It was intended to improve itch by lowering BNP with supportive communication between the patients and the dialysis staff. We planned to collect data, including visual analogue scale (VAS), dermatology life quality index (DLQI), blood samples and QOL score (KDQOL-SF), four times over a four week interval. The first data was obtained just prior to switching facilities due to a breakdown. The patients underwent hemodialysis in other facilities for two weeks and underwent determination of their clinical data, including QOL scores, three times.Results: While mean blood pressure showed no significant differences, serum albumin, iron and phosphate levels were worsened significantly between pre- and post-relocation. Serum BNP and DLQI showed no significant changes. VAS was significantly worsened. The mean values of the cognitive function in the KDQOL-SF and sleep and the role-physical, role-emotional, social function, dialysis staff’s encouragement in SF-36 analysis were identified as statistically significant items by MANOVA. Both SF-12 physical and mental composites were also statistically significant. Although SF-12 physical composites were significant among the patients under 66 yrs of age (median), eight factors were significant among those over 66 yrs. Independent analyses revealed every item that was detected worsened significantly after the switch of facilities.Conclusion: Unexpected switching of hemodialysis facilities severely impacts the QOL for a long duration as well as the patients’ symptom and laboratory data.Keywords: KDQOL-SF, switch of hemodialysis facilities, dermal itch

Recently, B-type (brain) natriuretic peptide (BNP) has been defined as an itch-selective neuropeptide in pruriceptive neurons in mice, and higher serum levels of BNP are frequently observed in hemodialysis patients. The objective of the present study was to evaluate the role of serum BNP in pruritus in patients undergoing hemodialysis. Patients and methods: The current cross-sectional study was performed on 43 patients undergoing maintenance hemodialysis. A visual analog scale (VAS) measuring the general severity of pruritus (values from 0 to 10, with higher values indicating more severe pruritus) in daytime and at night was self-reported by patients. Each patient's background and laboratory tests, including serum BNP in the post-hemodialysis period, were collected. The correlation between VAS and clinical parameters was evaluated. Results: Both daytime and nighttime VAS scores in diabetic patients were significantly less than those in nondiabetic patients. 9. Repellent effect of Gardenia jasminoidesethanol extracted oil on Blattella germanicaand Monomorium pharaonis

Abstract

The study examined the repellency of Gardenia jasminoidesethanol-extracted oil against the German cockroach, Blattella germanica, and the pharaoh ant, Monomorium pharaonis, which are serious pests in areas of public health hygiene. For the repellency tests, 31.4 μg of the oil was applied per cm2on one half of filter paper discs (9 or 15 cm diameter for the ant and cockroach, respectively), whereas the other half was treated as control (DMSO + Tween). Repellency effects were observed 1, 2, 3 and 4 h after the insect release. The oil showed high repellency against all life stages of cockroaches and worker ants. The maximum repellency was observed for the cockroach adults (81.7 ± 3.1%) followed by the fourth, third and second nymphal stages (76.7 ± 4.2, 75.0 ± 3.4, and 56.7 ± 8.4%, respectively), after 1h exposure. The repellence effect was strong against worker ants (78.3 ± 4.8%) after 1 h exposure. The repellence effect can last at least four hours for both species. Analysis of Gardeniaoil with Gas chromatography-mass spectrometry identified 14 major chemical components.

14. A dose-finding approach for genomic patterns in phase I trials

Abstract

Precision medicine is an emerging approach for disease treatment and prevention that accounts for individual variability in genes, environment, and lifestyle. Cancer is a genomic disease; therefore, the dose-efficacy and dose–toxicity relationships for molecularly targeted agents in cancer most likely differ, based on the genomic mutation pattern. The individualized optimal dose – the maximal efficacious dose with a clinically acceptable safety profile – may vary depending on the genomic mutation patterns and should be determined prior to the use of these agents in precision medicine. In addition, genes that influence the individualized optimal doses should be identified in early-phase development. In this study, we propose a novel dose-finding approach to identify the individualized optimal dose for molecularly targeted agents in phase I cancer trials. Individualized optimal dose determination and gene selection were conducted simultaneously based on L1 and L2 penalized regression. Similar to most reported dose-finding approaches, this study considers non-monotonic patterns for dose-efficacy and dose–toxicity relationships, as well as correlations between efficacy and toxicity outcomes based on multinomial distribution. Our dose-finding algorithm is based on the predictive probability calculated with an estimated penalized regression model. We compare the operating characteristics between the proposed and existing methods by simulation studies under various scenarios.

Abstract

BACKGROUND: The phase II J003 (N = 169) and phase III RECOURSE (N = 800) trials demonstrated a significant improvement in survival with trifluridine (FTD)/tipiracil (TPI) versus placebo in patients with refractory metastatic colorectal cancer. This post hoc analysis investigated pharmacokinetic data of FTD/TPI exposure and pharmacodynamic markers, such as chemotherapy-induced neutropenia (CIN) and clinical outcomes. PATIENTS AND METHODS: A total of 210 patients from RECOURSE were enrolled in this substudy. A limited sampling approach was used, with three pharmacokinetic samples drawn on day 12 of cycle 1. Patients were categorized as being above or below the median area under the plasma concentration-time curve (AUC) for FTD and TPI. We conducted a post hoc analysis using the entire RECOURSE population to determine the correlations between CIN and clinical outcome. We then carried out a similar analysis on the J003 trial to validate the results. RESULTS: In the RECOURSE subset, patients in the high FTD AUC group had a significantly increased CIN risk. Analyses of the entire population demonstrated that FTD/TPI-treated patients with CIN of any grade in cycles 1 and 2 had significantly longer median overall survival (OS) and progression-free survival (PFS) than patients who did not develop CIN and patients in the placebo group. Patients who required an FTD/TPI treatment delay had increased OS and PFS versus those in the placebo group and those who did not develop CIN. Similar results were obtained in the J003 cohort. CONCLUSIONS: In RECOURSE, patients with higher FTD drug exposure had an increased CIN risk. FTD/TPI-treated patients who developed CIN had improved OS and PFS versus those in the placebo group and those who did not develop CIN. Similar findings were reported in the J003 cohort, thus validating the RECOURSE results. The occurrence of CIN may be a useful predictor of treatment outcomes for FTD/TPI-treated patients. Abstract

The absorption rate of alcohol is one of the determinants of the peak blood alcohol concentration. Studies focusing on factors that cause inter-individual variability in absorption rate have not been conducted. To conduct a population pharmacokinetic analysis to identify variables that explain inter-individual differences in the absorption rate of alcohol, using blood alcohol concentrations immediately after consumption of a low dose of alcohol under fasting conditions. The study enrolled 34 healthy Japanese subjects, consisting of 21 males and 13 females (mean age, 29.4±12.9 years; mean body weight, 61.3±10.8 kg). All subjects consumed a single 350 mL can of beer (17.5 g of alcohol) after an overnight fast: an average dose of 0.232 g of alcohol per kg of body weight. A total of 157 samples were collected from 5 to 60 min after consumption of alcohol. A one-compartment model was fitted as a base model. The estimates of the population mean absorption rate and the population mean apparent volume of distribution were 0.023 1/min and 48.3 L, respectively. Age was positively correlated to the individual absorption rate and negatively correlated to the individual apparent volume of distribution, and body weight and ADH1B genotype affected the apparent volume of distribution. The key finding of this study is that age was positively correlated with alcohol absorption rate, suggesting that aging cause higher peak BAC due to higher absorption rate combined with lower apparent volume of distribution.

27. An interpretation of the neonatal period definition obtained with echocardiographic examination by using change point regression analysis.

Abstract

BACKGROUND: We had reported on the left ventricular end-diastolic dimension (LVDd) in normal children from the premature/neonatal period to the adolescence period by using two-dimensional echocardiography, and formulated equations to evaluate normal LVDd values by using body height as an index. There was an inflection point at around birth that seemed relevant to the fetal and neonatal periods for the relation of LVDd and body height. METHODS: We aimed to reveal the true inflection point and its meaning by using change point regression analysis. The study group consisted of 421 neonates and infants. The ages at examination ranged from 24 weeks' gestation to 1 year after birth. The subjects' body heights at examination were between 31 and 75 cm. RESULTS: The analysis showed no definite inflection point in height, and a flat bottom was observed between body heights of 48 and 55 cm. The inflection range seemed to mean the duration of the neonatal period, which connects the fetal and infantile periods. CONCLUSION: The results revealed that neonates reach the infantile period slower than usually imagined, and the end of the neonatal period may be at the age when the body height is around 55 cm— in other words, at 2 months after birth. This manuscript might be the first one to consider the definition of the neonatal period using cardiovascular methods. [ABSTRACT FROM AUTHOR]

Abstract

Creutzfeldt-Jakob disease (CJD) is a fatal degenerative neurological disease. Progressive dementia is its chief clinical sign, and its characteristic pathological feature is the formation of vacuoles within the degenerating brain. These vacuoles give the brain a 'spongy' appearance, and hence CJD and some similar diseases have been termed spongiform encephalopathies. Similar diseases include Kuru in man and scrapie in sheep. These diseases are often included among the so-called slow virus diseases, but the nomenclature is misleading, since the pathogenic agents responsible for the spongiform encephalopathies are unlike any known virus. In addition, Creutzfeldt-Jakob disease is unique among the spongiform encephalopathies. Unlike Kuru and scrapie, which have clearly infectious causes in nature, CJD occurs sporadically, and the majority of cases cannot be traced to any infectious transmission. Nevertheless, brain tissue from patients with CJD can be used to transmit a similar neurological disease to experimental animals. In most parts of the world, CJD affects primarily gray matter, that part of the brain where nerve cells and their interconnections predominate. In Japan, however, roughly 38 percent of cases affect the white matter of the brain as well; white matter consists of regions of the brain where long fibers insulated by their white myelin sheaths predominate. Since this form of CJD affects most areas of the brain, it has been termed 'panencephalopathic'. Japanese researchers have now shown that it is possible to isolate a pathogenic agent from panencephalopathic CJD and use it to transmit disease to laboratory animals. However, the disease caused in guinea pigs and hamsters affects specific areas of gray matter, and is not of the panencephalopathic type. There may be an interaction between the genetic characteristics of the pathogenic agent and its human host, and the panencephalopathic disease seen so commonly in Japan is likely to result from the influence of an unknown human gene or genes that occur more frequently in Japanese populations. Abstract

The aim of this longitudinal study was to examine the effects of 1-yr of volleyball practice on the bone mass development in the growing skeleton among prepubescent children. Twenty volleyball players and nine teen matched control boys (Tanner stage 1, at the start of the study) were followed over a 1-yr period. Bone mineral density (BMD, g/cm2), bone mineral content (BMC, g) were measured by dual-energy X-ray absorptiometry on the whole body, lumbar spine (L2-L4), legs, arms, femoral necks, hips and radii.
