BACKGROUND AND OBJECTIVE: The aim of this study was to compare the efficacy of photobiomodulation therapy (PBMT) and photodynamic therapy (PDT) as adjuncts to mechanical debridement (MD) for the treatment of peri-implantitis. The present study is based on the null hypothesis that there is no difference in the peri-implant inflammatory parameters (modified plaque index [mPI], modified gingival index [mGI], probing depth [PD]) and crestal bone loss (CBL) following MD either with PBMT or PDT in patients with peri-implantitis. METHODS: Forty-nine patients with peri-implantitis were randomly categorized into three groups. In Groups 1 and 2, patients underwent MD with adjunct PBMT and PDT, respectively. In Group 3, patients underwent MD alone (controls). Peri-implant inflammatory parameters were measured at baseline and 3-months follow-up. P-values < 0.01 were considered statistically significant. RESULTS: At baseline, peri-implant clinicoradiographic parameters were comparable in all groups. Compared with baseline, there was a significant reduction in mPI (P< 0.001), mGI (P< 0.001) and PD (P< 0.001) in Groups 1 and 2 at 3-months follow-up. In Group 3, there was no difference in the scores of mPI, mGI and PD at follow-up. At 3-months follow-up, there was no difference in mPI, mGI and PD among patients in Groups 1 and 2. The mPI (P< 0.001), mGI (P< 0.001) and PD (P< 0.001) were significantly higher in Group 3 than Groups 1 and 2. The CBL was comparable in all groups at follow-up. CONCLUSION: PBMT and PDT seem to be useful adjuncts to MD for the treatment of peri-implant soft-tissue inflammation among patients with peri-implantitis.

BACKGROUND The aims of this study were to develop a prediction model for identification of individuals with diabetes based on clinical and perceived periodontal measurements; and to evaluate its added value when combined with standard diabetes screening tools. METHODS The study was carried out among 250 adults attending primary care clinics in Riyadh (Saudi Arabia). The study adopted a case-control approach, where diabetes status was first ascertained, and the Finnish Diabetes Risk Score (FINDRISC), Canadian Diabetes Risk questionnaire (CANRISK), and periodontal examinations were carried out afterward. RESULTS A periodontal prediction model (PPM) including three periodontal indicators (missing teeth, percentage of sites with pocket probing depth ≥6 mm, and mean pocket probing depth) had an area under the curve (AUC) of 0.694 (95% Confidence Interval: 0.612-0.776) and classified correctly 62.4% of participants. The FINDRISC and CANRISK tools had AUCs of 0.766 (95% CI: 0.690-0.843) and 0.821 (95% CI: 0.763-0.879), respectively. The addition of the PPM significantly improved the AUC of FINDRISC (P = 0.048) but not of CANRISK (P = 0.144), with 26.8% and 9.8% of participants correctly reclassified, respectively. Finally, decision curve analysis showed that adding the PPM to both tools would result in net benefits among patients with probability scores lower than 70%. CONCLUSIONS This study showed that periodontal measurements could play a role in identifying individuals with diabetes, and that addition of clinical periodontal measurements improved the performance of FINDRISC and CANRISK.

The objective of this cross-sectional study was to evaluate the relationship between ABO blood groups and periodontal diseases.Four hundred sixteen subjects (223 females, 193 males) were recruited according to the eligibility criteria. Periodontal examination was performed, including full-mouth plaque index (PI), bleeding on probing (BOP), clinical attachment level (CAL), and interproximal bone loss (IBL). ABO blood group patterns were determined based on self-reports, confirmed by medical records. The chi-squared test was done to evaluate the data (p0.05).Out of the 416 subjects, 52.2% were blood group O, whereas 27.8% were blood group A. 46.8% of patients with blood group O had gingivitis and 49.6% had periodontitis. 31.2% of patients with blood group A had gingivitis,while 29.5% had periodontitis. The blood group with the lowest percentage among patients with gingivitis was AB, with a rate of 6.2%; in this blood group, 8.1% had periodontitis.There is no association between periodontal diseases and ABO blood group types.

BACKGROUND Emergence of peri-implant diseases led to the development of various methods for implant surface decontamination. This study was designed to compare the efficacy of biofilm removal from implant-like titanium surfaces by an erbium-doped yttrium-aluminum-garnet (Er:YAG) laser, titanium brush, and carbon fiber curet. METHODS Eight study subjects were recruited. A custom mouth appliance that held eight sandblasted and acid-etched titanium discs was fabricated for each subject. Subjects were asked to wear this appliance for 72 hours to allow for biofilm development. After retrieval, discs were removed and randomized to one of four treatment groups. The discs were stained with a two-component nucleic acid dye kit, and the residual biofilm was visualized under fluorescence microscopy. Quantification of residual biofilm was performed using an image analysis software and expressed as the percentage surface area. RESULTS Fifty-nine titanium discs were randomized to the four treatment groups. The percentage of titanium disc area covered by residual biofilm was 74.0% ± 21.6%, 32.8% ± 24.0%, 11.8% ± 10.3%, and 20.1% ± 19.2% in the control, Er:YAG, titanium brush and carbon fiber curet groups, respectively (mean ± SD). The biofilm-covered area significantly decreased in each of the three treatment groups compared with control (P < 0.008). Comparisons between treatment groups did not reveal statistical significance. CONCLUSIONS Er:YAG laser treatment is an effective method for reducing the bacterial biofilm on titanium discs. However, on a threadless titanium surface, Er:YAG laser does not exhibit a significantly greater efficacy in biofilm removal than commonly used titanium brushes or carbon fiber curets.

Hepatocyte growth factor (HGF) production by oral fibroblasts is enhanced by various molecules that are induced during inflammatory conditions including periodontitis. HGF plays an important role in the progression of periodontitis, by stimulating intense growth of epithelial cells and preventing regeneration of connective tissue attachments. Smokers have a greater risk factor in the pathogenesis and progression of periodontal disease. The objective of the study was to estimate the level of HGF in saliva and gingival crevicular fluid (GCF) in smokers with periodontitis and to compare these levels with that of nonsmokers with periodontitis and healthy controls. The HGF levels were found to be significantly high in the saliva and GCF of smokers with periodontitis compared to both never-smokers with periodontitis and the healthy control group. The elevated levels of HGF in the saliva and GCF in the study population could explain the intrinsic mechanism triggering the severity of the periodontitis in smokers. Further studies are necessary to validate the current observations and to establish a sensitive marker to predict periodontal disease activity.

Background/purpose There is limited information in the literature regarding the effect of different luting agents on the bond strength of zirconium oxide (ZrO 2 ) copings to prepared crown. The aim of this in vitro study was to evaluate the ability of different luting agents to retain ZrO 2 copings on prepared crowns under laboratory conditions. Materials and methods Forty-two extracted human maxillary first premolars were prepared with a flat occlusal surface, and rounded line angles, a 5° taper and approximately 3-mm occluso-gingival height. ZrO 2 copings were manufactured with buccal and lingual projections to assist removal of the crown after cementation. All copings were airborne-particle abraded with 50 μm aluminum oxide for 15 seconds. The specimens were randomly distributed into three equal groups ( n = 14) and cemented with one of three luting agents: resin-modified glass–ionomer cement, self-adhesive resin cement, and adhesive resin cement. The cemented specimens were thermocycled (3000 cycles, 5–55°C), and then removed along the path of insertion using a universal testing machine at 0.5 mm/minute. Statistical analyses of the data were performed using one-way ANOVA (α = 0.05). Results Mean copings bond strengths were 440 N, 416 N, and 360 N for resin-modified glass–ionomer cement, self-adhesive resin cement, and adhesive resin cement, respectively. There was no statistically significant difference in mean crown retention between the three cementation groups. Conclusion Retention of ZrO 2 copings to prepared crown was not influenced by the types of luting agents.

Osteoporosis has emerged as a major health problem affecting middle-aged and older individuals. It is characterized by a reduced bone mass and strength, resulting in increased susceptibility to fractures. The disease is associated with several risk factors, and increasing evidence suggests that it may be associated with oral health conditions such as periodontal disease, reduced jaw bone density and tooth loss. Besides the effects of osteoporosis on oral health, bisphosphonate-related osteonecrosis of the jaws is a major concern to the dentist. Bisphosphonate-related osteonecrosis of the jaws is a recently described adverse effect of bisphosphonate therapy. The exact mechanisms by which these drugs cause necrosis of the jaws remain unclear, and a true cause-and-effect relationship between osteonecrosis of the jaw and bisphosphonate use has not yet been established. Hence, any form of invasive dentoalveolar treatment should be performed with caution in patients taking bisphosphonates. This review discusses current evidence on osteoporosis and its treatment implications as a risk factor in the development of various oral diseases.

Maternal health has long been recognized as an important determinant in reducing the risk for pregnancy-related complications such as preterm birth and preeclampsia. Preterm (PTB) delivery and low birth weight (LBW) are considered to be the most relevant biological determinants of newborn infant survival in both developed and developing countries. The oral changes that can occur in pregnancy have been a focus of interest for many years. Physiological changes that occur in pregnant women can adversely affect oral health. Eleva‐ tions in estrogen and progesterone enhance the inflammatory response and consequently alter the gingival tissue (Mascarenhas et al., 2003). During pregnancy, the incidences of gingivitis and periodontitis are increased, and many pregnant women suffer from bleeding and spongy gums.

Periodontal diagnosis and treatment plan are based on the assessment of probing depth, clinical attachment level, plaque index, gingival index, bleeding on probing, suppuration, furcation involvement, mobility, and radiographic findings. However, these clinical parameters are not sufficiently sensitive and specific to identify disease activity in individual sites or to predict future attachment loss. Hence, attention is focused on the development of diagnostic tools that could screen and differentiate the active inflamed sites and predict future tissue destruction. Gingival crevicular fluid (GCF), has gained great interest on possible diagnostic value in periodontal disease. It contains a large number of proteins and peptides derived from inflamed host tissues. The analysis of the GCF components can reflect the disease status of individual sites and thus, identify potential biomarkers of periodontitis. A literature search was carried out to find out all the available tests that indicate periodontal disease markers in GCF. All major databases were searched to compile the information on published reports between 1999 and 2014. The list of GCF-biomarkers available to date is compiled and presented in a table format. Based on the available literature on GCF biomarkers, it can be concluded that several sensitive and reliable markers are present to detect the presence, severity, and response to treatment. Further studies are warranted to analyze the sensitivity and reliability of these indicators which might help in developing noninvasive tests that could help in the diagnosis of periodontal disease.

A literature search was carried out to find out all the available tests that indicate periodontal disease markers in saliva. All major databases were searched to compile the information on published reports between 1999 and 2014. The list of biomarkers available to date is compiled and presented in a table format. Each biomarker is discussed separately based on the available evidence. Based on the evidence, it can be concluded that several sensitive salivary indicators of periodontitis are available to detect the presence, severity and response to treatment. Further studies are warranted to analyze the sensitivity and reliability of these indicators that might help in developing non-invasive tests that could help in the diagnosis of periodontal disease.
