The aim: to analyze the current state of the issue of hemorrhoid disease in children of different age on the basisof first-hand experience.Materials and methods: author conducted examinations in 71 children with hemorrhoids: general clinical,laboratory, rectal finger, anoscopy, rectoscopy and additional research complexes. There were 56 (78,8%) boys and15 (21,2%) girls.Results and discussion: The features of the cause of the disease, clinical manifestation, course, localization havebeen studied and tactical treatment approaches in the age aspect have been developed. In infants, a common cause ofhemorrhoids were: a two-stage act of defecation with increased diarrheal syndrome; in children of early and preschoolage, periodic and prolonged potty sitting and congenital inferiority of the venous network of the small pelvis in schoolage children played an important role. The frequency of occurrence is more marked in preschool age – 71.7%. Theexternal form of hemorrhoids was revealed – in 90% and combined – in 10% of sick children. Frequent localizationof hemorrhoids occurred in children at 4 o'clock (35%) and 7 o'clock (30%).Comprehensive treatment of hemorrhoids in children was carried out: conservative - in 34 (83%) and surgical - in7 (17%) patients. In infancy and early age, the complex of conservative treatment included: treatment of the mainprimary disease (ChD, diarrheal syndrome, pathology of the colon, etc.) and local treatment; in preschool children- the use of medical enemas and rectal suppositories, ointments and the use of SCL, LL and school and older age- treatment of ChD. Indications for the operation were: the presence of large varicose nodes that violate the act ofdefecation, inflammation, prolapse, infringement and constantly falling out nodes and causing pain. In children, moregentle surgical methods are used. Surgical treatment is more susceptible to school age. There were no complicationsin the postoperative period.Conclusions. 1. Persistent constipation is a common cause of hemorrhoids in children; increased diarrhea, atwo-stage act of defecation, tension in sports, tense cough and urination are also causes of hemorrhoids.2. The clinical features of the course of hemorrhoids in children 1-3 years old is the transient nature of the coursein 70% of cases.3. Hemorrhoids are often found in children of preschool age, with the absence of pain and bleeding.4. The most frequent (65%) localization is 4 and 7 on the clock face.5. In children under 3 years of age with hemorrhoids, only conservative treatment is an effective method. Surgicaltreatment is more susceptible to school age.6. Indications for hemorrhoid surgery in children are: the presence of large varicose nodes that violate the act ofdefecation, inflammation, prolapse, infringement, as well as easily or constantly falling out nodes and causing pain.

This article discusses on the genesis of the plot of the "Tarkibadakhshan" epic, its historical foundations, issues related to the origin of motifs and images, its performance in the epic process, and theoretical views on its artistic evolution. The genesis of the plot of "Tarkibadakhshan" is distinguished by the fact that it was formed on the basis of the people's worldview, and the issues of connection of images and motives with the destiny of the people are described in a poetic form in the work. In the formation of the genesis and poetic development of the plot of Tarkibadakhshan, mythological imaginations, especially elements of shamanism, play a large role, and secondly, there is the issue of the relationship of some motifs and images used in the epic to the Islamic faith, which are the worldview of the epic poet and the Bakhshi-poets of the epic school and evaluated by circumstances in the harmony of the epic environment of the period. Such features urge to carry out special research and investigations.

Background An endocardial radiofrequency ablation is a common approach for the treatment of idiopathic ventricular arrhythmia. However, rare cases have been reported in which ventricular arrhythmia could not be ablated from endocardium due to an epicardial origin of the arrhythmia. Case summary In this article, we describe the rarely used, but acceptable approach to terminate ventricular arrhythmias in the summit of the left ventricle. We present a case of a 56-year-old patient with sustained monomorphic premature ventricular complexes, originating from the summit of the left ventricle, that were successfully eliminated. After unsuccessful ablation of the anterior wall right ventricular outflow tract, left coronary cusp, and distal coronary sinus, arrhythmia was eliminated by method of transvenous ethanol ablation. Complaints, such as palpitations and weakness, resolved after the procedure. Discussion This approach is used when an epicardial location of the substrate of arrhythmia is suspected and ablation through the right ventricular outflow tract, left coronary cusp, and great cardiac vein fails. The total effectiveness of eliminating ventricular arrhythmia increases if it is possible to use endo- and epicardial methods of mapping and ablation. In clinics with extensive experience in this area, ethanol ablation of epicardial ventricular arrhythmia is safe and effective.

The causes of postoperative complications in the correction of anorectal malformation were studied according to follow-up, general clinical and additional research method in 75 operated sick children. In the process of diagnosis and surgical correction, a number of diagnostics, tactical, and combined errors and shortcomings were identified that caused postoperative complications, which required a large number of reconstructive operations. The authors recommend various options for tactical and operational - technical treatment approaches when solving the problems of reconstructive operations. At the same time, it is proposed to study the causes, analyze errors, complications and failures, indications, terms, accesses, the volume of the operation and evaluate their effectiveness. The authors, thanks to the optimization of tactics, method of surgical and rehabilitation treatment, were able to improve the results of postoperative complications to 81.4%. In the course of the study of catamnestic and examination data 75 of children with postoperative non-NAS in the process of diagnosis and surgical correction errors and deficiencies were revealed as: unreliable primary diagnosis, insufficiency of preoperative preparation, inadequate indication and choice of the method of operation, inadequate postoperative rehabilitation. All these had diagnostic, tactical, technical and combined character, which led to the formation of the postoperative HCA, which required a large number of operations. Therefore, the authors suggest that to solve the problems of operations is not limited to the improvement of surgical techniques, and to study the causes, analysis of errors, complications and failures that lead to repeated interventions, indications, timing, access, scope of surgery and their evaluation of effectiveness, medical and social rehabilitation of patients. The authors, thanks to the optimisation of tactics, method of surgical and rehabilitation treatment, managed to correct the post-surgical NAC up to 81,4%.

Introduction. Atrial fibrillation is an irregular heartbeat that accelerates to form blood clots in the chambers of the heart and leads to stroke, heart failure, and other cardiovascular complications. Diabetes mellitus itself has been identified as a risk factor for atrial fibrillation, but the association between them is unclear. Material and methods. We analyzed 70 patients with type 2 insulin non-dependent diabetes mellitus. All patients were examined in parallel continuous glucose (CGM) and ECG for 14 days. The study population divided into documented atrial fibrillation (AF group, n = 16) and without atrial fibrillation (non-AF group, n = 54) groups. We assessed the relationship between hypoglycemia, fasting plasma insulin, insulin resistance using the homeostatic model assessment (HOMA-IR) equation, and the incidence of atrial fibrillation. Results. We found a total of 46 episodes of documented atrial fibrillation (AF be defined as an arrhythmia lasting ≥ 30 seconds) lasted on the whole 596.9 minute, which was the most significant by the number (2.87 ± 2.05 per patient, p 3 PACs (p = 0.0007). The work is devoted to Mossbauer studies of new building materials derived from wastes from coal-fired power plants. Measurements of Mossbauer spectrometers were carried out on the MC1104EM unit in the regime of constant acceleration with a source of 57Co (Cr). The elemental composition of each sample was determined by means of X-ray fluorescence analysis (XRF) on the RLP-21 installation. According to the results of studies of volume-surface concentric-zonal color effects in zoloceramic materials, the phase composition of iron compounds and their ratios is established by the Mossbauer method, and their elemental composition with 32 components is determined with high accuracy by means of XRF. The technology of obtaining gold-ceramic materials with volumetric-surface color effects is described.

This region-wide retrospective cohort study reports the treatment outcomes of patients registered in the TB electronic register and treated with first-line drugs in the TB Programme of the Republic of Karakalpakstan from 2005-2020 and factors associated with unfavourable outcomes. Among 35,122 registered patients, 24,394 (69%) patients were adults, 2339 (7%) were children, 18,032 (51%) were male and 19,774 (68%) lived in rural areas. Of these patients, 29,130 (83%) had pulmonary TB and 7497 (>22%) had been previously treated. There were 7440 (21%) patients who had unfavourable treatment outcomes. Factors associated with unfavourable treatment outcomes included: increasing age, living in certain parts of the republic, disability, pensioner status, unemployment, being HIV-positive, having pulmonary TB, and receiving category II treatment. Factors associated with death included: being adult and elderly, living in certain parts of the republic, having a disability, pensioner status, being HIV-positive, and receiving category II treatment. Factors associated with failure included: being adolescent, female, having pulmonary TB. Factors associated with loss to follow-up included: being male, disability, pensioner status, unemployment, receiving category II treatment. In summary, there are sub-groups of patients who need special attention in order to decrease unfavourable treatment outcomes.

In this article, the tasks of energy efficient operation of a mobile station in wireless communication system were considered. At the same time, the main attention was paid to the consideration of ensuring energy efficient data delivery to the mobile station while meeting the transmission delay requirements. In particular, the operation of a standard power saving mode control algorithm in an IEEE 802.16m network was investigated. Issues related to the functioning of this algorithm when receiving data streams with variable intensity were studied. To conduct such a study, a model of a data transmission system was developed and a universal characteristic was introduced for comparing various control algorithms for the energy saving mode-the energy efficiency coefficient. In addition, an optimization problem was formulated for choosing the parameters of the energy saving mode. After analyzing the existing control algorithm for the energy saving mode, an effective control algorithm was proposed, the use of which makes it possible to achieve a gain in the energy efficiency coefficient of up to 20% when receiving a data stream with variable intensity.

Normal, R 0 and anomalous, R S components of the Hall coefficient are determined from the results of experimental investigations of temperature dependences of the Hall coefficient, magnetic susceptibility, and specific electrical resistance for intermetallic Er2In, ErIn, and Er3In5 compounds. Effective parameters of spinorbital interaction λSO of intermetallic compounds are calculated from anomalous components RS of the Hall coefficient and specific electrical resistance. The results calculated for the band parameters and effective parameters of spin-orbital interaction λSO for Er–In system intermetallides coincide by orders of magnitude with the results obtained in [4,7,8] from the optical spectra of pure rare-earth metals.
