5. VERTEBROPLASTY COMBINED WITH TRANSPEDICULAR FIXATION FOR THE MANAGEMENT OF NON-TRAUMATIC OSTEOPOROTIC VERTEBRAL FRACTURES ASSOCIATED WITH PEDICLE FRACTURES.

Abstract

Objective: Non-traumatic osteoporotic vertebra fracture (OVF) in association with bilateral pedicle fracture (PD) is a rare condition. In particular, OVFs demonstrating vacuum cleft sign and superior endplate discontinuity extending to the posterior cortex may be associated with non-traumatic PDs. This coexistence results in an unstable fracture pattern, and when solely treated with vertebroplasty (VP), vertebral collapse develops, leading to dead bone formation. We aimed to evaluate the efficacy of VP combined with transpedicular fixation (TPF) for the management of OVFs associated with PD. Materials and Methods: Patients treated with VP combined with percutaneous TPF with a fenestrated screw at the same level of OVF were included. Prophylactic VP was performed at one level above and below. All patients underwent magnetic resonance imaging (MRI) and computerized tomography (CT) during preoperative evaluation. Preop and postop CT scans used for vertebral height measurements and comparison. Results: Thirty-two pts (10M, 22F), mean age 74 (47-92) years of OVF and f/up 30 (24-74) months. Quantative-CT analysis including bone mineral density and t-score mean values was 56.79 mg/cm³ and -4.38±0.538 respectively. VP combined with TPF was performed at 21 pts (Thoracolumbar), 8 pts (Thoracic), and 3 pts (Lumbar spine). Prophylactic VP was performed at 87 levels. The mean vertebral body angle improvement was 20.9% and the mean local kyphosis angle improvement was 17.2%. The mean anterior vertebral height and posterior vertebral height increased by 13.5% and 3.5%, respectively. None of the pts developed further vertebral collapse and none the pedicle screws pulled out at the final f/up. Conclusion: According to our study, VP combined with TPF provided stable fixation and prevented further vertebral collapse in patients with OVF associated with pedicle fractures. OVF with a vacuum cleft sign or superior endplate discontinuity extending to the posterior cortex must be evaluated for the coexistence of spontaneous pedicle fracture, which causes instability and vertebral collapse. We recommend routine preoperative CT scan evaluation to determine fracture pattern and check pedicle integrity in addition to MRI scans. [ABSTRACT FROM AUTHOR]

Abstract

Objective: Thoracolumbar/lumbar (TL/L) curves are a rare type of adolescent idiopathic scoliosis (AIS). Historically anterior selective fusion and posterior selective fusion provided satisfied results in terms of curve correction, maintenance of correction and spontaneous thoracic curve correction. Aim of our study was to present the results of selective posterior Cobb to Cobb TL/L fusion in patients lenke type 5c AIS patients with a single surgeon experience for up to 10 years of follow. Materials and Methods: Patients who underwent selective TL/L posterior fusion for a diagnose of Lenke type 5c AIS were retrospectively analyzed. Patients who were followed up minimum 2 years and underwent full preoperative, early postoperative and follow-up radiologic work up and last follow-up SRS22r scores were included in descriptive statistical analysis performed. Results: Fifty one patients (47 F, 4M) were included in the study. Mean age was 15 (12-17). Mean follow-up period was 84 months (24-120). The mean preoperative major TL/L curve improved to 6.3 (0-20) from 42.8 (38-71) with an 85% correction rate. The mean thoracic curve correction rate was %57. At follow main TL/L and upper thoracic curve did not show correction loss. Coronal imbalance has not been recorded. At last follow-up mean SRS22r was mean 4.3 (3.6-4.9). Conclusion: Selective TL/L posterior Cobb to Cobb fusion improves main TL/L and upper thoracic curves in AIS lenke type 5c patients and maintains long-term stability for the uninstrumented upper thoracic curve. Abstract

Objective: The treatment of femoral nonunion is challenging for both the surgeon and the patient. Strut allografts increase the bone stock, enhance fracture healing and increase stability by acting as a biological plate. In this study, we aimed to report the results of the sandwich technique with two-strut allograft in the treatment of oligotrophic or atrophic femoral nonunions. Methods: Medical records of the patients who were treated due to femoral nonunion in a single centre were retrospectively reviewed. Twenty-one patients (10 males, 11 females) with a mean age of 49 (range: 21 to 79) years were included in the study. The left side was affected in 11 patients, whereas the right side was affected in ten. The patients had 11 femoral shaft fractures, seven proximal femoral fractures and three distal femoral fractures. The mean time from the previous operation to the nonunion surgery was 9.6 (range: 6 to 22) months. Results: Union was achieved in all patients after a mean period of 6.2 (range: 4 to 10) months. The mean follow-up time was 46.8 (range: 12 to 86) months. One patient had superficial surgical site infection in the autologous graft donor site. Conclusion: The sandwich technique with two-strut allograft provides good results in the treatment of femoral nonunion. The technique can be used on any type of nonunion, at any segment of the femur and can be combined with different fixation techniques. [ABSTRACT FROM AUTHOR]

Abstract

Achondroplasia was first described in 1878 and is the most common form of human skeletal dysplasia. Spinal manifestations include thoracolumbar kyphosis, foramen magnum, and spinal stenosis. Progressive kyphosis can result in spinal cord compression and paraplegia due to the reduced size of spinal canal. The deficits are typically progressive, presenting as an insidious onset of paresthesia, followed by the inability to walk and then by urinary incontinence. Paraplegia can be the result of direct pressure on the cord by bone or the injury to the anterior spinal vessels by a protruding bone. Surgical treatment consists of posterior instrumentation, fusion with total wide laminectomy at stenosis levels, and anterior interbody support. Pedicle screws are preferred for spinal instrumentation because wires and hooks may induce spinal cord injury due to the narrow spinal canal. Pedicle lengths are significantly shorter, and 20–25 mm long screws are appropriate for lower thoracic and lumbar pedicles in adult achondroplastic There is no information about the appropriate length of screws for the upper thoracic pedicles. Tracheal injury due to inappropriate pedicle screw length is a rare complication. We report an extremely rare case of tracheal tear due to posterior instrumentation and its management in the early postoperative period.

Abstract

Amaç: Üçüncü kuşak anterior vida;tek çubuk sistemleri ve intervertebral titanyum mesh kullanılarak tedavi edilen adölesan idiopatik skolyozlu olgularda erken dönem tedavi sonuçları değerlendirildi. Çalışma planı: Adölesan idiopatik skolyozlu 17 hasta (15 kadın, 2 erkek; ort. Yaş 14.9:dağılım 11-20) değerlendirildi. Eğrilikler dört olguda King tip I, altı olguda tip II, iki olguda tip III, iki olguda tip IV ve üç olguda lomber olarak sınıflandırıldı. Lomber olgularda tüm seviyelerde, torakal olgularda ise enstrümantasyonun kranial ve kaudal sınırlarında intervertebral titanyum mesh$ler kullanıldı. Tüm olgular ameliyat sonrası 3-6 ay süreyle torakolumbosakral ortez içinde immobilize edildi. Hastaların ameliyat öncesi, ameliyat sonrası ve takiplerinde iki yönlü orthoröntgenografileri değerlendirildi. Ortalama takip süresi 28 ay (dağılım 18-41 ay) idi. Sonuçlar: Koronal eğrilik derecesi ortalamaları ameliyat öncesinde 51, ameliyat sonrasında 8.6 bulundu. Koreksiyon oranı ortalaması %83.9 idi. Ameliyat sonrası sagital açı değerleri ortalaması torakal kifoz için 27.5, lomberlordoz için 37.6 bulundu. Sekonder eğriliklerin hiçbirinde dekompensasyon gelişmedi. Posterior enstrümantasyon ile karşılaştırıldığında ortalama 0.93 seviye füzyondan tasarruf edildi. Torakolomber geçiş bölgesi kifozu saptanmadı. Bir torasik olguda vida sıyırması nedeniyle, erken dönemde posterior enstrümentasyon ile revizyon uygulandı. Takiplerde psödoartroz, implant yetersizliği ve koreksiyon kaybı gibi problemlerle karşılaşılmadı, Çıkarımlar: Sonuçlarımız üçüncü kuşak anterior tek çubuk enstrümentasyon ve intervertebral titanyum mesh'lerin adölesan idiopatik skolyozun cerrahi tedavisinde posterior enstrümentasyona iyi bir alternatif olduğunu gösterdi. Objectives: We evaluated early results of treatment in patients with adolescent idiopathic scoliosis, who underwent anterior fusion with rigid single-rod third generation instrumentation and titanium surgical mesh implants. Methods: Seventeen patients (15 females, 2 males; mean age 14.9 years; range 11 to 20 years) with adolescent idiopathic scoliosis were included. Of the patient group, four had King type I, six had King type II, two had King type III, two had King type IV, and three patients had lumbar curves. Titanium mesh cages were used in all lumbar procedures, and at the cranial and caudal ends of the instrumented area in thoracic cases. All patients were immobilized in a TLSO for three to six months postoperatively. Preoperative, postoperative and follow-up ortho-radiographs were evaluated. The mean follow-up was 28 months (range 18 to 41 months). Results: The mean pre- and postoperative primary coronal Cobb angles were 51∘ and 8.6∘, respectively. The mean correction was 83.9%. Sagittal balance was also restored with mean angles of 27.5∘ and 37.6∘ for thoracic kyphosis and lumbar lordosis, respectively. Spontaneous secondary curve decompensation did not occur. Compared with posterior surgery, the mean number of lumbar levels saved was calculated as 0.93. Postoperative thoracolumbar junctional kyphosis did not develop. Loss of correction, nonunion, and implant failure were not encountered; however, one thoracic case required revision with posterior surgery for proximal screw pull-out. Conclusion: Our early results indicate that anterior correction using third generation single solid rod instrumentation and titanium mesh cages offers a good alternative to posterior instrumentation.
