Abstract

INTRODUCTION: In the 330-patient ADVOCATE trial of avacopan for the treatment of antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis, in which 81% of patients had renal involvement, estimated glomerular filtration rate (eGFR) increased on average 7.3 ml/min per 1.73 m(2) in the avacopan group and 4.1 ml/min per 1.73 m(2) in the prednisone group (P = 0.029) at week 52. This new analysis examines the results in the patient subgroup with severe renal insufficiency at enrollment into the trial, i.e., eGFR ≤20 ml/min per 1.73 m(2). METHODS: eGFR was determined at baseline and over the course of the trial. Changes in eGFR were compared between the 2 treatment groups. RESULTS: In ADVOCATE, 27 of 166 patients (16%) in the avacopan group and 23 of 164 patients (14%) in the prednisone group had a baseline eGFR ≤20 ml/min per 1.73 m(2). At week 52, eGFR increased on average 16.1 and 7.7 ml/min per 1.73 m(2) in the avacopan and prednisone groups, respectively (P = 0.003). The last eGFR value measured during the 52-week treatment period was ≥2-fold higher than baseline in 41% of patients in the avacopan group compared to 13% in the prednisone group (P = 0.030). More patients in the avacopan group versus prednisone group had increases in eGFR above 20, 30, and 45 ml/min per 1.73 m(2), respectively. Serious adverse events occurred in 13 of 27 patients (48%) in the avacopan group and 16 of 23 patients (70%) in the prednisone group. CONCLUSION: Among patients with baseline eGFR ≤20 ml/min per 1.73 m(2) in the ADVOCATE trial, eGFR improved more in the avacopan group than in the prednisone group.

Abstract

Introduction: In the 330-patient ADVOCATE trial of avacopan for the treatment of antineutrophil cytoplasmic autoantibody (ANCA)-associated vasculitis, in which 81% of patients had renal involvement, estimated glomerular filtration rate (eGFR) increased on average 7.3 ml/min per 1.73 m2 in the avacopan group and 4.1 ml/min per 1.73 m2 in the prednisone group (P = 0.029) at week 52. This new analysis examines the results in the patient subgroup with severe renal insufficiency at enrollment into the trial, i.e., eGFR ≤20 ml/min per 1.73 m2. Methods: eGFR was determined at baseline and over the course of the trial. Changes in eGFR were compared between the 2 treatment groups. Results: In ADVOCATE, 27 of 166 patients (16%) in the avacopan group and 23 of 164 patients (14%) in the prednisone group had a baseline eGFR ≤20 ml/min per 1.73 m2. At week 52, eGFR increased on average 16.1 and 7.7 ml/min per 1.73 m2 in the avacopan and prednisone groups, respectively (P = 0.003). The last eGFR value measured during the 52-week treatment period was ≥2-fold higher than baseline in 41% of patients in the avacopan group compared to 13% in the prednisone group (P = 0.030). More patients in the avacopan group versus prednisone group had increases in eGFR above 20, 30, and 45 ml/min per 1.73 m2, respectively. Serious adverse events occurred in 13 of 27 patients (48%) in the avacopan group and 16 of 23 patients (70%) in the prednisone group. Conclusion: Among patients with baseline eGFR ≤20 ml/min per 1.73 m2 in the ADVOCATE trial, eGFR improved more in the avacopan group than in the prednisone group.

Abstract

BACKGROUND: The C5a receptor inhibitor avacopan is being studied for the treatment of antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis.METHODS: In this randomized, controlled trial, we assigned patients with ANCA-associated vasculitis in a 1:1 ratio to receive oral avacopan at a dose of 30 mg twice daily or oral prednisone on a tapering schedule. All the patients received either cyclophosphamide (followed by azathioprine) or rituximab. The first primary end point was remission, defined as a Birmingham Vasculitis Activity Score (BVAS) of 0 (on a scale from 0 to 63, with higher scores indicating greater disease activity) at week 26 and no glucocorticoid use in the previous 4 weeks. The second primary end point was sustained remission, defined as remission at both weeks 26 and 52. Both end points were tested for noninferiority (by a margin of 20 percentage points) and for superiority.RESULTS: A total of 331 patients underwent randomization; 166 were assigned to receive avacopan, and 165 were assigned to receive prednisone. The mean BVAS at baseline was 16 in both groups. Remission at week 26 (the first primary end point) was observed in 120 of 166 patients (72.3%) receiving avacopan and in 115 of 164 patients (70.1%) receiving prednisone (estimated common difference, 3.4 percentage points; 95% confidence interval [CI], -6.0 to 12.8; PCONCLUSIONS: In this trial involving patients with ANCA-associated vasculitis, avacopan was noninferior but not superior to prednisone taper with respect to remission at week 26 and was superior to prednisone taper with respect to sustained remission at week 52. All the patients received cyclophosphamide or rituximab. The safety and clinical effects of avacopan beyond 52 weeks were not addressed in the trial. Abstract

Parameters of humoral and cellular immunity were investigated in patients with chronic cadmium (Cd) disease and compared to values in an age matched control group. In the patient group, there was a slight reduction in WBC although T and B lymphocyte ratios were normal. RBC counts were significantly reduced (P < 0.05). No significant differences in serum immunoglobulin (Ig) concentrations were found. Blood Cd was significantly raised (P < 0.001) and urine Cd was elevated. In lymphocyte transformation tests, responses to Staphylococcus aureus, PHA and PPD were not impaired and no evidence of Cd hypersensitivity was found. Responses in PHA and ADCC cytotoxicity tests were normal and nitro-blue tetrazolium (NBT) reduction was only slightly decreased in the patient group. β2-microglobulin (β2M) was significantly raised in serum (P<0.001) and urine (P < 0.001) of patients. Patient 3 day lymphocyte cultures produced significantly more β2M than cultures from the control group (P < 0.01). β2M was increased in lymphocyte cultures, from patient and control groups, containing PHA (P < 0.01), low Cd concentrations (P < 0.01) or PPD. The results indicate that chronic Cd disease is not associated with significant alterations in immunological parameters in vitro. The high β2M levels associated with this disease may not result entirely from kidney damage but may in part be due to an increase in the release of β2M from cells, induced by Cd. 32. Plasma LH levels following electrochemical stimulation of the deafferented medial basal hypothalamus

Abstract

We have investigated the capability of a completely deafferented medial basal hypothalamus (MBH) pituitary complex to support LH release following electrochemical stimulation (ECS) of the arcuate-median eminence (ARC-ME) region. In adult female rats the MBH was completely deafferented (CD) on the morning of pro-oestrus (08.00-10.00 h of day 0). In the first experiment the animals were divided into 5 groups depending on the day of ECS (14.00–16.00 h) and oestradiol benzoate (Oe) treatment (08.00–10.00 h): group No. 1) ECS on day 0; 2) no ECS; 3) ECS on day 1; 4) Oe on day 0 and ECS on day 1; 5) Oe on day 4 and ECS on day 5. Blood samples were collected from the external jugular vein under ether anaesthesia for LH determinations just before and 1 and 2 h after ECS. ECS on day 0 resulted in a significant (P<0.01) rise in plasma LH at 1 and 2 h, while the rats subjected to CD, but not ECS failed to show any changes in plasma LH levels. ECS on days I and 5 (groups 3 and 5) failed to alter plasma LH levels, however, ECS on day 1 in Oe treated rats produced a significant (P< 0.01) elevation in plasma LH that was comparable to that of day 0. In a second experiment Oe was injected on days 1–5 and ECS of the ARC-ME was done bilaterally. ECS on day 5 resulted in a significant (P< 0.05) rise in plasma LH levels in rats with a completely deafferented MBH. In animals with an incomplete deafferentation ECS resulted in a much greater (P< 0.005) rise in plasma LH at l and 2 h. Since LH was released several days after complete hypothalamic deafferentation, these data suggest that LRH secreting cells may be present within the MBH of the rat. In a third experiment injection of LRH (400 ng) on days 0, 1 and 5 with Oe on days 0, 4 or 1–5 resulted in a significant (P< 0.001) rise of plasma LH at 30 and 60 min in all groups. Rats injected on day 0 showed the greatest elevation at 60 min in all groups. Saline injected rats did not show any changes in plasma LH levels.

33. EFFECT OF D-Trp6-LH-RH ON THE PITUITARY-GONADAL AXIS DURING THE LUTEAL PHASE IN THE BABOON

Abstract

An effect of D-Trp6-LH-RH (superactive LH-RH agonist) in the pituitary and ovarian function was examined. Four regularly cycling baboons were used for this study. After determination of control values of plasma levels of LH, oestrogen and progesterone during the entire menstrual cycle, D-Trp6-LH-RH was infused subcutaneously for 7 days during the early luteal phase in these four baboons. An infusion of D-Trp6-LH-RH increased plasma LH and oestrogen, but it failed to alter the plasma level of progesterone. From these results, it seems unlikely that 1) D-Trp6-LH-RH has a luteolytic effect, and 2) an increased ovarian oestrogen causes luteolysis in the baboon.

34. Prevention of Continuous Light-Induced Anovulation in Rats by Early Exposure to Continuous Light

Abstract

Continuous illumination (LL) beginning at 22 days of age caused precocious puberty followed by persistent estrus with anovulation in female offspring originating from mother rats exposed to a 14L:10D light-dark cycle prior to and during pregnancy. However, LL had no deleterious effect on reproductive cycles of offspring reared in LL and originating from mothers exposed to LL prior to and during pregnancy. These rats had a normal onset of puberty in LL, a normal 4-day estrous cycle, a periodic rise of plasma estrogen prior to the periodic appearance of the preovulatory luteinizing hormone (LH) surge, and spontaneous ovulation in LL continued until at least 300 days of age. Also, the female offspring of these rats showed a similar resistance to the deleterious effects of LL on cyclic ovulation.These results support the following interpretation: 1) offspring from mother rats exposed to LL prior to and during pregnancy become insensitive to the deleterious effects of LL on cyclic ovulation, 2) neural elements controlling cyclic release of LH are not totally photoperiod (14L:10D)-dependent, and 3) in the absence of daily 14L:10D signals, an endogenous clock, possibly timed by daily laboratory signals (temperature, noise, taking of vaginal smears), may provide time cues for cyclic LH release.

Abstract

An attempt was made to determine the synergistic effect of TRH stimulated prolactin with LH on the luteinization of corpus luteum in baboons. Four normally cycling female baboons were used in this study. Synthetic LHRH (100 micrograms) was given during the early luteal phase and blood samples were collected sequentially for assay of progestin. In additional experiments, 40 micrograms of synthetic TRH was given at 30 and 60 minutes after LHRH injection and blood samples were assayed for progestin. Injection of LHRH elevated the plasma level of progestin. Subsequent treatment with TRH in LHRH treated baboons enhanced the elevation of plasma progestin in three of four baboons. The evidence illustrates the nature of synergistic effect of prolactin with LH on the luteinization of corpus luteum in baboons.

Abstract

Objectives: To efficiently detect somatic UBA1 variants and establish a clinical scoring system predicting patients with pathogenic variants in VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome., Methods: Eighty-nine Japanese patients with clinically suspected VEXAS syndrome were recruited [81 males and 8 females; median age of onset 69.3 years (interquartile range 62.1-77.6)]. Peptide nucleic acid-clamping PCR (PNA-PCR), regular PCR targeting exon 3 clustering UBA1 variants and subsequent Sanger sequencing were conducted for variant screening. Partitioning digital PCR or targeted amplicon deep sequencing was also performed to evaluate the variant allele frequency (VAF). We developed our clinical scoring system to predict UBA1 variant-positive and -negative patients and assessed the diagnostic value of our system using receiver operating characteristics (ROC) curve analysis., Results: Forty patients (44.9%) with reported pathogenic UBA1 variants were identified, including a case having a variant with VAF of 1.7%, using a highly sensitive method. Our clinical scoring system considering age >50 years, cutaneous lesions, lung involvement, chondritis and macrocytic anaemia efficiently predicted patients with UBA1 variants (the area under the curve for the scoring total was 0.908)., Conclusion: Genetic screening with the combination of regular PCR and PNA-PCR detected somatic UBA1 variants with high sensitivity and specificity.
