5. A Deep learning Approach to Generate Contrast-Enhanced Computerised Tomography Angiography without the Use of Intravenous Contrast Agents

Abstract

Contrast-enhanced computed tomography angiograms (CTAs) are widely used in cardiovascular imaging to obtain a non-invasive view of arterial structures. However, contrast agents are associated with complications at the injection site as well as renal toxicity leading to contrast-induced nephropathy (CIN) and renal failure. We hypothesised that the raw data acquired from a non-contrast CT contains sufficient information to differentiate blood and other soft tissue components. We utilised deep learning methods to define the subtleties between soft tissue components in order to simulate contrast enhanced CTAs without contrast agents. Twenty-six patients with paired non-contrast and CTA images were randomly selected from an approved clinical study. Non-contrast axial slices within the AAA from 10 patients (n = 100) were sampled for the underlying Hounsfield unit (HU) distribution at the lumen, intra-luminal thrombus and interface locations. Sampling of HUs in these regions revealed significant differences between all regions (p<0.001 for all comparisons), confirming the intrinsic differences in the radiomic signatures between these regions. To generate a large training dataset, paired axial slices from the training set (n=13) were augmented to produce a total of 23,551 2-D images. We trained a 2-D Cycle Generative Adversarial Network (cycleGAN) for this non-contrast to contrast (NC2C) transformation task. The accuracy of the cycleGAN output was assessed by comparison to the contrast image. This pipeline is able to differentiate between visually incoherent soft tissue regions in non-contrast CT images. The CTAs generated from the non-contrast images bear strong resemblance to the ground truth. Here we describe a novel application of Generative Adversarial Network for CT image processing. Abstract

Existing methods to reconstruct vascular structures from a computed tomography (CT) angiogram rely on injection of intravenous contrast to enhance the radio-density within the vessel lumen. However, pathological changes can be present in the blood lumen, vessel wall or a combination of both that prevent accurate reconstruction. In the example of aortic aneurysmal disease, a blood clot or thrombus adherent to the aortic wall within the expanding aneurysmal sac is present in 70-80% of cases. These deformations prevent the automatic extraction of vital clinically relevant information by current methods. In this study, we implemented a modified U-Net architecture with attention-gating to establish a high-throughput and automated segmentation pipeline of pathological blood vessels in CT images acquired with or without the use of a contrast agent. Twenty-six patients with paired non-contrast and contrast-enhanced CT images within the ongoing Oxford Abdominal Aortic Aneurysm (OxAAA) study were randomly selected, manually annotated and used for model training and evaluation (13/13). Data augmentation methods were implemented to diversify the training data set in a ratio of 10:1. The performance of our Attention-based U-Net in extracting both the inner lumen and the outer wall of the aortic aneurysm from CT angiograms (CTA) was compared against a generic 3-D U-Net and displayed superior results. Subsequent implementation of this network architecture within the aortic segmentation pipeline from both contrast-enhanced CTA and non-contrast CT images has allowed for accurate and efficient extraction of the entire aortic volume. This extracted volume can be used to standardize current methods of aneurysmal disease management and sets the foundation for subsequent complex geometric and morphological analysis. Abstract

Vascular stability and tone are maintained by contractile smooth muscle cells (VSMCs). However, injury-induced growth factors stimulate a contractile-synthetic phenotypic modulation which increases susceptibility to abdominal aortic aneurysm (AAA). As a regulator of embryonic VSMC differentiation, we hypothesized that Thymosin [beta]4 (T[beta]4) may function to maintain healthy vasculature throughout postnatal life. This was supported by the identification of an interaction with low density lipoprotein receptor related protein 1 (LRP1), an endocytic regulator of platelet-derived growth factor BB (PDGF-BB) signaling and VSMC proliferation. LRP1 variants have been implicated by genome-wide association studies with risk of AAA and other arterial diseases. T[beta]4-null mice displayed aortic VSMC and elastin defects that phenocopy those of LRP1 mutants, and their compromised vascular integrity predisposed them to Angiotensin II-induced aneurysm formation. Aneurysmal vessels were characterized by enhanced VSMC phenotypic modulation and augmented PDGFR-[beta] signaling. In vitro, enhanced sensitivity to PDGF-BB upon loss of T[beta]4 was associated with dysregulated endocytosis, with increased recycling and reduced lysosomal targeting of LRP1-PDGFR-[beta]. Accordingly, the exacerbated aneurysmal phenotype in T[beta]4-null mice was rescued upon treatment with the PDGFR-[beta] antagonist Imatinib. Abstract

Lactate threshold testing is a commonly used method to gain a greater understanding of an athlete’s fitness. Indeed, endurance athletes such as marathon runners are unable to frequently perform maximal effort attempts for the entire event, as the recovery time needed would interfere with their training. As such, these tests are used as a proxy to help monitor fitness, and sometimes to help predict performance. This review aims to assess whether lactate tests can also be used to accurately predict 2km rowing ergometer performance, as no recent, published systematic review tackles this question. The findings of this review may help enable better monitoring of a rower’s athletic progress and the effects of training adaptations. It may also have wider consequences such as improving the ability of a coach to identify rowing performance potential in untrained athletes.

Abstract

The 6-minute walk test (6MWT) is a test that is frequently used in the assessment of patients with a wide range of cardiorespiratory conditions. It consists of measuring the maximal distance that the assessed individual is able to walk in 6 minutes. This is used as a surrogate indicator of disease presence and progression, as well as to assess the outcome of an intervention. Clear advantages of this exercise test include the fact that it is sub-maximal, which by being below the patient’s maximal threshold is more reflective of a patient’s everyday experience (especially as compared to its more extensive counterpart, the cardiopulmonary exercise test) and the ease with which it is carried out – all it requires is a hallway ideally 30m in length. However, in many ways these advantages also constitute the test’s main drawbacks: it lacks consistency and standardisation between studies, and whilst a useful surrogate indicator in many diseases it reveals little about a disease’s underlying pathophysiology. Pulmonary fibrosis (PF) is a progressive, irreversible interstitial lung disease characterised by extensive scarring and fibrous tissue in the lungs progressively leading to mortality. Whilst identifiable causes include environmental pollution, connective tissue diseases and infections, the most common cause is idiopathic (idiopathic pulmonary fibrosis, IPF). PF patients present in the clinic with breathlessness, chronic dry cough, fatigue, and weight loss. These symptoms severely compromise their quality of everyday life. Currently, a therapeutic cure for PF is not available and treatment options remain limited and are mainly focused on symptom relief and slow down of disease progression. Whilst the change in 6-minute walk distance (6MWD) is frequently used as a primary or secondary outcome in clinical trials, there has not been a recent systematic review to date assessing the test’s association with mortality in patients with PF, despite established associations between 6MWT parameters and prognosis in many diseases – a decline in 6MWD is a strong predictor of disease progression, and baseline 6MWD correlates well with a variety of lung function tests. A systematic approach will be helpful in outlining not only the test’s utility, but also its main weaknesses, in the hope of informing how these may be addressed.
