Abstract An improved interval-valued intuitionistic fuzzy multi-attribute group decision-making method considering the risk preference of decision-makers is proposed to solve the multi-attribute group decision-making problem with interval-valued intuitionistic fuzzy numbers and the condition that the attribute weight information is completely unknown. Firstly, the decision-maker weight of each attribute is determined by combining similarity and proximity. In order to consider the influence of the decision-maker's risk preference on the decision result and avoid the asymptotic behavior of interval-valued intuitionistic fuzzy matrix, the risk aversion coefficient of the decision-maker is introduced and combined with the determined decision-maker's weight aggregation to form a group decision matrix. Then, the information of group decision matrix is mined, and the interval-valued intuitionistic fuzzy entropy is used to determine the attribute weight and relative weight. Based on the interval-valued intuitionistic fuzzy distance measure formula and the TODIM method, the overall superiority of each scheme relative to other schemes is obtained by calculating the superiority between schemes, and the optimal scheme is determined by comparing and sequencing. Finally, the rationality and effectiveness of the proposed method are verified by an example of mechanical assembly supplier selection decision.

Retrohepatic vena cava (RHIVC) injury is often fatal and can be very difficult to manage. Total hepatic vascular isolation, a shunt, or bypass surgery is required for the surgical treatment of RHIVC injury in hemodynamically unstable patients; however, these are not easy procedures. Here, we present a case of RHIVC injury that was successfully treated by atriocaval shunt placement via a transdiaphragmatic incision without sternotomy or thoracotomy. In addition, we review the resuscitation and surgical procedures used for total hepatic vascular isolation in patients with RHIVC injury.

Abstract As a critical node for insulin/IGF signaling, insulin receptor substrate 1 (IRS-1) is essential for metabolic regulation. A long and unstructured C-terminal region of IRS-1 recruits downstream effectors for promoting insulin/IGF signals. However, the underlying molecular basis for this remains elusive. Here, we found that the C-terminus of IRS-1 undergoes liquid-liquid phase separation (LLPS). Both electrostatic and hydrophobic interactions were seen to drive IRS-1 LLPS. Self-association of IRS-1, which was mainly mediated by the 301–600 region, drives IRS-1 LLPS to form insulin/IGF-1 signalosomes. Moreover, tyrosine residues of YXXM motifs, which recruit downstream effectors, also contributed to IRS-1 self-association and LLPS. Impairment of IRS-1 LLPS attenuated its positive effects on insulin/IGF-1 signaling. The metabolic disease-associated G972R mutation impaired the self-association and LLPS of IRS-1. Our findings delineate a mechanism in which LLPS of IRS-1-mediated signalosomes serves as an organizing center for insulin/IGF-1 signaling and implicate the role of aberrant IRS-1 LLPS in metabolic diseases.

Abstract A multi-attribute group decision-making (MAGDM) method based on intuitionistic fuzzy preference information is proposed for the multi-attribute intuitionistic fuzzy group decision-making problem where the decision-makers weight and attribute weight are completely unknown and the decision-maker has preference information for the scheme. Firstly, an intuitionistic fuzzy interval judgment matrix is established to describe the original data of the key decision indicators for multiple network public opinion emergencies that erupt simultaneously. Secondly, the attribute weights are determined based on the improved intuitionistic fuzzy entropy construction method, and the expert weights are determined by using objective decision information, taking into account the intuitionistic fuzzy entropy of decision matrix. MAGDM can not only synthesize experts' professional experience in different aspects, but also avoid experts' decision-making errors caused by insufficient domain knowledge. It is necessary to continuously adjust the expert weight based on decision-making information to make the comprehensive decision-making information more accurate. Thirdly, a scheme preference model and an attribute weight optimization model are established to determine the ranking method of intuitionistic fuzzy interval values. Then, a modified distance measure of intuitionistic fuzzy sets (IFSs) is introduced to make the evaluation result more accurate and reasonable when it comes to solving the deviation between the evaluation value and ideal solution of each scheme. Finally, the effectiveness and practicability of the proposed decision-making method are verified by an example of emergency crisis severity, It assists decision makers in selecting and implementing the optimal emergency response plan in a timely and efficient manner, which improves the emergency treatment efficiency of network public opinion crisis, helps emergency departments to better deal with the network public opinion crisis, improves the ability of public opinion guidance and control, and provides a new method and idea for multi-attribute intuitionistic fuzzy group decision-making problem.

Objective: To determine a risk scoring system for predicting advanced colorectal neoplasia (ACN) within subcentimetric polyps in a large Asian population. Methods: A retrospective study was conducted in Hong Kong SAR, China involving participants who underwent colonoscopy between 2008 and 2015. A random sample of 20 072 subjects were included as the derivation cohort to assess ACN‐associated independent factors using logistic regression modeling. Another 8603 subjects formed a validation cohort. A risk scoring system was developed and its performance was assessed using the area under the receiver operating characteristic curve (AUROC). Results: The risk scores were assigned based on the following criteria: (a) patients who were admitted from inpatient colonoscopy (2.2) or not (1); (b) with three or more chronic diseases (hypertension, diabetes mellitus, hyperlipidemia, heart disease, or cancer) (1.7) or not (1); (c) anemia (1.3) or without anemia (1); (d) receiving aspirin (0.5) or not (1); (e) receiving other nonsteroidal anti‐inflammatory drugs (0.3) or not (1); (f) male (1.2) or female gender (1); (g) age <55 (1), 55–64 (1.4), 65–69 (2), 70 years or above (2.2). ACN was more common in those with scores of 2.192 or higher, and they were classified as high risk (HR). The prevalence of ACN in the validation cohort was 13.28% and 3.56% in the HR and low‐risk groups, respectively. In both the derivation and validation cohorts, AUROC of the risk‐scoring model was 0.7138. Conclusion: Physicians are recommended to utilize this validated score for risk‐stratification of patients having subcentimetric polyps. [ABSTRACT FROM AUTHOR]

Abstract Background Innovative technical approaches to controlling undesired sensory and motor activity, such as hyperalgesia or spasticity, may contribute to rehabilitation techniques for improving neural plasticity in patients with neurologic disorders. To date, transcutaneous electrical stimulation has used low frequency pulsed currents for sensory inhibition and muscle activation. Yet, few studies have attempted to achieve motor nerve inhibition using transcutaneous electrical stimulation. This study aimed to develop a technique for transcutaneous electrical nerve inhibition (TENI) using medium-frequency alternating current (MFAC) to suppress both sensory and motor nerve activity in humans. Methods Surface electrodes were affixed to the skin of eight young adults to stimulate the median nerve. Stimulation intensity was increased up to 50% and 100% of the pain threshold. To identify changes in sensory perception by transcutaneous MFAC (tMFAC) stimulation, we examined tactile and pressure pain thresholds in the index and middle fingers before and after stimulation at 10 kHz. To demonstrate the effect of tMFAC stimulation on motor inhibition, stimulation was applied while participants produced flexion forces with the index and middle fingers at target forces (50% and 90% of MVC, maximum voluntary contraction). Results tMFAC stimulation intensity significantly increased tactile and pressure pain thresholds, indicating decreased sensory perception. During the force production task, tMFAC stimulation with the maximum intensity immediately reduced finger forces by ~ 40%. Finger forces recovered immediately after stimulation cessation. The effect on motor inhibition was greater with the higher target force (90% MVC) than with the lower target (50% MVC). Also, higher tMFAC stimulation intensity provided a greater inhibition effect on both sensory and motor nerve activity. Conclusion We found that tMFAC stimulation immediately inhibits sensory and motor activity. This pre-clinical study demonstrates a novel technique for TENI using MFAC stimulation and showed that it can effectively inhibit both sensory perception and motor activity. The proposed technique can be combined with existing rehabilitation devices (e.g., a robotic exoskeleton) to inhibit undesired sensorimotor activities and to accelerate recovery after neurologic injury.

Immunotherapeutic agents have demonstrated encouraging signs of clinical utility in non-Hodgkin lymphoma. The goal of this study is to analyze the immune characteristics of Chinese patients with diffuse large B-cell lymphoma (DLBCL) to inform the development of immunotherapies in this patient population. Tumor samples from 211 DLBCL patients were analyzed for cell of origin (COO) and immune characteristics using the NanoString platform as well as MYC protein expression through immunohistochemistry. Lower incidence of the germinal center B-cell (GCB) subtype (93/211, 44.1%) was observed in this cohort. Compared to the GCB subtype, the activated B-cell (ABC) subtype was associated with significantly increased expression of multiple pro-inflammatory gene signatures and decreased expression of anti-inflammatory gene signatures. Instead of affecting the pro-inflammatory genes, MYC protein overexpression showed a negative correlation with the expression of T-cell receptor (TCR) and T regulatory genes as well as the OX40 gene. Regardless of COO, higher PD-L1 or IDO1 gene expression correlated with increased expression of T effector and Interferon-γ gene signatures while the expression of multiple oncogenes including ACTR3B, ERBB2, AKT2 and SMARCD1 was down-regulated. Our findings may thus be helpful in guiding further development of immunotherapies for the different subsets of Chinese DLBCL patients. Demagnetisation fault (DF) is a common rotor fault in the permanent magnet synchronous motor (PMSM). DF can cause obvious changes in the magnetic field of PMSMs. As a result, previous DF diagnosis methods mainly depends on magnetic field analysis. However, conventional analysis methods based on the magnetic potential‐permeability method, sub‐domain method and magnetic equivalent circuit method are not suitable for DF condition. In addition, DF diagnosis is further complicated by various operating conditions. To address these issues, a robust DF diagnosis method is proposed for PMSM based on d‐axis magnetic network model using magnetic field reconstruction. The proposed fault diagnosis method employs magnetic field reconstruction to obtain the radial air‐gap flux density under the open‐circuit condition. Subsequently, a d‐axis magnetic network is established to solve the demagnetisation coefficient matrix in the DF state. Finally, the effectiveness of the proposed method is validated through simulations and experiments. Both results demonstrate that the proposed method can accurately recognise uniform DF and partial DF with multiple demagnetised permanent magnets, exhibiting great robustness against different operating conditions. Modular multilevel converters (MMCs) are widely applied to medium and high voltage occasions. The total power consumption of the submodule (SM) and the maximum power consumption of power devices in the SM are related to the operating costs and lifetime of the MMC. Existing literature only considers total power loss optimization or maximal power consumption optimization in MMC's SM. In this article, a reduced loss and extended lifetime power loss optimum control (RLEL-PLOC) is introduced to inject the first-best second harmonic circulation into the MMC's arm current. Compared with the conventional power loss optimization control, the proposed control could decrease the maximal power consumption of the semiconductor devices without increasing the total loss of the SM. According to study results of the MMC, compared with the circulating current suppression control (CCSC) method, the total power consumption of the SM could be reduced by 4.2% and the maximal power dissipation in the SM could be reduced by 5.4% with RLEL-PLOC. PSCAD simulation and MMC prototype experiment are also carried out, and the research results verified the availability of the put forward RLEL-PLOC for MMCs. [ABSTRACT FROM AUTHOR]

Birt-Hogg-Dubé syndrome (BHDS) is a rare disease with autosomal dominant inheritance that manifests through skin tumors, pulmonary cystic lesions, and renal tumors. A mutation of FLCN located on chromosome 17p11.2, which encodes a tumor-suppressor protein (folliculin), is responsible for the development of BHDS. We report the case of a patient presenting with spontaneous pneumothorax, in whom a familial genetic study revealed a novel nonsense mutation: p.(Arg379*) in FLCN.

Background: Hybrid coronary revascularization (HCR) was developed to combine the advantages of coronary artery bypass graft (CABG) with percutaneous coronary intervention (PCI). However, it is still controversial whether it i s more optimal t o perform CABG or PCI first. T he p urpose o f this s tudy was t o compare the clinical outcomes of these 2 approaches. Methods: Eighty patients who underwent HCR from May 2010 to December 2015 were enrolled in this retrospective analysis. The CABG-first group comprised 12 patients and the PCI-first group comprised 68 patients. Outcomes of interest included in-hospital perioperative factors, major adverse cardiac and cerebrovascular events (MACCEs), and the incidence of repeated revascularization, especially for the target vessel lesion. Results: No significant difference was found in the amount of postoperative bleeding (p=0.239). The incidence of MACCEs was similar between the CABG-first and PCI-first groups (1 of 12 [8.3%] vs. 5 of 68 [7.4%], p＞0.999). Repeated revascularization was performed on 3 patients (25%) in the CABG-first and 9 patients (13.2%) in the PCI-first group (p=0.376). Conclusion: There were no significant differences in postoperative and medium-term outcomes between the CABG-first and PCI-first groups. Based on these results, it can be inferred that it is safe to opt for either CABG or PCI as the primary procedure in 2-stage HCR.
