Rifles are often involved in violent deaths such as homicide and suicide. Consequently, expert knowledge and experimental forensic investigations are important to clarify the nature of ballistic trauma when applied to the human head and neurocranium. This study investigated differences in entrance wound morphology with Synbone® spheres which are described as being comparable to human flat bones. A series of ballistic experiments were conducted using two different rifle calibers (5.56 × 45 mm and 7.62 × 39 mm Full Metal Jacket (FMJ)). Synbone® spheres were used for close-range 0.3 m simulated executions as well as at 25 m and 35 m to simulate urban and military engagements. Results were compared with previously published experimental studies using similar military ammunition. In our study, entry wound morphology closely resembles real forensic cases compared to exit wound and overall shape morphology independently of the distance and the caliber. Circumferential delamination was clearly visible with full metal jacket (FMJ) rounds, yielding similar damage pattern morphology to the human crania. This study documented the presence of hydraulic burst or shock in all ten rounds from all three distances. Krönlein shots were also observed in some cases. Synbone® spheres constitute an acceptable synthetic surrogate for ballistic experiments. The present study offers new initial data on the behavior of Synbone® proxies in ballistic testing of military ammunitions; FMJ gunshot injuries to the human head, for distances that have not previously been published, suggesting that efficient tests can take place under these conditions. Further research on experimental ballistics with a larger number of controlled factors and multiple repetitions is recommended to verify the results of this pilot study before applied in forensic simulations.

Realistic human head models are of great interest in traumatic brain injury research and in the forensic pathology courtroom and teaching. Due to a lack of biomechanical data, the human dura mater is underrepresented in head models. This study provides tensile data of 73 fresh human cranial dura mater samples retrieved from an area between the anterior middle and the posterior middle meningeal artery. Tissues were adapted to their native water content using the osmotic stress technique. Tensile tests were conducted under quasi-static uniaxial testing conditions with simultaneous digital image correlation. Human temporal dura mater is mechanically highly variable with regards to its elastic modulus of 70 ± 44 MPa, tensile strength of 7 ± 4 MPa, and maximum strain of 11 ± 3 percent. Mechanical properties of the dura mater did not vary significantly between side nor sex and decreased with the age of the cadaver. Both elastic modulus and tensile strength appear to have constant mechanical parameters within the first 139 hours post mortem. The mechanical properties provided by this study can help to improve computational and physical human head models. These properties under quasi-static conditions do not require adjustments for side nor sex, whereas adjustments of tensile properties accompanied with normal aging may be of interest.

There is much debate within the forensic community around the indications that suggest a head injury sustained by a child resulted from abusive head trauma, rather than from accidental causes, especially when a fall from low height is the explanation given by a caregiver. To better understand this problem, finite element models of the paediatric head have been and continue to be developed. These models require material models that fit the behaviour of paediatric head tissues under dynamic loading conditions. Currently, the highest loading rate for which skull data exists is 2.81 ms−1. This study improves on this by providing preliminary experimental data for a loading rate of 5.65 ± 0.14 ms−1, equivalent to a fall of 1.6 m. Eleven specimens of paediatric cranial bone (frontal, occipital, parietal and temporal) from seven donors (age range 3 weeks to 18 years) were tested in three-point bending with an impactor of radius 2 mm. It was found that prompt brittle fracture with virtually no bending occurs in all specimens but those aged 3 weeks old, where bending preceded brittle fracture. The maximum impact force increased with age (or thickness) and was higher in occipital bone. Energy absorbed to failure followed a similar trend, with values 0.11 and 0.35 mJ/mm3 for age 3 weeks, agreeing with previously published static tests, increasing with age up to 9 mJ/mm3 for 18-year-old occipital bone. The preliminary data provided here can help analysts improve paediatric head finite element models that can be used to provide better predictions of the nature of head injuries from both a biomechanical and forensic point of view.

Objectives: Conventional autopsies remain the gold standard of postmortem healthcare quality assurance and help gathering extended knowledge on diseases. In answer to constantly declining autopsy rates non- or minimally invasive autopsy methods were introduced. Ultrasound is a well-established tool for imaging commonly used in clinical practice. This narrative review aims to summarize the current literature regarding the feasibility and validity of ultrasound in a forensic context. Material and methods: A PubMed database search was carried out. Abstracts were scanned for pre-defined ex- and inclusion criteria, followed by a snowball search procedure applied to the primarily included articles. Results: Forty-five publications met our inclusion criteria. The selected articles concern the feasibility of ultrasound in pre- or postmortem settings, forensic age estimation, and minimally invasive approaches. For imaging, ultrasound was deemed a reliable tool for the examinatio n of epiphyses und superficial wounds, with limitations regarding internal organs and image quality due to postmortem changes. Ultrasound-guided minimally invasive approaches yielded higher success rates for adequate tissue sampling. Many investigations were carried out in low- and middle-income countries focusing on infectious diseases. Conclusion: Ultrasound seems a promising but underutilized imaging tool in legal medicine to date. Promising approaches on its feasibility have been conducted. Especially for minimally invasive methods, ultrasound offered significant improvements on qualified biopsy sampling and thus appropriate diagnostics. Moreover, ultrasonic evaluation of epiphyses for age estimation offered valuable results. Nevertheless, further assessment of ultrasonic feasibility in forensic contexts is needed.

The user experience (UX) of a product is important for a customer's decision, especially in the automotive sector. In recent years, a frontloading of UX assessments from physical prototypes to virtual prototypes has occurred. Nevertheless, there are clear indications that established UX assessments methods cannot simply be transferred 1-to-1 from real to virtual evaluation setups as results get biased. For the UX assessment of an in-car human machine interface (HMI) the haptic properties of the knobs and buttons are just as important as their visual features. Therefore, it is mandatory to also simulate the haptic behavior of the in-car HMI in virtual reality (VR) assessment scenarios. However, accurately simulating this haptic behavior adds considerable costs and effort, diminishing the beneficial effects of VR UX assessments. Due to confidentiality reasons the volunteering participants of VR UX studies in the automotive sectors are typically employees with a varying degrees of VR experience. It is unknown if this gap in VR experience biases the results of UX assessments in VR. This paper presents a UX assessment study of an in-car HMI in VR in comparison to reality. From a practitioners viewpoint two research question are investigated: (1) How much is the UX assessment impaired by missing haptic simulation? (2) Does VR experience influence UX assessment ratings in VR? The results show that missing haptic feedback severely diminishes UX ratings, leaving no doubt that meaningful UX assessments in VR can only be performed when haptic is included. This means that more effort has to be put into earlier VR assessments severely reducing its benefits compared to UX assessments with physical prototypes. However, the VR experience of the participants only has a slight effect on the UX assessment results.

Orthopedic surgeons endure high physical stresses when performing surgery, as large forces and torques are applied commonly. Occupational risks are consequently higher when compared to other surgical disciplines. One example is the reaming of the acetabula during total hip arthroplasty, using customized instruments. This surgery may predispose the surgeon to overuse-related wrist pathology. In this study, torques acting along the reaming tool were measured, and the resulting forces applied to the orthopedic surgeons' wrists were estimated based on the measured torque data from hip reaming. Different reamer sizes and tool velocities were analyzed to determine how both parameters may influence the torques applied at the surgeon's wrist. Using a highly standardized setup, torques were measured while the reamer was pushed into the acetabula to remove cartilage. Maximum torques and stoppage torques at blocking of the reamer were compared between feed rates and reamer sizes. Peak values of the maximum torques along the reamer axis averaged 1.5-1.8 Nm. No significant difference between maximum torques and reamer sizes was found. A significant difference in maximum torques was noted between feed rates with a large effect (p = 0.010; i2 = 0.214) and a large interaction effect (p = 0.017; i2 = 0.186). Based on this experimental setup, it can be hypothesized that the impulsive behavior of the torque when the milling tool reaches the subchondral lamella could potentially contribute to wrist pathology. These preliminary data warrant further study. Consequently, torque limiters should be implemented in reamers to minimize the risk of occupation-related pathology to the wrist.

Aim To investigate load‐deformation properties of Thiel‐embalmed human oral mucosa tissues and to compare three different anatomical regions in terms of mechanical, histological and ultrastructural characteristic with focus on the extracellular matrix. Materials and Methods Thirty specimens from three different regions of the oral cavity: attached gingiva, buccal mucosa and the hard palate were harvested from two Thiel‐embalmed cadavers. Mechanical properties were obtained, combining strain evaluation and digital image correlation in a standardised approach. Elastic modulus, tensile strength, strain at maximum load and strain to failure were computed and analysed statistically. Subsamples were also analysed using scanning electron microscopy (SEM) and histological analysis. Results The highest elastic modulus of 37.36 ± 17.4 MPa was found in the attached gingiva group, followed by the hard palate and buccal mucosa. The elastic moduli of attached gingiva differed significantly to the buccal mucosa (p = .01) and hard palate (p = .021). However, there was no difference in the elastic moduli between the buccal mucosa and hard palate (p > .22). The tensile strength of the tissue samples ranged from 1.54 ± 0.5MPa to 3.81 ± 0.9 MPa, with a significant difference between gingiva group and buccal mucosa or hard palate (p = .001). No difference was found in the mean tensile strength between the buccal mucosa and hard palate (p = .92). Ultrastructural imaging yielded a morphological basis for the various mechanical properties found intraorally; the attached gingiva showed unidirectional collagen fibre network whereas the buccal mucosa and hard palate showed multi‐directional network, which was more prone to tension failure and less elasticity. Conclusion This is the first study assessing the various morphological‐mechanical relationships of intraoral soft tissues, utilising Thiel‐embalmed tissues. The findings of this study suggest that the tissues from different intraoral regions showed various morphological‐mechanical behaviour which was also confirmed under the SEM and in the histological analysis.

Bone mineral density distribution patterns at the sacroiliac joint (SIJ) may reflect long-term adaptation patterns to the loading the joint endures. This study aims to display bone mineralisation patterns of the articular SIJ subchondral lamella using computed tomography (CT) osteoabsorptiometry and mechanical indenting, to determine whether a relationship exists between mineralisation and mechanical strength. Twenty hemipelves were CT-scanned before osteoabsorptiometry densitograms were derived. Each articular side of eleven SIJs was mechanically indented following a 10-mm grid scheme. The sacral surface displayed lower Hounsfield unit (HU) values (< 700 HU) than the iliac side (> 700 HU). The apex, superior corner and borders yielded the highest HU scores (> 700 HU). Penetration strength was significantly higher on the iliac side (p < 0.04). Mineral density correlated positively with penetration strength of the subchondral bone layer (p < 0.05). No correlations were found between the HU values, nor between penetration strength of corresponding sides of the same SIJ in the majority of cases (p > 0.05). The iliac subchondral lamella is mechanically denser than the sacral aspect. The non-correlation between density and bone strength of articulating sides indicates biomechanical non-conformity. Loading throughout the SIJ may follow a complex distribution pattern involving the surrounding soft tissues, suspending the sacrum between the ilia.

Left sided non-recurrent laryngeal nerves (NRLN) are very rarely observed during surgery in the head and neck region. Arising directly from the cervical aspect of the vagus nerve, the NRLN lies in a vulnerable position distant from its normal location. NRLNs are normally associated with embryological branchial arch aberrations and subsequent vascular anomalies. The anomalous course of the NRLN makes it more susceptible to injury during surgery in the neck region. Knowledge of this anatomical variant will reduce the potential for injury and resultant vocal cord paralysis. During microsurgical dissection of the carotid sheath for the implantation of a vagus nerve stimulator in a 19-year-old female patient with refractory epilepsy, a moderate-sized branch of the main vagus nerve trunk was identified postero-medially within the carotid sheath. Intra-operative stimulation of this nerve resulted in a compound muscle evoked potential from the left vocal cord. Thus, this branch was confirmed to be a left-sided NRLN. The patient had no associated vascular anomalies. This is first reported case of a left-sided NRLN found during VNS insertion. Awareness of the possibility of an NRLN is imperative to prevent iatrogenic injury. A medial location of the vagus nerve within the carotid sheath should alert the surgeon to the possible presence of an NRLN. The absence of fourth branchial arch remnant anomalies is not a guarantee as to the absence of a left-sided NRLN. The addition of intra-operative nerve monitoring for vagus nerve stimulator (VNS) implantation procedures should be strongly considered to help avoid iatrogenic injury.

For 50 years now, sacrospinous ligament fixation (SSLF) has been used to treat pelvic organ prolapse consequent on altered integrity of the pelvic myofascial structures. It is usually performed vaginally, but it has recently been performed laparoscopically through either an anterior or a posterior approach, with the broad ligament as a landmark to differentiate the two. In the present study, these two laparoscopic approaches were assessed using Thiel-embalmed cadavers. The anterior and posterior approaches were compared in terms of the closest distance to anatomical structures at risk, including pelvic viscera, the obturator nerve, and vascular structures. The posterior approach was more often closer to the investigated vessels and the rectum. The obturator nerve and the ureter were close to both the anterior and posterior approaches. The urinary bladder was closer using the anterior approach. From an anatomical standpoint, therefore, the anterior laparoscopic approach for SSLF is more likely to cause injury to the urinary bladder, whereas the posterior approach is more prone to causing rectal and vessel injuries. This study illustrates, from a basic science perspective, the importance of combining fascia research, novel endoscopic or minimally invasive surgical exposures informed by anatomy, and contemporary trends in gynecology in order to improve patient outcomes.
