2. MEASUREMENT OF ANTIPLATELET ACTIVITY OF ANTILYMPHOCYTE SERA BASED ON THE INHIBITION OF 5-HYDROXYTRYPTAMINE UPTAKE BY BLOOD PLATELETS

Abstract

A method for the measurement of antiplatelet antibodies (APA) in antilymphocyte sera is described. It is based on the percentage of inhibition induced by 180 min of incubation with serum on the uptake of 5-hydroxytryptamine by platelets. An APA unit is denned as the amount of APA which inhibits 50 of the control uptake. This test is far more sensitive than the classical complement-fixation and agglutination tests. All of the antilymphocyte sera tested have shown the presence of some antiplatelet activity with this new test. When the APA titrc was less than 20 units, the other tests were usually negative and no thrombopenic effect was observed in patients. Since the new test may detect as little as 0.1 APA unit, it may be particularly useful for studying preparations with a low APA content.

Abstract

La description d'une coupe géologique est actuellement réalisée par des représentations numériques. Ce type de représentation est inadapté pour effectuer des raisonnements géologiques. Nous proposons une méthode pour reconnaître les objets géologiques et un formalisme FROG(1) de représentation symbolique de ces objets. Nous utilisons pour parvenir à cette fin les méthodes et techniques de programmation d'Intelligence Artificielle. En particulier, nous substituons à la reconnaissance de formes qui n'est pas applicable dans ce cas, ce que nous appelons la reconnaissance génétique où les modèles d'objets à reconnaître sont définis non pas par leurs caractéristiques géométriques, mais leurs caractéristiques historiques, autrement dit par leur genèse. L'identification de ces caractéristiques, ainsi que la compréhension de leur signification géologique forment une partie de l'expertise d'un géologue. Le programme GROG met en oeuvre ces idées et fournit une validation de ce travail. (1) Formalisme de Représentation des Objets Géologiques. A picking of cross-section is currently represented in a numerical way. This kind of representation is not adapted to perform geological reasoning. We propose a method of recognition of geological objects and the FROG (1) symbolic representation formalism for geological objects. To achieve this goal, we use Artifical Intelligence methods and techniques. Particularly, we do not use pattern recognition methods which are hardly helpful, but what we call genetic recognition: geological models are not defined by their geometrical features, but by their historical features, i. e. by their genesis. Identification of these features and understanding of their geological meaning are a part of the geologist expertise. The GROG program provides a FROG implementation and a validation of this work. Abstract

BACKGROUND: Patients with obstructive left main coronary artery disease are usually treated with coronary-artery bypass grafting (CABG). Randomized trials have suggested that drug-eluting stents may be an acceptable alternative to CABG in selected patients with left main coronary disease. METHODS: We randomly assigned 1905 eligible patients with left main coronary artery disease of low or intermediate anatomical complexity to undergo either percutaneous coronary intervention (PCI) with fluoropolymer-based cobalt-chromium everolimus-eluting stents (PCI group, 948 patients) or CABG (CABG group, 957 patients). Anatomic complexity was assessed at the sites and defined by a Synergy between Percutaneous Coronary Intervention with Taxus and Cardiac Surgery (SYNTAX) score of 32 or lower (the SYNTAX score reflects a comprehensive angiographic assessment of the coronary vasculature, with 0 as the lowest score and higher scores [no upper limit] indicating more complex coronary anatomy). The primary end point was the rate of a composite of death from any cause, stroke, or myocardial infarction at 3 years, and the trial was powered for noninferiority testing of the primary end point (noninferiority margin, 4.2 percentage points). Major secondary end points included the rate of a composite of death from any cause, stroke, or myocardial infarction at 30 days and the rate of a composite of death, stroke, myocardial infarction, or ischemia-driven revascularization at 3 years. Event rates were based on Kaplan-Meier estimates in time-to-first-event analyses. RESULTS: At 3 years, a primary end-point event had occurred in 15.4% of the patients in the PCI group and in 14.7% of the patients in the CABG group (difference, 0.7 percentage points; upper 97.5% confidence limit, 4.0 percentage points; P=0.02 for noninferiority; hazard ratio, 1.00; 95% confidence interval, 0.79 to 1.26; P=0.98 for superiority). Abstract

The aim of the study was to evaluate the levels of physical activity in individuals with primary Sjögren’s syndrome (PSS) and its relationship to the clinical features of PSS. To this cross-sectional study, self-reported levels of physical activity from 273 PSS patients were measured using the International Physical Activity Questionnaire-short form (IPAQ-SF) and were compared with healthy controls matched for age, sex and body mass index. Fatigue and other clinical aspects of PSS including disease status, dryness, daytime sleepiness, dysautonomia, anxiety and depression were assessed using validated tools. Individuals with PSS had significantly reduced levels of physical activity [median (interquartile range, IQR) 1572 (594–3158) versus 3708 (1732–8255) metabolic equivalent of task (MET) × min/week, p < 0.001], but similar levels of sedentary activity [median (IQR) min 300 (135–375) versus 343 (223–433) (MET) × min/week, p = 0.532] compared to healthy individuals. Differences in physical activity between PSS and controls increased at moderate [median (IQR) 0 (0–480) versus 1560 (570–3900) MET × min/week, p < 0.001] and vigorous intensities [median (IQR) 0 (0–480) versus 480 (0–1920) MET × min/week, p < 0.001]. Correlation analysis revealed a significant association between physical activity and fatigue, orthostatic intolerance, depressive symptoms and quality of life. Sedentary activity did not correlate with fatigue. Stepwise linear regression analysis identified symptoms of depression and daytime sleepiness as independent predictors of levels of physical activity. Physical activity is reduced in people with PSS and is associated with symptoms of depression and daytime sleepiness. Sedentary activity is not increased in PSS. Clinical care teams should explore the clinical utility of targeting low levels of physical activity in PSS.

Abstract

Sjogren's syndrome (SS) is a disorder in which the mucous membranes become dry, with dryness of the mouth and eyes. Although the cause of SS is unknown, it may be an autoimmune disease (in which the body manufactures antibodies to its own tissues) precipitated by environmental factors, such as viruses. In particular, Epstein-Barr virus (EBV), associated with the salivary glands, has been implicated. Several studies have examined salivary gland cells from SS patients for the presence of EBV, with conflicting results. In one study, 78 percent of the salivary glands from SS patients were found to contain EBV DNA (genetic material from EBV), while only 13 percent of the salivary glands from patients without SS contained EBV DNA. In two other studies, EBV DNA was present in the salivary glands of patients with and without SS. In an attempt to clarify this issue, salivary gland cells from 10 patients with SS, eight patients with rheumatoid arthritis (often associated with SS), and seven normal individuals were examined for the presence of EBV. The salivary gland cells were tested for EBV using two techniques: labeled antibodies that bind to antigens (proteins) associated with EBV (immunocytochemistry); and the polymerase chain reaction (PCR), a method of amplifying the amount of DNA present so it can be analyzed. None of the salivary gland samples tested positive for EBV using immunocytochemistry. When PCR was performed, no differences were found in the presence of EBV DNA between SS patients (present in 90 percent of the salivary gland samples) and normal individuals or rheumatoid arthritis patients (present in 71 percent of samples from both groups). 43. Molecular Technique Utilising Sputum For Detecting Wuchereria bancrofti Infections In Malindi, Kenya

Abstract

Background: Lymphatic filariasis is a tropical parasitic disease which has been identified for elimination by 2020 through mass drugs administration. There is a major problem in its diagnosis and sensitive surveillance methods for monitoring the disease elimination programs need to be sought. Objectives: To establish and evaluate the usefulness of a Polymerase Chain Reaction, PCR assay employing sputum for diagnosis of Wuchereria bancrofti infections in an endemic location. Design: Community based samples collection and a molecular laboratory technologies study. Setting: Mpirani, Malindi District and Centre for Biotechnology Research and Development, Kenya Medical Research Institute. Subjects: Sputum samples were obtained from 304 willing and consenting participants, aged between 5 and 73 years resident in Mpirani, Malindi District. Results: Prevalence of W. bancrofti infection was found to be 42.8% (130/304) by PCR assay employing sputum compared with 22.0 % (67/304) and 38.8% (119/304) respectively for microfilaria counts and ICT. The sensitivity and specificity of the PCR sputum assay was 97.5 and 92.4% respectively. Predictive values were 89.2 and 98.3% for positive (PPV) and negative (NPV) respectively while accuracy was 94.4%. Conclusions: The molecular PCR assay using sputum was found to have a great potential for use in mass diagnosis and in epidemiological studies in patients with W. bancrofti infections East African Medical Journal Vol. 85 (3) 2008: pp. 118-122
