Abstract

Abstract Background Antibiotic-associated diarrhea is one of the most frequent side effects of antimicrobial therapy. We assessed the epidemiological data of antibiotic-associated diarrhea in pediatric patients in our region. Methods The prospective multi-center study included pediatric patients who were initiated an oral antibiotic course in outpatient clinics and followed in a well-established surveillance system. This follow-up system constituded inclusion of patient by the primary physician, supply of family follow-up charts to the family, passing the demographics and clinical information of patient to the Primary Investigator Centre, and a close telephone follow-up of patients for a period of eight weeks by the Primary Investigator Centre. Results A result of 758 cases were recruited in the analysis which had a frequency of 10.4% antibiotic-associated diarrhea. Among the cases treated with amoxicillin-clavulanate 10.4%, and cephalosporins 14.4% presented with antibiotic-associated diarrhea. In the analysis of antibiotic-associated diarrhea occurrence according to different geographical regions of Turkey, antibiotic-associated diarrhea episodes differed significantly (p = 0.014), particularly higher in The Eastern Anatolia and Southeastern Anatolia. Though most commonly encountered with cephalosporin use, antibiotic-associated diarrhea is not a frequent side effect. Conclusion This study on pediatric antibiotic-associated diarrhea displayed epidemiological data and the differences geographically in our region.

Abstract

The clinical manifestations of SARS-CoV-2 infection mainly involve the respiratory system. However, there is increasing evidence that this virus can affect other organs, causing a wide range of clinical symptoms. This is the report of a 40-day-old patient who presented with sepsis and had no risk factors other than SARS-CoV-2 infection, whose radiological findings were compatible with cerebral sinus vein thrombosis.

Abstract

Objectives: The aim of this study is to identify the epidemiological, clinical, and laboratory features of coronavirus disease 2019 (COVID-19) in children.Methods: A retrospective study was conducted by pediatric infectious disease specialists from 32 different hospitals from all over Turkey by case record forms. Pediatric cases who were diagnosed as COVID-19 between March 16, 2020, and June 15, 2020 were included. Case characteristics including age, sex, dates of disease onset and diagnosis, family, and contact information were recorded. Clinical data, including the duration and severity of symptoms, were also collected. Laboratory parameters like biochemical tests and complete blood count, chest X-ray, and chest computed tomography (CT) were determined.Results: There were 1,156 confirmed pediatric COVID-19 cases. In total, male cases constituted 50.3% (n = 582) and females constituted 49.7% (n = 574). The median age of the confirmed cases was 10.75 years (4.5–14.6). Of the total cases, 90 were younger than 1 year of age (7.8%), 108 were 1–3 years of age (9.3%), 148 were 3–6 years of age (12.8%), 298 were 6–12 years of age (25.8%), 233 were 12–15 years of age (20.2%), and 268 cases were older than 15 years of age (23.2%). The most common symptom of the patients at the first visit was fever (50.4%) (n = 583) for a median of 2 days (IQR: 1–3 days). Fever was median at 38.4°C (38.0–38.7°C). The second most common symptom was cough (n = 543, 46.9%). The other common symptoms were sore throat (n = 143, 12.4%), myalgia (n = 141, 12.2%), dyspnea (n = 118, 10.2%), diarrhea (n = 112, 9.7%), stomachache (n = 71, 6.1%), and nasal discharge (n = 63, 5.4%). When patients were classified according to disease severity, 263 (22.7%) patients were asymptomatic, 668 (57.7%) patients had mild disease, 209 (18.1%) had moderate disease, and 16 (1.5%) cases had severe disease. One hundred and forty-nine (12.9%) cases had underlying diseases among the total cases; 56% of the patients who had severe disease had an underlying condition (p < 0.01). The need for hospitalization did not differ between patients who had an underlying condition and those who do not have (p = 0.38), but the need for intensive care was higher in patients who had an underlying condition (p < 0.01). Forty-seven (31.5%) of the cases having underlying conditions had asthma or lung disease (38 of them had asthma).Conclusions: To the best of our knowledge, this is one of the largest pediatric data about confirmed COVID-19 cases. Children from all ages appear to be susceptible to COVID-19, and there is a significant difference in symptomatology and laboratory findings by means of age distribution.

5. The Effect Of Goal-Directed Fluid Therapy On The Development Of Acute Renal Failure In Patients With Cardiac Surgery

Abstract

Objective: AKI occurs in 5% to 42% of patients after cardiac surgery. The primary aim of this study was to investigate the effect of goal-directed fluid therapy in the perioperative period on the development of AKI in the early period in patients undergoing cardiac surgery.Methods: The study was designed prospectively at single academic center. Patients undergoing cardiac surgery were divided into two groups according to perioperative fluid management: Group Control (Group C, n=30): Fluid management was performed with traditional monitoring method. Group Study (Group S, n=30): It was performed with transesophageal doppler and transthoracic doppler fluid management. Clinical and hemodynamic data were recorded at preoperative (t0), postoperative 4th hour (t2), 24th hour (t3) and 48th hour (t4). Serum creatinine, GFR, Cystatin-C and KIM-1 values were measured as AKI indicators.Results: Postoperative fluid requirement was significantly lower in the study group (p=0.002). Postoperative total balance was significantly more negative in the study group than in the control group (p0.05).Conclusion: GDFT should be considered in the foreground because it shortens the length of hospital stay and reduces unnecessary fluid load in patients who are scheduled for major surgery. Cystatin-C can be used more prominently in the evaluation of AKI as a biomarker.

Abstract

Aims: The frontal QRS-T (fQRS) angle has been investigated in the general population, including healthy people and patients with heart failure. The fQRS angle can predict mortality due to myocarditis, ischaemic and nonischaemic cardiomyopathies, idiopathic dilated cardiomyopathy, and chronic heart failure in the general population. Moreover, no studies to date have investigated fQRS angle in coronavirus disease 2019 (COVID-19) patients. Thus, the purpose of this retrospective multicentre study was to evaluate the fQRS angle of COVID-19 patients to predict in-hospital mortality and the need for mechanical ventilation.Methods and results: An electrocardiogram was performed for 327 COVID-19 patients during admission, and the fQRS angle was calculated. Mechanical ventilation was needed in 119 patients; of them, 110 died in the hospital. The patients were divided into two groups according to an fQRs angle >90 degrees versus an fQRS angle 90 degrees group and 26.1% and 29.9% in the fQRS 90 degrees patient group versus the fQRS 90 degrees was a predictor of in-hospital mortality and associated with the need for mechanical ventilation among COVID-19 patients.(c) 2022 Elsevier Inc. Abstract

Peanut that is leguminous plant is grown in most Osmaniye and Adana in Turkey. It is the most important agricultural product of Osmaniye due to trade and industrial contribution. Most of the produced ground peanuts are consumed as snacks, but they are also used in the food industry as additives in peanut oil, peanut butter, confectionery and desserts. In this study, the usability of Anthocyanin (Cyanidin) dyestuff found in peanut seed shell in natural dyeing was investigated. In this context, raw and dried peanut skins were supplied firstly, then the skins were cleaned, ground and extracted. As the first stage of dyeing, wool hank yarns were dyed with traditional techniques in Karatepe Kilim Cooperative and it was used mordant (şap) for dyeing. The color analyzes were made for the dyed samples in the spectrophotometer and the results were evaluated. The fastness tests were performed in order to determine the color resistance of rubbing, washing and light to dyed hanks.As a result of natural dyeing, brown and shades of colors were obtained from peanut shell. According to fastness tests, the values are above acceptable values. As a result, it has been revealed in our project that peanut skins can be used easily in textile finishing and especially in natural dyeing., Baklagiller familyasından olan yer fıstığı Türkiye’de en çok Osmaniye ve Adana illerinde yetiştirilmekte ve tarımsal üretimin yanında ticaret ve sanayiye katkısı açısından Osmaniye’nin en önemli tarım ürünü konumundadır. Üretilen yer fıstığının büyük bölümü kuruyemiş olarak çerez olarak tüketilse de özellikle fıstık yağı, fıstık ezmesi, şekerleme ve tatlıcılıkta katkı maddesi olarak gıda sanayisinde kullanılmaktadır. Çalışma kapsamında, yer fıstığının tohum kabuğunda bulunduğu tespit edilen Antosiyanin (Siyanidin) boyarmaddesinin doğal boyamacılıkta kullanılabilirliği araştırılmıştır. Bu kapsamda ilk önce kam ve kavrulmuş fıstık kabukları tedarik edilmiş, daha sonra kabuklar temizlenmiş, öğütülmüş ve ekstrakte edilmiştir. Elde edilen boyarmadde ile Karatepe Kilim Kooperatifi’nde yün çile ipliklerin doğal boyaması şap mordanı kullanılarak gerçekleştirilmiştir. Boyanmış numunelerin spektrofotometrik renk değerleri tespit edilmiştir. Boyalı kumaşlara sürtünme, yıkama ve ışığa karşı renk dayanımlarını tespit edebilmek için haslık testleri yapılmış ve sonuçlar değerlendirilmiştir. Doğal boyamalar sonucunda, fıstık kabuğundan genel itibariyle kahverengi tonlarında renkler elde edilmiş, haslık testleri sonucunda da kullanım açısından kabul edilebilir sınırların üzerindeki değerlere ulaşılmıştır. Sonuç olarak fıstık iç kabuğunun tekstil terbiyesinde ve özellikle doğal boyamacılık alanında rahatlıkla kullanılabileceği ortaya konulmuştur.

Abstract

A 75-year-old woman presented with an 18-month history of severe, slowly worsening dysphonia. She was a smoker and known to have multiple benign cystic thyroid lesions. She reported no associated symptoms and other medical and social history was unremarkable. Fibreoptic nasendoscopy revealed a right-sided supraglottic cyst appearing to arise from the right false vocal cord. Further bedside examination was unremarkable. She underwent microlaryngoscopy and biopsy which showed a cyst originating from the right anterior ventricle, successfully removed without rupture using cold steel. Formal histopathology revealed a 14×10×7 mm unilocular, completely excised cyst lined by oncocytic epithelium and composed of columnar cells with darkly stained nuclei and abundant granular, eosinophilic cytoplasm. Three weeks postoperatively the patient’s voice had returned to normal. To date, 11 months postoperatively, there is no evidence of recurrence, and she will continue long-term follow-up.

10. Pelvic osteomyelitis in a 15-year-old girl. Case report

Abstract

Pelvic osteomyelitis is a rare infection. Ilium, ischium, pubis and acetabulum are commonly affected sites. Pelvic radiography, pelvic magnetic resonance and scintigraphy are useful imaging techniques for diagnosis. However, diagnoses should be confirmed with bone biopsy from the lesion. Sometimes diagnosis is delayed because of deep localization of infection site. Here in, we report a 15-year-old girl with left groin pain who was diagnosed as pelvic osteomyelitis one month after initial symptoms. Patient was successfully treated with antibiotics for 8 weeks. In conclusion, although pelvic osteomyelitis is an unusual infection in childhood, it should be considered as differential diagnoses of pain in groin, hips, thigh, abdomen and spine. If diagnosis is suspicious, empiric treatment should be started quickly, because it will be possible to achieve satisfactory clinical results with effective treatment.La osteomielitis pélvica es una infección poco frecuente, que suele afectar el ilion, el isquion, el pubis y el acetábulo. La radiografía de pelvis, la resonancia magnética de pelvis y el centellograma óseo son técnicas de diagnóstico por imágenes de mucha utilidad. Sin embargo, el diagnóstico debe confirmarse con la biopsia ósea de la lesión. A veces, el diagnóstico se retrasa debido a la localización profunda del foco infeccioso. Aquí presentamos el caso de una paciente de 15 años con dolor inguinal izquierdo, con diagnóstico de osteomielitis pélvica al mes de la manifestación de los síntomas iniciales. Recibió tratamiento antibiótico durante 8 semanas, con resultados satisfactorios. En conclusión, si bien la osteomielitis pélvica es una infección inusual en la infancia, debe considerarse como diagnóstico diferencial para el dolor en la ingle, la cadera, el muslo, el abdomen y la columna. Si se sospecha este diagnóstico, es necesario comenzar el tratamiento empírico de inmediato, porque es posible lograr resultados clínicos satisfactorios con el tratamiento eficaz.

11. Campylobacter jejuni: A rare agent in a child with peritoneal dialysis-related peritonitis

Abstract

Peritonitis is a serious problem in children receiving peritoneal dialysis. Campylobacter jejuni is an unusual cause of peritonitis. A 10-year-old boy who had end stage renal failure due to atypical hemolytic uremic syndrome was admitted to our hospital with abdominal pain and fever. Peritoneal dialysis fluid was cloudy and microscopic examination showed abundant leukocytes. Intraperitoneal cefepime treatment was started. Campylobacter jejuni was isolated from peritoneal dialysis fluid culture and oral clarithromycin was added to the treatment. At the end of therapy, peritoneal fluid culture was negative. To our knowledge, C. jejuni peritonitis was not reported in children previously.Although C. jejuni peritonitis is rarely encountered in children, it should be considered as an etiologic factor for peritonitis.La peritonitis es un problema grave en los niños que reciben diálisis peritoneal. La bacteria Campylobacter jejuni es una causa infrecuente de peritonitis. Un niño de 10 años de edad con insuficiencia renal terminal causada por síndrome urémico hemolítico atípico ingresó a nuestro hospital con dolor abdominal y fiebre. El líquido de la diálisis peritoneal era turbio; en el examen microscópico se observaron leucocitos abundantes. Se inició tratamiento con cefepime intraperitoneal. En el cultivo del líquido peritoneal se aisló Campylobacter jejuni, por lo que se agregó claritromicina oral al tratamiento. Al finalizar el tratamiento, el resultado del cultivo del líquido peritoneal era negativo. Hasta donde sabemos, no se había informado previamente peritonitis por C. jejuni en niños. Conclusión. Si bien la peritonitis por C. jejuni es rara en los niños, debe considerarse como factor etiológico de la peritonitis.

During the 6-year study using the Centers for Disease Control and Prevention's (CDC) U.S. National Healthcare Safety Network (NHSN) definitions for device-associated health care-associated infection (DA-HAI), we collected prospective data from 605,310 patients hospitalized in the INICC's ICUs for an aggregate of 3,338,396 days. Although device utilization in the INICC's ICUs was similar to that reported from ICUs in the U.S. in the CDC's NHSN, rates of device-associated nosocomial infection were higher in the ICUs of the INICC hospitals: the pooled rate of central line-associated bloodstream infection in the INICC's ICUs, 4.9 per 1,000 central line days, is nearly 5-fold higher than the 0.9 per 1,000 central line days reported from comparable U. S. ICUs. The overall rate of ventilator-associated pneumonia was also higher (16.8 vs 1.1 per 1,000 ventilator days) as was the rate of catheter-associated urinary tract infection (5.5 vs 1.3 per 1,000 catheter days). Frequencies of resistance of Pseudomonas isolates to amikacin (42.8% vs 10%) and imipenem (42.4% vs 26.1%) and Klebsiella pneumoniae isolates to ceftazidime (71.2% vs 28.8%) and imipenem (19.6% vs 12.8%) were also higher in the INICC's ICUs compared with the ICUs of the CDC's NHSN. 14. Incomplete Kawasaki disease in an infant presenting with only prolonged fever

Abstract

A 2.5-month-old boy admitted to our hospital with irritability, poor feeding and fever of 12 hours duration. On physical examination, he was febrile and extremely irritable. Initial whole blood count revealed a hemoglobin level of 10.1 g/dl, white blood count of 17,800/mm3 and platelet count of 454,000/mm3. Erythrocyte sedimentation rate was 80 mm/h and C-reactive protein was 3.96 mg/dl. Biochemical examinations of serum, urinalysis, chest X-ray, and analysis of cerebrospinal fluid (CSF) were normal. He was started on intravenous ampicillin and ceftriaxone empirically for provisional occult bacteremia. His blood, urine and CSF cultures were negative. On the 7th day of the treatment, there were no additional symptoms or findings other than fever. Echocardiography revealed aneurysms in both the left and right coronary arteries. Intravenous immunoglobulin (IVIG) and per oral aspirin were administered, and the fever resolved after IVIG infusion. Two years later, the echocardiography showed disappearing of the saccular aneurysm on the right coronary artery, but the dilatation of the left coronary artery was persisting. In conclusion, incomplete Kawasaki disease should always be included in the differential diagnosis of an infant with persistent fever, especially one younger than three months of age, when the conventional work-up fails to reveal the underlying cause.

15. An unusual case of neurobrucellosis presenting as demyelination disorder

Abstract

Brucellosis is a public health problem in most countries in the Mediterranean. Involvement of the central nervous system is seen in 4-13% of patients with brucellosis. A 13-year-old girl was admitted because of gait disturbance, diplopia, and dizziness. Her complaints began about 1.5 years ago. The second symptomatic episode repeated about three months ago and the third two months ago. In total, attacks repeated 3 times over 1.5 years. The magnetic resonance imaging (MRI) and the clinical features mimicked multiple sclerosis. The patient was given pulse steroid treatments. After steroid treatment, her gait disturbance and diplopia improved over the short term. Following positive developments, her symptoms recurred. The tests were repeated; the MRI showed increasingly high signal abnormalities, and Brucella melitensis was grown in cerebrospinal fluid. The patient was started on an oral combination of rifampin, doxycycline, and ciprofloxacin. MRI findings improved markedly after nine months of treatment. Although neurobrucellosis is associated rarely with demyelination in adults, this finding has not been reported previously in children or adolescents. Additionally, this case is the first in terms of involvement of the corpus callosum in neurobrucellosis. In this article, we present an unusual case of neurobrucellosis.

Abstract

Malaria is an acute and chronic illness characterized by paroxysms of fever, chills, sweats, fatigue, anemia, and splenomegaly. It is still an important health problem in malaria-endemic countries. Children living in malaria-endemic areas have elevated Epstein-Barr Virus (EBV) loads in the circulation and acute malaria infection leads to increased levels of circulating EBV that are cleared after anti-malaria treatment. There are many reports about the association of Plasmodium falciparum (P. falciparum) malaria and EBV infection. Here we report a case who had coinfection of Plasmodium vivax (P. vivax) malaria and EBV infection. To the best of our knowledge this is the first case indicating the association of P. vivax malaria and EBV infection.

Abstract

The multidrug-resistant bacterial infections cause high mortality in immunocompromised patients because of the limited antibacterial choices. Tigecycline, first member of the glycylcyclines, has in vitro activity against a wide variety of organisms, including multidrug-resistant pathogens; however, it has not yet been approved for use in children. Herein, we report a nine-year-old girl with acute myeloid leukemia who was treated successfully with tigecycline due to multidrug-resistant Escherichia coli bacteremia.

18. Chickenpox complications, incidence and financial burden in previously healthy children and those with an underlying disease in Ankara in the pre-vaccination period

Abstract

The aim of this study was to determine the complications, financial burden and mortality caused by chickenpox using the data of Ankara, Turkey in the pre-vaccination period. The study was conducted as a retrospective sectional study. Of the 65 patients admitted to our hospital, 34 (52.3%) had been previously healthy, 10 (15.4%) had previous chronic disease and 21 (32.3%) were immunocompromised. The most common complications of chickenpox in those patient groups were skin and soft tissue infections (41.2%), hematological complications (50%) and gastrointestinal complications (38.1%), respectively. We found 10.6/100,000 and 8.7/100,000 rates of hospitalization due to chickenpox in Ankara for all children and for previously healthy children, respectively. The chickenpox-related mortality rate for the 0-17 age group was 3.03/1,000,000 in Ankara. In conclusion, we feel that a national vaccination program for chickenpox will lead to a significant decrease in the overall cost to our country.

19. Nosocomial infections due to Acinetobacter baumannii in a pediatric intensive care unit in Turkey

Abstract

The aim of this study is to document the clinical characteristics and outcomes of Acinetobacter baumannii infections in pediatric patients in a pediatric intensive care unit (PICU) in Turkey. The ages ranged from 1 month to 16 years with a mean age of 55.5 months, and the male-to-female ratio was 1:1.5. Ventilator-associated pneumonia (10 patients) was the leading diagnosis, followed by catheter-related blood stream infection (4 patients), and bacteremia and ventilator-associated pneumonia associated with meningitis (1 patient) due to A. baumannii. Mechanical ventilation (93.3%), central venous catheter (73.3%), urinary catheter (93.3%), and broad spectrum antibiotic usage (80%) were the frequently seen risk factors. Neuromuscular (40%) and malignant (26.7%) disorders were the most common underlying diseases. Nosocomial A. baumannii is commonly multidrug-resistant, prolongs the length of stay in the PICU and increases the mortality rates in pediatric critical care.

Abstract

Neurologic manifestations of seasonal influenza 2009 pandemic influenza A (H1N1) are now known to include encephalitis, acute disseminated encephalomyelitis, Guillain-Barré syndrome, transverse myelitis, and acute necrotizing encephalopathy. We report a case of 2009 pandemic influenza A (H1N1) meningitis in a previously healthy six-year-old girl who presented with fever, headache, abdominal pain, and vomiting. The infection was confirmed via nasopharyngeal and throat swabs. She was treated with oseltamivir successfully. To our knowledge, she is the first child diagnosed as pandemic influenza A (H1N1) meningitis.

Abstract

The 2009 pandemic influenza A (H1N1) virus spread throughout the world and caused different clinical situations. Here, we report a 4.5-year-old boy with spontaneous pneumomediastinum (SPM) complicating pneumonia associated with H1N1 virus. SPM could be associated with bronchial obstruction or necrotizing pneumonia, and the prognosis of patients with SPM associated with necrosis is worse than of patients with SPM associated with bronchial obstruction.

Abstract

We present the case of a three-year-old boy who developed a special exanthem after oral intake of an antitussive-decongestant agent. The clinical findings were compatible with baboon syndrome. To our knowledge, this is one of the rare reported cases of baboon syndrome associated with use of an antitussive-decongestant including pseudoephedrine HCI, dextromethorphan HBr and chlorpheniramine maleate.

Abstract

Hyponatremia is the most frequent electrolyte disorder in critically ill neurological patients. The major differential diagnoses in this situation are the syndrome of inappropriate antidiuretic hormone secretion, marked by inappropriate retention of free water, and cerebral salt wasting, characterized by excessive urinary loss of sodium and resulting in polyuria and extracellular volume contraction. Cerebral salt wasting is a syndrome of hyponatremia due to increased urine output and excessive natriuresis described in patients with central nervous system disease. Although cerebral salt wasting has been well described in neurosurgical patients, data regarding pediatric patients is sparse. We present a 34-month-old boy with lissencephaly who developed cerebral salt wasting after brain biopsy. The patient was treated with hypertonic saline and multiple antiepileptic drugs. Fludrocortisone supplementation effectively treated cerebral salt wasting.

WOS: 000259244900009, PubMed ID: 18388064, Coronary collateral circulation determines the severity of ischemic myocardial damage. Increased P-wave dispersion is an independent predictor for atrial fibrillation. Consistent evidence is little about the relation between coronary collateral circulation and arrhythmia risk. In this article, the effect of coronary collateral circulation on P-wave dispersion was evaluated. Collateral grade and p-wave dispersion were ascertained in 100 patients with >= 85% diameter stenoses in left anterior descending or right coronary arteries. Left ventricular function score was also determined in all patients. Coronary collateral circulation was absent in 32 patients, whereas 68 patients had coronary collateral circulation. Patients with collateral grade A had greater left ventricular function score than did patients with collateral grade 0 (P = .048). However, there was no significant difference between P-wave dispersion of patients with and without coronary collateral circulation (P = .45). The presence of coronary collateral circulation failed to exert a beneficial decreasing effect on P-wave dispersion.
