1. "A sticking plaster over a burst artery" : an explanatory theory of moral distress : frontline workers' experience of supporting rough sleepers with a mental illness through austerity, welfare reform and the COVID-19 pandemic

Abstract

For a decade, prior to the COVID-19 pandemic, frontline homelessness workers in England have worked within national and local policies of welfare reform and austerity, within which there was a major cut to public spending. After the COVID-19 outbreak frontline workers began working within policies relating to the pandemic and homelessness. There is little empirical research on how these policies have impacted frontline workers who support rough sleepers with a mental illness as previous research focuses on people experiencing homelessness and/or mental illness during austerity and welfare reform, rather than the experience of the frontline homelessness worker. The purpose of this empirical research was to explore the experiences of homelessness frontline workers supporting rough sleepers with a mental illness post austerity, welfare reform and during the COVID-19 pandemic in the Midlands geographical area. Using a constructivist grounded theory approach, ten frontline workers, who worked within a variety of statutory and third sector organisations, took part in sixteen semi-structured interviews. The study offers an explanation of how working within welfare reform, austerity and COVID-19 has affected frontline workers who support rough sleepers with a mental illness. An explanatory theory of moral distress was co-constructed with the research participants. The frontline workers worked within disconnected systems across, housing, health, social care and the department of work and pensions, with the COVID-19 pandemic exacerbating this. They were frequently restricted in supporting their service users as they saw fit. This caused them to experience moral distress. The findings have significance going forward as due to the cost-of-living crisis, homelessness may increase, and planned cuts to public services will put additional pressure across housing, health, and social care services, which in turn will impact on homelessness organisations and frontline workers in the sector. If this does occur without any increase to funding to homelessness and mental health services, along with changes to policy and legislation, frontline workers will be under even higher risk of experiencing moral distress.

Abstract

This project explores principles of "meaning" and "the meaningful" as active and embodied "gestures" or "lived-realities", via concepts of "sharing" and communicative relationality. These concepts were ethnographically researched in the far northern, religiously-plural Indian state of Uttarakhand. This is a state with faith-based populations variously self-identifying as Hindu, Christian, Buddhist, Sikh, or Muslim. I met members of all of these faiths regularly, although my interlocutors were predominantly Christian and Hindu. Formerly a region of Uttar Pradesh, Uttarakhand is known for its close proximity to the Himalaya, the Indian borders with Tibet and Nepal, and for its religious sanctity as an area of devout (usually Hindu) pilgrimage. All of these diverse factors have led to the consistent and complex intermingling of a number of sub-communities and regional populations. This was evident within many of the larger towns, such as Mussoorie, found in the Garhwal district of Uttarakhand, and the central-base for my fieldwork. My methodology was ethnographic and qualitative, focusing on oral-verbal communications and person-to-person interactions. These processes defined my principal concepts within this project by demonstrating the lived-realities of religious identities, religious communities, and the prevalence of inter-faith communication (or "sharing") within this region. Most significantly, my research highlighted the ongoing importance of meanings - as well as the notable variance in what "the meaningful" was actually considered to be across and between faiths. Such diversity in understanding was made even more complex by different meanings being expressed in the same language, forms, narratives, or verbal images. Within these communications, metaphor was a creative, commonly used, and actively-engaged principle, evidenced within the practice of (different) faiths. This demonstrated a form of "relationality" or "inter-relationality" that seemed to reinforce contemporary, anthropological concepts of embodied lived-realities being a fundamental aspect of the relational, but not the same as "definitionality" or as "meaning". Yet, when in use in Uttarakhand, principles, concepts (and uses) of expressive form, representation, communication, "sharing", and "meaning", were frequently entangled . Many apparent "oral metaphors" and "descriptions" - including images of "sacrifice", "heart-speech", God, Gods, or the cosmological/transcendent - had extended-meanings and purposes in their Garhwali context. Such meanings and actions were in contrast to what I had previously understood metaphor "to be". These were not solely representational-metaphors, they were, instead, acts of agency (via expression). These observations led to my own evolution of the term "ontological-metaphor". The metaphorical was persistently utilised not only as a "symbolic- tool", but as an overlapping, entwined lived-form that was capable of encapsulating the relational and definitional-meaningful. This was not merely in a simultaneous representation, but as a co-creational state-of-being or "ontology".

Abstract

Contexte: La forme grave de COVID-19 semble affecter de maniere disproportionnee les gens immunovulnerables, meme si les donnees canadiennes dans ce contexte sont limitees. Nous avons voulu determiner quels facteurs sont associes aux parametres de la forme grave de COVID-19 chez les receveurs de transplantations au Canada. Methodes: Nous avons procede a une etude de cohorte multicentrique prospective regroupant tous les receveurs d'une transplantation d'organe plein ayant requ un diagnostic de COVID-19 suivis dans 9 programmes de transplantation au Canada entre mars 2020 et novembre 2021. Les donnees ont ete analysees afin de degager les facteurs de risque a l'egard du recours a l'oxygenotherapie et autres criteres de la gravite de la maladie. Nous avons compare les parametres selon le type d'organe transplante et suivi devolution des parametres au fil du temps. Nous avons procede a une analyse multivariee pour determiner quelles variables sont associees au recours a l'oxygenotherapie. Resultats: En tout, 509 patients ayant requ une transplantation d'organe plein ont contracte la COVID-19 durant la periode de l'etude. Les facteurs de risque associes au recours a l'oxygenotherapie (n = 190) ou non (n = 319) incluaient l'age (age median 62,6 ans, intervalle interquartile [II] 52,5-69,5 ans c. age median 55,5 ans, II 47,5-66,5; p < 0,001) et le nombre de comorbidites (nombre median 3, II 2-3 c. nombre median 2, II 1-3; p < 0,001), de meme que les parametres concernant l'immunosuppression. Les receveurs d'une transplantation pulmonaire (n = 48) etaient plus susceptibles de souffrir d'une forme grave de la maladie, avec un taux de mortalite eleve (n = 15, 31,3%) comparativement aux receveurs d'autres organes, y compris le rein (n = 48, 14,8%), le creur (n = 1, 4,4%), le foie (n = 9, 11,4%) et le reinpancreas (n = 3, 12,0%) (p = 0,02). Les facteurs protecteurs contre le recours a l'oxygenotherapie incluaient le fait d'avoir subi une transplantation hepatique et de recevoir de l'azathioprine. Le fait d'avoir requ 2 doses de vaccin anti-SRAS-CoV-2 n'a pas eu d'influence appreciable sur le recours a l'oxygenotherapie. L'analyse multivariee a montre que l'age avance (rapport des cotes [RC] 1,04, intervalle de confiance [IC] de 95% 1,02-1,07) et le nombre de comorbidites (RC 1,63, IC de 95% 1,30-2,04), entre autres facteurs, etaient associes au recours a l'oxygenotherapie. La gravite de la maladie n'a pas considerablement diminue au fil du temps. Interpretation: Malgre les progres therapeutiques et la vaccination des receveurs d'une transplantation d'organe plein, les signes de gravite accrue de la COVID-19, en particulier chez les receveurs d'une transplantation pulmonaire, justifient le maintien des mesures de sante publique pour proteger ces personnes a risque, et l'utilisation hative de traitements contre la COVID-19 chez les receveurs d'une transplantation d'organe plein., Les receveurs d'une transplantation d'organe plein doivent prendre des immunosuppresseurs la vie durant pour prevenir le rejet. Methods: We performed a multicentre, prospective cohort study of all recipients of solid organ transplants from 9 transplant programs in Canada who received a diagnosis of COVID-19 from March 2020 to November 2021. Data were analyzed to determine risk factors for oxygen requirement and other metrics of disease severity. We compared outcomes by organ transplant type and examined changes in outcomes over time. We performed a multivariable analysis to determine variables associated with need for supplemental oxygen. Results: A total of 509 patients with solid organ transplants had confirmed COVID-19 during the study period. Risk factors associated with needing (n = 190), compared with not needing (n = 319), supplemental oxygen included age (median 62.6 yr, interquartile range [IQR] 52.5-69.5 yr v. median 55.5 yr, IQR 47.5-66.5; p < 0.001) and number of comorbidities (median 3, IQR 2-3 v. median 2, IQR 1-3; p < 0.001), as well as parameters associated with immunosuppression. Recipients of lung transplants (n = 48) were more likely to have severe disease with a high mortality rate (n = 15, 31.3%) compared with recipients of other organ transplants, including kidney (n = 48, 14.8%), heart (n = 1, 4.4%), liver (n = 9, 11.4%) and kidney-pancreas (n = 3, 12.0%) transplants (p = 0.02). Protective factors against needing supplemental oxygen included having had a liver transplant and receiving azathioprine. Having had 2 doses of SARS-CoV-2 vaccine did not have an appreciable influence on oxygen requirement. Multivariable analysis showed that older age (odds ratio [OR] 1.04, 95% confidence interval [CI] 1.02-1.07) and number of comorbidities (OR 1.63, 95% CI 1.30-2.04), among other factors, were associated with the need for supplemental oxygen. Over time, disease severity did not decline significantly. Interpretation: Despite therapeutic advances and vaccination of recipients of solid organ transplants, evidence of increased severity of COVID-19, in particular among those with lung transplants, supports ongoing public health measures to protect these at-risk people, and early use of COVID-19 therapies for recipients of solid organ transplants., Recipients of solid organ transplants take life-long immunosuppressive agents to prevent rejection. 34. Computational Modeling of Deep Brain Stimulation for Parkinson Disease to Evaluate Levodopa and Dyskinesia Reduction

Abstract

Parkinson disease (PD) is a neurodegenerative disorder resulting from the death of dopamine releasing neurons. Levodopa is used to treat PD and has side effects such as dyskinesia, involuntary and uncontrollable movements. When levodopa-induced side effects diminish the effectiveness of treatment, another option for PD patients is deep brain stimulation (DBS). DBS is the implantation of electrodes into the brain to deliver electrical impulses to specific regions. For PD, the standard target is the subthalamic nucleus (STN). Volume of tissue activation (VTA) modeling can be used to quantify the amount of stimulation in and around the target area. STN DBS may reduce dyskinesia indirectly through reducing medication or directly through stimulation. The primary aim of this study was to determine the optimal stimulation locations for levodopa and dyskinesia reduction to see if the locations were distinct or overlapping. Patient-specific STN and VTA point cloud data from 40 PD patients who received bilateral STN DBS were created from Analyze and MATLAB software. Stimulated tissue activation in the dorsal-ventral, lateral-medial, and anterior-posterior subthalamic regions was calculated using STN and VTA point clouds. Levodopa equivalent daily dosage (LEDD) and dyskinesia both reduced significantly from pre- to post-operative appointments; however, LEDD and dyskinesia reduction did not correlate with one another. Overall, stimulation within the STN appears to explain LEDD reduction, whereas stimulation above the STN appears to explain dyskinesia reduction. Dyskinesia reduction after STN DBS may be due to stimulation of the pallidothalamic fibers dorsal to the STN (Kim et al., 2015), specifically the lenticular fasciculus, which originates from the globus pallidus internus (GPi). Abstract

An ongoing, severe outbreak of highly pathogenic avian influenza virus (HPAI) A H5N1 clade 2.3.4.4b has been circulating in wild and domestic bird populations throughout the world, reaching North America in 2021. This HPAI outbreak has exhibited unique characteristics when compared to previous outbreaks. The global distribution of disease, prolonged duration, extensive number of species and individual wild birds affected, and the large impact on the global poultry industry have all exceeded historical impacts of previous outbreaks in North America. In this study, we describe the results of HPAI surveillance conducted at The Raptor Center, a wildlife rehabilitation hospital at University of Minnesota (Saint Paul, MN, U.S.A.), from March 28th–December 31, 2022. All wild raptors admitted to the facility were tested for avian influenza viruses using polymerase chain reaction (PCR) testing. All non-negative samples were submitted to the United States Department of Agriculture (USDA) Animal and Plant Health Inspection Service (APHIS) National Veterinary Services Laboratories for confirmatory HPAI testing and genetic sequencing. During the study period, 996 individual birds representing 20 different species were tested for avian influenza, and 213 birds were confirmed HPAI positive. Highly pathogenic avian influenza surveillance conducted at The Raptor Center contributed 75% of the HPAI positive raptor detections within the state of Minnesota, located within the Mississippi flyway, significantly augmenting state wildlife surveillance efforts. The viral genotypes observed in birds sampled at The Raptor Center were representative of what was seen in wild bird surveillance within the Mississippi flyway during the same time frame. Wildlife rehabilitation centers provide an opportune situation to augment disease surveillance at the human, wildlife and domestic animal interface during ongoing infectious disease outbreaks.
