Abstract

X-chromosomal genetic variants are understudied but can yield valuable insights into sexually dimorphic human traits and diseases. We performed a sex-stratified cross-ancestry X-chromosome-wide association meta-analysis of seven kidney-related traits (n = 908,697), identifying 23 loci genome-wide significantly associated with two of the traits: 7 for uric acid and 16 for estimated glomerular filtration rate (eGFR), including four novel eGFR loci containing the functionally plausible prioritized genes ACSL4, CLDN2, TSPAN6 and the female-specific DRP2. Further, we identified five novel sex-interactions, comprising male-specific effects at FAM9B and AR/EDA2R, and three sex-differential findings with larger genetic effect sizes in males at DCAF12L1 and MST4 and larger effect sizes in females at HPRT1. All prioritized genes in loci showing significant sex-interactions were located next to androgen response elements (ARE). Five ARE genes showed sex-differential expressions. This study contributes new insights into sex-dimorphisms of kidney traits along with new prioritized gene targets for further molecular research.

Abstract

Land fires in South Sumatra are an annual problem during the long dry season. It was recorded that in 2015, 2016, 2017, and 2018, the land fires spread massively in the four districts of South Sumatra. The peatlands located within oil palm plantations in the Districts of Ogan Komering Ilir, Banyuasin, Musi Banyuasin, and the district of Ogan Ilir were the source of the fire. The haze not only attracts national but also international attention. Besides human contribution to land fire, climate change should also be considered. The role of El Nino makes the season uncertain. Land fires affect human health and other human activities in the affected areas. Three legal instruments guarantee and protect the people's right to the environment, i.e., The 1945 Indonesian Constitution, the 2009 Law No. 32 on the Environment, and the 1999 Law No. 39 on Human Rights. The problem raised herein is to what extent people can claim the right to a clean environment as human rights guaranteed and protected in those legal instruments. The results of the discussion show that those three legal instruments do not protect people whose human rights have been violated. This is because 2000 Law No. 26 on Human Rights has no jurisdiction over environmental matters. It is suggested that establishing a special Environmental Court is the solution to protect community environmental human rights cases.

5. Orientalism: From Expeditions to Colonialisation

Abstract

The aim of this paper is to examine the relationship between orientalism and colonialism. More specifically, it analyses orientalism in its various ways before and during the nineteenth century to the present day, as well as its actual aims. For this purpose, the expeditions of some European orientalists, missionaries, and explorers are analysed and discussed. In addition to these expeditions, numerous writings and various viewpoints expressed by some authors in the social and human sciences are examined. The methodology used in the study is mainly based on critical literature sources. Overall, it appears that despite the attempts of some authors to challenge it, there is a direct and close link between orientalism, postcolonialism, and colonialism. Indeed, far from being a simple and innocent approach or discipline studying the relationship between the East and the West, orientalism served as a colonial ideology and prepare the ground to facilitate Europe's dominance over the rest of the world. Furthermore, the structure of international relations and the forms of relationship that certain European powers maintain with their former colonies reveal orientalist and neo-colonialist patterns.

Abstract

Conversations between a clinician and a patient, in natural conditions, are valuable sources of information for medical follow-up. The automatic analysis of these dialogues could help extract new language markers and speed-up the clinicians' reports. Yet, it is not clear which speech processing pipeline is the most performing to detect and identify the speaker turns, especially for individuals with speech and language disorders. Here, we proposed a split of the data that allows conducting a comparative evaluation of speaker role recognition and speaker enrollment methods to solve this task. We trained end-to-end neural network architectures to adapt to each task and evaluate each approach under the same metric. Experimental results are reported on naturalistic clinical conversations between Neuropsychologist and Interviewees, at different stages of Huntington's disease. We found that our Speaker Role Recognition model gave the best performances. In addition, our study underlined the importance of retraining models with in-domain data. Abstract

Disease-modifying treatments are currently assessed in neurodegenerative diseases. Huntington's Disease represents a unique opportunity to design automatic sub-clinical markers, even in premanifest gene carriers. We investigated phonatory impairments as potential clinical markers and propose them for both diagnosis and gene carriers follow-up. We used two sets of features: Phonatory features and Modulation Power Spectrum Features. We found that phonation is not sufficient for the identification of sub-clinical disorders of premanifest gene carriers. Abstract

Rapid decline of glomerular filtration rate estimated from creatinine (eGFRcrea) is associated with severe clinical endpoints. In contrast to cross-sectionally assessed eGFRcrea, the genetic basis for rapid eGFRcrea decline is largely unknown. To help define this, we meta-analyzed 42 genome-wide association studies from the Chronic Kidney Diseases Genetics Consortium and United Kingdom Biobank to identify genetic loci for rapid eGFRcrea decline. Two definitions of eGFRcrea decline were used: 3 mL/min/1.73m2/year or more ("Rapid3"; encompassing 34,874 cases, 107,090 controls) and eGFRcrea decline 25% or more and eGFRcrea under 60 mL/min/1.73m2 at follow-up among those with eGFRcrea 60 mL/min/1.73m2 or more at baseline ("CKDi25"; encompassing 19,901 cases, 175,244 controls). Seven independent variants were identified across six loci for Rapid3 and/or CKDi25: consisting of five variants at four loci with genome-wide significance (near UMOD-PDILT (2), PRKAG2, WDR72, OR2S2) and two variants among 265 known eGFRcrea variants (near GATM, LARP4B). All these loci were novel for Rapid3 and/or CKDi25 and our bioinformatic follow-up prioritized variants and genes underneath these loci. The OR2S2 locus is novel for any eGFRcrea trait including interesting candidates. For the five genome-wide significant lead variants, we found supporting effects for annual change in blood urea nitrogen or cystatin-based eGFR, but not for GATM or LARP4B. Individuals at high compared to those at low genetic risk (8-14 vs. 0-5 adverse alleles) had a 1.20-fold increased risk of acute kidney injury (95% confidence interval 1.08-1.33). Thus, our identified loci for rapid kidney function decline may help prioritize therapeutic targets and identify mechanisms and individuals at risk for sustained deterioration of kidney function.

Abstract

Marine biota are redistributing at a rapid pace in response to climate change and shifting seascapes. While changes in fish populations and community structure threaten the sustainability of fisheries, our capacity to adapt by tracking and projecting marine species remains a challenge due to data discontinuities in biological observations, lack of data availability, and mismatch between data and real species distributions. To assess the extent of this challenge, we review the global status and accessibility of ongoing scientific bottom trawl surveys. In total, we gathered metadata for 283,925 samples from 95 surveys conducted regularly from 2001 to 2019. We identified that 59% of the metadata collected are not publicly available, highlighting that the availability of data is the most important challenge to assess species redistributions under global climate change. Given that the primary purpose of surveys is to provide independent data to inform stock assessment of commercially important populations, we further highlight that single surveys do not cover the full range of the main commercial demersal fish species. An average of 18 surveys is needed to cover at least 50% of species ranges, demonstrating the importance of combining multiple surveys to evaluate species range shifts. We assess the potential for combining surveys to track transboundary species redistributions and show that differences in sampling schemes and inconsistency in sampling can be overcome with spatio-temporal modeling to follow species density redistributions. In light of our global assessment, we establish a framework for improving the management and conservation of transboundary and migrating marine demersal species. We provide directions to improve data availability and encourage countries to share survey data, to assess species vulnerabilities, and to support management adaptation in a time of climate-driven ocean changes.

13. Endotracheal tube biofilm in critically ill patients during the COVID-19 pandemic : description of an underestimated microbiological compartment

Abstract

Abstract Biofilm (BF) growth is believed to play a major role in the development of ventilator-associated pneumonia (VAP) in the intensive care unit. Despite concerted efforts to understand the potential implication of endotracheal tube (ETT)-BF dispersal, clinically relevant data are lacking to better characterize the impact of its mesostructure and microbiological singularity on the occurrence of VAP. We conducted a multicenter, retrospective observational study during the third wave of the COVID-19 pandemic, between March and May 2021. In total, 64 ETTs collected from 61 patients were included in the present BIOPAVIR study. Confocal microscopy acquisitions revealed two main morphological aspects of ETT-deposited BF: (1) a thin, continuous ribbon-shaped aspect, less likely monobacterial and predominantly associated with Enterobacter spp., Streptococcus pneumoniae or Viridans streptococci, and (2) a thicker, discontinuous, mushroom-shaped appearance, more likely characterized by the association of bacterial and fungal species in respiratory samples. The microbiological characterization of ETT-deposited BF found higher acquired resistance in more than 80% of analyzed BF phenotypes, compared to other colonization sites from the patient’s environment. These findings reveal BF as a singular microbiological compartment, and are of added clinical value, with a view to future ETT-deposited BF-based antimicrobial stewardship in critically ill patients. Abstract

Environmental surveillance for poliovirus is increasingly used in poliovirus eradication efforts as a supplement to acute flaccid paralysis (AFP) surveillance. Environmental surveillance was officially established in 2017 in Senegal, where no poliovirus had been detected since 2010. We tested sewage samples from 2 sites in Dakar monthly for polioviruses. We identified a vaccine-derived poliovirus serotype 2 on January 19, 2021, from a sample collected on December 24, 2020; by December 31, 2021, we had detected 70 vaccine-derived poliovirus serotype 2 isolates circulating in 7 of 14 regions in Senegal. Sources included 18 AFP cases, 20 direct contacts, 17 contacts in the community, and 15 sewage samples. Phylogenetic analysis revealed the circulation of 2 clusters and provided evidence on the virus introduction from Guinea. Because novel oral polio vaccine serotype 2 was used for response activities throughout Senegal, we recommend expanding environmental surveillance into other regions.

Abstract

Increased levels of the urinary albumin-to-creatinine ratio (UACR) are associated with higher risk of kidney disease progression and cardiovascular events, but underlying mechanisms are incompletely understood. Here, we conduct trans-ethnic (n = 564,257) and European-ancestry specific meta-analyses of genome-wide association studies of UACR, including ancestry- and diabetes-specific analyses, and identify 68 UACR-associated loci. Genetic correlation analyses and risk score associations in an independent electronic medical records database (n = 192,868) reveal connections with proteinuria, hyperlipidemia, gout, and hypertension. Fine-mapping and trans-Omics analyses with gene expression in 47 tissues and plasma protein levels implicate genes potentially operating through differential expression in kidney (including TGFB1, MUC1, PRKCI, and OAF), and allow coupling of UACR associations to altered plasma OAF concentrations. Knockdown of OAF and PRKCI orthologs in Drosophila nephrocytes reduces albumin endocytosis. Silencing fly PRKCI further impairs slit diaphragm formation. These results generate a priority list of genes and pathways for translational research to reduce albuminuria.

17. Benthic Fauna Assessment along the Navigation Channel from the Mouth of the Casamance Estuary to Ziguinchor City

Abstract

This study aimed at studying the benthic fauna of the Casamance estuary (Senegal) through the analysis of its composition at 13 stations located along the channel from the estuary mouth to Ziguinchor port (estuary upstream), which may provide indications of the potential environmental impact of the navigation channel dredged on the estuary. Sixty taxa distributed in twelve phyla were identified, of which the most abundant were bivalve mollusks, annelids, and nematodes. The species abundance and species richness varied from five (estuary mouth) to thirty species (upstream). Correspondence factorial analysis (CFA) and factor analysis for mixed data (FAMD) showed, respectively, that 34% (Dim 1 = 15.6% and Dim 2 = 18.4%) and 35.5% (Dim 1 = 20.8% and Dim 2 = 14.7%) of total inertia was explained by the first two axes and a significant correlation between the dominant species distribution, sediment type, and depth of the sampled station. Overall, Spearman’s rank correlation indicated a significant negative correlation of the species abundance and species richness with distance from the estuary mouth. From these results, it appears that sediment type, depth of the sampled stations, salinity, and the mangrove forests are key factors that influence the distribution and abundance of benthic fauna in the Casamance estuary. The monitoring plans for the preservation of the health and biological diversity of this ecosystem, especially benthic fauna biodiversity, should take into consideration the seasonal variations of rainfalls and related changes in terms of physicochemical factors. This should include evaluating the long-term responses of benthic organisms to dredging activities, boat traffic, and especially physical habitat modifications, changes in migration ways, and pollution pressure.

Bu araştırma Nijer'in eğitim sistemi ile kültür ve kimliğinin inşası arasındaki mevcut ilişkiyi ortaya koymayı amaçlamaktadır. Bu makalede, sömürgecilik döneminden günümüze kadar Nijer’in siyasî tarihine odaklanılmıştır. Nitekim geçmişte yaşanan çeşitli gelişmeler Nijer’in siyasi tarihinde önemli bir yere sahiptir. Bununla birlikte yaşanan gelişmelerinin ardında Nijer’de yönetim biçimi demokratik sürece doğru geçişi tecrübe etmektedir. Nijer’in bağımsızlığı dâhil olmak üzere, istikrarın olmadığı dönemlerde iktidar tarafından uygulamaya konulan eğitim ve kamu politikalarının kültürel hayat ve kimlik üzerindeki etkileri tartışmaya açıktır. Genel olarak, Nijer’in toplumsal gerçekliği ve kültürel dokusu dikkate alınarak, yeni bir eğitim sistemi müfredatı uygulamaya konulmuş olmakla birlikte, birçok eksikliğin varlığı da göze çarpmaktadır. Dolaysıyla, bağımsız bir ülke olmasına rağmen, başta Fransa olmak üzere dış devletler tarafından Nijer’in iç politikasına yapılan yoğun müdahalelerden dolayı ülkenin eğitim standartları hedeflenenden daha aşağı seviyededir.

23. Driver drowsiness estimation using EEG signals with a dynamical encoder–decoder modeling framework

Abstract

Abstract Drowsiness is a leading cause of accidents on the road as it negatively affects the driver’s ability to safely operate a vehicle. Neural activity recorded by EEG electrodes is a widely used physiological correlate of driver drowsiness. This paper presents a novel dynamical modeling solution to estimate the instantaneous level of the driver drowsiness using EEG signals, where the PERcentage of eyelid CLOSure (PERCLOS) is employed as the ground truth of driver drowsiness. Applying our proposed modeling framework, we find neural features present in EEG data that encode PERCLOS. In the decoding phase, we use a Bayesian filtering solution to estimate the PERCLOS level over time. A data set that comprises 18 driving tests, conducted by 13 drivers, has been used to investigate the performance of the proposed framework. The modeling performance in estimation of PERCLOS provides robust and repeatable results in tests with manual and automated driving modes by an average RMSE of 0.117 (at a PERCLOS range of 0 to 1) and average High Probability Density percentage of 62.5%. We further hypothesized that there are biomarkers that encode the PERCLOS across different driving tests and participants. Using this solution, we identified possible biomarkers such as Theta and Delta powers. Results show that about 73% and 66% of the Theta and Delta powers which are selected as biomarkers are increasing as PERCLOS grows during the driving test. We argue that the proposed method is a robust and reliable solution to estimate drowsiness in real-time which opens the door in utilizing EEG-based measures in driver drowsiness detection systems.

Abstract

Uterus measurements are useful for assessing both the treatment and follow-ups of gynaecological patients. The aim of our study was to develop a deep learning (DL) tool for fully automated measurement of the three-dimensional size of the uterus on magnetic resonance imaging (MRI). In this single-centre retrospective study, 900 cases were included to train, validate, and test a VGG-16/VGG-11 convolutional neural network (CNN). The ground truth was manual measurement. The performance of the model was evaluated using the objective key point similarity (OKS), the mean difference in millimetres, and coefficient of determination R2. The OKS of our model was 0.92 (validation) and 0.96 (test). The average deviation and R2 coefficient between the AI measurements and the manual ones were, respectively, 3.9 mm and 0.93 for two-point length, 3.7 mm and 0.94 for three-point length, 2.6 mm and 0.93 for width, 4.2 mm and 0.75 for thickness. The inter-radiologist variability was 1.4 mm. A three-dimensional automated measurement was obtained in 1.6 s. In conclusion, our model was able to locate the uterus on MRIs and place measurement points on it to obtain its three-dimensional measurement with a very good correlation compared to manual measurements.

ContextBehavioral observation scales are important for understanding and assessing social skills. In the context of collaborative problem-solving (CPS) skills, considered essential in the 21st century, there are no validated scales in French that can be adapted to different CPS tasks. The aim of this study is to adapt and validate, by annotating a new video corpus of dyadic interactions that we have collected, two observational scales allowing us to qualitatively assess CPS skills: the Social Performance Rating Scale (SPRS) and the Social Skills of Collaboration Scale (SSC).MethodThe construct validity of these two scales was assessed by exploratory factor analysis and inter-item correlations. We also checked inter-judge agreement using inter-class correlation coefficients. Internal consistency was determined using Cronbach’s alpha and convergent and divergent validity by assessing correlations between the two scales and measures of depression and alexithymia. Finally, the discriminative properties of the two scales were analyzed by comparing the scores obtained by a group of anxious individuals and a non-anxious control group.ResultsThe results show that our two scales have excellent inter-item correlations. Internal consistency is excellent (alpha SPRS =0.90; SSC = 0.93). Inter-rater agreement ranged from moderate to high. Finally, convergent validity was significant with the alexithymia scale, as was divergent validity with the depression scale. Anxious individuals had lower scores on both scales than non-anxious individuals.ConclusionBoth scales show good psychometric properties for assessing social skills relevant to different collaborative tasks. They also identify individuals with difficulties in social interaction. Thus, they could allow monitoring the effectiveness of training social skills useful in CPS.

Abstract

Mauritania is characterized by fast-growing fisheries that have developed over the past several decades from low levels of fishing to intense fishing and have led to severe decreases in biomass and severe signs of overexploitation. To ensure sustainable fisheries, a primary goal of science-based fisheries policy is to provide an adequate conservation status of marine habitats that are directly, or indirectly, fundamental for the lifecycle of fish populations of commercial interest. The National Park of Banc d’Arguin has been recognized for its nursery and fish biomass export functions. In this paper, we present the economic value of the fish provisioning service of marine coastal ecosystems of the protected area. We estimated the added value of artisanal fisheries, the spatial distribution of fishing activities, the contribution of the Park to the Mauritanian fisheries and the heritage value of artisanal fisheries.Overall, the average added value for artisanal fisheries is about 43 million MRU (1 million euros) over the period 2006–2017. It mainly benefits to the ship-owners, which generates inequalities and threatens the livelihoods of the Imraguen. The spatial distribution of fishing activities and associated revenues shows high variability throughout the years, the seasons and the geographic areas, and reveals the high adaptability of Imraguen fishers. Fishing grounds are mostly located close to the shore, associated with seagrass beds, confirming the service of fish provisioning rendered by this ecosystem. Furthermore, the high productivity of seagrass beds contributes to the spill-over of biomass. The contribution of the Park to fishing activities that occur within the Mauritanian Exclusive Economic Zone is estimated to 15%, representing an added value creation of 3.2 billion MRU (71 million euros). The heritage value of the fisheries is assessed, through fishers' willingness to accept to stop their fishing activities, at 17.5 million MRU (0.4 million euros). Thus, this paper highlights the necessity of incorporating an ecosystem-based approach along with fishers' knowledge and adaptability into future conservation plan of the National Park of Banc d’Arguin and fisheries management at the local, national and regional scale.

Abstract

Globally, over 85% of oyster reefs have been lost, and the combined effects of climate change, ocean acidification, and environmental degradation, including pollution and mangrove overharvesting, could further reduce global oyster fisheries in the coming decades. To understand the level of impact of climate change on the oyster fishery in West Africa, an expert-based vulnerability assessment to climate change was conducted for the West African mangrove oyster (Crassostrea tulipa, Lamarck 1819). Using a combination of the exposure of the oyster to climatic stressors (estuarine temperature, salinity, river flow, surface run-off, sea level rise, and estuarine circulation) together with an assessment of sensitivity to these stressors, we estimate the overall vulnerability of C. tulipa to climate change. A very high overall climate vulnerability score of 12 on a scale of 16 was calculated for C. tulipa. While the overall climate exposure score in the West African coastal region remained high, the high sensitivity of C. tulipa to hydrographic conditions of its habitat, in particular salinity, coupled with its sessile and habitat-specific nature, pushed the overall vulnerability to very high. Early life history settlement requirements, adult mobility, and sensitivity to salinity were the three most important biological and sensitivity attributes that determined the vulnerability score. By leaving each of these three sensitivity attributes out of the analysis, the overall vulnerability score was reduced to 9 (i.e., from very high to high). A negative directional effect of climate change, coupled with a low potential for change in distribution, threatens the C. tulipa fishery in a long-term adverse climate scenario. We recommend management efforts that incorporate climate resilience and adaptation practices to prioritize recruitment success, as well as the development of breeding lines with climate-resilient traits.

Accurate tracking of ball trajectories on a platform using a 2-DOF balancer system poses significant challenges in the existing literature due to its inherent nonlinearity and instability. This research addresses these challenges through a two-fold approach. Firstly, we focus on designing a robust 2-DOF ball balancer system model. Secondly, we perform a comparative study of three control techniques: Linear Quadratic Regulator-based Proportional (LQR_P), Full State Feedback-based Proportional (FSF_P), and Classical Proportional Derivative-based Proportional (PD_P) control. To evaluate the effectiveness of the designed controllers, we conduct both simulation and experimental tests using MATLAB Simulink integrated with Quarc software and the 2DOF ball balancer system Quanser hardware. The results demonstrate that the Linear Quadratic Regulator-based proportional control exhibits superior performance in terms of transient response, including percentage overshoot, settling time, and peak time. Abstract

Polioviruses have been eliminated in many countries; however, the number of acute flaccid paralysis cases has not decreased. Non-polio enteroviruses are passively monitored as part of the polio surveillance program. Previous studies have shown that some enteroviruses do not grow in conventional cell lines used for the isolation of poliovirus according to the WHO guidelines. In order to evaluate the presence of enteroviruses, real-time RT-PCR was performed on Human Rhabdomyosarcoma (RD)-positive and RD-negative stool samples. A total of 310 stool samples, collected from children under the age of 15 years with acute flaccid paralysis in Senegal in 2017, were screened using cell culture and real-time RT-PCR methods. The selected isolates were further characterized using Sanger sequencing and a phylogenetic tree was inferred based on VP1 sequences. Out of the 310 stool samples tested, 89 were positive in real-time RT-PCR. A total of 40 partial VP1 sequences were obtained and the classification analysis showed that 3 (13%), 19 (82.6%), and 1 (4.4%) sequences from 23 RD-positive non-polio enterovirus isolates and 3 (17.6%), 7 (41.1%), and 7 (41.1%) sequences from 17 RD-negative stool samples belonged to the species EV-A, B, and C, respectively. Interestingly, the EV-B sequences from RD-negative stool samples were grouped into three separate phylogenetic clusters. Our data exhibited also a high prevalence of the EV-C species in RD-negative stool samples. An active country-wide surveillance program of non-polio enteroviruses based on direct RT-PCR coupled with sequencing could be important not only for the rapid identification of the involved emergence or re-emergence enteroviruses, but also for the assessment of AFP’s severity associated with non-polio enteroviruses detected in Senegal.

Transport measurements carried out on twinned ybco films are compared to the predictions of a previously proposed model suggesting that the vortices move along the films twin boundaries that behave as rows of Josephson weak links [P.Bernstein and J.F.Hamet, J.Appl.Phys.95 (2004) 2569]. The obtained results suggest that, except if the films are very thin, the twin boundaries consist of superimposed rows of weak links with mean height,ds, whose mean length along the TBs is an universal function of T/Tc, the reduced temperature. This conclusion yields a general expression for the critical surface current density of the films as a function of T/Tc and of the number of superimposed weak links rows, while the critical current density depends on ds. A comparison of the measurements reported by various authors shows that the nature of the substrate and the growth technique have both a strong effect on ds . Abstract

We propose a description of the vortex dynamics in YBCO films from the critical to the normal states. This description supposes that the vortex motion is thermally activated along the twin boundaries of the films. The discontinuity observed in the current-voltage curves at the transition to the normal state is explained by the sudden increase in the dissipated power rate due to vortex depinning. However, near the critical temperature, this phenomenon does not occur because the vortex activation energy is near zero. We also show how the current at the transition to the normal state can be computed from the current-voltage curves measured at low currents. The predictions of this description are compared to the data published by Gonzalez et al. [Phys.Rev.B68,054514 (2003)].

Abstract

BackgroundEfficient cognitive tasks sensitive to longitudinal deterioration in small cohorts of Huntington's disease (HD) patients are lacking in HD research. We thus developed and assessed the digitized arithmetic task (DAT), which combines inner language and executive functions in approximately 4 minutes.MethodsWe assessed the psychometric properties of DAT in three languages, across four European sites, in 77 early-stage HD patients (age: 52 ± 11 years; 27 females), and 57 controls (age: 50 ± 10, 31 females). Forty-eight HD patients and 34 controls were followed up to one year with 96 participants who underwent MRI brain imaging (HD patients = 46) at baseline and 50 participants (HD patients = 22) at one year. Linear mixed models and Pearson correlations were used to assess associations with clinical assessment.ResultsAt baseline, HD patients were less accurate (p = 0.0002) with increased response time (pConclusionsDAT is fast, reliable, motor-free, applicable in several languages, and able to unmask cognitive decline correlated with striatal atrophy in small cohorts of HD patients. This likely makes it a useful endpoint in future trials for HD and other neurodegenerative diseases.

Abstract

Background and objectives Hyperkalemia after starting renin-angiotensin system inhibitors has been shown to be subsequently associated with a higher risk of cardiovascular and kidney outcomes. However, whether to continue or discontinue the drug after hyperkalemia remains unclear. Design, setting, participants, & measurements Data came from the Action in Diabetes and Vascular Disease: Preterax and Diamicron Modified Release Controlled Evaluation (ADVANCE) trial, which included a run-in period where all participants initiated angiotensin-converting enzyme inhibitor–based therapy (a fixed combination of perindopril and indapamide). The study population was taken as patients with type 2 diabetes with normokalemia (serum potassium of 3.5 to,5.0 mEq/L) at the start of run-in. Potassium was remeasured 3 weeks later when a total of 9694 participants were classified into hyperkalemia (≥5.0 mEq/L), normokalemia, and hypokalemia (,3.5 mEq/L) groups. After run-in, patients were randomized to continuation of the angiotensin-converting enzyme inhibitor–based therapy or placebo; major macrovascular, microvascular, and mortality outcomes were analyzed using Cox regression during the following 4.4 years (median). Results During active run-in, 556 (6%) participants experienced hyperkalemia. During follow-up, 1505 participants experienced the primary composite outcome of major macrovascular and microvascular events. Randomized treatment of angiotensin-converting enzyme inhibitor–based therapy significantly decreased the risk of the primary outcome (38.1 versus 42.0 per 1000 person-years; hazard ratio, 0.91; 95% confidence interval, 0.83 to 1.00; P=0.04) compared with placebo. The magnitude of effects did not differ across subgroups defined by short-term changes in serum potassium during run-in (P for heterogeneity =0.66). Abstract

Background: Standard measures of blood pressure (BP) do not account for both the magnitude and duration of exposure to elevated BP over time. Objectives: The purpose of this study was to assess the association between cumulative systolic blood pressure (SBP) load and risk of cardiovascular events in patients with type 2 diabetes. Methods: A post hoc analysis of patients with type 2 diabetes followed by the ADVANCE-ON (Action in Diabetes and Vascular Disease: Preterax and Diamicron Modified Release Controlled Evaluation - Observational Study). Cumulative SBP load was defined as the area under curve for SBP values ≥130 mm Hg divided by the area under curve for all measured SBP values over a 24-month exposure period. HRs for the association between cumulative SBP load with major cardiovascular events and death were estimated using Cox models. Results: Over a median 7.6 years of follow-up, 1,469 major cardiovascular events, 1,615 deaths, and 660 cardiovascular deaths were observed in 9,338 participants. Each 1-SD increase in cumulative SBP load was associated with a 14% increase in major cardiovascular events (HR: 1.14; 95% CI: 1.09-1.20), 13% increase in all-cause mortality (HR: 1.13; 95% CI: 1.13-1.18), and 21% increase in cardiovascular death (HR: 1.21; 95% CI: 1.13-1.29). For the prediction of cardiovascular events and death, cumulative SBP load outperformed mean SBP, time-below-target SBP, and visit-to-visit SBP variability in terms of Akaike information criterion and net reclassification indexes. Conclusions: Cumulative SBP load may provide better prediction of major cardiovascular events compared with traditional BP measures among patients with type 2 diabetes. These findings reinforce the importance of both the magnitude and duration of exposure to elevated SBP in assessing cardiovascular risk.
