Studies indicate that commencing an opioid during a hospital admission increases the likelihood of long-term use. This study aimed to identify the prevalence of opioid persistence amongst opioid-naïve patients following surgery as well as the indication for use.Methods: A retrospective review of patients who underwent a surgical procedure at the Royal Hobart Hospital, Tasmania, Australia, between August and September 2016 was undertaken. Patients were linked to the Tasmanian real-time prescription monitoring database to ascertain if they were subsequently dispensed a Schedule 8 opioid (morphine, codeine oxycodone, buprenorphine, hydromorphone, fentanyl, methadone, or tapentadol) and the indication for use.Results: Of the 3275 hospital admissions, 1015 opioid-naïve patients were eligible for inclusion. Schedule 8 opioids were dispensed at or within 2 days of discharge in 41.7% of admissions. Thirty-nine (3.9%) patients received prescribed opioids 2-months post-discharge; 1.8% of the patients were approved by State Health to be prescribed Schedule 8 opioids regularly for a chronic condition at 6 months, and 1.3% received infrequent or one-off prescriptions for Schedule 8 opioids at 6 months. Thirteen (1.3%) patients continued Schedule 8 opioids for at least 6 months following their surgery, with the indication for treatment either related to the surgery or the condition which surgery was sought for.Conclusion: This study found that there was a low rate of Schedule 8 opioid persistence following surgery, indicating post-surgical pain is not a significant driver for persistent opioid use.Keywords: opioids, post-operative, persistence, prescribing

22. Talk of Family: How Institutional Overlap Shapes Family-Related Discourse across Social Class

- Abstract
We develop a novel application of machine learning and apply it to the interview transcripts from the American Voices Project (N = 1,396), using discourse atom topic modeling to explore social class variation in the centrality of family in adults' lives. We take a two-phase approach, first analyzing transcripts at the person level and then at the line level. Our findings suggest that family, as represented by talk, is more central in the lives of those without a college degree than among the college educated. However, the degree of institutional overlap between family and other key institutions -- health, work, religion, and criminal justice -- does not vary by education. We interpret these findings in the context of debates about the deinstitutionalization of family in the contemporary United States. This demonstrates the value of a new method for analyzing qualitative interview data at scale. We address ways to expand the use of this method to shed light on educational disparities.

Automated segmentation of Cardiac Magnetic Resonance (CMR) plays a pivotal role in efficiently assessing cardiac function, offering rapid clinical evaluations that benefit both healthcare practitioners and patients. While recent research has primarily focused on delineating structures in the short-axis orientation, less attention has been given to long-axis representations, mainly due to the complex nature of structures in this orientation. Performing pixel-wise segmentation of the left ventricular (LV) myocardium and the four cardiac chambers in 2-D steady-state free precession (SSFP) cine sequences is a crucial preprocessing stage for various analyses. However, the challenge lies in the significant variability in contrast, appearance, orientation, and positioning of the heart across different patients, clinical views, scanners, and imaging protocols. Consequently, achieving fully automatic semantic segmentation in this context is notoriously challenging. In recent years, several deep learning models have been proposed to accurately quantify and diagnose cardiac pathologies. These automated tools heavily rely on the accurate segmentation of cardiac structures in magnetic resonance images (MRI). Hence, there is a need for new methods to handle such structures' geometrical and textural complexities. We proposed 2D and 3D two-stage self-supervised deep learning segmentation hybrid transformer and CNN-based architectures for 4CH whole heart segmentation. Accurate segmentation of the ventricles and atria in 4CH views is crucial for analyzing heart health and reconstructing four-chamber meshes, which are essential for estimating various parameters to assess overall heart condition. BackgroundHealth professionals have expressed unmet needs, including lacking the skills, confidence, training, and resources needed to properly attend to the psychological needs of people with diabetes. ObjectiveInformed by needs assessments, this study aimed to develop practical, evidence-based resources to support health professionals to address the emotional needs of adults with type 1 or type 2 diabetes. MethodsWe developed a new handbook and toolkit informed by formative evaluation, including literature reviews, stakeholder consultation and review, and a qualitative study. In the qualitative study, health professionals participated in interviews after reading sections of the handbook and toolkit. ResultsThe literature review uncovered that psychological problems are common among adults with diabetes, but health professionals lack resources to provide related support. We planned and drafted resources to fill this unmet need, guided by stakeholder consultation and an Expert Reference Group (ERG). Before finalizing the resources, we implemented feedback received from stakeholders (ERG, health professionals, academics, and people with diabetes). The resulting resources were the practical, evidence-based Diabetes and Emotional Health handbook and toolkit. A total of 19 health professionals took part in the qualitative study about the handbook and toolkit. They viewed the resources favorably, felt empowered to support people with diabetes experiencing psychological problems, and felt motivated to share the resources with others. Some gave examples of how they had used the handbook in clinical practice. A perceived highlight was the inclusion of a process model outlining 7 steps for identifying and supporting people with emotional problems: the 7 A’s model. With funding from the National Diabetes Services Scheme (NDSS), more than 2400 copies of Diabetes and Emotional Health have been distributed. It is freely available on the Web. The NDSS is an initiative of the Australian Government administered with the assistance of Diabetes Australia. ConclusionsThe new evidence-based resources are perceived by stakeholders as effective aids to assist health professionals in providing emotional support to adults with diabetes. The 7 A’s model may have clinical utility for routine monitoring of other psychological and health-related problems, as part of person-centered clinical care.

We present new exclusion bounds obtained at the European X-ray Free Electron Laser facility (EuXFEL) on axion-like particles (ALPs) in the mass range 10^{-3} eV < m_a < 10^{4} eV. Our experiment exploits the Primakoff effect via which photons can, in the presence of a strong external electric field, decay into axions, which then convert back into photons after passing through an opaque wall. While similar searches have been performed previously at a 3^rd generation synchrotron [1], our work demonstrates improved sensitivity, exploiting the higher brightness of X-rays at EuXFEL., Comment: 7 pages, 6 figures

We present a detailed characterization of the structure and evolution of differentially rotating plasmas driven on the MAGPIE pulsed-power generator (1.4 MA peak current, 240 ns rise-time). The experiments were designed to simulate physics relevant to accretion discs and jets on laboratory scales. A cylindrical aluminium wire array Z pinch enclosed by return posts with an overall azimuthal off-set angle was driven to produce ablation plasma flows that propagate inwards in a slightly off-radial trajectory, injecting mass, angular momentum, and confining ram pressure to a rotating plasma column on the axis. However, the plasma is free to expand axially, forming a collimated, differentially rotating axial jet that propagates at $\approx 100$ km/s. The density profile of the jet corresponds to a dense shell surrounding a low-density core, which is consistent with the centrifugal barrier effect being sustained along the jet's propagation. We show analytically that, as the rotating plasma accretes mass, conservation of mass and momentum implies plasma radial growth scaling as $r \propto t^{1/3}$. As the characteristic moment of inertia increases, the rotation velocity is predicted to decrease and settle on a characteristic value $\approx 20$ km/s. We report on a measurement of astrophysical tau neutrinos with 9.7 years of IceCube data. Using convolutional neural networks trained on images derived from simulated events, seven candidate $\nu_\tau$ events were found with visible energies ranging from roughly 20 TeV to 1 PeV and a median expected parent $\nu_\tau$ energy of about 200 TeV. Considering backgrounds from astrophysical and atmospheric neutrinos, and muons from $\pi^\pm/K^\pm$ decays in atmospheric air showers, we obtain a total estimated background of about 0.5 events, dominated by non-$\nu_\tau$ astrophysical neutrinos. Thus, we rule out the absence of astrophysical $\nu_\tau$ at the $5\sigma$ level. The measured astrophysical $\nu_\tau$ flux is consistent with expectations based on previously published IceCube astrophysical neutrino flux measurements and neutrino oscillations., Comment: Accepted for publication in Physical Review Letters. The IceCube Neutrino Observatory relies on an array of photomultiplier tubes to detect Cherenkov light produced by charged particles in the South Pole ice. IceCube data analyses depend on an in-depth characterization of the glacial ice, and on novel approaches in event reconstruction that utilize fast approximations of photoelectron yields. Here, a more accurate model is derived for event reconstruction that better captures our current knowledge of ice optical properties. When evaluated on a Monte Carlo simulation set, the median angular resolution for in-ice particle showers improves by over a factor of three compared to a reconstruction based on a simplified model of the ice. The most substantial improvement is obtained when including effects of birefringence due to the polycrystalline structure of the ice. A measurement of the diffuse astrophysical neutrino spectrum is presented using IceCube data collected from 2011-2022 (10.3 years). We developed novel detection techniques to search for events with a contained vertex and exiting track induced by muon neutrinos undergoing a charged-current interaction. Searching for these starting track events allows us to not only more effectively reject atmospheric muons but also atmospheric neutrino backgrounds in the southern sky, opening a new window to the sub-100 TeV astrophysical neutrino sky. The event selection is constructed using a dynamic starting track veto and machine learning algorithms. We use this data to measure the astrophysical diffuse flux as a single power law flux (SPL) with a best-fit spectral index of $\gamma = 2.58 ^{+0.10}_{-0.09}$ and per-flavor normalization of $\phi^{\mathrm{Astro}}_{\mathrm{per-flavor}} = 1.68 ^{+0.19}_{-0.22} \times 10^{-18} \times \mathrm{GeV}^{-1} \mathrm{cm}^{-2} \mathrm{s}^{-1} \mathrm{sr}^{-1}$ (at 100 TeV). The sensitive energy range for this dataset is 3 - 550 TeV under the SPL assumption. This data was also used to measure the flux under a broken power law, however we did not find any evidence of a low energy cutoff., Comment: 27 pages, 28 figures

Frontotemporal dementia (FTD) is the second most common cause of early-onset dementia after Alzheimer disease (AD). Efforts in the field mainly focus on familial forms of disease (fFTDs), while studies of the genetic etiology of sporadic FTD (sFTD) have been less common. In the current work, we analyzed 4,685 sFTD cases and 15,308 controls looking for common genetic determinants for sFTD. We found a cluster of variants at the MAPT (rs199443; p = 2.5 × 10-12, OR = 1.27) and APOE (rs6857; p = 1.31 × 10-12, OR = 1.27) loci and a candidate locus on chromosome 3 (rs1009966; p = 2.41 × 10-8, OR = 1.16) in the intergenic region between RPSA and MOBP, contributing to increased risk for sFTD through effects on expression and/or splicing in brain cortex of functionally relevant in-cis genes at the MAPT and RPSA-MOBP loci. The association with the MAPT (H1c clade) and RPSA-MOBP loci may suggest common genetic pleiotropy across FTD and progressive supranuclear palsy (PSP) (MAPT and RPSA-MOBP loci) and across FTD, AD, Parkinson disease (PD), and cortico-basal degeneration (CBD) (MAPT locus). Our data also suggest population specificity of the risk signals, with MAPT and APOE loci associations mainly driven by Central/Nordic and Mediterranean Europeans, respectively. This study lays the foundations for future work aimed at further characterizing population-specific features of potential FTD-discriminant APOE haplotype(s) and the functional involvement and contribution of the MAPT H1c haplotype and RPSA-MOBP loci to pathogenesis of sporadic forms of FTD in brain cortex.

41. The Relationship between Traditional and Cyber Bullying Victimization in Early Adolescence and Emotional Wellbeing: A Cross-Sectional, Population-Based Study

- Abstract
Despite the wealth of knowledge about the impact of bullying victimization, information gaps exist about how traditional and cyber bullying in early adolescence is associated with emotional wellbeing, namely, indicators of positive wellbeing. Therefore, this study investigated associations between different types of bullying victimization and positive and negative emotional wellbeing indicators, in addition to examining the prevalence of different types of bullying in conjunction with child, peer, and school factors. The study used data from an annual survey of student wellbeing conducted in South Australian schools. The sample comprised of 9019 grade 6 students aged 10-13 years (49.6% female). One-third of students experienced bullying victimization. Verbal bullying was most prevalent in this sample (24%), followed by social (21%), physical (10%), and cyberbullying (7%). Males were significantly more likely to experience physical and verbal bullying, and students living in more socioeconomically disadvantaged communities were significantly more likely to experience physical, verbal, social, and cyber bullying victimization. Additionally, all types of bullying victimization were significantly associated with lower scores on positive emotional wellbeing indicators (happiness, life satisfaction, and emotion regulation) and significantly higher scores on negative indicators (sadness and worries), all with small effect sizes after accounting for child-, peer-, and school-level factors. This research suggests that bullying is associated with both positive and negative aspects of emotional wellbeing, and both aspects of wellbeing are crucial to consider when developing school-based interventions.

Name that Neutrino is a citizen science project where volunteers aid in classification of events for the IceCube Neutrino Observatory, an immense particle detector at the geographic South Pole. From March 2023 to September 2023, volunteers did classifications of videos produced from simulated data of both neutrino signal and background interactions. Name that Neutrino obtained more than 128,000 classifications by over 1,800 registered volunteers that were compared to results obtained by a deep neural network machine-learning algorithm. Possible improvements for both Name that Neutrino and the deep neural network are discussed.

We present the results of a search for 10--1,000 GeV neutrinos from 2,268 gamma-ray bursts over 8 years of IceCube-DeepCore data. This work probes burst physics below the photosphere where electromagnetic radiation cannot escape. Neutrinos of tens of GeVs are predicted in sub-photospheric collision of free streaming neutrons with bulk-jet protons. In a first analysis, we searched for the most significant neutrino-GRB coincidence using six overlapping time windows centered on the prompt phase of each GRB. In a second analysis, we conducted a search for a group of GRBs, each individually too weak to be detectable, but potentially significant when combined. No evidence of neutrino emission is found for either analysis. The most significant neutrino coincidence is for Fermi-GBM GRB bn 140807500, with a p-value of 0.097 corrected for all trials. The binomial test used to search for a group of GRBs had a p-value of 0.65 after all trial corrections. The binomial test found a group consisting only of GRB bn 140807500 and no additional GRBs. The neutrino limits of this work complement those obtained by IceCube at TeV to PeV energies. We compare our findings for the large set of GRBs as well as GRB 221009A to the sub-photospheric neutron-proton collision model and find that GRB 221009A provides the most constraining limit on baryon loading. For a jet Lorentz factor of 300 (800), the baryon loading on GRB 221009A is lower than 3.85 (2.13) at a 90% confidence level.
