functioning in healthy individuals. Healthy individuals showed impairments on a resource-allocation task that were due to alexithymic but not autistic traits. These findings suggest that alexithymic rather than autistic traits impair prosocial behavior across the autism-spectrum.

6. Structural Alterations in the Corpus Callosum Are Associated with Suicidal Behavior in Women with Borderline Personality Disorder

Structural alterations in the corpus callosum (CC), the major white matter tract connecting functionally related brain regions in the two hemispheres, have been shown to be associated with emotional instability, impulsivity and suicidality in various mental disorders. To explore whether structural alterations of the CC would be similarly associated with emotional instability, impulsivity and suicidality in borderline personality disorder (BPD), we used diffusion tensor imaging (DTI) to assess the structural integrity of the CC in 21 BPD and 20 healthy control (HC) participants. Our hypothesis-driven analyses revealed a positive correlation between BPD participants’ suicidal behavior and fractional anisotropy (FA) in the splenium and genu of the CC and a negative correlation between BPD participants’ suicidal behavior and mean diffusivity (MD) in the splenium of CC. Our exploratory analyses suggested that suicidal BPD participants showed less FA and more MD in these regions than HC participants but that non-suicidal BPD participants showed similar FA and MD in these regions as HC participants. Taken together, our findings suggest an association between BPD participants’ suicidal behavior and structural alterations in regions of the CC that are connected with brain regions implicated in emotion regulation and impulse control. Structural alterations of the CC may, thus, account for deficits in emotion regulation and impulse control that lead to suicidal behavior in BPD. However, these findings should be considered as preliminary until replicated and extended in future studies that comprise larger samples of suicidal and non-suicidal BPD participants.

Background and aims Existing evidence suggests that text message interventions can help people to reduce their alcohol consumption. However, studies with alcohol-dependent patients are lacking. In this study a 1-year automatic mobile phone-based short messaging service (SMS) intervention on alcohol consumption in patients after alcohol detoxification in hospital was compared with treatment as usual. Design Multi-center, randomized, controlled, two parallel-group, observer-blinded trial. Setting and participants Primary and secondary care: four hospitals and community (1 million residents, 7600 km2 area in Germany). A total of 462 patients with alcohol dependence (ICD-10) were included during inpatient detoxification treatment. Patients were randomly assigned (1 : 1) to an SMS intervention and treatment as usual (SMS + TAU; n = 230; mean age: 45.4 years; 22.6% women) or TAU alone (n = 232 mean age: 44.5 years; 22.8% women). Planned, automated messages were sent to patients over 1 year to record assistance needs. A 'yes' or missing response triggered a telephone call from a hospital therapist. Outcome was assessed by an independent survey center. Measurements The primary end-point was a three-category alcohol consumption measure covering months 10-12 after discharge: abstinence, non-heavy drinking, heavy drinking [men > 60 g/day; women > 40 g/day equal to World Health Organization (WHO) criteria: high risk and very high risk, mean consumption]. Secondary end-points were number of abstinent days over 12 months and frequency of abstinence. Results The arms differed primarily in the heavy drinking category (intervention group 22.2%, TAU-only group 32.3%) in months 9-12. This is reflected by an odds ratio (OR) = 1.68, 95% confidence interval (CI) = 1.11-2.54, P = 0.015 for heavy drinking versus non-heavy drinking/abstinence. No difference between treatments was found with respect to any drinking versus abstinence (OR = 1.13). These results were confirmed by models adjusting for randomization strata. Conclusions In Germany, a 12-month mobile phone short messaging service-based intervention enhanced the reduction in heavy drinking for 1 year in routine care among adults with alcohol dependence discharged from inpatient alcohol detoxification.

ZusammenfassungDer vorliegende Text betrachtet Posttraumatische Belastungsstörungen und Traumafolgestörungen in der Adoleszenz und geht dabei insbesondere auf die Transitionsproblematik ein. Zum einen bilden sich auf der diagnostischen Ebene die Traumatisierungen häufig nicht PTBS-spezifisch, sondern in Selbsthilfe und Selbstmedikationsmaßnahmen sowie Alkohol- und Drogenabhängigkeit und Dissozialität ab, die im erwachsenenpsychiatrischen System oft nicht ausreichend hinterfragt werden. Zum anderen geht es um die Weiterentwicklung und Implementierung traumapädagogischer Ansätze insbesondere für Jugendliche, die in kritischen Hochrisikokonstellationen aufwachsen oder sich bereits Institutionen der Jugendhilfe oder der Eingliederungshilfe befinden. Hier wird nicht nur ein eklatanter Mangel in der Aus-, Fort- und Weiterbildung der beteiligten Berufsgruppen, sondern auch im Hinblick auf empirische Therapiestudien festgestellt.

The study evaluated a telephone call and text message intervention to improve adherence to medication among patients with severe mental illness.A randomized clinical trial was conducted, and outpatients with schizophrenia or bipolar disorder were assigned to the intervention group or to a usual care control group. The intervention was provided by trained nurses. Medication adherence was measured with the Medication Adherence Report Scale.The study sample comprised 120 participants. Logistic regression analysis showed that intervention group participants were significantly more likely than control group participants to be medication adherent at 6 months (odds ratio=4.11, p=.007). The superiority of the intervention emerged during months 4 to 6. Social desirability, diagnosis, and medication did not affect the results.Telemedicine via telephone can deliver low-threshold support to patients who are otherwise at high risk of progressive nonadherence to their psychotropic medication after 6 months.

In diesem Beitrag werden die vorliegenden Vorschlage von Qualifikationskriterien fur Supervisoren vorgestellt und diskutiert. Zusatzlich werden einige Trainings- und Ausbildungskonzepte vorgestellt. Hieraus werden Vorschlage fur einen spezifizierten Kompetenzerwerb und fur die Zertifizierung von Supervisoren abgeleitet.

- Abstract
Es wird der Behandlungsfall einer Patientin mit Borderline-Personlichkeitsstorung und komplexer Traumatisierung vorgestellt, bei dem ein Teil der autoaggressiven Verhaltensanteile durch temporare Episoden von Prostitution ausagiert werden. In diesem Kontext begegnet die Patientin einem »Freier«, der selbst psychotherapeutisch tatig ist und in eine laufende ambulante Behandlung massiv eingreift. Die damit verbundenen Handlungen des Therapeuten konnen als Traumareinszenierung der Patientin verstanden werden und verstosen eklatant und auf vielfaltige Weise gegen die ethischen Richtlinien jedweder Psychotherapie. Dabei zeigt sich auch, welche grosen Uberlappungsbereiche zwischen dem narzisstischen und dem sexuellen Missbrauch in der Psychotherapie bestehen.

BackgroundPrevious studies suggested that exposure to traumatic events during childhood and adulthood and post-traumatic stress disorder (PTSD) are associated with a dysregulation of different neuroendocrine systems. However, the activity of the renin–angiotensin–aldosterone-system (RAAS) in relation to trauma/PTSD has been largely neglected.MethodsTraumatization, PTSD, and plasma concentrations of renin and aldosterone were measured in 3092 individuals from the general population. Subgroups according to the status of traumatization (‘without trauma’; ‘trauma, without PTSD’, ‘PTSD’) were formed and compared regarding renin and aldosterone concentrations. Additionally, we calculated the associations between the number of traumata, renin, and aldosterone concentrations. Finally, associations of PTSD with renin/aldosterone levels were controlled for the number of traumata (‘trauma load’).ResultsLevels of renin, but not aldosterone, were increased in traumatized persons without PTSD (p= 0.02) and, even stronger, with PTSD (p< 0.01). Moreover, we found a dose–response relation between the number of traumata and renin levels (β= 0.065;p< 0.001). Regression analyses showed PTSD as a significant predictor of renin (β= 0.38;p< 0.01). This effect was only slightly attenuated when controlled for trauma load (β= 0.32;p< 0.01).ConclusionsOur results suggest that traumatization has lasting and cumulative effects on RAAS activity. Finding elevated renin levels in PTSD independent from trauma load supports the concept of PTSD as a disorder with specific neuroendocrine characteristics. Alternatively, elevated renin levels in traumatized persons may increase the risk for developing PTSD. Our findings contribute to explain the relationship between traumatic stress/PTSD and physical disorders.

Zusammenfassung. In der klinischen Praxis gewinnen evidenzbasierte Behandlungsleitlinien zunehmend an Bedeutung. Innerhalb dieser finden sich jedoch nur unbefriedigende Informationen zur Diagnostik, speziell im Hinblick auf eine therapie-begleitende Diagnostik. Der Erfolg einer Therapie, ob Psycho- und/oder Pharmakotherapie, hängt jedoch von der Verzahnung von Therapie und Diagnostik ab, um z.B. den Therapieverlauf und -erfolg zu bewerten und bei Bedarf die therapeutische Strategie zu ändern. Der Ansatz des Evidence-Based Assessment (EBA) bietet hier ein hilfreiches Rahmenmodell und konkrete Vorschläge sowie Kriterien zur Realisierung dieser Verzahnung. Im Beitrag soll EBA vorgestellt sowie anhand ausgewählter Störungsgruppen illustriert werden.

Nach dem Ende der Deutschen Demokratischen Republik (DDR) 1990 wurde umfassend uber das systematische DDR-Staatsdoping im Leistungssport – insbesondere mit androgenen Steroiden – berichtet. Das DDR-Dopingsystem beruhte auf einer systematischen und heimlichen Kooperation zwischen Wissenschaftlern, Trainern, Sportarzten, der pharmazeutischen Industrie und dem Staatssicherheitsdienst. Im Verlauf der Zeit wurden etwa 15 000 Sportler mit Androgenen und anderen Substanzen gedopt. In diesem Beitrag wird ein multimodaler Forschungsansatz vorgestellt, mit dem die somatischen, psychischen und sozialen Langzeitfolgen der geheimen Dopingaktivitaten etwa 25 Jahre spater bei den Betroffenen evaluiert werden sollen. Erste Ergebnisse zeigen vergleichsweise hohe Raten Posttraumatischer Belastungs- und anderer psychischer Storungen sowie substantielle individuelle Veranderungen in den weiteren Lebensverlaufen.

Background: Life events (LEs) are associated with future physical and mental health. They are crucial for understanding the pathways to mental disorders as well as the interactions with biological parameters. However, deeper insight is needed into the complex interplay between the type of LE, its subjective evaluation and accompanying factors such as social support. The "Stralsund Life Event List" (SEL) was developed to facilitate this research. Methods: The SEL is a standardized interview that assesses the time of occurrence and frequency of 81 LEs, their subjective emotional valence, the perceived social support during the LE experience and the impact of past LEs on present life. Data from 2265 subjects from the general population-based cohort study "Study of Health in Pomerania" (SHIP) were analysed. Based on the mean emotional valence ratings of the whole sample, LEs were categorized as "positive" or "negative". For verification, the SEL was related to lifetime major depressive disorder (MDD; Munich Composite International Diagnostic Interview), childhood trauma (Childhood Trauma Questionnaire), resilience (Resilience Scale) and subjective health (SF-12 Health Survey). Results: The report of lifetime MDD was associated with more negative emotional valence ratings of negative LEs (OR = 2.96, p < 0.0001). Negative LEs (b = 0.071, p < 0.0001, beta = 0.25) and more negative emotional valence ratings of positive LEs (b = 3.74, p < 0.0001, beta = 0.11) were positively associated with childhood trauma. In contrast, more positive emotional valence ratings of positive LEs were associated with higher resilience (b = -7.05, p < 0.0001, beta = 0.13), and a lower present impact of past negative LEs was associated with better subjective health (b = 2.79, p = 0.001, beta = 0.05). The internal consistency of the generated scores varied considerably, but the mean value was acceptable (averaged Cronbach's alpha > 0.75). Conclusions: The SEL is a valid instrument that enables the analysis of the number and frequency of LEs, their emotional valence, perceived social support and current impact on life on a global score and on an individual item level. Zusammenfassung.Hintergrund: Die Bindungsorganisation ist ein zentrales Merkmal, um die Psychopathologie der Borderline-Persönlichkeitsstörung (BPS) im Kontext affektiver und interpersonaler Probleme zu verstehen. In dieser Pilotstudie wurden anhand verschiedener Methoden bindungsbezogene Merkmale von 26 BPS-Patientinnen und 26 alters- und bildungsgleich gemachten gesunden Kontrollprobandinnen vor, während und nach einer einjährigen ambulanten DBT-Intervention erhoben. Methode: Die Bindungsrepräsentationen wurden mittels Adult Attachment Projective Picture System (AAP) erfasst. Mittels parametrischer- und non-parametrischer Tests wurden Veränderungen des Bindungsstils (ASQ) und subjektive Einschätzungen zu Affektivität, autobiografischer Relevanz und emotionaler Valenz personalisierter Sätze aus den eigenen AAP-Interviews vor, während und nach der einjährigen DBT-Intervention erhoben. Ergebnis: Wie erwartet wiesen die BPS-Patientinnen im AAP signifikant häufiger unverarbeitete Traumata (desorganisierte Bindungsrepräsentation), einen höheren Anteil an traumatisch-dysregulierenden Markern in Narrativen zu AAP Bildern, die potentiell Alleinsein und Verlassenwerden induzieren (monadische Bilder), eine stärkere Bindungsangst und -vermeidung (ASQ) sowie mehr BPS-assoziierte Emotionen bei stärkerer autobiografischer Relevanz und emotionaler Valenz in der Konfrontation mit personalisierten Sätzen des eigenen AAP-Interviews auf. Nach einjähriger DBT wurden Bindungsangst und -vermeidung in der BPS-Gruppe signifikant reduziert, Bindungssicherheit wurde signifikant gestärkt (ASQ). Bei erhalten gebliebener autobiografischer Relevanz gaben die Patientinnen nach DBT in der Konfrontation mit personalisierten Sätzen des eigenen AAP-Interviews weniger BPS-assoziierte Emotionen, sowie mehr Akzeptanz und eine reduzierte emotionale Valenz an und glichen sich damit den Kontrollprobanden an. Schlussfolgerung: Es ergaben sich Hinweise, dass Veränderungen in Richtung einer stärkeren Bindungssicherheit und intrapsychischen Integration bindungsbezogener autobiografischer Aspekte im Rahmen der ambulanten DBT für BPS-Patienten zu erzielen sind. Zum Aufbau einer sicheren therapeutischen Allianz mit positiven Bindungserfahrungen ist ein ausreichender, hier einjähriger, Interventionszeitraum vorauszusetzen.

Beispiele fur die damit verbundenen Inhalte sind die lange Zeit kustodialen Lebens- und Behandlungsbedingungen in den psychiatrischen Krankenhausern, die lange relevante Aversion wichtiger Fachvertreter gegen die judische Psychoanalyse und die Institutionalisierung der Psychotherapie sowie der konzeptionelle und therapeutische Umgang mit Traumafolgestorungen, die durch den Krieg und den Holocaust entstanden waren.

Zusammenfassung Ziel der Studie Studierende mit Spezialisierungspräferenz für eines von 3 nervenheilkundlichen Fächern wurden im Hinblick auf ihre Motivation zum Medizinstudium retrospektiv verglichen. Methodik Mit dem Ziel der Identifikation früher Bedingungen von Spezialisierungspräferenzen wurde eine deutschlandweite Online-Befragung an 9079 Medizinstudierenden durchgeführt. Die Angaben der 669 Studierenden mit nervenheilkundlicher Präferenz wurden varianzanalytisch ausgewertet. Ergebnisse Angehende Neurologen wurden vergleichsweise stärker von inhaltlichen Interessen am Fach Medizin und den Naturwissenschaften, weniger jedoch von Aspekten der Lebensführung motiviert. Hingegen spielten bei der Spezialisierungspräferenz für die psychologischen Fächer der Wunsch, aktiv Hilfe zu leisten, und die eigene Krankheitsanamnese eine wichtigere Rolle. Schlussfolgerung Die aufgezeigten Motive zeigen thematische Richtungen auf, die bei der fachspezifischen Werbung um Nachwuchs verfolgt werden können.

Der Wechsel von der kinderpsychiatrischen in die erwachsenenpsychiatrische Behandlung wird durch das Erreichen der Volljahrigkeit gemeinhin festgelegt. Diese festgesetzte Altersgrenze wird jedoch den unterschiedlichen Entwicklungsstanden und Reifungsgraden junger Erwachsener im Alter von 18 bis 23 Jahren mit psychischen Storungen nicht gerecht. Das Konzept und die ersten Behandlungserfahrungen in einer Tagesklinik fur Adoleszenzpsychiatrie und -psychotherapie werden vorgestellt und diskutiert. Wahrend die institutionelle in- und externe Transition sich als erfolgreich erweisen, sollten diese Ansatze auf die komplementaren Hilfesysteme ubertragen bzw. neue Versorgungsformen entwickelt und implementiert werden.

Der 1970 von Michael Verhoeven gedrehte Film o.k. thematisiert sexualisierte Kriegsgewalt an einem authentischen Beispiel im Vietnamkrieg, verlegt aber die Handlungsfolge analog zu Brechts Konzeption der Verfremdung im epischen Theater in den bayerischen Wald. Der Zuschauer wird Zeuge der erschreckenden Logik, nach der Vergewaltigungen unvermeidliche Realitaten innerhalb von Kriegen sind und in Militarhierarchien gedeckt werden („o.k.“). Die politische Konstellation Ende der 1969er und Anfang der 1970er Jahre, die u. a. durch die Polarisierung der Ost-West-Thematik und durch den Vietnamkrieg gekennzeichnet war, fuhrte dazu, dass der Film von vielen Rezipienten als Skandal betrachtet und wegen ihm die Berliner Filmfestspiele abgebrochen wurden.

Trotzdem lasst sich eine Vielzahl von Ein- und Einschlusskriterien beschreiben, die sich insbesondere auf storungsbezogene, das Krankheitsverhalten betreffende Merkmale sowie auf emotionale und interpersonelle Variablen beziehen. Eine Auswahl der Patienten fur Gruppenpsychotherapien ist ebenso notwendig wie deren Vorbereitung durch Informationsmaterialien oder „pre-group training“, um die vergleichsweise hohen Abbruchraten zu reduzieren.

Retrospektiv wurden fruhe Traumatisierungen in Form von Vernachlassigung, physischer Misshandlung und sexuellem Missbrauch bei erwachsenen Patienten in stationarer psychotherapeutischer Behandlung erfasst. Gepruft wurden Assoziationen zwischen den 3 Traumaformen untereinander und mit dem Geschlecht der Patienten. Auch wurden Effekte fruher Traumatisierung i. Allg. und des Geschlechts sowie spezifischer Traumamuster auf Symptomniveau, Funktionstuchtigkeit im Alltag und Therapieverlauf untersucht. Hierzu wurden 191 Patienten einer psychosomatisch-psychotherapeutischen Universitatsklinik mithilfe standardisierter Fragebogen zu soziodemografischen und klinischen Basisdaten bei Aufnahme und Entlassung befragt. Art und Umfang einer Traumatisierung vor dem 18. Lebensjahr wurden im Anschluss an das Eingangsinterview anhand standardisierter Behandlerangaben eingeschatzt. Die Befunde weisen darauf hin, dass die Expositionswahrscheinlichkeiten fur die untersuchten Formen traumatischer Erfahrung nicht voneinander unabhangig sind. Weibliche Patienten waren zudem sexuellem Missbrauch, aber auch Vernachlassigung in hoherem Mas ausgesetzt als mannliche. Traumatisierte Patienten wiesen im Vergleich zu nichttraumatisierten hohere Symptombelastungen (Symptom-Checkliste [SCL]-90-R) und ein geringeres Funktionsniveau (Global Assessment of Functioning, GAF) auf als nichttraumatisierte, erzielten aber einen vergleichbaren relativen Therapiegewinn. Sexueller Missbrauch (SCL-90-R und GAF) und Vernachlassigung (nur SCL-90-R) wirkten sich auf die Hohe der Beeintrachtigung der Patienten bei Aufnahme und Entlassung aus; es fanden sich aber nur schwache Hinweise auf differenzielle Therapieverlaufe. Neben methodischen Beschrankungen des Untersuchungsansatzes werden Implikationen fur die Gestaltung stationarer Therapiemasnahmen bei fruh traumatisierten Patienten diskutiert.

Die Langzeitfolgen insbesondere komplexer sequenzieller Realtraumatisierungen haben wegen der mit ihnen verbundenen vielschichtigen Symptomatik zur Entwicklung des Konzepts der sog. komplexen posttraumatischen Belastungsstorung (PTBS) gefuhrt. Diese Storung ist durch anhaltende Veranderungen des affektiven Systems, der Selbstwahrnehmung sowie der interpersonellen und sozialen Beziehungen gekennzeichnet. Die Zahl kontrollierter und randomisierter Studien zur Therapie komplexer PTBS ist relativ gering. Die Ergebnisse dieser Studien zeigen, das traumafokussierte kognitiv-behaviorale Interventionen wirksam sind und gute Effektstarken hervorbringen.

Das Pflegepraktikum als Zugangsweg in den psychiatrischen Fachbereich ist weitgehend unbeachtet. Eine deutschlandweite Umfrage analysiert das medizinische Pflegepraktikum unter den Aspekten: praktische Fahigkeiten, Lehre im Pflegepraktikum, Kompetenzerwerb und Zufriedenheit. Die wenigen Studierenden im psychiatrischen Pflegepraktikum weisen auf entscheidende praktische inhaltliche Aspekte hin, geben einen Einblick in die Bewertung der Zufriedenheit und zu dem Zugang in das Fachpflegepraktikum. Eine gelungene inhaltliche Strukturierung des praktischen Initialkontaktes kann Studierende bei der Fachauswahl unterstutzen.

OBJECTIVE: The FKBP5 gene codes for a co-chaperone that regulates glucocorticoid receptor sensitivity and thereby impacts the reactivity of the hypothalamic-pituitary-adrenal (HPA)-axis. Evidence suggested that subjects exposed to childhood abuse and carrying the TT genotype of the FKBP5 gene single nucleotide polymorphism (SNP) rs1360780 have an increased susceptibility to stress-related disorders. METHOD: The hypothesis that abused TT genotype carriers show changes in gray matter (GM) volumes in affect-processing brain areas was investigated. About 1,826 Caucasian subjects (age ≤ 65 years) from the general population [Study of Health in Pomerania (SHIP)] in Germany were investigated. The interaction between rs1360780 and child abuse (Childhood Trauma Questionnaire) and its effect on GM were analyzed. RESULTS: Voxel-based whole-brain interaction analysis revealed three large clusters (FWE-corrected) of reduced GM volumes comprising the bilateral insula, the superior and middle temporal gyrus, the bilateral hippocampus, the right amygdala, and the bilateral anterior cingulate cortex in abused TT carriers. These results were not confounded by major depressive disorders. In region of interest analyses, highly significant volume reductions in the right hippocampus/parahippocampus, the bilateral anterior and middle cingulate cortex, the insula, and the amygdala were confirmed in abused TT carriers compared with abused CT/CC carriers. CONCLUSION: The results supported the hypothesis that the FKBP5 rs1360780 TT genotype predisposes subjects who have experienced childhood abuse to widespread structural brain changes in the subcortical and cortical emotion-processing brain areas. Zusammenfassung. Das Ziel der vorliegenden Analyse war es, herauszufinden welche emotionalen Reaktionen mit einer Fallvignette Depression und deren Einschätzung als Burnout oder Depression verbunden sind. Datengrundlage war eine Bevölkerungsumfrage in Deutschland aus dem Jahr 2011. Die Bezeichnung einer Depression als Depression war mit stärkerer Angst und Unbehagen, jedoch mit weniger Unverständnis verbunden. Die Bezeichnung als Burnout war mit einem stärkeren Bedürfnis zu helfen sowie mit weniger Verunsicherung, Genervtsein und Unverständnis verbunden. Weiterhin ergaben die Analysen, dass es bei Erwerbstätigen zu größerer Verunsicherung führt, wenn sie das Problem Depression, und zu größerem Mitleid führt, wenn sie das Problem Burnout genannt haben. Insgesamt zeigt die vorliegende Arbeit, dass die emotionalen Reaktionen eher positiver sind, wenn das Problem Burnout genannt wird und eher negativer sind, wenn es Depression genannt wird. Wir diskutieren mögliche Ursachen für diese Unterschiede.

Zusammenfassung. Vor dem Hintergrund des Zusammenhangs zwischen politischer Gesellschaftsgestaltung, der Prävalenz psychischer Erkrankungen und dem Organisationsgrad des psychosozialen Hilfesystems stellt sich die Frage nach einer politischen Rolle der Sozialpsychiatrie. In einer offenen Gesellschaft ist die vorurteilsfreie und solidarische Begegnungskultur eine wichtige Voraussetzung, um die Interessen von Menschen mit schweren psychischen Erkrankungen zu vertreten. Nicht zuletzt im Kontext der PEGIDA-Bewegung und «Asylkritiker» offenbart sich jedoch eine unübersichtliche Gemengelage von simplifizierenden Stereotypen, Sorgen und teils menschenfeindlichen Auswüchsen von Wut und Hass. Hier kann die Sozialpsychiatrie einen politischen Beitrag leisten: Indem sie Eskalationsspiralen frühzeitig benennt und erklärt, kann sie eine kritische Begegnung im öffentlichen Raum beeinflussen. Stereotype können das Selbstkonzept eines Bürgers kurzfristig stabilisieren. Eine schnelle und nicht diskursive Abwertung von Stereotypen im öffentlichen Raum kann zu deren Immunisierung sowie Wut und Hass führen. «Angst» und «Wutbürger» sind diminutive Ferndiagnosen mit entlastender Funktion, die diesen Prozess katalysieren. Akteure des öffentlichen Raumes wie Politiker und Journalisten nutzen ebenfalls Stereotype und sind häufig gezwungen, zwischen Wählbarkeit, wirtschaftlichen Interessen und der Kontrolle einer gesellschaftsspaltenden Eskalationsdynamik abzuwägen. Politik im Sinne einer engagierten Sozialpsychiatrie sollte einen reflektierten und hartnäckigen Diskurs auf Augenhöhe zum Abbau von Unbehagen, Wut und Stereotypen moderieren. Das aktive und selbstbewusste Werben für die Zumutung des diskursiven Aneinanderwachsens sollte ein Teil des Selbstverständnisses einer politisch engagierten Sozialpsychiatrie sein.

Attachment characteristics play a key role in understanding borderline-specific problems with respect to childhood maltreatment. The aim of this study was to investigate how attachment representations may influence the trajectory of change in a 1-year outpatient dialectical behavior therapy (DBT) for patients with borderline personality disorder (BPD). Attachment representations were assessed in 26 BPD patients and 26 healthy controls (HC) using the Adult Attachment Projective Picture System (AAP) before treatment. Borderline and global symptom severity and interpersonal problems were examined before, during, and after completing the intervention. Analysis of variance and stepwise hierarchical regression analyses were used to explore the course of symptomatology. As expected, BPD patients displayed a predominance of unresolved attachment in the AAP compared with HC, by showing a lack of ability to integrate attachment related trauma. Whereas both resolved and unresolved attachment groups revealed significant improvement in symptom severity during treatment, dimensional AAP scores showed differences. Patients with higher scores in "synchrony" demonstrated more indicators of mutual care in their narratives to dyadic pictures and displayed a significantly stronger decrease of interpersonal problems than patients with lower synchrony scores. Assessing attachment representations prior to DBT might provide a helpful insight into individual attachment related resources or lack of these capacities. Responsiveness and synchrony in dyadic interactions with significant others are crucial for healthy interpersonal relations. A stronger therapeutic focus on the patient's capacity to show synchrony in dyadic attachment situations might improve the patient's interpersonal problems towards sensitive and mutual interaction.

Previous evidence showed associations of alexithymia with altered declarative memory performance. However, these findings were not fully consistent and the underlying mechanism remains unclear. Alexithymic subjects may be at specific risk for chronic psychosocial stress, which in turn represents a predictor for poorer memory performance. We investigated independent and interaction effects of alexithymia and chronic perceived stress on declarative memory performance. Data were used from two independent general-population samples from the Study of Health in Pomerania (SHIP). In sample 1 (N = 1981), the Auditory Verbal Learning Test, the Toronto Alexithymia Scale (TAS-20) and the Screening Scale for Chronic Stress (SSCS) were applied. In sample 2 (N = 3799), the word list of the Nuremburg Age Inventory and TAS-20 were administered to replicate findings. Alexithymia was significantly associated with poorer immediate and delayed word recall. Chronic stress negatively predicted immediate, but not delayed recall. Alexithymia and particularly “Difficulties Identifying Feelings” showed significant associations with chronic perceived stress. Our findings provide clear evidence for an association of alexithymia with impaired declarative memory performance for words. The strong association of alexithymia with perceived chronic stress could contribute to explain the association of alexithymia with stress-related disorders.

Background Schizophrenia and bipolar disorder are characterized by a high disease burden. Antipsychotic medication is an essential part of the treatment. However, non-adherence is a major problem. Our aim was to examine potential determinants of non-adherence for patients with severe mental disorders. Methods Baseline data of the study “Post stationary telemedical care of patients with severe psychiatric disorders” (Tecla) were used. Medication adherence was assessed with the Medication Adherence Report Scale German version (MARS-D). A logistic regression was calculated with age, sex, education, employment status, level of global functioning, social support and intake of typical and atypical antipsychotics as predictors. Results N = 127 participants were included in the analysis (n = 73 men, mean age 42 years). The mean MARS-D Score was 23.4 (SD 2.5).
