Dyslipidemia is one of the common disorders in the diabetes mellitus (DM) patients causes cardiovascular disorders. Bacterial infections and sepsis remain significant causes of morbidity and mortality in critical cases in Intensive Care Units (ICU) around the world. A quick and accurate diagnosis for sepsis is a challenge for clinicians and laboratory. The problem of an existing marker for sepsis with high sensitivity and specificity is still not satisfactory. Serum procalcitonin test is still widely used but cannot be supported by hospital laboratories especially in the regional public hospitals and needs more cost. Neutrophil-lymphocyte ratio is a potential index, easy, fast and cheap for bacteremia in patients with sepsis. This study aimed to determine the agreement of neutrophil-lymphocyte ratio with serum procalcitonin as a bacterial infection marker in sepsis patients. This study was an observational, analytical study with a cross-sectional design of 30 patients who had been diagnosed as sepsis by clinicians based on clinical signs of sepsis in the Intensive Care Unit of the Dr. M. Djamil Hospital Padang. The study was conducted from February to July 2016. Neutrophil and lymphocyte count was performed by flow cytometry method and procalcitonin test with Enzyme Linked Fluorescent Assay (ELFA). Results were analyzed by Kappa test with a statistical significance if p < 0.05. There were 30 subjects, consisting of 15 females (50%) and 15 males (50%) with the age mean of 48 years. Based on the Kappa test, the value obtained was kappa=0.634 (p=0.001) for the ratio neutrophil lymphocyte with procalcitonin. There was a substantial agreement between ratio neutrophil lymphocyte with procalcitonin test.

Sepsis neonatorum merupakan penyebab utama morbiditas dan mortalitas neonatus di negara berkembang. Baku emasnya adalah kultur darah, namun sulit dilakukan. Pemeriksaan hematologi sederhana diperlukan untuk membantu klinisi menegakkan diagnosis sepsis neonatorum. Hematologic Scoring System (HSS) memiliki keunggulan terutama kemudahan dan kemurahannya. Tujuan penelitian untuk mengetahui gambaran HSS pada tersangka sepsis neonatorum. Penelitian ini merupakan penelitian deskriptif dengan rancangan potong lintang pada 34 pasien dengan diagnosis tersangka sepsis neonatorum awitan dini. Penelitian dilaksanakan mulai Desember 2015 hingga September 2016. Masing-masing parameter dalam skor HSS diperoleh dari pemeriksaan dengan alat hematologi otomatis dan sediaan hapus darah tepi. Hasil penelitian disajikan dalam bentuk tabel distribusi frekuensi. Tersangka sepsis neonatorum lebih banyak bayi laki-laki (58,8%), bayi berat badan lahir rendah (61,8%), dan bayi kurang bulan (64,7%). Sebanyak 14 pasien (41,2%) memiliki skor HSS 5 (sepsis). Metabolic acidosis is prevalent among critically ill patients and the common cause of metabolic acidosis in ICU is lactic acidosis. However, not all ICUs can provide lactate measurement. The traditional method that uses Henderson-Hasselbach equation (completed with BE and AG) and alternative method consisting of Stewart and its modification (BDEgap and SIG), are acid-base balance parameters commonly used by clinicians to determine metabolic acidosis in critically ill patients. The objective of this study was to discover the association between acid-base parameters (BE, AGobserved, AGcalculated, SIG, BDEgap) with lactate level in critically ill patients with metabolic acidosis. This was an analytical study with a cross-sectional design. Eighty-four critically ill patients hospitalized in the ICU department Dr. M. Djamil Padang Hospital were recruited in this study from January to September 2016. Blood gas analysis and lactate measurement were performed by potentiometric and amperometric method while electrolytes and albumin measurement were done by ISE and colorimetric method (BCG). Linear regression analysis was used to evaluate the association between acid-base parameters with lactate level based on p-value less than 0.05. Fourty five (54%) were females and thirty-nine (46%) were males with participant's ages ranged from 18 to 81 years old. Postoperative was the most reason for ICU admission (88%). Linear regression analysis showed that p-value for BE, AGobserved, AGcalculated, SIG and BDEgap were 119; 0.967; 0.001; 0.001; 0.689, respectively. Acid-base balance parameters which were mostly associated with lactate level in critically ill patients with metabolic acidosis were AGcalculated and SIG.

Sepsis is a systemic inflammatory response with the presence of suspected or proven infection. The inflammation will increase the production of proinflammatory cytokines that will activate coagulation and suppress fibrinolytic system. An imbalance of hemostasis mechanism and inflammation in sepsis can progress into Disseminated Intravascular Coagulation (DIC). The objective of this study was to determine the coagulation activity (platelet count, Prothrombin Time (PT), activated Partial Thromboplastin Time (aPTT)), and D-dimer in sepsis patients. A descriptive study was conducted at the Dr. M. Djamil Hospital on December 2012-July 2013. Subjects were adult patients with two or more of four criteria for Systemic Inflammatory Response Syndrome (SIRS) and presence of suspected or proven infection admitted to the Department of Internal Medicine. Laboratory examination was conducted in the Clinical Pathology Laboratory Dr. M. Djamil Hospital including the coagulation activity and D-dimer level. Data were processed manually and presented in distribution tables, and diagrams. Subjects of this study were 54 sepsis patients, consisting of 57% males and 43% females with a median age of 53.5 year-old, platelet count 188.5 x 103/µL, PT 13.7 seconds, aPTT 39.3 seconds, and D-dimer level 1.15 μg/mL. Based on this research it can be concluded that coagulation activity in sepsis patients showed that platelets count were still in the normal range while PT and aPTT were prolonged and D-dimer levels were increased.

Malaria masih merupakan masalah kesehatan di daerah tropis dan sub tropis terutama Asia dan Afrika dengan angka kesakitan dankematian yang tinggi. Parasit masuk ke dalam darah selain menimbulkan gejala klinis berupa demam, juga diduga memicu terjadinyaperubahan hematologi antara lain monositosis dan trombositopenia. Penelitian ini bertujuan mengetahui uji diagnostik tolok ukurhematologi di pasien terduga malaria. Penelitian uji diagnostik potong lintang ini dilakukan terhadap 60 orang pasien terduga malariayang memenuhi patokan kesertan dan nonkesertaan masa waktu Juli 2015 sampai Maret 2016 di Instalasi Laboratorium Sentral RSUP.Dr. M. Anemia usually happened in childhood. Babies 6–12 years old as are those group that are sensitive to iron deficiency anemia (IDA)Hemoglobin is a mean corpuscular volume (MCV), red-cell distribution width (RDW) examination with blood film evaluation that canbe used to screen IDA, especially in high risk babies. This study was carried out by the cross sectional method in healthy babies 6-12years old in Padang city, on November 2006 until January 2007. They were examined in the Clinical Pathology Laboratory of Dr. MDjamil hospital Padang. The samples consisted of 2 ml venous blood EDTA and laboratory examinations. The results were Hb, MCV,and RDW with ABX micros 60. Also blood film evaluation if Hb were 11g/dL. The means of Hb, MCV and RDW were 11.1 ± 1.07 g/dL,71.7 ± 4.70 fl and 14. 2 ± 1.37%. In 62 (45.9%) babies were anemic as a result of IDA examination and the rest of 33 babies (532%) the blood film were normocytic normochromic 49% and normocytic hypochromic (+), mild anisocytosis 51%. Myelodysplastic syndrome (MDS) merupakan gangguan klonal sel stem hematopoietik berupa sitopenia, displasia, dan cenderung bertransformasi menjadi leukemia mieloid akut. Diagnosis MDS ditegakkan berdasarkan pemeriksaan hematologi, morfologi sel darah tepi, sumsum tulang, pemeriksaan lanjutan sitogenetika dan imunofenotiping, namun dua pemeriksaan terakhir ini tidak tersedia di semua rumah sakit di Indonesia sehingga diagnosis MDS terbatas pada pemeriksaan morfologi sel darah tepi dan sumsum tulang. Tujuan penelitian ini adalah mengetahui gambaran hematologi pasien MDS di RSUP Dr. M. Djamil Padang, merupakan penelitian deskriptif di laboratorium sentral RSUP Dr. M. Djamil Padang dari bulan November 2016 sampai dengan Oktober 2017. Populasi adalah semua pasien yang melakukan pemeriksaan sumsum tulang. Sampel penelitian adalah semua pasien yang telah didiagnosis MDS dari hasil pemeriksaan hematologi, sediaan hapus darah tepi dan sumsum tulang. Didapatkan 19 penderita MDS yang terdiri dari laki-laki 52,6% dan perempuan 47,4%, dengan perbandingan 1,1 : 1; rata-rata usia 40,6 tahun. Hasil pemeriksaan fisik: anemia 68,4%, anemia+organomegali 31,6%. Pemeriksaan hematologi: anemia+leukopenia+trombositopenia 57,8%, anemia+leukopenia 21,1%, anemia+trombositopenia 21,1%. Pemeriksaan sediaan hapus darah tepi: gambaran eritosit, ukuran: anisopoikilositosis 73,6%, normositik 26,4%; warna: polikrom 57,8%, normokrom 42,2%, ditemukan blast pada 42,1% penderita. Pemeriksaan sediaan hapus sumsum tulang, selularitas: hiposelular 73,6%, hiperselular 21,1%, normoselular 5,3%. Displasia seri eritropoietik+granulopoietik+trombopoietik 47,4%; seri eritropoietik+granulopoietik 31,5%; seri eritropoietik+trombopoietik 15,8%; seri eritropoietik saja 5,3%. Gambaran hematologi pasien MDS terbanyak pada penelitian ini adalah anemia, anisopoikilositosis, polikrom, selularitas hiposelular, dan ditemukannya displasia seri eritropoietik, granulopoietik dan trombopoietik.
