Apelin, an endogenous peptide, has recently gained attention due to its positive inotropic effects in heart failure physiopathology. We investigated the relationship between serum apelin levels and the severity of calcific aortic stenosis (AS).A total of 68 consecutive patients diagnosed with calcific AS and a control group of 32 subjects were included in the study. The subjects were divided into three group as follows: the control group, the mild-moderate AS group and the severe AS group. Blood samples were obtained from all of the subjects, which were used for biochemical comparisons of apelin 36 and high-sensitive C-reactive protein (hsCRP) levels.Plasma apelin 36 levels were significantly lower in the patients with severe AS [490 (247-1074) pg/ml] compared to both the mild-moderate AS [209 (97-453) pg/ml] and control [660 (378-1200) pg/ml] groups (p0.001). Correlation analysis between the left ventricular mass index and apelin concentrations revealed a significant negative correlation between the two parameters (p0.001, r = -0.478).Our study demonstrated decreased apelin levels and increased hsCRP concentrations in patients with severe calcific AS. Our findings may help to clarify the exact pathophysiologic role of apelin in cardiovascular diseases.

Experimental and animal models suggest that oxidative stress, which is characterized by excessive production of reactive oxygen species and reduction of antioxidant defense capacity, might play an important role in the pathophysiology of heart failure (HF) [1]. In HF patients, high serum oxidative stress (OS) levels correlate with advanced disease and known markers of poor prognosis [2]. Recent experimental studies have clearly demonstrated the therapeutic effects of antioxidants on progression of HF [1, 3]. Consequently, antioxidant therapy has been identified as a promising intervention to attenuate the oxidative damage and prevent the progression of HF. Ozone therapy (O3T) is a kind of treatment involving the administration of a certain amount of ozone/oxygen mixture into body cavities or the circulation system [4]. The result of repeated ozone applications is development of resistance to oxidative stress by stimulation of the antioxidant system. Ozone therapy can be used as an assisting treatment method especially for physiological situations involving the inflammatory system and suppressed the immune system [5, 6]. Some such situations include wound healing, macular degeneration secondary to advanced age, and ischemic and infectious diseases. Many experimental studies about these applications have shown that oxidative stress is reduced and the antioxidant system is increased [7]. In this context, the present study aimed to demonstrate the possible effects of O3T on oxidative stress, cardiac functions and clinical findings in patients with heart failure reduced ejection fraction (HFrEF)., The aim of study was to determine the effects of ozone therapy on the oxidative stress, cardiac functions and clinical findings in patients with heart failure reduced ejection fraction (HFrEF). A total of 40 patients with New York Heart Association 2 and 3 HF with left ventricular ejection fraction (LVEF)\35%, and 40 subjects without HF as control group were included in the study. Patients with HFrEF were given additional ozone therapy of major and minor administrations along with conventional HF treatment for 5 weeks. Before and after ozone therapy, left ventricular end-systolic and enddiastolic volumes (LVESV, LVEDV) and the 6 minute walk distance (6MWD) and blood levels of the superoxide dismutase (SOD), catalase (CAT), glutathione (GSH), glutathione peroxidase (GSHPx), malondialdehyde (MDA), nitric oxide (NO) and N-terminal pro-brain natriuretic peptide (NTproBNP) were measured. Ozonetherapy significantly reduced the serum levels of NO and MDA (p\0.001, respectively) and significantly increased the levels of SOD, CAT, GSH and GSHPx (p\0.001, respectively). LVEDV and LVESV were found to be significantly reduced; however, LVEF was not found to be significantly increased (p = 0.567). As the biochemical improvement marker of HF, NT-proBNP was significantly reduced (p\0.001). The clinical HF improvement marker of 6 minute walk distance was also modestly increased (p\0.001). Ozone therapy might be beneficial in terms of activating antioxidant system and merit further therapeutic potential to conventional HF treatment in patients with HFrEF.

Giriş:Koroner arter ektazisi (KAE), koroner arterlerinanormal genişlemesi ile karakterize edilir. Ortalama trombosit hacmi (OTH),trombosit dağılım genişliği (TDG), plateletkrit (PKT) dahil olmak üzeretrombosit hacmi endeksleri ve trombosit sayısı trombosit aktivasyonunungöstergeleridir. Bu çalışmada KAE’li hastalarda trombosit hacmi endeksleriincelendi.Hastalarve Yöntem: Çalışmaya, izole KAE’si olan51 (38 erkek; ortalama yaş: 52 + 9.9 yıl) hasta ve koroner arterleri normalolan 50 (39 erkek; ortalama yaş: 54 + 11.3 yıl) sağlıklı birey dahil edildi.Başvuruda trombosit hacmi endeksleri otomatik tam kan sayımı parçası olarakölçüldü.Bulgular:Trombosit sayımı, OTH, PKT ve TDG, KAE grubundakontrol grubuna göre daha yüksekti (p< 0.05). Çok değişkenli lojistikregresyon analizinde, TDG (Odds oranı: 0.22, %95 güven aralığı: 0.06-0.73, p=0.013) ve PKT’nin (Odds oranı: 3.41, %95 güven aralığı: 1.66-6.98, p≤ 0.001)KAE’nin bağımsız öngördürücüleri olduğu gösterildi.Sonuç:Bu çalışma, PKT ve TDG’nin KAE’nin bağımsızbelirleyicileri olduğunu göstermektedir., Introduction:Coronary artery ectasia (CAE) is characterised byan abnormal dilatation of the coronary arteries. Platelet volume indices,including the mean platelet volume (MPV), platelet distribution width (PDW),plateletcrit (PCT) and platelet count, are indicators of platelet activation.In this study, we investigated platelet volume indices in patients with CAE.Patientsand Methods: The study group included 51 patients (38 men;mean age: 52 ± 9.9 years) with isolated CAE and 50 individuals with normalcoronary arteries (39 men; mean age: 54 ± 11.3 years). Admission plateletvolume indices were measured as part of the automated complete blood count.Results:Platelet count, MPV, PCT and PDW were higher in CAEthan in the control group (p< 0.05). Multivariate analysis revealed PDW(odds ratio: 0.22, 95% confidence interval: 0.06-0.73, p= 0.013) and PCT (oddsratio: 3.41, 95% confidence interval: 1.66-6.98, p≤ 0.001) as independentpredictors of CAE.Conclusion:This study demonstrates that PCT and PDW areindependent predictors of CAE.

Aims: In this study, we aim to evaluate the left atrial global longitudinal strain (LAGLS), total atrial conduction time (TACT) and left ventricle (LV) functions in recently diagnosed pulmonary sarcoidosis (PS) patients. Method: Fifty recently diagnosed PS patients (group 1) and a control group of 50 healthy individuals (group 2) were evaluated in the study. Two-dimensional echocardiography images were obtained from LV apical 4-chamber (4C), long-axis (L) and 2-chamber (2C) views. Peak longitudinal strain and strain rate (SR) were obtained from 4C, L and 2C views. Mean values of the three views were calculated. LV global longitudinal strain and LV-SR torsion were determined as the net differences in the mean rotation between the apical and basal levels. LAGLS and TACT values were calculated. Results: The study found that LAGLS was significantly lower in group 1 than in group 2 (P < 0.05). TACT was also significantly longer in group 1 than in group 2 (respectively group 1: 111.6 ± 15.1 ms; group 2: 103.4 ± 5.8 ms, P < 0.001). There was a significant moderate negative correlation between LAGLS and TACT (r = −0.36, P < 0.05). Patients with sarcoidosis had significantly lower LV longitudinal strain and SR measurements than the control group. Although LV basal rotation (LVR) values were similar in both groups, LVR-apical and LV-torsion (LVTR) values were significantly higher in the patient group (group 1). Conclusion: The identification of left atrial and left ventricular myocardial deformation using speckle tracking echocardiography in patients with PS allows subclinical LV dysfunction and subclinical electrophysiologic changes to be detected earlier.

Erectile dysfunction (ED) is an inability to get and maintain an erection for a satisfying sexual relationship [1]. It has been showed in a recent study that the incidence of ED adjusted by age is 48.1% [2]. The International Index of Erectile Function (IIEF) is, today, one of the most commonly used forms for men admitting because of the sexual complaints. According to IIEF, the severity of ED is classified from mild to severe [3]. Organic ED and coronary artery disease (CAD) are closely related as a result of endothelial dysfunction leading to the restriction of blood flow [4, 5]. The similar risk factors including obesity, diabetes mellitus (DM), smoking, hypertension (HT) and dyslipidemia are defined for both [6-9]. Diffuse atherosclerotic process affects the arterial blood flow and leads to the major pathophysiologic changes in both cardiovascular and peripheral vascular diseases including ED [10]. It was reported that the prevalence of ED has a high ratio like 75% in the patients diagnosed with CAD [11-16]. There are evidences supporting that ED is the predictor of CAD. An important part of the men with ED show the early signs of CAD. The men who previously had ED may get more severe CAD in proportion to the ones without CAD. The time between the beginning of the symptoms of ED and CAD is 2-3 years; also, the occurrence time of cardiovascular event is approximately 3-5 years. ED has substantially increased mortality due to the relation with the mortality of CAD. As a result, the severity of ED is associated with the severity of CAD [17]. When the previous studies about the severity of ED and the severity of CAD were researched, it was seen that the patients with acute coronary syndrome were generally included in CAD group in the studies. The underlying root cause of acute coronary syndrome is atherosclerotic plaque rupture. However, the underlying pathology in the patients with stable coronary artery disease is endothelial dysfunction and atherosclerotic plaque burden. Also, in the previous studies, the numbers of epicardial vessels with stenosis above 50% and Gensini score were used in order to evaluate the extent of CAD. In our study, we used Syntax score, which is more current and provides better information about the complexity and extent of CAD, instead of Gensini score which is a classical scoring system. In the literature, among the studies evaluating the severity of ED and the extent of CAD, no study using Syntax score was encountered. Therefore, we think that this is the first study. The aim of our study is to investigate the prevelance of ED in the patients with stable coronary artery disease, to evaluate the relation between the severity of ED and the extent of CAD by using Syntax score and to determine the time between the beginning of the symptoms of ED and CAD., Introduction: Erectile dysfunction (ED) and coronary artery disease (CAD) are closely related as a result of endothelial dysfunction leading to the restriction of blood flow. ED is a potential independent risk factor of CAD. We investigated the prevalence and severity of ED, the extent of CAD and the time interval between the symptoms of ED and CAD in the stable coronary artery patients. Materials and methods: 161 patients applied coronary angiography were divided into two groups according to SYNTAX score as group 1 (n=81) SYNTAX score ≤22, and group 2 (n=80) SYNTAX score >22. The prevalence and severity of ED was determined by using The International Index of Erectile Function (IIEF). Results: The prevalence of ED was 43.2% in group 1 and 61.3% in group 2 (P=0.022). The score of IIEF was 23.1 (15-29) in group 1, 19.3 (6-29) in group 2; there was a significant difference (P=0.000). In the multivariate logistic regression analysis carried out in order to determine the independent predictors on Syntax score, it was found that LDL (odds ratio: 1.032, 95% confidence interval: 1.009-1.055, P=0.007) and IIEF score (odds ratio: 0.825, 95% confidence interval: 0.733-0.928, P=0.001) were the independent predictors. The time between the symptoms of ED and CAD 30.1 ± 4.8 months in group 1, and 40.5 ± 4.3 months in group 2 (P=0.000). Conclusion: The severity of ED is an independent factor predicting the extent of CAD. The early detection of ED enables to make a cardiovascular evaluation. Therefore, taking the cardiovascular risk factors under an aggressive treatment may contribute to prevent the cardiovascular cases which may develop in the future.

Bifurkasyon lezyonu; kaybetmek istemediğiniz bir yan dala hemen komşu olan ve/veya o yan dalın ostiumunu da içeren lezyonlara denir.1 Darlık %50’nin üstünde ve yan dal çapı 2 mm’nin üstünde olmalıdır. Perkütan koroner girişim uygulanan lezyonların %8-15 kadarını oluştururlar.2 Bifurkasyon lezyonlarında önemli bir sorun vardır. Hangi damarı ana damar olarak değerlendireceğiz? Bu sorunun cevabını şöyle verebiliriz; Hangi damar daha geniş ve/veya daha uzun ise o damar, ana damarın devamı olarak kabul edilmelidir., Koroner bifurkasyon lezyonları tüm gelişmelere rağmen düşük işlem başarısı, yüksek restenoz ve işlem sırasında ve sonrasında istenmeyen kardiyak olay oranı nedeniyle girişimsel kardiyologlar için önemli sorun olmaya devam etmektedir. Bifurkasyon lezyonlarının perkütan koroner girişim yoluyla tedavisinde en önemli nokta, her lezyon için ayrı ve en uygun stratejinin belirlenmesi gerektiğidir. Bifurcation lesions stil represent technical challenge fort the interventional cardiologist. Although proposed different techniques, percutaneous coronary intervention for bifurcation lesions is still associated with lower procedural success rate, higher major and adverse cardiac events and poor long term out come compared with non-bifurcation lesions. The most important point in the treatment of bifurcation lesions is determination of the most appropriate strategy for each lesion.

Kardiyovasküler hastalıklar günümüzde gelişmiş ülkelerde ölümün en sık nedenidir ve gelişmekte olan ülkelerde de önümüzdeki 10 yıl içerisinde sıklığının giderek artması beklenmektedir.1,2 Erişkin kalp girişimlerinde mortalite ve morbidite hakkında objektif kriterlere dayanarak önceden risk belirleyebilmek hem hasta ve hasta yakınlarını bilgilendirmek hem de operasyon öncesi cerrahi ekibin süreç planlaması açısından önemlidir.3,4 Bu önemden dolayı risk skorlama sistemleri yaygın bir şekilde klinik kullanıma girmiştir. SYNTAX (Syner gy between PCI with TAXUS™ and Cardiac Surgery) skorlama sistemi bu risk skorlama sistemlerinden biridir. Syntax skorlama sistemi çok damar koroner arter hastalığında (ÇDKAH) hasta için kullanımı uygun olan revaskülarizasyon yönteminin belirlenmesinde kullanılabilmektedir.5 Ancak daha yeni bir skorlama sistemi olan EuroSCORE II izole KABG sonrası postoperatif morbidite ve mortaliteyi EuroSCORE’ dan daha iyi tespit etmektedir. Ayrıca, EuroSCORE II geç dönem sağ kalımın da iyi bir belirleyicisidir.6 Mev - cut risk skorlama sistemlerinden esinlenerek yaptığımız çalışmanın amacı ilk defa koroner arter revaskülarizasyonu (PKG veya KABG) uygulanmış olan çok damar koroner arter hastalarının Syntax skoru, EuroSCORE II, klinik ve demografik verilerini tespit etmek ve her iki revaskülarizasyon yönteminde gerçekleşen mortaliteyi belirleyip, çok damar koroner arter hastalarında uygulanan PKG ya da KABG ile ilgili klinik tecrübeyi bilimsel yansıtmaktır, Amaç: Bu çalışmanın amacı, perkütan koroner girişim (PKG) ya da koroner arter baypas greftleme (KABG) gibi revaskülarizasyon yöntemlerinin ilk defa uygulandığı çok damar koroner arter hastaları ya da sol ana koroner arter (SAKA) hastalarının Syntax skoruna göre sınıflandırılması ve her iki revaskülarizasyon yönteminin sonuçlarının karşılaştırılmasıdır. Gereç ve Yöntem: Kliniğimizde koroner anjiyografi uygulanıp çok damar KAH ya da SAKA hastalığı tespit edilmiş ve ilk defa revaskülarizasyon kararı (PKG ya da KABG) verilen toplam 170 hasta (PKG:80 hasta, KABG:90 hasta) çalışmaya dâhil edilmiştir. Hem PKG hem de KABG yapılan hastalar Syntax skoruna göre düşük (32) olmak üzere üç gruba ayrıldı. Ayrıca KABG uygulanan hastaların EuroSCORE II ve lojistik EuroSCORE değerleri hesaplandı. Her iki revaskülarizasyon yönteminin hastane içi ve taburculuk sonrası ilk bir yıllık mortalite oranları değerlendirildi. Bulgular: PKG yapılan hastaların ortalama yaşı 61 iken KABG yapılan hastaların ortalama yaşı 63 idi. Son yıllarda, subklinik aterosklerozdan akut koroner sendroma kadar olan yelpaze içinde rol oynayan birçok enflamasyon belirteci keşfedilmiş ve her geçen gün bunlara yenileri eklenmektedir. Bunlar arasında NLO, MPOa, hs-CRP, IL-6 ve fetuin-A sistemik ve vasküler enflamasyonda rol oynayan biyobelirteçlerdir. Bunların koroner arter hastalığında (KAH) risk gruplarının saptanması, hastalığın tanısına yönelik yeri ve önemi yanı sıra hangi test modellerinde belirleyici oldukları üzerinde araştırmalar devam etmektedir.1, Objective: We investigated the serum levels of inflammation markers [neutrophil to lymphocyte ratio (NLR), myeloperoxidase activity (MPOa), high sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6), and fetuin-A] in patients with acute coronary syndrome (ACS) and their association with SYNTAX score. Material and Method: 126 consecutive subjects underwent coronary angiography were grouped: subjects with normal coronary arteries (control group, n=36) and ACS (n=90). The latter group were further subgrouped into unstable angina pectoris (n=30), non-ST elevation myocardial infarction (NSTEMI, n=30), and ST elevation MI (STEMI, n=30). Serum markers were measured by fluorometric or ELISA kits. The SYNTAX scores were calculated by scoring all coronary lesions. Results: Serum markers were higher in the ACS, except for fetuin-A. But, multiple logistic regression analysis showed that NLR levels and left ventricular ejection fraction were independently associated with ACS [odds ratio: 2.471, 95% confidence interval: 1.166-5.238, p=0.018, and odds ratio: 0.795, 95% confidence interval: 0.711-0.888, p

Background. The present study aims to evaluate the parameters for aortic stiffness by comparing gestational diabetes mellitus (GDM) with a healthy control group via transthoracic echocardiography. Methods. This was a cross-sectional study involving monitoring of 62 pregnant women (33 with GDM and 29 with uncomplicated pregnancy as controls) during the third trimester. The aortic strain, aortic distensibility, and aortic stiffness values were measured via transthoracic echocardiography. Measurements of GDM group were repeated after 6 months. Results. Metabolik sendrom; bozulmuş glukoz toleransı, hipertansiyon, dislipidemi ve abdominal obezite gibi aterosklerotik risk faktörleri ile karakterize bir hastalıktır.1 MS prevalansı genel popülasyonda erişkinlerde ortalama %22 olarak bildirilmektedir.2 Birçok epidemiyolojik çalışmada MS’un kardiyovasküler morbidite ve mortalite artışı ile ilişkili olduğu gösterilmiştir. MS’lu hastalarda normal popülasyona kıyasla koroner arter hastalığında 3 kat artış olduğu saptanmıştır.3,4 Ateroskleroza bağlı hastalıklar oluşmadan subklinik ateroskleroz yaşamın erken dönemlerinde başlar ve hayat boyu devam eder. Aterosklerozun yol açtığı koroner arter hastalıklarından ölümlerin ve geri dönüşsüz sakatlıkların oluşmasının önlenmesi, hastalığın tedavisinde elde edilen başarılardan çok daha önemlidir. Ateroskleroz gelişiminin subklinik evrede tespiti ve değerlendirilmesi, risk faktör modifikasyonu, ilgili patolojiye bağlı olarak ortaya çıkabilecek komplikasyonların engellenmesi ve risk sınıflamasının yapılması açısından önemlidir.5,6 Bu anlamda karotis arterlerin ultrasonografik gorüntülemesi subklinik aterosklerozun değerlendirilmesinde invazif olmayan ve tekrarlanabilir bir yöntemdir. Pek çok çalışmada ultrasonografik değerlendirilen karotis intima medya kalınlığı (KİMK)’ nın kardiyovasküler olayları tahmin etmede kullanılabileceği ve MS ile ilişkili olduğu gösterilmiştir.7,8, Amaç: Çalışmamızın amacı, metabolik sendrom (MS)’lu hastalarda aortik yayılım hızı (AYH) ve karotis intima medya kalınlığı (KİMK) ile değerlendirilen subklinik ateroskleroz arasındaki ilişkiyi araştırmaktı. Giriş ve Amaç: Sol dal bloğu (LBBB) hem sağlıklı bireylerde hem de yapısal kalp hastalığı olanlarda görülebilen elektriksel ileti kusurudur. Diğer taraftan, kalp yetersizliği ve akut koroner olaylarda beyin natriüretik peptid (BNP) düzeyinin yükseldiği ve bu artışın morbidite ve mortaliteyi olumsuz etkilediği birçok çalışmada gösterilmiştir. Literatürde sol ventrikülün sistolik ve diyastolik fonksiyon bozukluğu ile BNP arasındaki ilişki üzerine çok sayıda klinik araştırma olmasına karşın, izole LBBB'li hastalarda BNP düzeyini araştıran klinik çalışmaya rastlamadık. Çalışmamızda hem klinik ve biyokimyasal kriterlere göre kalp yetersizliği bulgusu olmayan hem de ekokardiyografik olarak sol venrikül sistolik fonksiyonları korunmuş izole LBBB'li hastalarda, LBBB'nin plazma BNP düzeyine etkisini araştırdık.Metod: Bu çalışma, Atatürk Üniversitesi Tıp Fakültesi Kardiyoloji kliniğinde Ocak 2008 ile Ağustos 2011 tarihleri arasında EKG'de LBBB saptanmış, koroner anjiyografileri normal ve ekokardiyografide sol ventrikül sistolik fonksiyonları korunmuş olup izole LBBB tanısı almış hastalarda plazma BNP düzeyi ölçülerek yapıldı. İzole LBBB tanısı elektrokardiyografik olarak LBBB belirlenen hastalardan klinik öykü, fizik muayene ve ekokardiyografi ile yapısal kalp hastalığı olmayan, DM ve kronik böbrek yetersizliği delili olmayanlar olarak tanımlandı. Sol ventrikül sistolik fonksiyonlar korunmuş olması LV çaplarının normal sınırlarda ve LVEF'nun %50'nin üzerinde olması olarak tanımlandı. Çesitli nedenlerle kliniğimize başvuran 8432 hastanın taraması yapılarak LBBB'lu toplam 378 hasta tespit edildi. Bunlar arasından alınma ve dışlanma kriterlerine uygun 42 izole LBBB'li hasta bu çalışmaya dahil edildi. Ayrıca yaş ve cinsiyet açısından hasta grubuna benzer, koroner risk faktörü olmayan, klinik öykü ve fizik muayene olarak tamamen sağlıklı, elekrokardiyografik ve ekokardiyografik olarak normal 30 kişi kontrol grubu olarak alındı. Hastalardan ve kontrol grubundan plazma BNP düzeyi ölçümü için uygun tüpe kan numuneleri ayrıldı. Çalışmaya alınan tüm denekler (hasta ve kontrol grubu birlikte) için plazma BNP düzeyinin medyan değeri 14,6 pg/mL olarak hesaplandı. Böylece, tüm denekler (n=72) plazma BNP düzeylerine göre düşük BNP grubu (plazma BNP?14,6 pg/mL) ve yüksek BNP grubu (plazma BNP>14,6 pg/mL) olmak üzere sınıflandırıldı. Böylece, izole LBBB hasta grubu (n=42) ile kontrol grubu (n=30) ve düşük BNP grubu (n=36) ile yüksek BNP grubu (n=36) demografik ve klinik özellikleri, elekrokardiyografik ve ekokardiyografik parametrelere göre karşılaştırıldı. Ayrıca, tüm çalışma grubunda ölçülen plazma BNP düzeyini etkileyen bağımsız prediktörler belirlendi.Bulgular: Klinik ve ölçülen ekokardiyografik parametreler açısından; İnterventriküler septum kalınlığı ve LA çapı izole LBBB grubunda kontrol grubuna göre artmış LVEF ise azalmıştı. Plazma BNP düzeyleri izole LBBB olan grupta 30,16±22,04 pg/mL, kontrol grubunda ise 15,81±14,24 pg/mL idi (p=0,003). Yine yüksek BNP grubunda düşük BNP grubuna göre yaş ortalaması daha düşük, QRS süresi ise anlamlı olarak daha yüksekti. Plazma BNP düzeyi yaş ve QRS süresi ile anlamlı olarak pozitif korelasyon gösterdi. Lojistik regresyon analizinde; BNP düzeyini LBBB ve yaş bağımsız olarak etkilemekteydi (sırasıyla OR: 3.9, p= 0,038; OR 1.1 p= 0.001).Sonuç: İzole LBBB'li ve sol ventrikül sistolik fonksiyonları korunmuş hastalarda plazma BNP düzeyi dal bloğu olmayan sağlıklı kişilere göre artmaktadır. Bu artış normal popülasyondaki plazma BNP düzeyi aralığında (0-100 pg/ml) olmaktadır. Bu aralıkta ayrıca QRS süresi ile plazma BNP düzeyi arasında güçlü pozitif korelasyon mevcuttur. Ekokardiyografik olarak sol ventrikül sistolik fonksiyonları korunmuş olsa da LVEF normal sınırlarda kalmasına rağmen sağlıklı kişilere göre düşme eğilimindedir. İVS kalınlığı ve LA çapı izole LBBB'de hafif artış gösterirken plazma BNP düzeyi ile İVS kalınlığı çok zayıf pozitif korelasyon göstermektedir. Ölçülen klinik ve ekokardiyografik parametreler arasında plazma BNP düzeyini bağımsız olarak etkileyen faktörlerin yaş ve izole LBBB olduğu saptandı.Sonuç olarak, izole LBBB'li ve sol ventrikül sistolik fonksiyonları korunmuş hastalarda, LBBB plazma BNP düzeyini bağımsız olarak artırmaktadır. Böylece izole LBBB'li asemptomatik kişilerde plazma BNP düzeyin belirlenmesi subklinik sol ventrikül disfonksiyonlarının önceden saptanmasına katkı sağlayabilir. Ancak, normal plazma BNP düzeyi aralığında olan asemptomatik izole LBBB'li kişilerde sol ventrikül disfonksiyonunun belirlenmesinde BNP'nin cutoff değeri saptanmasına yönelik ek çalışmalara gerek vardır.Anahtar Kelimeler: İzole sol dal bloğu, brain natriüretik peptid Introduction and Purpose: Left bundle branch block (LBBB) is an electrical conduction defect that can be seen in both healthy individuals and the ones with structural heart disease. On the other hand, many studies demonstrate that brain natriuretic peptid (BNP) level increases and this has negative effects on morbidity and mortality in case of heart failure and acute coronary syndromes. In the Literature, although there are many clinical researches on the relationship of left ventricular (LV) systolic and diastolic dysfunction with plasma BNP levels, no clinical research on BNP level in patients with isolated LBBB is found. We studied the effect of LBBB on plasma BNP level in patients both with having no signs of heart failure according to the clinical and biochemical criteria and with isolated LBBB and echocardiographically preserved LV systolic function.Method: This study was carried out by measuring plasma BNP level in the patients who were detected with LBBB on EKG, had normal coronary angiography and were diagnosed with isolated LBBB and preserved LV systolic function on echocardiography at Ataturk University Medical Faculty Cardiology Clinic between January 2008 and August 2011. The diagnosis of isolated LBBB, among the patients who were diagnosed with LBBB on electrocardiography, are defined as the ones who didn?t have structural heart disease, DM and chronic renal failure by clinical history, physical examination and echocardiography. Preserved LV systolic function define diameter of normal limits for LV and left ventricular ejection fraction (LVEF) over 50%. 8432 patients who applied to our clinic with various reasons were screened, and total 378 patients with LBBB were detected. Among from these 42 patients with isolated LBBB who met inclusion and exclusion criteria were included in the study. 30 individuals similar to the patient group from age and gender, having no coronary risk factor, healthy from clinical history and physical examination, normal on electrocardiography and echocardiography were defined as control group. Blood samples from the patient and control group were divided into appropriate tubes for plasma BNP level measurement. The median value for plasma BNP level was calculated as 14,6 pg/mL for all subjects in the study (patient and control group). So, all subjects (n=72) were classified in regard to the plasma BNP level as low BNP group (plasma BNP?14,6 pg/mL) and high BNP group (plasma BNP>14,6 pg/mL). Thus, isolated LBBB patient group (n=42) with the control group (n=30) and low BNP group (n=36) with high BNP group (n=36) were compared in regard to demographic and clinical characteristics, electrocardiographic and echocardiographic parameters. The independent predictors effecting plasma BNP level measured in all study group were determined.Results: From the clinical and measured echocardiographic parameters interventricular septum thickness and diameter of left atrium (LA) increased in isolated LBBB group in according to the control group, and LVEF decreased. Plasma BNP levels were 30,16±22,04 pg/mL in isolated LBBB group and 15,81±14,24 pg/mL in the control group(p=0,003). The mean age of the patients were lower in high BNP group in according to low BNP group. However, QRS duration was significantly higher. Plasma BNP level showed a significantly positive correlation with age and QRS duration. According to the logistic regression analysis, LBBB and age independently effect BNP level (OR 3.9, p= 0,038; OR 1.1 p= 0.001, respectively).Conclusion: Plasma BNP level increases in patients with isolated LBBB and preserved left ventricular systolic function in proportion to the healthy individuals without bundle branch block. This increase is in the range of plasma BNP level of normal population (0-100 pg/ml). In this range, there is also a strongly positive correlation between QRS duration and plasma BNP level. LVEF tend to fall in according to the healthy individuals despite of its normal limits and echocardiographically preserved LV systolic function. While IVS thickness and diameter of LA show a slight increase in isolated LBBB, plasma BNP level and IVS thickness show a weak positive correlation. It was detected that among measured clinical and echocardiographic parameters the factors independently effecting plasma BNP level were age and isolated LBBB.As a result, plasma BNP level independently increases in patients with isolated LBBB and preserved LV systolic function. Therefore, determining plasma BNP level in asymptomatic individuals with isolated LBBB can contribute to the pre-detection of subclinical LV dysfunctions. However, there is need in additional studies for the detection of cut off value of BNP in diagnosing LV dysfunction in the asymptomatic patients with isolated LBBB and in the range of normal plasma BNP level.Key Words: Isolated left bundle branch block, brain natriuretic peptid 50
