Abstract

Introduction: Medullary thyroid carcinoma constitutes 5% to 10% of all thyroid cancers. Metastatic adenopathies may pose challenges in intricate anatomical locations, such as the parapharyngeal space. A rare case of metastatic medullary thyroid carcinoma in the parapharyngeal space has been treated in our unit using combined trans-cervical trans-oral robotic surgery. Our objective was to provide a detailed description of the surgery performed on this patient. Method: We reported a singular case report worth of interest. Result: A 42-year-old woman was addressed in our unit for the management of a medullary thyroid carcinoma adenopathy located in the right parapharyngeal space. A parapharyngeal 40.0 mm × 25.0 mm × 12.0 mm adenopathy was removed using a combined trans-cervical and trans-oral robotic approach without sacrifice or injury of vascular or nervous structure. Neither the tracheostomy nor the feeding tube was implemented. Feeding was resumed on postoperative day 1 and hospitalization spanned 7 days. Conclusion: An innovative combined trans-cervical and trans-oral robotic surgery approach was conducted to address a metastatic medullary thyroid carcinoma in the parapharyngeal space. This surgical technique allowed us to circumvent the need for a trans-mandibular approach, tracheostomy, and feeding tube and enabling successful tumor removal without fragmentation. Postoperative care was significantly eased. The sole complication observed was dysphonia, likely resulting from intra-operative stretching of the vagus nerve during the dissection of the carotid artery. [ABSTRACT FROM AUTHOR]

Rooted in the observations and experiences of multiple physicians in the late 19th century, immunological interventions are currently integral to the oncological therapeutic repertoire. This article seeks to delineate the evolution of cancer immunotherapy, tracing its inception in 1891 with the pioneering work of an American surgeon, William B. Coley, who achieved the first documented cure of a cancer case involving a malignant head and neck tumor. 42. Management of Bilateral Vocal Fold Paralysis: A Systematic Review.

Abstract

Objective: To review the current literature about epidemiology, etiologies and surgical management of bilateral vocal fold paralysis (BVFP). Data Sources: PubMED, Scopus, and Cochrane Library. Review Methods: A systematic review of the literature on epidemiology, etiologies, and management of adult patients with BVFP was conducted through preferred reporting items for systematic reviews and meta‐analyses statements by 2 investigators. Results: Of the 360 identified papers, 245 were screened, and of these 55 were considered for review. The majority (76.6%) of BVFP cases are iatrogenic. BVFP requires immediate tracheotomy in 36.2% of cases. Laterofixation of the vocal fold was described in 9 studies and is a cost‐effective alternative procedure to tracheotomy while awaiting potential recovery. Unilateral and bilateral posterior transverse cordotomy outcomes were reported in 9 and 7 studies, respectively. Both approaches are associated with a 95.1% decannulation rate, adequate airway volume, but voice quality worsening. Unilateral/bilateral partial arytenoidectomy data were described in 4 studies, which reported lower decannulation rate (83%) and better voice quality outcome than cordotomy. Revision rates and complications vary across studies, with complications mainly involving edema, granuloma, fibrosis, and scarring. Selective posterior cricoarytenoid reinnervation is being performed by more surgeons and should be a promising addition to the BVFP surgical armamentarium. Conclusion: Depending on techniques, the management of BVFP may be associated with several degrees of airway improvements while worsened or unchanged voice quality. The heterogeneity between studies, the lack of large‐cohort controlled randomized studies and the confusion with posterior glottic stenosis limit the draw of clear conclusion about the superiority of some techniques over others. Abstract

Recently, several experiments were reported using ambient vibration surveys in buildings to estimate the modal parameters of buildings. Their modal properties are full of relevant information concerning its dynamic behaviour in its elastic domain. The main scope of this paper is to determine relevant, though simple, beam modelling whose validity could be easily checked with experimental data. In this study, we recorded ambient vibrations in 3 buildings in Grenoble selected because of their vertical structural homogeneity. First, a set of recordings was done using a 18 channels digital acquisition system (CityShark) connected to six 3C Lennartz 5s sensors. We used the Frequency Domain Decomposition (FDD) technique to extract the modal parameters of these buildings. Second, it is shown in the following that the experimental quasi-elastic behaviour of such structure can be reduced to the behaviour of a vertical continuous Timoshenko beam. A parametric study of this beam shows that a bijective relation exists between the beam parameters and its eigenfrequencies distribution. Consequently, the Timoshenko beam parameters can be estimated from the experimental sequence of eigenfrequencies. Having the beam parameters calibrated by the in situ data, the reliability of the modelling is checked by complementary comparisons. For this purpose, the mode shapes and eigenfrequencies of higher modes are calculated and compared to the experimental data. A good agreement is also obtained. In addition, the beam model integrates in a very synthetic way the essential parameters of the dynamic behaviour.
