4. A web-based mindfulness-based cognitive therapy for couples dealing with chronic cancer-related fatigue: Protocol for a single-arm pilot trial

Abstract

Background Chronic fatigue is a common symptom among patients who have been treated for cancer. Current psychosocial interventions typically target the patient alone, despite growing evidence suggesting that a couples’ approach can increase and broaden the efficacy of an intervention. Therefore, based on an existing web-based mindfulness-based cognitive therapy for patients, the couple intervention COMPANION was developed. Objective The primary objectives of this study are to determine the acceptability of COMPANION and its potential efficacy in reducing fatigue in patients with cancer. Our secondary objectives are to examine the feasibility of the trial procedures and the potential working mechanisms of the couple intervention. Methods We will conduct a single-arm pilot trial for couples (ie, patients with cancer with chronic fatigue and their partners). All couples are allocated to the web-based couple intervention that consists of psychoeducation, mindfulness, and cognitive-behavioral exercises. The 9 sessions of the intervention are supervised remotely by a trained therapist. Patients and partners will complete questionnaires before starting the intervention (T0), 2 weeks after completing the intervention (T1), and 1 month after T1 (T2). They will also fill out weekly diaries during the intervention period. A subsample of patients (n≈5) and partners (n≈5) as well as all the therapists providing COMPANION will participate in the final focus groups. Benchmark values have been defined to determine the acceptability (ie, ≥60% of couples complete the intervention and/or ≥70% of the participants are satisfied with the intervention) and potential efficacy (ie, a significant improvement in fatigue and/or a clinically relevant improvement in fatigue in 45% of the patients between T0 and T1) of the intervention. 5. The interpersonal process model of intimacy, burden and communal motivation to care in a multinational group of informal caregivers

Abstract

The interpersonal process model of intimacy suggests that intimacy within dyads develops when open communication from one dyad member is met with validating, understanding and caring responses from the other dyad member. Little is known about interpersonal processes between informal caregivers and care recipients in different illness contexts (i.e., coping with neurological, physical or other impairments/comorbidities) and relationship types (i.e., spouses, adult children and others), and how interpersonal processes are linked to the caregivers' wellbeing. In this cross-sectional study, caregivers (N = 882) from nine countries reported on their open communication with care recipients, care recipients' responsiveness, and caregivers' wellbeing (i.e., intimacy, burden and communal motivation to care). Although caregivers of care recipients with neurological impairments and adult child caregivers reported lower mean levels of open communication and perceived responsiveness, similar mediating effects of perceived responsiveness between open communication and intimacy (effect = 0.05; CI95 = [0.04, 0.06]), burden (effect = -0.42; CI95 = [-0.49, -0.35]), and communal motivation to care (effect = 0.28; CI95 = [0.21, 0.36]) were reported across different illness contexts and relationship types. Only the negative relationship between perceived responsiveness and burden was stronger for spouses (b = -0.23, CI95 = [-0.26, -0.19]) than for adult children and others. Results suggest the interpersonal process model of intimacy may be used to identify caregivers at risk of poor wellbeing in different illness contexts and with different relationships with their care recipients. Lack of open communication and caregivers' misperceptions of care recipients' responsiveness may represent opportunities to intervene to enhance caregivers' intimacy to the care recipients, communal motivation to care, and to decrease caregiver burden.

11. Trajectories of fatigue in cancer patients during psychological care

Abstract

Objective Psycho-oncological institutions offer specialized care for cancer patients. Little is known how this care might impact fatigue. This study aimed to identify fatigue trajectories during psychological care, examined factors distinguishing these trajectories and predicted fatigue severity after nine months of psychological care. Design Naturalistic, longitudinal study of 238 cancer patients receiving psycho-oncological care in the Netherlands. Data were collected before initiation of psychological care (T1) and three (T2) and nine months (T3) afterwards. Latent class growth analysis, repeated measure analyses (RMA) and linear regression analysis were performed. Main Outcome Measures Fatigue severity: Checklist Individual Strength. Results Three fatigue trajectories were identified: high- (30%), moderate- (62%) and low-level fatigue (8%). While statistically significant decreases in fatigue were found, this decrease was not clinically relevant. RMA showed main effects for time for fatigue trajectories on depression, anxiety, personal control and illness cognitions. Fatigue severity and physical symptoms at T1, but not demographic or clinical factors, were predictive of fatigue severity at T3. Conclusions Fatigue is very common during psycho-oncological care, and notably not clinically improving. As symptoms of fatigue, depression, anxiety and physical symptoms often cluster, supplementary fatigue treatment should be considered when it is decided to treat other symptoms first.

Abstract

Introduction Prevalence of obesity and associated diseases, including type 2 diabetes mellitus, dyslipidaemia and non-alcoholic fatty liver disease (NAFLD), are increasing. Underlying mechanisms, especially in humans, are unclear. Bariatric surgery provides the unique opportunity to obtain biopsies and portal vein blood-samples. Methods The BARIA Study aims to assess how microbiota and their metabolites affect transcription in key tissues and clinical outcome in obese subjects and how baseline anthropometric and metabolic characteristics determine weight loss and glucose homeostasis after bariatric surgery. We phenotype patients undergoing bariatric surgery (predominantly laparoscopic Roux-en-Y gastric bypass), before weight loss, with biometrics, dietary and psychological questionnaires, mixed meal test (MMT) and collect fecal-samples and intra-operative biopsies from liver, adipose tissues and jejunum. We aim to include 1500 patients. A subset (approximately 25%) will undergo intra-operative portal vein blood-sampling. Fecal-samples are analyzed with shotgun metagenomics and targeted metabolomics, fasted and postprandial plasma-samples are subjected to metabolomics, and RNA is extracted from the tissues for RNAseq-analyses. Data will be integrated using state-of-the-art neuronal networks and metabolic modeling. Patient follow-up will be ten years. Results Preoperative MMT of 170 patients were analysed and clear differences were observed in glucose homeostasis between individuals. Repeated MMT in 10 patients showed satisfactory intra-individual reproducibility, with differences in plasma glucose, insulin and triglycerides within 20% of the mean difference. Conclusion The BARIA study can add more understanding in how gut-microbiota affect metabolism, especially with regard to obesity, glucose metabolism and NAFLD. Identification of key factors may provide diagnostic and therapeutic leads to control the obesity-associated disease epidemic.

Among married persons, marital satisfaction is associated with better mental health. Studies on mental health in married and nonmarried persons that consider marital satisfaction have categorized patients as satisfied versus unsatisfied, which reduces statistical power and does not generate clinically useful information on mental health across the satisfaction spectrum. Our objective was to demonstrate a novel regression approach to evaluate mental health in women with systemic sclerosis (SSc), comparing married and unmarried women, accounting for continuously measured marital satisfaction. Methods: Depressive symptoms were assessed using the Center for Epidemiologic Studies Depression Scale (CES-D) and marital satisfaction with the Dyadic Adjustment Scale-7. A single multiple linear regression model was used to predict CES-D scores from marital status and, among married women, continuously measured marital satisfaction, controlling for demographic and clinical characteristics. Results: Of 725 women, 494 (68%) were married or living as married. On average, married women had mean CES-D scores that were 2.0 points (0.19 SDs) lower than unmarried women (P = 0.013). 21. The effect of an attachment-oriented couple intervention for breast cancer patients and partners in the early treatment phase:A randomised controlled trial

Abstract

OBJECTIVE: Patients and partners both cope individually and as a dyad with challenges related to a breast cancer diagnosis. The objective of this study was to evaluate the effect of a psychological attachment-oriented couple intervention for breast cancer patients and partners in the early treatment phase.METHODS: A randomised controlled trial including 198 recently diagnosed breast cancer patients and their partners. Couples were randomised to the Hand in Hand (HiH) intervention in addition to usual care or to usual care only. Self-report assessments were conducted for both patients and partners at baseline, postintervention (5 months), and follow-up (10 months), assessing cancer-related distress, symptoms of anxiety and depression, and dyadic adjustment. Patients' cancer-related distress was the primary outcome.RESULTS: Cancer-related distress decreased over time in both patients and partners, but the intervention did not significantly affect this decrease at postintervention (P = .08) or follow-up (P = .71). A significant positive effect was found on dyadic adjustment at follow-up for both patients (P = .04) and partners (P = .02).CONCLUSIONS: There was no significant effect of the HiH intervention cancer-related distress. The results suggest that most couples can cope with cancer-related distress in the context of usual care. However, the positive effect on dyadic adjustment implies that the HiH intervention benefitted both patients and partners. Future studies should investigate how to integrate a couple focus in usual cancer care to improve dyadic coping in the early treatment phase.

Abstract

Objective: To examine how social comparison orientation (SCO) moderates the effects of three types of social comparison information on the global quality of life of cancer patients 2 weeks and 3 months later. Design: Cancer patients (n = 226) were provided with social comparison information just prior to undergoing radiation therapy, using audiotapes. Each participant was confronted with one of three tapes: (1) focusing on procedural aspects, (2) focusing on emotional reactions and (3) focusing on coping strategies. Main outcome measures: Quality of life as measured with the Cantril self-anchoring scale [Cantril, H. (1965). The pattern of human concerns. New Brunswick, NJ: Rutgers University Press]. Results: With increasing SCO, a lower quality of life was reported after listening to the emotion tape, while a higher quality of life was reported after listening to the coping tape. These effects were found 2 weeks as well as 3 months after the radiation therapy had ended. Conclusion: Social comparison information may have longitudinal effects on quality of life, but these effects are to an important extent dependent on the nature of the information and individual differences in SCO. © 2012 Copyright Taylor and Francis Group, LLC.

Abstract

The present study examined how the effects of three audiotapes containing different types of social comparison information on the mood of cancer patients depended on the level of neuroticism. On the procedural tape, a man and woman discussed the process of radiation therapy, on the emotion tape, they focussed on emotional reactions to their illness and treatment, while on the coping tape they focussed on the way they had been coping. A validation study among 115 students showed that the tapes were perceived as they were intended. The main study was conducted among 226 patients who were about to undergo radiation therapy. Compared to patients in the control group, as patients were higher in neuroticism, they reported less negative mood after listening to the procedural and the coping tape. Furthermore, as patients were higher in neuroticism, they reported less negative mood after listening to the coping tape than to the emotion tape. © 2009 John Wiley & Sons, Ltd.

Abstract

Objective. The aim of this study was to investigate whether a breast cancer diagnosis, personal assertiveness (i.e. frequency of assertive behaviour and tension associated with this behaviour), and partners' relationship-focused coping (i.e. active engagement and protective buffering) were sources of variation in patients' relationship satisfaction, cross-sectionally and over time. Design and method. This longitudinal case-control study assessed the two dimensions of personal assertiveness and relationship satisfaction in both women with cancer (N = 72) and comparison controls (N = 62). In addition, patients completed a measure assessing their partners' active engagement and protective buffering. Results. Cases (i.e. women with breast cancer) were not found to report more relationship problems than controls. Women with breast cancer who tended not to express their concerns and feelings and who experienced much tension when they did, reported relatively low marital satisfaction. Moreover, partners' protective buffering was associated with less relationship satisfaction in especially more assertive (i.e. high frequency of assertive behaviour and low tension) women with cancer, while active engagement was associated with more relationship satisfaction, regardless of the women's personal assertiveness. Discussion. The results of the present study indicate that a breast cancer diagnosis by itself may not be a risk factor for relationship problems. However, in the context of an illness such as cancer, personal assertiveness, and a partner's relationship-focused coping strategies do seem to play a role in maintaining a satisfactory relationship with one's partner.

Abstract

Patient navigation ( PN) and patient activation ( PA) interventions are widely used to help patients with cancer to manage the disease and the care trajectory. However, the usability and impact of these interventions on older patients and their well-being are unclear. This study aims to show which PN and PA interventions are being used and what impact they have. After systematically searching the literature, we assessed the quality of the publications we found. The publications had to involve at least a subgroup of older people with minimally one abnormal oncologic test result each, and they had to focus on PN or PA interventions. Six PA publications examined the interventions exclusively for elderly patients. Each of the 11 PN and PA publications contained at least one comment about the impact on older patients. The types of impact varied substantially, but there was scant attention to the quality of life. The type of intervention, the outcome measures and the quality of publications also varied considerably. Generally, age is not an important factor in PN and PA studies. To facilitate adjustment of navigation and activation interventions to the individual patient, more rigorous research into the impact of PN and PA interventions on older patients is necessary. Abstract

(from the journal abstract) The communication of support plays a central role in the adaptation process following the cancer diagnosis, for both patients and their partners. This study deals with the question how marital communication relates to marital satisfaction and depressive symptoms. The 174 participating patients suffered from various types of cancer and were recruited through patient organisations or an oncologist. In order to measure communication styles, both patients and partners filled out the Communication Skills Inventory (CSI). The CSI distinguishes four subscales: intimacy, avoidance, discongruent and destructive communication. The communication style of the couples was characterised by a high degree of intimacy, and in a lesser degree by successively avoidance, destructive and discongruent communication. These four communication factors could explain a high percentage of the variance of marital satisfaction, namely 60% for patients and 52% for healthy partners. The negative correlation between destructive communication and marital satisfaction appeared to be stronger for women than for men, irrespective of their role of patient or partner. In addition, the negative relationship between discongruent communication and marital satisfaction was found to be stronger for healthy female partners than for male partners. Unexpectedly, the communication patterns reported by the spouses explained only a small portion of the variance of depressive symptoms, namely 3% for patients and 14% for healthy partners. The results of this study might encourage scientists in the field of psychosocial oncology to include aspects of the interaction when studying determinants of well-being of both patients and their intimate partners. Abstract

BACKGROUND. The present study examined the structure and reliability of the Dutch version of the Patient Satisfaction Questionnaire III (PSQ 111). The PSQ III was designed to measure technical competence, interpersonal manner, communication, time spent with doctor, financial aspects, and access to care. In the Dutch version, the financial items were left aside because these are not appropriate for the Dutch socialized system. OBJECTIVES. The main objectives were to assess response bias, the number of dimensions needed to describe the PSQ III items, and the reliability of the scales. In addition, distribution characteristics were examined and norm scores to interpret satisfaction scores in an oncological setting were presented. RESEARCH DESIGN. A cross-sectional survey study. SUBJECTS. The study was comprised of 1594 cancer patients from eight hospitals. MEASURES. The Dutch version of the PSQ III. RESULTS. Approximately 14% of the respondents were found to demonstrate considerable response bias. Confirmative factor analyses were performed to test three theoretical models with a varying number of dimensions among those participants who did not demonstrate response bias. The original structure did not fit the data well, but support was found for a three-factor model (with interpersonal manner, communication, and time spent with doctor loading on one factor instead of separate factors) and a one-dimensional model. CONCLUSIONS. The PSQ III seems to be an appropriate measure of cancer patients' satisfaction, with the note that the number of dimensions may vary for different patient groups and/or care settings and that it is important to be aware of response bias.

Abstract

This study examined evaluations of a proposal to equalize the budget for special education across regions among 1,114 participants affiliated with either mainstream or special education in The Netherlands. In line with predictions, it was found that equalization was more positively evaluated as expected outcomes for the own region were more favorable, and as the procedures and distribution were regarded as more fair. Moreover, equalization was evaluated more positively as referent distributions were rated as less fair. This research shows that perceived quality and fairness of a policy measure are strongly related to the perceived favorability of collective outcomes.

Abstract

This study examined the hypothesis that patients' persistence in comparing themselves with others who are perceived as being worse off might induce a response shift in self-reported quality of life (QL). Specifically, we tested whether patients' ratings of how they are doing compared to others (i.e., relative evaluation) moderates the link between physical deterioration and decline in self-reported QL. Two hundred forty patients with various cancer diagnoses who were undergoing palliative chemotherapy rated their physical functioning and QL (i.e., emotional functioning and global quality of life) twice with an interval of 3 months. In addition, a significant other and a research assistant rated the physical condition of the patients. Consistent with the buffering model of response shift, patients who felt that they were better off than others appeared to be able to sustain their quality of life in the face of a worsening physical condition.
