Objective\ud To develop a core outcome set for pre‐eclampsia.\ud \ud Design\ud Consensus development study.\ud \ud Setting\ud International.\ud \ud Population\ud Two hundred and eight‐one healthcare professionals, 41 researchers and 110 patients, representing 56 countries, participated.\ud \ud Methods\ud Modified Delphi method and Modified Nominal Group Technique.\ud \ud Results\ud A long‐list of 116 potential core outcomes was developed by combining the outcomes reported in 79 pre‐eclampsia trials with those derived from thematic analysis of 30 in‐depth interviews of women with lived experience of pre‐eclampsia. Forty‐seven consensus outcomes were identified from the Delphi process following which 14 maternal and eight offspring core outcomes were agreed at the consensus development meeting. Maternal core outcomes: death, eclampsia, stroke, cortical blindness, retinal detachment, pulmonary oedema, acute kidney injury, liver haematoma or rupture, abruption, postpartum haemorrhage, raised liver enzymes, low platelets, admission to intensive care required, and intubation and ventilation. Offspring core outcomes: stillbirth, gestational age at delivery, birthweight, small‐for‐gestational‐age, neonatal mortality, seizures, admission to neonatal unit required and respiratory support.\ud \ud Conclusions\ud The core outcome set for pre‐eclampsia should underpin future randomised trials and systematic reviews. Such implementation should ensure that future research holds the necessary reach and relevance to inform clinical practice, enhance women's care and improve the outcomes of pregnant women and their babies.

Protein-tyrosine phosphatase receptor type G (RPTPγ/PTPRG) interacts in vitro with contactin-3-6 (CNTN3-6), a group of glycophosphatidylinositol-anchored cell adhesion molecules involved in the wiring of the nervous system. In addition to PTPRG, CNTNs associate with multiple transmembrane proteins and signal inside the cell via cis-binding partners to alleviate the absence of an intracellular region. Here, we use comprehensive biochemical and structural analyses to demonstrate that PTPRG·CNTN3-6 complexes share similar binding affinities and a conserved arrangement. Furthermore, as a first step to identifying PTPRG·CNTN complexes in vivo, we found that PTPRG and CNTN3 associate in the outer segments of mouse rod photoreceptor cells. In particular, PTPRG and CNTN3 form cis-complexes at the surface of photoreceptors yet interact in trans when expressed on the surfaces of apposing cells. Further structural analyses suggest that all CNTN ectodomains adopt a bent conformation and might lie parallel to the cell surface to accommodate these cis and trans binding modes. Taken together, these studies identify a PTPRG·CNTN complex in vivo and provide novel insights into PTPRG- and CNTN-mediated signaling.

Single-wavelength anomalous dispersion of S atoms (S-SAD) is an elegant phasing method to determine crystal structures that does not require heavy-atom incorporation or selenomethionine derivatization. Nevertheless, this technique has been limited by the paucity of the signal at the usual X-ray wavelengths, requiring very accurate measurement of the anomalous differences. Here, the data collection and structure solution of the N-terminal domain of the ectodomain of HCV E1 from crystals that diffracted very weakly is reported. By combining the data from 32 crystals, it was possible to solve the sulfur substructure and calculate initial maps at 7 Å resolution, and after density modication and phase extension using a higher resolution native data set to 3.5 Å resolution model building was achievable.

Macromolecular crystallography (MX) is the most powerful technique available to structural biologists to visualize in atomic detail the macromolecular machinery of the cell. Since the emergence of structural genomics initiatives, significant advances have been made in all key steps of the structure determination process. In particular, third-generation synchrotron sources and the application of highly automated approaches to data acquisition and analysis at these facilities have been the major factors in the rate of increase of macromolecular structures determined annually. A plethora of tools are now available to users of synchrotron beamlines to enable rapid and efficient evaluation of samples, collection of the best data, and in favorable cases structure solution in near real time. Here, we provide a short overview of the emerging use of collecting X-ray diffraction data directly from the crystallization experiment. These in situ experiments are now routinely available to users at a number of synchrotron MX beamlines. A practical guide to the use of the method on the MX suite of beamlines at Diamond Light Source is given.

Headaches are a common problem in pregnancy and often receive scant attention from the clinician. This may be due to an incomplete understanding of the condition. Although sometimes innocuous, headaches are also associated with some very dangerous conditions that occur during pregnancy. Background: To describe the incidence and understanding of early postmenopausal symptoms in rural Xhosa women. Methods: After written informed consent, 137 women were interviewed within five years after natural menopause using the Greene Climacteric Scale. Body mass indices were calculated, and blood levels of FSH, LH and oestradiol were determined. Results: The mean age of menopause was 50 years amongst the 118 women who met the study criteria. Ninety-four (80%) patients experienced hot flushes, with 20% describing them as extreme. Sixty-nine percent of patients experienced nocturnal sweating. Most of the women (111; 94%) had never heard of hormonal therapy. Serum oestradiol levels rose with increasing body mass index, but did not achieve significance. Conclusions: These rural South African women experienced the menopause at a comparable age to Caucasian women. Although they had a high incidence of vasomotor symptoms, they were unaware of the potential benefits of hormonal therapy.Keywords: menopause, hot flushes, African, body mass indexSouth African Family Practice Vol. 47(5) 2005: 56-58

Regional lung function was studied with xenon-133 in 10 patients with bronchial carcinoma before and again three to six months after pneumonectomy. It was found that ventilation and perfusion were often considerably reduced in the tumour-bearing lung and that the greater part of the cardiac output had apparently been diverted to the unaffected lung before operation. The regional distribution of ventilation and perfusion in the lung remaining after surgery was essentially unchanged from that measured in the same lung preoperatively and no different from the pattern seen in 10 normal volunteers.
