Abstract

Le lipome du corps calleux est une lésion congénitale bénigne très rare, qui peut être isolée ou associée à des degrés divers de dysgénésie du corps calleux. Il peut être asymptomatique ou se révélé par des signes non spécifiques tels que des crises épileptiques, des céphalées, de déficit neurologique ou de démences. L´imagerie en coupe fait le diagnostic aisément. Nous rapportons le cas d´un adolescent de 18 ans qui a présenté une crise convulsive chez qui l´imagerie a révélé un lipome du corps calleux.

Abstract

Objectif : Le but de l’étude était de déterminer les caractéristiques non exhaustives des diverses séquelles et complications de la tuberculose thoracique au CHU Hassan II de Fès. Méthodologie : Il s’est agi d’une étude transversale descriptive à collecte rétrospective portant sur 40 observations de patients ayant présenté une tuberculose à localisation thoracique, colligées de janvier 2017 à décembre 2018 au CHU Hassan II de Fès. Tous les patients ont bénéficié d’une radiographie du thorax de face et d’un scanner thoracique. Résultats : L'âge moyen des patients était de 55 ans avec un sex-ratio H/F de 2,33. Les signes fonctionnels étaient dominés par la toux (95% des patients), la dyspnée (80%) et l’hémoptysie (62,5%). Les séquelles broncho-pulmonaires étaient prédominantes avec les cavités résiduelles (20% des patients), les destructions parenchymateuses (17,5%) et les bronchectasies (15%). Conclusion : Reconnaitre les manifestations radiologiques des séquelles et complications de tuberculose est essentiel pour tout radiologue et pneumologue dans l’optique de faciliter le diagnostic. La TDM reste l’examen clé pour asseoir ce diagnostic et assurer le suivi évolutif des patients. English title & abstract Imaging of sequelae and complications of thoracic tuberculosis about 40 observations at HASSAN II Teaching Hospital of Fez (Morroco) Objective : The aim of the study was to determine the non-exhaustive characteristics of the various sequelae and complications of thoracic tuberculosis at CHU Hassan II in Fez. Methodology: This was a descriptive cross-sectional study with retrospective collection of 40 observations of patients who presented thoracic tuberculosis localization, collected from January 2017 to December 2018 at the Hassan II University Hospital in Fez. All patients underwent a frontal chest X-ray and chest CT scan. Results: The mean age of the patients was 55 years with an M / F sex ratio of 2.33. The functional signs were dominated by cough (95% of patients), dyspnea (80%) and hemoptysis (62.5%). Broncho-pulmonary sequelae were predominant, with residual cavities (20% of patients), parenchymal destruction (17.5%) and bronchiectasis (15%). Conclusion: Recognizing the radiological manifestations of the sequelae and complications of tuberculosis is essential for any radiologist and pulmonologist in order to facilitate the diagnosis. Abstract

Thymoma is a rare tumor arising from the epithelium of the thymus gland and is usually located in the anterior mediastinum. Ectopic thymoma is very rare and can be located in the neck, trachea, lung, and pleura. Through this manuscript, we report a rare case of a 50-year-old woman presenting with dyspnea and recurrent pleural effusion. Her computed tomography of the thorax showed a unilateral left pleural mass. Biopsy of the pleural mass confirmed the diagnosis of pleural thymoma and she was started on chemotherapy. To our knowledge, this is the first reported pleural thymoma in our region.

Abstract

Coeliac artery compression stenosis caused by the median arcuate ligament can lead to aneurysm formation in the pancreatico-duodenal arteries that can eventually result in a spontaneous rupture leading to retroperitoneal haemorrhage. In this case series, we describe the cases of three patients, all presenting with sudden epigastric pain, diagnosed as spontaneous haematoma, complicating a median arcuate ligament syndrome.

Abstract

Aim of the study To study clinical, laboratory and imaging features correlated with complete response (CR) to transarterial chemoembolization (TACE) in patients with unresectable hepatocellular carcinoma (HCC) through 162 patients collected in Hassan II University Hospital of Fez. Material and methods From January 2015 to December 2019, 162 patients diagnosed with 225 HCC were treated by TACE. Among them, 14 showed CR during the follow-up. Imaging response was evaluated using the modified Response Evaluation Criteria in Solid Tumors (mRECIST). A multivariate analysis was performed including demographic parameters, etiology, α-fetoprotein (AFP) rates, hepatic function scores, imaging and TACE features. In cases with complete response and remission, follow-up duration was considered from the first to the last imaging control showing no viable tumor and eventually nodule retraction. Results Among the 162 patients with 225 nodules, 14 (9%) of them showed remission and 148 (91%) did not. There was no significant difference between the two groups in age, performance status (PS), AFP, nodularity, size nodule or number of TACE cures. Sex, etiology, Child-Pugh and MELD scores, location, BCLC stage and blush extinction were all found to have a significant impact on therapeutic response. Conclusions This study demonstrates that CR of HCC treated by TACE is strongly correlated with male sex, etiology (viral hepatitis C), location (segments VI and VII) and complete blush extinction on digital subtraction angiography (DSA). No significant correlation was found, particularly that of tumor size and segment IV (as a pejorative location).
