7. Medical Measurement Against The Mega-Disaster : The Necessity of Systematization of The Disaster Medicine or The Disaster Medicine Compendium

Abstract

How to treat them are researched, mainly based on the Japanese experience. Method The Tokyo Subway Sarin attack, 1995, which was caused by the Japanese Aum religeous group with more than 5,000 casualties with intoxication was analyzed, first. In this situation our medical experience of intoxicated patients treated exceeded 230, with more than 20 serious patients.Other C hazards are Matsumoto sarin attack by the Aum, 1994, Osaka VX attack 1994 by Aum, Kuala Lumpur airport VX attack caused by suspicion of North Korea, 2017, etc.Results Among patients involved in the 1995 sarin attack, we actually treated more than 100 policemen and necessitated medical care, in which PAM, Steroids, Atropine phosphate, etc. wereused.u3000Five years after mental influence was researched. Nearly half of seriously contaminated patients(11 among 23, researched) had long-term mental damage, or diagnosed to be PTSD. Most medical doctors are not enough to treat or measure against such catastrophe. In addition to the basic disaster medicine capability, special knowledge, skill, idea regarding the safety/security are essential, considering the possibility of NBC hazards.u3000Discussion The status that most medical staff had been laypersons in the field, seems to be lasting at present, including the Japanese DMAT member.u3000Insufficient medical measure increases victims as well as arouses anxiety of the society and ultimately the society. Basic disaster management are important without question: so-called, CSCATTT or Command Control, Safety, Communication, Assessment, Triage, Treatment and Transportation. However, it should be stressed that it should be promptly done to systematize the disaster medicine or make the compendium, considering the possibility of these special situations.

Abstract

Introduction: The risks of NBC (Nuclear, Biological and Chemical) or CBRNE (R: radiological, E: explosive) hazards are rapidly increasing even in civilian areas, as well as those of natural disasters (earthquakes, hurricanes, etc.). Therefore, one of the most important and emergent issues for medical staff, especially for general surgeons is the necessity of skills to deal with various mega- or major disasters to help people as well as, protecting themselves. This has been a point of emphasis since 2005, when the Disaster Medicine Compendium was published and continues to be updated today. Method: The research focuses on NBC/CBRNE hazards: Pandemics such as COVID-19, Monkeypox, influenza, the Tokyo Subway Sarin Incident, and the 2011 Tōhoku Earthquake, followed by Fukushima Plant Incident, Chernobyl, earthquakes with tsunami, such as the 2004 Indian Ocean earthquake, and the September 11 terrorist attacks in the US. Results: The skills that should be accustomed to are protection, prevention, diminution of toxicity, decontamination, as well as routine medical/surgical treatments. The relevant education is varied and not easily performed. For example, it was found that Japan DMAT or disaster medical assistant teams struggled with a lack of techniques to deal with the nuclear plant hazard during the above-mentioned Fukushima plant accident. Conclusion: In the event of hazards including NBC/CBRNE, surgical skills are necessary. However, medical teams require training in advance. Surgical methods and other skills, intensive care, and examinations performed wearing PPE or personal protection equipment is important, as well as the safety and security of the medical teams, in addition, to supporting the vulnerable/weak victims, ensured using an Incident Command System. 41. Close relationship of tissue plasminogen activator-plasminogen activator inhibitor-1 complex with multiple organ dysfunction syndrome investigated by means of the artificial pancreas

Abstract

Background: Glucose tolerance (GT) has not been taken into consideration in investigations concerning relationships between coagulopathy and multiple organ dysfunction syndrome (MODS), and endothelial cell activation/endothelial cell injury (ECA/ECI) in septic patients, although coagulopathy is known to be influenced by blood glucose level. We investigated those relationships under strict blood glucose control and evaluation of GT with the glucose clamp method by means of the artificial pancreas in nine septic patients with glucose intolerance. The relationships between GT and blood stress related hormone levels (SRH) were also investigated. Methods: The amount of metabolized glucose (Mvalue), as the parameter of GT, was measured by the euglycemic hyperinsulinemic glucose clamp method, in which the blood glucose level was clamped at 80 mg/dl under a continuous insulin infusion rate of 1.12 mU/kg per min, using the artificial pancreas, STG-22. Multiple organ failure (MOF) score was calculated using the MOF criteria of Japanese Association for Critical Care Medicine. Regarding coagulopathy, the following parameters were used: disseminated intravascular coagulation (DIC) score (calculated from the DIC criteria of the Ministry of Health and Welfare of Japan) and the parameters used for calculating DIC score, protein-C, protein-S, plasminogen, antithrombin III (AT-III), plasminogen activator inhibitor-1 (PAI-1), and tissue plasminogen activator-PAI-1 (tPA-PAI-1) complex. Thrombomodulin (TM) was measured as the indicator of ECI. Results: There were no significant correlations between Mvalue and SRH, parameters indicating coagulopathy and the MOF score. The MOF score and blood TM levels were positively correlated with DIC score, thrombin-AT-III complex and tPA-PAI-1 complex, and negatively correlated with blood platelet count. Conclusions: GT was not significantly related to SRH, coagulopathy and MODS under strict blood glucose control. Hypercoagulability was closely related to MODS and ECI. Among the parameters indicating coagulopathy, tPA-PAI-1 complex, which is considered to originate from ECA, seemed to be a sensitive parameter of MODS and ECI, and might be a predictive marker of MODS. The treatment for reducing hypercoagulability and ECA/ECI were thought to be justified as one of the therapies for acutely ill septic patients.
