1. The role of community healthcare professionals in discussing sexual assault experiences during obstetrics and gynecological healthcare appointments

Abstract: Abstract Background Nearly half of adult women in the US report experiencing sexual assault, with almost one-fifth reporting rape. For many sexual assault survivors, healthcare professionals are the first point of contact and disclosure. This study aimed to understand how healthcare professionals working in community settings perceived their role in discussing sexual violence experiences with women during obstetrical and gynecological healthcare appointments. The secondary purpose was to compare healthcare professionals’ perspectives with the patients’ to determine how sexual violence conversations should occur in these environments. Methods Data were collected in two phases. Phase 1 consisted of 6 focus groups (Sept-Dec, 2019) with women aged 18–45 (n = 22) living in Indiana who sought community-based or private healthcare for women’s reproductive healthcare needs. Phase 2 included 20 key-informant interviews with non-physician healthcare professionals (i.e., NP, RN, CNM, doula, pharmacist, chiropractor) living in Indiana (September 2019-May 2020) who provided community-based women’s reproductive healthcare. Focus groups and interviews were audio-recorded, transcribed, and analyzed using thematic analyses. HyperRESEARCH assisted in data management and organization. Results There were three resulting themes: (1) healthcare professionals’ approaches to screening for a history of sexual violence varied depending on how they ask, what setting they work in, and type of professional asking; (2) healthcare experiences can compound traumatic experiences and create distrust with survivors; and (3) sexual violence impacts patient healthcare experiences through what services they seek, how professionals may interact with them, and what professionals they are willing to utilize. Conclusions Findings offered insight into actionable and practical strategies for enhancing sexual violence screening and discussions in community-based women’s reproductive health settings. The findings offer strategies to address barriers and facilitators among community healthcare professionals and the people they serve. Incorporating healthcare professional and patient experiences and preferences for violence-related discussions during obstetrical and gynecological healthcare appointments can assist in violence prevention efforts, improve patient-professional rapport, and yield better health outcomes.

Abstract: Experimental craniotomies are a common surgical procedure in neuroscience. Because inadequate analgesia appears to be a problem in animal-based research, we conducted this review and collected information on management of craniotomy-associated pain in laboratory mice and rats. A comprehensive search and screening resulted in the identification of 2235 studies, published in 2009 and 2019, describing craniotomy in mice and/or rats. While key features were extracted from all studies, detailed information was extracted from a random subset of 100 studies/year. Reporting of perioperative analgesia increased from 2009 to 2019. However, the majority of studies from both years did not report pharmacologic pain management. Moreover, reporting of multimodal treatments remained at a low level, and monotherapeutic approaches were more common. Among drug groups, reporting of pre- and postoperative administration of non-steroidal anti-inflammatory drugs, opioids, and local anesthetics in 2019 exceeded that of 2009. In summary, these results suggest that inadequate analgesia and oligoanalgesia are persistent issues associated with experimental intracranial surgery. 3. Exploring the Accent Mix Perceptually and Automatically: French Learners of English and the RP–GA Divide

Abstract: Acquiring a consistent accent and targeting a native standard like Received Pronunciation (RP) or General American (GA) are prerequisites for French learners who plan to become English teachers in France. Reliable methods to assess learners’ productions are therefore extremely valuable. We recorded a little over 300 students from our English Studies department and performed auditory analysis to investigate their accents and determine how close to native models their productions were. Inter-rater comparisons were carried out; they revealed overall good agreement scores which, however, varied across phonetic cues. Then, automatic speech recognition (ASR) and automatic accent identification (AID) were applied to the data. We provide exploratory interpretations of the ASR outputs, and show to what extent they agree with and complement our auditory ratings. AID turns out to be very consistent with our perception, and both types of measurements show that two thirds of our students favour an American, and the remaining third, a British pronunciation, although most of them have mixed features from the two accents.

4. 'They’re very passionate about making sure that women stay healthy': a qualitative examination of women’s experiences participating in a community paramedicine program

Abstract: Abstract Background Community paramedicine programs (i.e., physician-directed preventive care by emergency medical services personnel embedded in communities) offer a novel approach to community-based health care. Project Swaddle, a community paramedicine program for mothers and their infants, seeks to address (directly or through referrals) the physical, mental, social, and economic needs of its participants. The objective of this process evaluation was to describe women’s experiences in Project Swaddle. By understanding their experiences, our work begins to build the foundation for similar programs and future examinations of the efficacy and effectiveness of these approaches. Methods We completed 21 interviews with women living in Indiana (July 2019–February 2020) who were currently participating in or had graduated from Project Swaddle. Interviews were audio-recorded, transcribed, and analyzed using a six-phase approach to thematic analysis. Results Program enrollment was influenced by the community paramedics’ experience and connections, as well as information received in the community from related clinics or organizations. Participants viewed the community paramedic as a trusted provider who supplied necessary health information and support and served as their advocate. In their role as physician extenders, the community paramedics enhanced patient care through monitoring critical situations, facilitating communication with other providers, and supporting routine healthcare. Women noted how community paramedics connected them to outside resources (i.e., other experts, tangible goods), which aimed to support their holistic health and wellbeing. Conclusions Results demonstrate Project Swaddle helped women connect with other healthcare providers, including increased access to mental health services. The community paramedics were able to help women establish care with primary care providers and pediatricians, then facilitate communication with these providers. Women were supported through their early motherhood experience, received education on parenting and taking control of their health, and gained access to resources that met their diverse needs.

5. Grimace scale, burrowing, and nest building for the assessment of post-surgical pain in mice and rats—A systematic review

Abstract: Several studies suggested an informative value of behavioral and grimace scale parameters for the detection of pain. However, the robustness and reliability of the parameters as well as the current extent of implementation are still largely unknown. In this study, we aimed to systematically analyze the current evidence-base of grimace scale, burrowing, and nest building for the assessment of post-surgical pain in mice and rats. The following platforms were searched for relevant articles: PubMed, Embase via Ovid, and Web of Science. Only full peer-reviewed studies that describe the grimace scale, burrowing, and/or nest building as pain parameters in the post-surgical phase in mice and/or rats were included. Information about the study design, animal characteristics, intervention characteristics, and outcome measures was extracted from identified publications. In total, 74 papers were included in this review. The majority of studies have been conducted in young adult C57BL/6J mice and Sprague Dawley and Wistar rats. While there is an apparent lack of information about young animals, some studies that analyzed the grimace scale in aged rats were identified. The majority of studies focused on laparotomy-associated pain. Only limited information is available about other types of surgical interventions. While an impact of surgery and an influence of analgesia were rather consistently reported in studies focusing on grimace scales, the number of studies that assessed respective effects was rather low for nest building and burrowing. Moreover, controversial findings were evident for the impact of analgesics on post-surgical nest building activity. Regarding analgesia, a monotherapeutic approach was identified in the vast majority of studies with non-steroidal anti-inflammatory (NSAID) drugs and opioids being most commonly used. In conclusion, most evidence exists for grimace scales, which were more frequently used to assess post-surgical pain in rodents than the other behavioral parameters. However, our findings also point to relevant knowledge gaps concerning the post-surgical application in different strains, age levels, and following different surgical procedures. Future efforts are also necessary to directly compare the sensitivity and robustness of different readout parameters applied for the assessment of nest building and burrowing activities.

6. Hormonal Male Contraception: Formative Research to Develop and Test Communication Messaging

Abstract: Male-controlled contraception is limited, yet a promising hormonal male contraceptive (HMC) development in may increase options for men to share in the work of pregnancy prevention. This study examined college-aged (18–26 years) men’s and women’s perceptions of influential marketing strategy for an experimental novel HMC method, a transdermal gel. Six focus groups ( n = 29) were conducted to glean insights for ad prototype creation. Ad prototypes were designed and then tested in semi-structured interviews ( n = 20) to understand whether the messages were adequately representing the attributes intended while also allowing participants to contribute insights on improving appeal. A constant comparative approach was used for data contextualization and theme identification. Results provided a foundation for the creation of HMC message prototypes, aimed at increasing knowledge and uptake of a novel family planning method for men. Representation of a relatable character, or drawing on authentic experiences, aligned best with participant values. Focus groups illuminated salient concepts for HMC marketing, including humor, destigmatization, information, and credibility. Interview message testing yielded an understanding of elements participants responded most strongly to, including (1) ad sentiment and (2) trustworthiness. Findings provide a basis for understanding the placement of a potential marketing campaign to effectively frame HMC uptake as easy to use, safe, and popular while building on peer norms and group acceptability among a college-aged demographic. Healthcare providers and other practitioners can utilize these findings while engaging in family planning conversations, interventions, and message development.

7. Increased breadth of HIV-1 neutralization achieved by diverse antibody clones each with limited neutralization breadth.

Abstract: Broadly neutralizing antibodies (bNAbs) are rarely elicited by current human immunodeficiency virus type 1 (HIV-1) vaccine designs, but the presence of bNAbs in naturally infected individuals may be associated with high plasma viral loads, suggesting that the magnitude, duration, and diversity of viral exposure may contribute to the development of bNAbs. Here, we report the isolation and characterization of a panel of human monoclonal antibodies (mAbs) from two subjects who developed broadly neutralizing autologous antibody responses during HIV-1 infection. In both subjects, we identified collections of mAbs that exhibited specificity only to a few autologous envelopes (Envs), with some mAbs exhibiting specificity only to a subset of Envs within the quasispecies of a particular sample at one time point. Neutralizing antibodies (NAbs) isolated from these subjects mapped mostly to epitopes in the Env V3 loop region and the CD4 binding site. None of the individual neutralizing mAbs recovered exhibited the cumulative breadth of neutralization present in the serum of the subjects. Surprisingly, however, the activity of polyclonal mixtures comprising individual mAbs that each possessed limited neutralizing activity, could achieve increased breadth of neutralizing activity against autologous isolates. While a single broadly neutralizing antibody targeting one epitope can mediate neutralization breadth, the findings presented here suggest that a cooperative polyclonal process mediated by diverse antibodies with more limited breadth targeting multiple epitopes also can achieve neutralization breadth against HIV-1.

9. Implications for the NHS of inward and outward medical tourism: a policy and economic analysis using literature review and mixed-methods approaches

Abstract: Background: The study examined the implications of inward and outward flows of private patients for the NHS across a range of specialties and services. Objectives: To generate a comprehensive documentary review; to better understand information, marketing and advertising practices; examine the magnitude and economic and health-related consequences of travel; understand decision-making frames and assessments of risk; understand treatment experience; elicit the perspectives of key stakeholder groups; and map out medical tourism development within the UK. Design and participants: The study integrated policy analysis, desk-based work, economic analysis to estimate preliminary costs, savings and NHS revenue, and treatment case studies. The case studies involved synthesising data sources around bariatric, fertility, cosmetic, dental and diaspora examples. Overall, we drew on a mixed-methods approach of qualitative and quantitative data collection. The study was underpinned by a systematic overview and a legal and policy review. In-depth interviews were carried out with those representing professional associations, those with clinical interests and representative bodies (n = 16); businesses and employees within medical tourism (n = 18); NHS managers (n = 23); and overseas providers. We spoke to outward medical travellers (46 people across four treatment case studies: bariatric, fertility, dental and cosmetic) and also 31 individuals from UK-resident Somali and Gujarati populations. Results: The study found that the past decade has seen an increase in both inward and outward medical travel. Europe is both a key source of travellers to the UK and a destination for UK residents who travel for medical treatment. Inward travel often involves either expatriates or people from nations with historic ties to the UK. The economic implications of medical tourism for the NHS are not uniform. The medical tourism industry is almost entirely unregulated and this has potential risks for those travelling out of the UK. Existing information regarding medical tourism is variable and there is no authoritative and trustworthy single source of information. Those who travel for treatment are a heterogeneous group, with people of all ages spread across a range of sociodemographic groups. Medical tourists do not appear to inform their decision-making with hard information and consequently often do not consider all risks. They make use of extensive informal networks such as treatment-based or cultural groups. Motivations to travel are in line with the findings of other studies. Notably, cost is never a sole motivator and often not the primary motivation for seeking treatment abroad. Limitations: One major limitation of the study was the abandonment of a survey of medical tourists. We sought to avoid an extremely small survey, which offers no real insight. Instead we redirected our resources to a deeper analysis of qualitative interviews, which proved remarkably fruitful. In a similar vein, the economic analysis proved more difficult and time consuming than anticipated. Data were incomplete and this inhibited the modelling of some important elements. Conclusions: In 2010 at least 63,000 residents of the UK travelled abroad for medical treatment and at least 52,000 residents of foreign countries travelled to the UK for treatment. Inward referral and flows of international patients are shaped by clinical networks and longstanding relationships that are fostered between clinicians within sender countries and their NHS counterparts. Our research demonstrated a range of different models that providers market and by which patients travel to receive treatment. There are clearly legal uncertainties at the interface of these and clinical provision. Patients are now travelling to further or ‘new’ markets in medical tourism. Future research should: seek to better understand the medium- and long-term health and social outcomes of treatment for those who travel from the UK for medical treatment; generate more robust data that better capture the size and flows of medical travel; seek to better understand inward flows of medical travellers; gather a greater level of information on patients, including their origins, procedures and outcomes, to allow for the development of better economic costing; explore further the issues of clinical relationships and networks; and consider the importance of the NHS brand. 10. Understanding Gender and Sexuality: The Hidden Curriculum in English Schools

Abstract: Schools continue to produce regimes of gender and sexuality, including overt and covert curricula based on assumed essentialist differences between girls and boys, reinforced and regulated through uniform, sport and peer pressure. The recent focus on the experiences of trans and non-binary children in schools makes visible the ways in which all children are subject to heteronormatively gendered regulatory and disciplinary techniques in everyday school life. This article discusses the findings from a pilot study drawing on participatory action research techniques with 42 young people in six workshops in north-east England. Recruitment methods were required to be flexible given the context within which the study was conducted, which was with COVID-19 mitigations in place. This meant that we were not able to be fully inclusive of young people from local youth groups as they were either not meeting or only meeting online. We thus had to mainly recruit from university student societies and student residences from which we organised three workshops; sports organisations from which we organised one workshop, and a local youth group with which we ran one workshop. The final workshop was conducted with young people who had attended one of the previous five workshops, to enable feedback on our analysis. All participants were over 16 years of age. The majority of participants were women (25) with 16 men, including one transman, and one non-binary person. Most identified as white (31) with the rest identifying as Black, East Asian and British 'Other' (11). The focus of the workshops was to explore with young people their memories about where and how they first encountered being 'gendered' and/or having a sexuality. The data has been collected, recorded and transcribed within strict ethical guidelines. The workshop data has been analysed using a grounded theory approach, where we developed the theoretical models from the data. This article focuses on those key moments when their behaviours, presentation and/or ideas were subject to facilitators and/or regulators of their gender and/or sexuality. We draw out the contradictions inherent in, on the one hand, the essentialist rationales for difference and inequalities between genders and sexualities in schools and, on the other hand, the apparent need to enforce these 'natural' differences and inequalities. Participatory creative approaches were adopted in each workshop to promote conversations and drawings about who regulated/facilitated their gender and/or sexuality and how they did so. Each workshop cumulatively informed the next, leading to a sixth synthesising workshop that collectively analysed young people's reflections. Drawing on the conceptual frameworks of epistemic injustice (M. Fricker, "Epistemic Injustice: Power and the Ethics of Knowing," Oxford University Press, 2007) and 'space for action' we conclude that young people want and need brave active spaces to discuss and 'do' gender and sexuality, and to resist essentialism and social control. Schools can be both places where control is created and entrenched and where it can also be resisted. Our research suggests that better whole school responses to dismantle regimes of gender and sexuality can be created by and for young people.

12. Labiodentals /r/ here to stay: Deep learning shows us why

Abstract: The secondary labial articulation which accompanies the post-alveolar approximant /r/ in English has attracted far less attention from linguists than the primary lingual one. However, the lips may be particularly important in the variety of English spoken in England, Anglo-English, because non-lingual labiodental articulations ([ʋ]) are on the rise. Labiodentalisation may be due to speakers retaining the labial gesture at the expense of the lingual one, implying that /r/ is always labiodental even in lingual productions. We verify this assumption by comparing the labial postures of /r/ and /w/ in Anglo-English speakers who still present a lingual component. If post-alveolar /r/ is labiodental, the labial gesture for /w/, which is unequivocally considered rounded, should differ considerably. Techniques from deep learning were used to automatically classify and measure the lip postures for /r/ and /w/ from static images of the lips in 23 speakers. Our results suggest that there is a recognisable difference between the lip postures for /r/ and /w/, which a convolutional neural network is able to detect with a very high degree of accuracy. Measurements of the lip area acquired using an artificial neural network suggest that /r/ indeed has a labiodental-like lip posture, thus providing a phonetic account for labiodentalisation. We finish with a discussion of the methodological implications of using deep learning for future analyses of phonetic data.

15. Perceptions of safety and experiences of gender-based violence at UK music festivals

Abstract: Crime and safety at UK music festivals is a subject of growing concern for festival management, police and festival-goers, bolstered by increasing media coverage of incidents of sexual harassment and sexual assault. To date, however, there has been limited evidence regarding festival-goers’ experiences and perspectives concerning safety, particularly in relation to gender-based violence at music festivals. Using data from a mixed methods pilot study, this article presents the findings of a self-selecting survey of 450 festival-goers which asked respondents about their perceptions of safety and experiences of different crime and harms including gender-based violence at UK music festivals. The findings reveal that most respondents report feeling safe at festivals, but various personal, social and environmental factors may increase or reduce these feelings of safety, and these are gendered. Similarly, although experiences of acquisitive crime, hate crime and stalking were low and broadly similar for women and men, a third of women experienced sexual harassment and 8% experienced sexual assault – significantly higher than the reported levels among male respondents. We argue that festivals must work proactively with key stakeholders and agencies, as well as artists and patrons, to develop clear policies and initiatives to prevent sexual violence.

Abstract: Background: Hypertrophic pyloric stenosis in otherwise healthy neonates frequently requires urgent surgical procedure but anaesthesia care may result in respiratory complications, such as hypoxaemia, pulmonary aspiration of gastric contents, and postoperative apnoea. The primary aim was to study whether or not the incidence of difficult airway management and of hypoxaemia in neonates undergoing pyloric stenosis repair was higher than that in neonates undergoing other surgeries. Methods: Data on neonates and infants undergoing anaesthesia and surgery for pyloric stenosis were extracted from the NEonate and Children audiT of Anesthesia pRactice In Europe (NECTARINE) database, for secondary analysis. Results: We identified 310 infants who had anaesthesia for surgery for pyloric stenosis. Difficult airway management (more than two attempts at laryngoscopy) was higher in children with pyloric stenosis when compared with the entire NECTARINE cohort (7.9% [95% confidence interval {CI}, 5.22–11.53] vs 4.4% [95% CI, 1.99–6.58]; relative risk [RR]=1.81 [95% CI, 1.21–2.69]; P=0.004), whereas transient hypoxaemia with oxygen saturation

Abstract: This paper investigates the influence of visual cues in the perception of the /r/-/w/ contrast in Anglo-English. Audio-visual perception of Anglo-English /r/ warrants attention because productions are increasingly non-lingual, labiodental (e.g., [ʋ]), possibly involving visual prominence of the lips for the post-alveolar approximant [ɹ]. Forty native speakers identified [ɹ] and [w] stimuli in four presentation modalities: auditory-only, visual-only, congruous audio-visual, and incongruous audio-visual. Auditory stimuli were presented in noise. The results indicate that native Anglo-English speakers can identify [ɹ] and [w] from visual information alone with almost perfect accuracy. Furthermore, visual cues dominate the perception of the /r/-/w/ contrast when auditory and visual cues are mismatched. However, auditory perception is ambiguous because participants tend to perceive both [ɹ] and [w] as /r/. Auditory ambiguity is related to Anglo-English listeners' exposure to acoustic variation for /r/, especially to [ʋ], which is often confused with [w]. It is suggested that a specific labial configuration for Anglo-English /r/ encodes the contrast with /w/ visually, compensating for the ambiguous auditory contrast. An audio-visual enhancement hypothesis is proposed, and the findings are discussed with regard to sound change.

Abstract: Throughout the COVID-19 pandemic there has been a corner of society where the spotlight has not fallen. As in ‘ordinary’ times, this is the black hole of prisons, whose population consists of predominantly poor, minoritised and often younger adults. Globally, during the pandemic, people detained in prison have been locked away in solitary, or near solitary, confinement for up to 23-hours a day. In the UK, many have had to choose between fresh air, exercise or a phone call to loved ones each day. There has been little mention of education. This has meant that those in custody endured over a year locked in a cell without access to basic education let alone Higher Education (HE). In examining the state’s responsibility to provide ‘education for all’, we demonstrate, through our collective participation in the Inside-Out Prison Exchange Programme, the value and importance of prison education beyond the current focus on risk, responsibility and recidivism. We evidence the transformative and humanising potential of HE in prison through three key elements–the space and learning environment; the role of voice, recognition and agency; and the power of disruptive and transgressive teaching practice. In this article we shine a light on education in prison during the COVID-19 pandemic. The various impacts of COVID-19 on prison learners exposes new and deeper forms of structural disadvantage that shape the educational experiences and journeys of people in custody. We consider how we can expedite ‘education as the practice of freedom’ (hooks, 1994) for those who are incarcerated during and beyond the pandemic. We conclude by reimagining HE in UK prisons, reflecting upon alternative, more positive, approaches to prison education.

Abstract: Background Community paramedicine programs (i.e., physician-directed preventive care by emergency medical services personnel embedded in communities) offer a novel approach to community-based health care. Project Swaddle, a community paramedicine program for mothers and their infants, seeks to address (directly or through referrals) the physical, mental, social, and economic needs of its participants. The objective of this process evaluation was to describe women’s experiences in Project Swaddle. By understanding their experiences, our work begins to build the foundation for similar programs and future examinations of the efficacy and effectiveness of these approaches. Methods We completed 21 interviews with women living in Indiana (July 2019–February 2020) who were currently participating in or had graduated from Project Swaddle. Interviews were audio-recorded, transcribed, and analyzed using a six-phase approach to thematic analysis. Results Program enrollment was influenced by the community paramedics’ experience and connections, as well as information received in the community from related clinics or organizations. Participants viewed the community paramedic as a trusted provider who supplied necessary health information and support and served as their advocate. In their role as physician extenders, the community paramedics enhanced patient care through monitoring critical situations, facilitating communication with other providers, and supporting routine healthcare. Women noted how community paramedics connected them to outside resources (i.e., other experts, tangible goods), which aimed to support their holistic health and wellbeing. Conclusions Results demonstrate Project Swaddle helped women connect with other healthcare providers, including increased access to mental health services. The community paramedics were able to help women establish care with primary care providers and pediatricians, then facilitate communication with these providers. Women were supported through their early motherhood experience, received education on parenting and taking control of their health, and gained access to resources that met their diverse needs.

Abstract: The COVID-19 global pandemic has impacted everyone’s lives—young and old. When the World Health Organization declared the COVID-19 outbreak a pandemic on 11 March 2020 and one country after another went into lockdown, we as editors of YOUNG and youth researchers living in five different countries naturally started to think about and reflect on what impact the pandemic would have on young people’s everyday lives, their well-being and futures across different national settings. In response to this uncertainty and in the interest of capturing young people’s experiences, we as editors called for this special issue to focus on young people and COVID-19, exploring their changed everyday lives and how they adapted to the global pandemic. To accommodate the immense interest and the high quality of many of the submissions, this special issue of YOUNG is a double issue with 11 articles.

Abstract: Selective serotonin reuptake inhibitors (SSRIs) are commonly prescribed drugs for treating human depression, but the role of serotonin—often called the happy chemical—is complex, associated with the regulation of appetite, anxiety, aggression, and more. With such psychoactive pharmaceuticals being increasingly detected in aquatic environments and their effects on non-target species an uncertainty, this study seeks to investigate how inhibiting the serotonin pathway with the SSRI fluoxetine affects territorial aggression, using the cichlid fishNyassachromiscf.microcephalusas a model. Males of this sand-dwelling Lake Malawi species build bowers to attract females within a lekking system, where male-male competition is intense. Being aggressive in defending against intruders would serve to maximise mating opportunities and reproductive success for the territory holding male. However, after a one-week exposure to fluoxetine at environmentally relevant concentrations, a decline in aggression was observed in the male cichlids. This implies the serotonergic system plays an important role in modulating aggression and therefore sexual selection in cichlid fishes, and that environmental pollution has the potential to disrupt these behaviours.

Abstract: Image sensing technologies are rapidly increasing the cost-effectiveness of biodiversity monitoring efforts. Species differences in the reflectance of electromagnetic radiation can be used as a surrogate estimate plant biodiversity using multispectral image data. However, these efforts are often hampered by logistical difficulties in broad-scale implementation. Here, we investigate the utility of multispectral imaging technology from commercially available unmanned aerial vehicles (UAVs, or drones) in estimating biodiversity metrics at a fine spatial resolution (0.1–0.5 cm pixel resolution) in a temperate calcareous grassland in Oxfordshire, UK. We calculate a suite of moments (coefficient of variation, standard deviation, skewness, and kurtosis) for the distribution of radiance from multispectral images at five wavelength bands (Blue 450 ± 16 nm; Green 560 ± 16 nm; Red 650 ± 16 nm; Red Edge 730 ± 16 nm; Near Infrared 840 ± 16 nm) and test their effectiveness at estimating ground-truthed biodiversity metrics from in situ botanical surveys for 37–1 × 1 m quadrats. We find positive associations between the average coefficient of variation in spectral radiance and both the Shannon–Weiner and Simpson's biodiversity indices. Furthermore, the average coefficient of variation in spectral radiance is consistent and highly repeatable across sampling days and recording heights. Positive associations with biodiversity indices hold irrespective of the image recording height (2–8 m), but we report reductions in estimates of spectral diversity with increases to UAV recording height. UAV imaging reduced sampling time by a factor of 16 relative to in situ botanical surveys. We demonstrate the utility of multispectral radiance moments as an indicator of biodiversity in this temperate calcareous grassland at a fine spatial resolution using a widely available UAV monitoring system with a coarse spectral resolution. Abstract: Background: Neonates and infants are susceptible to hypoxaemia in the perioperative period. The aim of this study was to analyse interventions related to anaesthesia tracheal intubations in this European cohort and identify their clinical consequences. Methods: We performed a secondary analysis of tracheal intubations of the European multicentre observational trial (NEonate and Children audiT of Anaesthesia pRactice IN Europe [NECTARINE]) in neonates and small infants with difficult tracheal intubation. The primary endpoint was the incidence of difficult intubation and the related complications. The secondary endpoints were the risk factors for severe hypoxaemia attributed to difficult airway management, and 30 and 90 day outcomes. Results: Tracheal intubation was planned in 4683 procedures. Difficult tracheal intubation, defined as two failed attempts of direct laryngoscopy, occurred in 266 children (271 procedures) with an incidence (95% confidence interval [CI]) of 5.8% (95% CI, 5.1e6.5). Abstract: In many U.S. metropolitan areas housing costs have skyrocketed in recent years relative to average incomes. A worsening shortage of affordable housing in these metros may push households away from job-rich cities and expensive neighborhoods into outlying areas, where housing is cheaper but jobs are more distant. To examine this issue, we revisit the jobs–housing balance, a popular topic of research in the 1990s, with a focus on the relationship between housing and the spatial location of workers relative to jobs. Our analysis draws on data from the Longitudinal Employer–Household Dynamics Origin–Destination Employment Statistics (LODES) for cities in California in 2002 and 2015. In contrast to earlier jobs–housing balance research, we find that California cities are becoming less self-contained over time, defined as a decline in the number of workers who both live and work within a jurisdiction relative to the number of commuters who travel either into or out of a city for work. Statistical models show that self-containment was higher in cities with lower housing costs and, in 2015, in cities with a greater balance between jobs and employed residents. The deepening housing affordability crisis in many metropolitan areas like those found in California are pushing workers and jobs farther apart, increasing the economic, social, and environmental costs of commuting. Policies to increase the supply of housing in job-rich and high–housing cost areas could help reverse this troubling trend, though they are likely to meet with considerable resistance. Our findings also underscore the importance of efforts that include but extend beyond housing production, such as policies to better match job skills and housing prices to the characteristics of workers.

Abstract: BACKGROUND Little is known about current clinical practice concerning peri-operative red blood cell transfusion in neonates and small infants. Guidelines suggest transfusions based on haemoglobin thresholds ranging from 8.5 to 12��g��dl-1, distinguishing between children from birth to day 7 (week 1), from day 8 to day 14 (week 2) or from day 15 (��week 3) onwards. OBJECTIVE To observe peri-operative red blood cell transfusion practice according to guidelines in relation to patient outcome. DESIGN A multicentre observational study. SETTING The NEonate-Children sTudy of Anaesthesia pRactice IN Europe (NECTARINE) trial recruited patients up to 60 weeks' postmenstrual age undergoing anaesthesia for surgical or diagnostic procedures from 165 centres in 31 European countries between March 2016 and January 2017. PATIENTS The data included 5609 patients undergoing 6542 procedures. Inclusion criteria was a peri-operative red blood cell transfusion. MAIN OUTCOME MEASURES The primary endpoint was the haemoglobin level triggering a transfusion for neonates in week 1, week 2 and week 3. Secondary endpoints were transfusion volumes, 'delta haemoglobin' (preprocedure - transfusion-triggering) and 30-day and 90-day morbidity and mortality. RESULTS Peri-operative red blood cell transfusions were recorded during 447 procedures (6.9%). The median haemoglobin levels triggering a transfusion were 9.6 [IQR 8.7 to 10.9] g��dl-1 for neonates in week 1, 9.6 [7.7 to 10.4] g��dl-1 in week 2 and 8.0 [7.3 to 9.0] g��dl-1 in week 3. The median transfusion volume was 17.1 [11.1 to 26.4] ml��kg-1 with a median delta haemoglobin of 1.8 [0.0 to 3.6] g��dl-1. Thirty-day morbidity was 47.8% with an overall mortality of 11.3%. CONCLUSIONS Results indicate lower transfusion-triggering haemoglobin thresholds in clinical practice than suggested by current guidelines. The high morbidity and mortality of this NECTARINE sub-cohort calls for investigative action and evidence-based guidelines addressing peri-operative red blood cell transfusions strategies. Abstract: The reciprocal nature of the relationship between the concepts of responsibilisation and resilience appears, in policy and political circles at least, almost natural. Whilst both concepts have been subjected to sustained academic critique for their presentation as largely individual or familial qualities, and their negation of social and structural pressures, there has been more limited attention paid to the potential of the concepts if they were put to work in a different way. This article attempts to sketch out ways in which the fundamentally relational aspects of the concepts of responsibility and resilience can be brought to the fore. In doing so, it builds on Rose and Lentzos’s argument that we should perhaps ‘argue not against responsibility and resilience but on the territory of responsibilities and resiliencies’ and sets out the case for engaging with, rather than withdrawing from or resisting discussions of the meanings and uses of these concepts, in tandem. Extending the work of Bourdieu and Wacquant, it argues for the need to turn the lens on the structures and mechanisms of power which promote and maintain inequality and divisive complex social relations, which undermine the possibility of collective ‘resilience’. The article advocates our collective ‘responsibility’ as engaging in processes that challenge and redefine these practices and structures to enable resistance and progressive action.

It presents studies focused on drug use and supply, sex work, sexual exploitation and experiences of imprisonment. Bringing together cutting-edge feminist research, this book exposes the intersecting oppressions and social control often central to women’s experiences of the justice system and offers invaluable insights for developing penal policies that account for the needs of women.

Abstract: Since 1980, many have marveled at Los Angeles’“innovation” of funding transportation through ballot measures that are raising billions for transportation improvements. In fact, historically much transportation infrastructure in Los Angeles was financed by local voter-approved revenues. It began in 1868 with a narrowly approved $225,000 bond measure to build the region’s first railroad, followed by an 1876 measure to grant the Southern Pacific railroad a $602,000 subsidy to entice the company to route its transcontinental line through the region. Angeleno voted on an additional 23 different transportation-related ballot measures between the passage of the Good Roads Act (1908) and the end of the New Deal (1937)—a key period of Los Angeles’ history that saw dramatic population increase and with it political contention over the direction of the region’s growth. Overall, these early transportation measures fared well with voters. Of the 25 transportation-related ballot measures in Los Angeles County from 1860 to 1960, only seven (28%) failed to pass, a far better record than nontransportation measures of which 21 of 31 (71%) went down to defeat. Regardless of whether, as some contend, Los Angeles missed a golden opportunity to create the backbone of an effective transit system that would have reduced the need for automobiles and spending many billions on freeways, it is clear that local voters have long faced competing visions for the future of Los Angeles and arguments over whether to fund transportation systems to serve these visions.

50. Low-Income Workers, Residential Location, and the Changing Commute in the United States

Abstract: Numerous reports suggest that rising rents in some U.S. metropolitan areas are pushing workers to live further from their workplaces over time and contributing to lengthening commutes. Drawing on data from the last three vintages of the U.S. National Household Travel Survey (2001, 2009, and 2017), we test whether the relationship between residential location and commute distance varies significantly between low-income and higher-income workers and has changed over time. The data show that commute distances have increased for both low- and higher-income workers with much of the increase occurring in lower density areas. Statistical models show a strong positive relationship between living in a low-density neighbourhood and commute distance for all workers. This relationship appears stronger for low-income than higher-income workers but the strength of the relationship has not increased over time. The findings suggest that the growth in commute distance among low-income workers is largely due to a shift in their residential location towards low-density neighbourhoods.
