Conventional aortic valve replacement in patients with porcelain aorta can be technically challenging and is even sometimes seen as contraindication. Trans-aortic valve implantation has been proposed as an alternative to other trancutaneous routes of implantation and is feasible when there is a small healthy area in the right anterolateral aspect of the ascending aorta. Herein we report an interesting case of combined transaortic valve implantation and off-pump coronary artery bypass grafting in a patient with severe aortic stenosis, severe ostial left main coronary artery disease and porcelain aorta. Transcatheter Aortic-Valve Implantation (TAVI) is considered to be highly effective in the treatment of high-risk patients with severe aortic stenosis. After TAVI, the rate of pacemaker implantation is 6.5%-40%. Some reports of sudden death after TAVI are mostly attributed to bradyarrhythmias. We report the case of three patients who experienced sudden cardiac death or aborted sudden cardiac death after TAVI. All patients were affected from ischemic heart disease with an ejection fraction of approximately 40% and underwent pacemaker implantation (PM) after the procedure due to 1rst degree atrioventricular block (AV) and left bundle branch block (LBBB). One of the patients died suddenly 30 days after the procedure. The PM interrogation revealed many episodes of non sustain ventricular tachycardias (NSVT) and one episode of ventricular fibrillation (VF) that led to death. The other two patients had syncope and during PM interrogation episodes of ventricular tachycardia >12 sec were recorded. Patients affected by ischemic heart disease undergoing TAVI, especially with borderline coronary lesions should receive particular attention in order to avoid potentially lethal ventricular arrhythmias. In addition, the physiopathologic mechanism of sudden arrhythmic death in these patients needs to be clarified. [ABSTRACT FROM AUTHOR]

The MitraClip procedure is an important procedure for the treatment of high-risk patients with severe MR, offering an effective and clinically meaningful reduction in the degree of MR, with low perioperative morbidity and mortality. Careful screening of prospective patients and evaluation by the multidisciplinary Heart Team are essential. A dedicated interventional team combining technical skills and advanced imaging is critical for success.

Introduction: Transcatheter aortic valve replacement (TAVR) is now an established treatment for certain patients with severe aortic valve stenosis (AS). However, as the number of patients screened for TAVR increases, many are found to have absolutely no option for peripheral artery access. Transaortic valve replacement (TAoVR) has been proposed as a new alternative route in patients deemed unsuitable for conventional approaches. We present our first series of TAoVR cases using the Edwards Sapien-XT and the Medtronic CoreValve prostheses., Methods: Twenty-five (25) symptomatic patients (mean age 78 ± 8 years, mean logistic EuroSCORE I 25 ± 11%) with severe AS underwent TAoVR using the Sapien-XT valve (10 patients) or the CoreValve (15 patients)., Results: The mean fluoroscopy time was 15.6 ± 4.2 minutes, the mean time in the intensive care unit was 1.9 ± 1.0 days, and the mean hospital stay was 6.4 ± 1.6 days. The mean effective aortic valve area increased (from 0.68 ± 0.15 cm(2) to 1.82 ± 0.34 cm(2), p<0.001) and the mean transvalvular pressure gradient declined (from 48 ± 15 mmHg to 9 ± 5 mmHg, p<0.05) post implantation. The procedural mortality was 0% and the in-hospital mortality was 4% (one death at day 3 due to cardiogenic shock). The mean NYHA functional class improved from 3.2 ± 0.4 to 1.5 ± 0.9 at 30 days., Conclusions: Our initial experience with the TAoVR approach using both the Edwards Sapien-XT and the Medtronic CoreValve prosthesis demonstrated that it could be performed safely, resulting in substantial acute echocardiographic and early clinical improvement.

The combination of a giant coronary aneurysm with multiple coronary aneurysms in adults is an extremely rare entity--especially in atherosclerotic patients, since it is most commonly associated with Kawasaki disease in children. We report an interesting case of a 59-year-old male patient with multiple atherosclerotic aneurysms of the left coronary system and a giant aneurysm of the right coronary artery. The patient was admitted to our hospital because of a non-ST myocardial infarction. Diagnosis was established by echocardiography, computed tomography angiogram, and coronary angiography. In view of the clinical symptoms and the extent of the giant right coronary aneurysm, with the associated risk of rupture, the patient was successfully treated with urgent surgical intervention. We also present a review of the current literature on this anomaly and a statistical analysis of all atherosclerotic giant coronary artery aneurysms previously reported.

Introduction: Mitral valve (MV) repair is the treatment of choice for degenerative mitral regurgitation (MR), but the surgical options for functional MR are limited. Percutaneous edge-to-edge MV repair using the MitraClip system has evolved as a new tool for the treatment of certain anatomical subsets of both functional and degenerative MR. We present the first Greek series of MV repair using the MitraClip., Methods: Five symptomatic patients (age 75 ± 9 years, logistic EuroSCORE 29 ± 11%) with moderate-tosevere (n=2) or severe (n=3) MR underwent MV repair using the MitraClip. All patients were treated with one clip and there were no primary adverse events., Results: Acute MR reduction by 3 grades was achieved in 2 patients and by 2 grades in 3 patients (reduction 2 grades in 100%). The total procedure time was 2.9 ± 2 hours (median 2 hours), the length of the ICU stay was 1 ± 0 day and the total length of hospital stay was 3.2 ± 0.6 days. All patients reported functional status improvement by 1 month (mean NYHA class improved from 3.0 ± 0.3 to 1.6 ± 0.6)., Conclusion: Our initial experience with percutaneous MV repair using the MitraClip system demonstrated that it can be performed safely, resulting in substantial acute echocardiographic and early clinical improvement.

The expression patterns of transforming growth factor beta 1 (TGFβ₁), urokinase-type plasminogen activator (uPA) and uPA receptor (uPAR) were analysed after artery ligation-induced myocardial infarction (MI) in the rat myocardium. uPA and uPAR expressions were significantly increased both at transcriptional and protein level during early phase post MI period (uPA at 1 hour and uPAR at 24 hours post infarction). TGFβ1 mRNA expression profile revealed a significant increase of TGFβ1 expression from day 4 up to 8 weeks post infarction. These data suggest that the need for an increasing TGFβ₁ bioavailability during the post-infarction period in rat myocardium is achieved in the early post MI period by an increased expression of uPA/uPAR proteolytic system (indirect activation of latent TGFβ₁) and in the late post MI period by direct regulation of TGFβ₁ expression. It is therefore concluded that differential regulation of the TGFβ₁ bioavailability may be a crucial step of the repair mechanisms during the post MI infarction period in the rat myocardium.

Animal models have enormously contributed to the study of diabetes mellitus, a metabolic disease with abnormal glucose homeostasis, due to some defect in the secretion or the action of insulin. They give researchers the opportunity to control in vivo the genetic and environmental factors that may influence the development of the disease and establishment of its complications, and thus gain new information about its handling and treatment in humans. Most experiments are carried out on rodents, even though other species with human-like biological characteristics are also used. Animal models develop diabetes either spontaneously or by using chemical, surgical, genetic or other techniques, and depict many clinical features or related phenotypes of the disease. In this review, an overview of the most commonly used animal models of diabetes are provided, highlighting the advantages and limitations of each model, and discussing their usefulness and contribution in the field of diabetes research.

We report the case of a young female patient who presented with chest pain, having a background of systemic complaints and a history of essential thrombocytosis. Transesophageal echocardiography (TEE) was suggestive of an intramural hematoma of the distal ascending aorta and transverse arch, confirming computed tomography findings. However, surgery revealed only diffuse aortic wall thickening without hematoma or dissection, despite an epiaortic ultrasonogram corroborating the TEE findings. Histopathology demonstrated inflammation involving all layers of the aortic wall, with intimal fibrosis, disrupted elastic fibers in the media, and marked fibrous thickening of the adventitia with multiple necrobiotic foci; the latter were apparently the cause of the markedly hypodense region mimicking intramural hematoma on imaging. The findings were consistent with Takayasu's arteritis, a large vessel vasculitis of unknown etiology that predominantly affects females.

Background: Osteoprotegerin (OPG) and receptor activator of nuclear factor kappa-B ligand (RANKL) are critical regulators of bone remodeling and RANKL/RANK signaling could also play an important role in the remodeling process of several tissues, such as myocardium. Therefore, we investigated whether the serum concentrations of OPG and RANKL correlate with the serum levels of metalloproteinase-1 (MMP-1), MMP-9 and tissue inhibitors of MMP-1 (TIMP-1), which are known regulators of myocardial healing in acute myocardial infarction (AMI) patients., Methods: We analyzed blood samples from 51 consecutively hospitalized men with AMI, 12 men with established ischemic heart failure (New York Heart Association category II, NYHA-II) and 12 healthy men age-matched to the NYHA-II patients. Serum levels of MMP-1, MMP-9, TIMP-1, OPG and RANKL were quantified using commercially available ELISA kits. AMI patients were sampled 4 days and 6 months after MI., Results: Our data revealed increased serum levels of OPG, RANKL, MMP-1 and TIMP-1 levels and significant correlations between increased RANKL levels and MMP-1 and TIMP-1 serum levels 6 months after MI. In addition, the ratio OPG/RANKL was very low 6 months after MI, suggesting that the nuclear factor kappa-B signaling is possibly more active 6 months post-MI than it is on day 4 post-MI., Conclusions: Our data suggest that OPG, RANKL, MMP-1 and TIMP-1 serum levels can be potential mediators of myocardial healing after MI. However, further large studies are needed to confirm the utility of OPG and RANKL as markers of healing after ST elevation in MI.
