Tutkimuksessa arvioitiin yhteistoimintaa kuntien lapsiperheiden palveluissa päihdehoitoyksiköiden työntekijöiden näkökulmasta. Poikkileikkausaineisto kerättiin postikyselyllä päihdehuollon toimipaikoista koko maassa Ahvenanmaata lukuun ottamatta. Tutkimusaineisto muodostui 132 vastauksesta. Tutkimuksen tulokset osoittavat, että yhteisistä toimintatavoista oli sovittu useimmiten lastensuojelun ja muiden päihdepalvelujen sekä harvemmin mielenterveyspalvelujen kanssa. Yhteistoimintaa tukevia rakenteita oli useimmiten muiden päihdepalvelujen ja lastensuojelun kanssa sekä vähiten muiden erityis- ja peruspalvelujen kanssa. Yhteistyön arvioitiin toimivan parhaiten sosiaalipalvelujen ja opetuksen kanssa ja heikoimmin terveyspalvelujen kanssa. Päihdepalveluissa tunnettiin melko hyvin lapsiperheille tarjolla olevat palvelut ja erilaiset tukimuodot. Sovitut toimintatavat, yhteistoimintaa tukevat rakenteet sekä hyvä yhteistyö ja tietoisuus palveluista mahdollistavat yhteistoiminnan lapsiperheille palveluja tuottavien eri tahojen välillä kunnissa.

Vanhempien osallisuuden vahvistaminen sekä oikeus tulla kuulluksi ovat keskeisiä lasten palveluja ohjaavia periaatteita. Osallisuuden toteutumisessa on kuitenkin edelleen puutteita. Tämän tutkimuksen tarkoituksena oli selvittää päihdepalvelujen esimiesten käsityksiä siitä, miten päihdepalvelujen piirissä olevien vanhempien osallisuutta tuetaan. Osallisuuden tukemista arvioitiin työntekijöille modifiodulla Family Empowerment Scale -mittarilla (FES). Aineisto kerättiin päihdepalveluissa toimivilta esimiehiltä (n=372). Vastausprosentti oli 36. Taustamuuttujien perusteella määräytyvien vastaajaryhmien väliset erot analysoitiin riippumattomien otosten t-testillä sekä yksisuuntaisella varianssianalyysillä. Vanhempien osallisuuden tukeminen toteutui kohtuullisesti kaikissa toimipisteissä. Merkittävimmät kehittämiskohteet liittyivät perheiden riittämättömään tiedonsaantiin, heikkoihin palautteen antomahdollisuuksin, perheen ja henkilöstön välisen yhteistyön sekä vanhempien yhteiskunnallisten vaikutusmahdollisuuksien lisäämiseen. Vastaajien mukaan toimipisteen luonne oli yhteydessä osallisuuden tukemiseen päihdepalveluissa.

Rakenneyhtälömallien (SEM) avulla voidaan tutkia monimutkaisia, useiden eri tekijöiden välisiä yhteyksiä. Menetelmä sopii siten hyvin palvelujärjestelmää tarkastelevaan tutkimukseen, jossa tutkittavat yhteydet eivät ole yksinkertaisia. SEM -analyysit mahdollistavat kompleksisten avun antamista kuvaavien tekijöiden välisten suhteiden ja niiden muodostamien rakenteiden tarkastelun. Tässä artikkelissa kuvataan rakenneyhtälömallin muodostaminen konkreettisen tutkimusesimerkin avulla. Tutkimuksessa kehitettiin ja arvioitiin lineaarinen rakenneyhtälömalli, joka kuvaa palvelujen riittävyyttä ja niiden tuntemista, avun antamista sekä yhteistyön toimivuutta eri tahojen kanssa lasten ja perheiden parissa toimivien työntekijöiden näkökulmasta. Lisäksi artikkelissa esitellään SEM mallin kehittämisen eteneminen ja estimoinnissa käytetyt menetelmät ja kriteerit sekä arvioidaan SEM -mallin luotettavuutta.

Isän ja leikki-ikäisen lapsen yhdessäolo Väitöstyössä selvitettiin suomalaisen isän arkielämää: yhdessäoloa lapsen kanssa, tyytyväisyyttä parisuhteessa, vanhemmuuteen liittyvää stressiä sekä pienten lasten isien alkoholin käyttöä. Isän ja leikki-ikäisen lapsen yhdessäolossa painottui fyysinen yhdessä tekeminen. Suurimmalle osasta isiä yhdessäolo oli luonnollinen ja tärkeä osa arkielämää. Noin neljännes isistä olisi toivonut voivansa olla nykyistä enemmän lapsensa kanssa. Elämäntilanteiden muuttuminen, esimerkiksi avioero tai uusperheen muodostaminen vaikuttivat myös isän ja lapsen yhdessäoloon. Leikki-ikäisen lapsen isät kokivat melko yleisesti vanhempana toimimiseen liittyvää stressiä; huomattava osa isistä koki puutteita omissa vanhemmuuden taidoissaan ja kyvyissään tulkita lapsen tunteita ja tarpeita. Lisäksi stressiä lisäsivät lapsen vaativuus ja itkuinen mieliala. Isillä esiintyi myös runsaasti parisuhteeseen liittyviä ongelmia. Etenkin seksuaalisuuteen ja tunteiden ilmaisuun oltiin usein tyytymättömiä. Hätkähdyttävänä voidaan myös pitää tulosta, jonka mukaan yli kolmannes leikki-ikäisten lasten isistä voitiin luokitella alkoholin riskikuluttajiksi. Alkoholin käyttö lisäsi tyytymättömyyttä parisuhteessa, isänä toimimiseen liittyvää stressiä sekä vaikutti negatiivisesti isän ja lapsen yhdessäoloon ja sen määrään. Alkoholin käyttö, isänä toimimiseen liittyvä stressi ja ongelmat parisuhteessa liittyivät kiinteästi toisiinsa ja olivat yhteydessä isän vaikeuteen toimia sitoutuneena ja vastuullisena vanhempana. Tulokset pohjautuvat 1082 kolmesta kuuteen vuotiaan lapsen isältä kerättyyn aineistoon. Tutkimus tuotti sekä kansallisesti että kansainvälisesti uutta tietoa isän ja lapsen yhdessäolosta ja siihen yhteydessä olevista tekijöistä. Tätä tietoa voidaan käyttää apuna kehitettäessä isyyden tukemisen käytäntöjä neuvoloissa, päivähoidossa, varhaiskasvatuksessa, koulussa sekä muualla sosiaali- ja terveydenhuollossa. Time spent together by fathers and small children: Developing and evaluating a structural equation model of stress pertaining to being a father and satisfaction with the pair relationship The aim of the study was to develop and test a structural equation model describing time spent together by fathers and small children, of stress pertaining to being a father and father s satisfaction with the pair relationship. For this purpose an instrument was developed to evaluate time spent together by fathers and small children. The study further elicited the stress of parenthood experienced by fathers and satisfaction with the pair relationship, and moreover evaluated the relation between these and time spent together. The research was carried out in two phases. In the first phase an instrument (Father-Child Instrument, here abbreviated to FCI) of time spent together by fathers and small children was developed and piloted and a more precise assessment was made of the amount of time during which the father was present, activities shared by father and child, father s attitude to his child and being together, potential time spent together and conflict situations in time spent together. In the first phase 261 fathers of small children participated. In the second phase of the study an evaluation was conducted on stress pertaining to parenthood and being a father, father s satisfaction with the pair relationship and a connection between background variables and time spent together by fathers and small children. Data collection was by postal structured questionnaire, fathers from all over Finland being selected by random sampling from information held in the population database. In the second phase of the research 821 fathers of children aged 3 to 6 participated. The functionality of the model was tested on the empirical data formed from various background variables, the socio-economic position of the families, fathers alcohol consumption, stress pertaining to being a father and the relation between fathers satisfaction with their pair relationships and time spent together with the small children. Stress pertaining to being a father was assessed using the items from the Parenting Stress Index (PSI), satisfaction with the pair relationship using the items from the Marital Satisfaction Inventory (MSI)-R) and alcohol consumption using the AUDIT measure. LISREL modelling program was used for modeling structural relations. The findings showed that time spent together by fathers and small children is a complex phenomenon affected by numerous factors. The emphasis was on physical activity together. For most of the fathers being together was a natural part of everyday life. About one fourth of fathers wished that they could spend more time with their children. Changes in the life situation, for example, divorce or forming a new pair relationship also affected the time spent together by fathers and small children. The fathers of the small children felt fairly generally that there was stress related to being a parent: a considerable share of them felt that there were shortcomings in their parenting skills and abilities to interpret the children s feelings and needs. Moreover, this stress was increased by the demanding or fretful mood of the child. The fathers also reported many problems related to the pair relationship. Dissatisfaction with sexuality and the expression of feelings was frequently a cause of dissatisfaction. It can be considered surprising that over a third of the fathers of small children could be classified as risky consumers of alcohol. Alcohol consumption increased dissatisfaction in the pair relationship, stress pertaining to being a father and had a negative effect on time spent together and its duration. Alcohol consumption, stress about being a father and problems in the pair relationship had a combined negative effect on time spent together and its duration. The study generated new knowledge about time spent together by fathers and small children and factors related to them. On the basis of the findings measures supporting parenting can be targeted where they are most needed; present practices supporting fatherhood can be further developed to better meet the needs of families with small children. Development and testing of the FCI-instrument and model on longitudinal data give indication for further research.

Background: Coronary artery bypass grafting patients often suffer from persistent fears, anxiety and depression after surgery, but no studies are available to show how these psychological symptoms are linked to each other and affect the perceived social support. Objectives: We developed and estimated a hypothetical model to assess the relations of anxiety, fears and depression on perceived social support and the quality of patient education among coronary artery bypass grafting patients. Design and methods: The sample consisted of 171 coronary artery bypass grafting patients three months after surgery. The State Trait Anxiety Inventory, the Hospital Anxiety and Depression scale and the Zung Self-Rating Depression Scale were used to assess anxiety and depression. Fear was measured using the Bypass Grafting Fear Scale. Support received from nurses and the quality of patient education was measured using the Social Support from Nursing Staff scale and the Quality of Basic Cardiac Information scale. The proposed model was assessed using structural equation modelling. Results: The fears, anxiety and depressiveness were associated with patients’ feelings about perceived support from the nurses and the quality of patient education. Patients’ fear and anxiety increased depressiveness, leading to situations in which it is more difficult for a patient to receive emotional, informational and tangible support and patient education in general. Conclusions: Psychological symptoms of CABG patients undermine the effects of patient education and social support from nurses.
