Abstract

Simple Summary: Myeloproliferative neoplasms (MPNs) represent a subset of blood cancers, a member of which is chronic neutrophilic leukemia (CNL). This entity is very rare in comparison to the more common MPNs. The increased usage of next-generation sequencing (NGS) has increased our diagnostic and prognostic ability regarding this entity. There is no specified treatment approach for CNL; however, there are clinical trials that show promising results and experimental drugs that are being investigated to reach a unified treatment approach. This review aims to comprehensively review this rare entity in terms of epidemiology, diagnosis, pathogenesis, and treatment. CNL is a rare subtype of MPNs characterized by persistent neutrophilia, bone marrow hypercellularity, and specific genetic mutations, particularly in the CSF3R gene. Advances in molecular diagnostics have greatly enhanced our understanding of CNL, distinguishing it from other myeloproliferative disorders and refining diagnostic criteria. This review provides an updated overview of CNL, focusing on breakthroughs in genetic profiling, including novel mutations with potential prognostic value and implications for targeted therapy. We discuss current management strategies, emphasizing the role of JAK inhibitors, allogeneic stem cell transplantation, and evolving investigational treatments. Challenges in early diagnosis, therapeutic resistance, and future directions in research are also addressed, underscoring the need for a personalized medicine approach to improve outcomes for patients with CNL. Abstract

The study aimed to “measure the impact of the environmental review in disclosing the environmental burdens and their implications in raising the rates of profitability”. To achieve the objectives of the study, the descriptive approach was used. Analytical study in the theoretical study and preparation of the field side, as the researcher relied upon collecting the primary data for the study on the method of observation and investigation, implementing the survey list directed to the sample items, which amount to 384 items, and statistical analysis was used to ensure the validity of the study hypotheses. The study reached the following results: The first hypothesis was accepted: “There is a significant relationship between the environmental audit and the accounting disclosure of environmental burdens,” as there was a positive correlation with a significant significance between the environmental audit and the accounting disclosure of the environmental burdens, where the correlation coefficient reached 0.751, so the greater the review Environmental The greater the accounting disclosure of environmental burdens. The second hypothesis was accepted: “There is a significant and significant relationship between the accounting disclosure of environmental burdens and raising the rates of profitability.” There was a positive correlation with a significant significance between the accounting disclosure of environmental burdens and raising the rates of profitability, as the correlation coefficient reached 0.594. The need for auditors to be familiar with the laws and legislations to which economic establishments are subject, information systems, and related scientific disciplines. The necessity for the environmental review team to include experts in specialized scientific fields related to the environment, or to seek assistance from experts if they are not present on the team.

36. Analytic Study of Cough and Wheezes in Children Attended to Al-Azhar Assuit University Hospital.

Abstract

Background: There is a large deal of heterogeneity in the clinical appearance, duration, and course of the disease among children who wheeze and have recurrent coughs. Objectives: To evaluate clinically, epidemiologically and laboratory the children attended to Al-Azhar Assuit University Hospital by coughing and wheezing. Subjects and Methods: This study is a retrospective study between 1st March 2020 and the 28th February 2021 and prospective study between 1st March 2021 and 30th November 2021 and was carried out in outpatient and inpatient Pediatric Chest Unit in Al-Azhar Assuit University Hospital. Result: Among the studied cases (Total n=324), there were 106 (32.8%) suffered from bronchial asthma, 81 (25%) suffered from pneumonia, 63 (19.4%) suffered from bronchiolitis, 27 (8.3%) suffered from COVID-19 and 47 (14.5%) suffered from other cases such as bronchitis, GERD, lung abscess and F.B. Conclusion: Cough in pediatric is due to multiple underlying causes, the most common are bronchial asthma, pneumonia, bronchiolitis, COVID-19, foreign body aspiration, and gastroesophageal reflux. Bronchial asthma was more prevalent among age (6-12) years but pneumonia was among age (1-6) years, bronchiolitis was among age (1-2) years and COVID-19 was among age (12-18) years. Bronchial asthma and infections as pneumonia and bronchiolitis had declined among breast feeding cases. [ABSTRACT FROM AUTHOR]

41. Assessment of Thyroid Functions in Late Preterm Infants of Mothers on Antenatal Steroids

Abstract

Purpose: The study examined the effect of antenatal steroids on thyroid functions in late preterm infants on the third to the seventh day of life. Patients and Methods: A comparative Cross-Sectional study was conducted on 75 neonates admitted to NICU in the first week of life. They were divided according to exposure to antenatal steroids into three groups. First group: exposed to complete course of ANS. Second group: exposed to partial course of ANS. The third group: not exposed to ANS. Serum samples were obtained from selected cases free T3, Free T4, and TSH levels in the third day of life, compared to the cut of levels currently available (the TSH reference range is (1.7 to 9.1 mU per L), T4 should be greater than( 10 mcg per dL). Using Eleusis and Cubase analyzers kits (Roche Diagnostics, Indianapolis, IN, USA) by ELISA (enzyme-linked immune sorbent assay) technique. Results: The study showed that there were significantly higher serum T4 levels in group 1 that was exposed to a complete course compared to group 2 (partial course) and 3 (Third group). There were no significant differences in serum TSH, T3 levels between groups. The current study found a decreased incidence of the ROP, NEC and BPD among group 1 as compared to the other two groups although was non statistically significant. In addition, the complete course of ANS had delayed the date of delivery (P=0.04) as compared to the partial course in the current study. Conclusion: Antenatal corticosteroids can influence thyroid function in late preterm infants as serum T4 was significantly higher in infants exposed to complete course compared to those who were exposed to partial course or did not receive antenatal corticosteroids.

Abstract

Abstract Background The relationship between zinc (Zn) and growth hormone-insulin growth factor (GH-IGF) system and how Zn therapy stimulates growth in children has not been clearly defined in humans. Thus, we aimed to assess GH-IGF axis in short children with Zn deficiency and to investigate the effect of Zn supplementation on these parameters. Methods Fifty pre-pubertal Egyptian children with short stature and Zn deficiency were compared to 50 age-, sex-, and pubertal stage- matched controls. All subjects were subjected to history, auxological assessment and measurement of serum Zn, IGF-1, insulin growth factor binding protein-3 (IGFBP-3); and basal and stimulated GH before and 3 months after Zn supplementation (50 mg/day). Results After 3 months of Zn supplementation in Zn-deficient patients, there were significant increases in height standard deviation score (SDS, P = 0.033), serum Zn (P Conclusion Serum IGF-1 and IGFBP-3 levels were low in short children with Zn deficiency, and increased after Zn supplementation for 3 months but their levels were still lower than the normal reference ranges in most children; therefore, Zn supplementation may be necessary for longer periods.
