Abstract

The workforce shortage in health care is a major challenge worldwide. In Switzerland, this shortage is not only demographically caused but also due to workload. Adequate measures require a good data basis. The aim of this study is to test instrument specially developed for the health professions to measure work-related stress in practice. Cognitive pretesting was used for the evaluation. Overall, 18 semi-structured single-interviews with nurses and nursing assistants were conducted. In two rounds, the question-answer-process was analysed for 11 questions. The STRAIN questionnaire was then pretested by 17 health professionals. In the first round, 42 problems were identified based on the phases of the “Cognitive Aspects of Survey Methodology”. Of these, 57 % were comprehension problems. This was shown by the polysemantic usage or unknowingness of terms. Further 38 % were problems with the answer categories, such as a missing category or an inappropriate formulation. Further questions were modified through a final pretest. The uniform comprehensibility has been achieved. Due to the questionnaire length, an online tool with a save function should be used. The results of this study are similar to those of comparable studies, which also show the most common problems with understanding. This is due to changes of concepts’ meanings over time and culture. A cognitive pretest is therefore recommended, especially for a heterogeneous target group such as health professions.

10. Highly conserved serine residue 40 in HIV-1 p6 regulates capsid processing and virus core assembly

Abstract

Abstract Background The HIV-1 p6 Gag protein regulates the final abscission step of nascent virions from the cell membrane by the action of two late assembly (L-) domains. Although p6 is located within one of the most polymorphic regions of the HIV-1 gag gene, the 52 amino acid peptide binds at least to two cellular budding factors (Tsg101 and ALIX), is a substrate for phosphorylation, ubiquitination, and sumoylation, and mediates the incorporation of the HIV-1 accessory protein Vpr into viral particles. As expected, known functional domains mostly overlap with several conserved residues in p6. In this study, we investigated the importance of the highly conserved serine residue at position 40, which until now has not been assigned to any known function of p6. Results Consistently with previous data, we found that mutation of Ser-40 has no effect on ALIX mediated rescue of HIV-1 L-domain mutants. However, the only feasible S40F mutation that preserves the overlapping pol open reading frame (ORF) reduces virus replication in T-cell lines and in human lymphocyte tissue cultivated ex vivo. Most intriguingly, L-domain mediated virus release is not dependent on the integrity of Ser-40. However, the S40F mutation significantly reduces the specific infectivity of released virions. Further, it was observed that mutation of Ser-40 selectively interferes with the cleavage between capsid (CA) and the spacer peptide SP1 in Gag, without affecting cleavage of other Gag products. This deficiency in processing of CA, in consequence, led to an irregular morphology of the virus core and the formation of an electron dense extra core structure. Moreover, the defects induced by the S40F mutation in p6 can be rescued by the A1V mutation in SP1 that generally enhances processing of the CA-SP1 cleavage site. Conclusions Overall, these data support a so far unrecognized function of p6 mediated by Ser-40 that occurs independently of the L-domain function, but selectively affects CA maturation and virus core formation, and consequently the infectivity of released virions.

Abstract

Abstract Background Standardised translation and cross-cultural adaptation (TCCA) procedures are vital to describe language translation, cultural adaptation, and to evaluate quality factors of transformed outcome measures. No TCCA procedure for objectively-assessed outcome (OAO) measures exists. Furthermore, no official German version of the Canadian Chedoke Arm and Hand Activity Inventory (CAHAI) is available. Methods An eight-step for TCCA procedure for OAO was developed (TCCA-OAO) based on the existing TCCA procedure for patient-reported outcomes. The TCCA-OAO procedure was applied to develop a German version of the CAHAI (CAHAI-G). Inter-rater reliability of the CAHAI-G was determined through video rating of CAHAI-G. Validity evaluation of the CAHAI-G was assessed using the Chedoke-McMaster Stroke Assessment (CMSA). All ratings were performed by trained, independent raters. In a cross-sectional study, patients were tested within 31 hours after the initial CAHAI-G scoring, for their motor function level using the subscales for arm and hand of the CMSA. Inpatients and outpatients of the occupational therapy department who experienced a cerebrovascular accident or an intracerebral haemorrhage were included. Results Performance of 23 patients (mean age 69.4, SD 12.9; six females; mean time since stroke onset: 1.5 years, SD 2.5 years) have been assessed. A high inter-rater reliability was calculated with ICCs for 4 CAHAI-G versions (13, 9, 8, 7 items) ranging between r = 0.96 and r = 0.99 (p < 0.001). Correlation between the CAHAI-G and CMSA subscales for hand and arm was r = 0.74 (p < 0.001) and r = 0.67 (p < 0.001) respectively. Internal consistency of the CAHAI-G for all four versions ranged between α = 0.974 and α = 0.979. Conclusions The TCCA-OAO procedure was validated regarding its feasibility and applicability for objectively-assessed outcome measures. The resulting German CAHAI can be used as a valid and reliable assessment for bilateral upper limb performance in ADL in patients after stroke.

Abstract

Introduction: No brief questionnaire comprising knowledge, skills, and attitudes is available to measure digital competence among clinical practice nurses. Objective: The aim was to evaluate the structural validity and internal consistency of the Digital Competence Questionnaire (DCQ) for Clinical Practice Nurses. Methods: A cross-sectional study was conducted with English-speaking clinical practice nurses. Twenty-six items from an initial item pool, developed in a prior conducted Delphi Study, were included. Exploratory factor analysis for structural validity with "oblimin" rotation and a two-factor solution as well as internal consistency test using Cronbach's alpha were conducted. Results: Data from 185 nurses was obtained. The final questionnaire comprised of 12 items allocated to two factors: knowledge & skills and attitude. Factor "attitude" explained 33% of the variance and factor "knowledge & skills" 24%, resulting in a cumulative explanation of the variance of 57% by both factors. Internal consistency per factor was satisfactory, with 0.81 and 0.91, respectively. Conclusion: The DCQ for clinical practice nurses is valid and has satisfactory internal consistency. Researchers and nurse managers can use it to assess the level of digital competence among clinical practice nurses. Future psychometric validation of the DCQ for clinical practice nurses is required to allow a conclusion on the goodness of fit and reliability. [ABSTRACT FROM AUTHOR]

45. Pain responses in preterm infants and parental stress over repeated painful procedures: a randomized pilot trial

Abstract

OBJECTIVES In this pilot study, the aims were to determine the feasibility of whether pain behavior in extremely and very preterm infants and perceived parental stress change when parents are involved in pain reducing measures, either actively, performing facilitated tucking or passively, observing the intervention, in comparison to the involvement of nurses only. In addition, the infant's pain reactivity and parental stress over three time points of measurement was of interest. METHODS Extremely and very preterm infants in need of subcutaneous erythropoietin were randomly assigned to the two intervention groups. The intervention encompassed that one parent of each infant was involved during the painful procedure: Either parents executed facilitated tucking themselves or stood by, observing the procedure. Usual care involved that nurse executed facilitated tucking. All infants received 0.5 ml of 30% oral glucose solution via cotton swab before the painful procedure. Infant pain was observed with the Bernese Pain Scale for Neonates (BPSN) and measured with the MedStorm skin conductance algesimeter (SCA) before, during, and after the procedure. Parents' stress levels were measured before and after the painful procedure on the infant, using the Current Strain Short Questionnaire (CSSQ). Feasibility of a subsequent trial was determined by assessing recruitment, measurement and active parental involvement. Quantitative data collection methods (i.e. questionnaires, algesimeter) were employed to determine the number of participants for a larger trial and measurement adequacy. Qualitative data (interviews) was employed to determine parents' perspectives of their involvement. RESULTS A total of 13 infants (98% participation rate) were included along with their mothers. Median gestational age was 27 weeks (IQR 26-28 weeks), 62% were female. Two infants (12.5%) dropped out of the study as they were transferred to another hospital. Facilitated tucking turned out to be a good method to actively involve parents in pain reducing measures. No significant differences between the two intervention and control groups were found concerning parental stress and infant pain (p = .927). Power analysis indicated that at least N = 741 infants (power of 81%, α = .05) would be needed to obtain statistically significant results in a larger trial, as effect sizes were smaller than expected. Two of the three measurement tools - i.e. the BPSN and CSSQ) - proved easy to implement and were well accepted. owever, the SCA was challenging in this context. Measurements were also found to be time-consuming and resource-intense (i.e. health professionals as assistants). CONCLUSIONS Although the intervention was feasible and was readily accepted by parents, the study design was found to be challenging along with the SCA. In preparation of the larger trial, the study design needs to be revisited and adjusted. Thus, issues of time and resources may be resolved. In addition, national and international collaboration with similar neonatal intensive care units (NICU) needs to be considered. Thus, it will be possible to conduct an appropriately powered larger trial, which will yield important results to improve pain management in extremely and preterm infants in NICU.
