Brown tumor (BT), also known as osteoclastoma, may appear in the context of primary and secondary hyperparathyroidism. Spinal cord compression due to the BT is extremely rare. We present here an unusual case of BT involving thoracal spine and mandible. A 26-year-old woman, who had been on hemodialysis for chronic renal failure for over 6 years, got admitted with dorsal pain and progressive weakness in her lower extremities and gait disturbances. Neurological examination revealed spastic paraparesis and symmetrically hyperactive tendon reflex in the lower extremities. She had hypoesthesia under T10 level. On physical examination, a swelling on the left side of her jaw was also detected. Magnetic resonance imaging (MRI) showed cord compression due to an extradural mass lesion at T8 level. A computerized tomography (CT) scan showed that this expansile lytic lesion was caused by the collapse of vertebra corpus (T8) at that level. CT of the mandible revealed an expansile lytic lesion on left arm of the mandible. Laboratory findings were nearly normal except parathormone level elevation to 1289 pg/mL (normal 30-70 pg/mL). Ultrasound examination showed enlargement of the parathyroid glands. The patient underwent an emergency decompression and stabilization surgery. The lesion was fragile and reddish in appearance and was easy to aspirate. The tumor was reported as "BT." Her weakness in the lower extremities improved in the early postoperative period. Following surgical intervention, the patient was transferred to nephrology clinic for additional medical treatment.

Spontaneous regression of herniated lumbar discs was reported occasionally. The mechanisms proposed for regression of disc herniation are still incomplete. This paper describes and discusses a case of spontaneous regression of herniated lumbar discs with a new disc protrusion in the adjacent level. A 41-year-old man was admitted with radiating pain and numbness in the left lower extremity with a left posterolateral disc extrusion at L5-S1 level. He was admitted to hospital with low back pain due to disc herniation caudally immigrating at L4-5 level three years ago. He refused the surgical intervention that was offered and was treated conservatively at that time. He had no neurological deficit and a history of spontaneous regression of the extruded lumbar disc; so, a conservative therapy, including bed rest, physical therapy, nonsteroidal anti-inflammatory drugs, and analgesics, was advised. In conclusion, herniated lumbar disc fragments may regress spontaneously. Reports are prone to advise conservative treatment for extruded or sequestrated lumbar disc herniations. However, these patients should be followed up closely; new herniation at adjacent/different level may occur. Furthermore, it is important to know which herniated disk should be removed and which should be treated conservatively, because disc herniation may cause serious complications as muscle weakness and cauda equine syndrome.

In healthy people without a coagulopathy or who are not treated by anti-coagulant, appearance of a femoral neuropathy due to the pressure exerted by an iliac hematoma is very rare. Most of the cases are young individuals in whom iliac muscle is tearing during sporting, which causes bleeding from the intramuscular vessels ending up with an iliac hematoma which exerts pressure on the neighboring femoral nerve through iliac fascia. Pressure is usually on the inguinal ligament level. All the clinical symptomatology regarding a pure femoral nerve lesion is apparent. To achieve femoral nerve decompensation, inguinal ligament must be dissected and the hematoma must be drained., Femoral sinirin iliak hematom nedeni ile basıya uğrayarak bir nöropati oluşturması, koagülopatisi olmayan veya antikoagülan kullanmayan sağlıklı kişilerde çok seyrek görülür. Daha çok gençlerde spor yaparken kapalı ve minör bir travmadan sonra iliak kasın gerilerek yırtılması ile intramusküler damarların kanaması sonucu oluşan iliak hematom, iliak fasiayı şişirip gererek komşu olduğu femoral sinirde basıya sebep olur. Bası, en çok inguinal ligaman seviyesindedir ve pür femoral sinir lezyonunun tüm klinik bulguları ortaya çıkar. Femoral sinirin dekompresyonu için inguinal ligamanı kesmek ve hematomu boşaltmak gerekir. Sunulan olgu bir futbol oyuncusunun kaleci vuruşandan sonra akut olarak sağ kasığında, uyluk ve dizinde ağrı, dizini açamama yakınmaları ile gelerek iliak nematom nedeni ile femoral nöropati tanısı alan bir erkek hastaya aittir. Yapılan ameliyat ile inguinal ligaman eksize edilmiş ve hematom drene edilmiştir.
