Abstract

OBJECTIVES:. To compare the performance of Sequential Organ Failure Assessment, systemic inflammatory response syndrome, Red Flag Sepsis, and National Institute of Clinical Excellence sepsis risk stratification tools in the identification of patients at greatest risk of mortality from sepsis in nonintensive care environments. DESIGN:. Secondary analysis of three annual 24-hour point-prevalence study periods. SETTING:. The general wards and emergency departments of 14 acute hospitals across Wales. Studies were conducted on the third Wednesday of October in 2017, 2018, and 2019. PATIENTS:. We screened all patients presenting to the emergency department and on the general wards. MEASUREMENTS AND MAIN RESULTS:. We recruited 1,271 patients, of which 724 (56.9%) had systemic inflammatory response syndrome greater than or equal to 2, 679 (53.4%) had Sequential Organ Failure Assessment greater than or equal to 2, and 977 (76.9%) had Red Flag Sepsis. When stratified according to National Institute of Clinical Excellence guidelines, 450 patients (35.4%) were in the “High risk” category in comparison with 665 (52.3%) in “Moderate to High risk” and 156 (12.3%) in “Low risk” category. In a planned sensitivity analysis, we found that none of the tools accurately predicted mortality at 90 days, and Sequential Organ Failure Assessment and National Institute of Clinical Excellence tools showed only moderate discriminatory power for mortality at 7 and 14 days. Furthermore, we could not find any significant correlation with any of the tools at any of the mortality time points. CONCLUSIONS:. Our data suggest that the sepsis risk stratification tools currently utilized in emergency departments and on the general wards do not predict mortality adequately. This is illustrated by the disparity in mortality risk of the populations captured by each instrument, as well as the weak concordance between them. We propose that future studies on the development of sepsis identification tools should focus on identifying predicator values of both the short- and long-term outcomes of sepsis.

2. What matters to families about the healthcare of preterm or low birth weight infants: A qualitative evidence synthesis

Abstract

BackgroundPreterm and low birth weight (LBW) infants have complex healthcare needs. In this qualitative evidence synthesis, we aimed to understand what matters to families about the healthcare provided from birth to preterm or LBW infants, to inform appropriate planning and delivery of care.MethodsWe searched nine databases and the reference lists of included studies to identify eligible studies that used qualitative methods to examine the views of families relating to healthcare for preterm or LBW infants. Studies in all countries (low, middle, and high-income) and healthcare settings (home, community, primary, secondary and tertiary care) were eligible for inclusion. Study quality was assessed using the Critical Appraisal Skills Programme checklist for qualitative studies. The GRADE-CERQual approach was used to assess confidence in each review finding. Thematic synthesis techniques were used for analysis.Results203 studies (in 208 papers) were eligible for inclusion. 49 studies (in 51 papers) from 25 countries were sampled for the analysis, based on pre-specified criteria (methodological quality; data richness; and ensuring representation across continents and from settings with varying resources). Eight analytical themes were identified. What mattered to carers was: a positive outcome for the child; active involvement in care; support to cope at home after discharge; emotional support for the family; the healthcare environment; information needs were met; logistical support was available; and positive relationships with staff. Confidence in most results was high to moderate.ConclusionWe found high consistency in what matters to families regardless of location or resources, even though carers reported a variety of experiences in the care of their preterm or LBW infant. Families views on care in neonatal units have been extensively studied. Abstract

RationaleMany expectant and new mothers have been impacted by the restructuring of maternal services that has occurred in response to the pandemic. Although there is hopefully an end in sight, restrictions may continue for some months. It is therefore important to assess the changes that have occurred, in order to prevent further harm to mothers, or to realise and implement any positive changes permanently. Aim: To analyse the different ways in which antenatal, obstetric, and postnatal services in the UK have adapted to the COVID-19 pandemic.MethodsKey-word searches were used to systematically scope three databases for papers that reported on any aspect of maternal care or services and COVID-19 until 03/12/2020. In total, 2,299 papers’ titles and abstracts were screened according to pre-set eligibility criteria. Half were double-screened by a second researcher, and discrepancies were discussed between three researchers until resolved. Full-text screening (n=344) excluded papers that weren’t relevant, or didn’t include primary data. Excluded papers and papers that the lead researcher flagged for a second opinion were checked by a second researcher. At this point, the decision was made to focus on UK services, due to the high number of papers. Next steps will be to extract data from the final sample of papers (n=12) and appraise their quality. Narrative synthesis will form a coherent report on how services in the UK have changed.ResultsPreliminary results show that there have been shifts in some maternal services towards virtual delivery. Additionally, there has been a change in the number of antenatal and postnatal appointments, including the number and nature of antenatal emergency presentations. There has also been a reduction in the care choices available to pregnant women, including changes to birth plans. Furthermore, there have been changes to obstetric unit layouts and care delivery, including changes to the rates of C-sections and general anaesthesia. Additional results will be submitted before the June 28th.Population Health RelevanceThis review is expected to highlight some interesting changes that have occurred in UK maternal services since the beginning of the pandemic. Some may have changed maternal services for the better, while others may highlight some difficulties that new mothers have faced, and will be important to address in case of continuing restrictions or future pandemics. Further research will focus on qualitatively assessing the experiences of new mothers during the pandemic, to gain first-hand insights into how they were affected by the changes.

Abstract

The new NHS 'Long Term Plan' has a particular focus on the public health issues of disease prevention and reducing health inequalities. However, medical students often perceive public health as abstract and irrelevant to clinical practice. We believe students need to be encouraged to appreciate wider public health issues and ultimately be able to apply clinical and public health tools to achieve change for patients and populations. Our aim is for all medical graduates to be able to apply public health and evidence-based principles to their chosen specialties. In the undergraduate curriculum, we hope to extend problem-, case-, and simulation-based learning into public health education, emphasise the context around statistics and epidemiology teaching, and make the teaching more relevant, tangible and enjoyable. Intercalated BSc students will gain an interdisciplinary perspective by joining Master of Public Health (MPH) students to learn about the prevention and control of disease and the promotion of health and wellbeing. They will also have opportunities to join the new Health Intelligence Team (H.I.T.), on a voluntary reserve list to support Public Health Wales in the event of real investigations. We will evaluate these strategies, and we hope that medical educators worldwide will share their experience of innovative approaches to public health and evidence-based medicine teaching in response to this article, so that public health teaching may be improved.
