Aims and MethodProgramme budgeting and marginal analysis (PBMA) is a recognised tool for commissioning healthcare. the objectives of this project were to test the acceptability, data availability and practical value of PBMA within the sphere of mental health. the PBMA methodology was applied to the consideration of Norfolk Primary Care Trust's National Health Service expenditure on mental health for the fiscal year 2006/7.ResultsThe project successfully attracted the interest of, and contribution from, important stakeholders with the exception of general practitioners. the process led to the identification of areas for disinvestment, releasing funds to be made available for the development of new services, or enhancement of existing services.Clinical ImplicationsProgramme budgeting and marginal analysis is a useful structured tool for the commissioning of mental health services. It is essential, however, that psychiatrists fully engage with the process in order to have an influence over the future direction of mental health services.

Little empirical research has been published concerning sexual offending against older female victims. We analysed all recorded sexual offences against adult women over 5.25 years in a circumscribed semi-rural area, calculated prevalence rates and attributable risk values, and used case control and descriptive methodology to analyse the crime scene characteristics. Sexual offences committed against older or elderly women are uncommon. Sexual offences against women aged 60+ years are not characterized by excessive violence and are typically carried out in a residential domestic setting by a single male assailant who is significantly older than is the case for typical sex offenders. Sexual offences where the female victim is 30+ years older than the male offender are typically minor assaults perpetrated by young drunk males in public places. Despite the not infrequent occurrence of the rape of an elderly woman and the subsequent public shock that is generated, this type of offence remains poorly reported in the medical and scientific literature. This article reviews what little literature is available and brings together the existing knowledge about the phenomenon. The paucity of hard data is emphasized along with the almost complete lack of understanding in respect of the underlying motivations within this offender group. Further work is required, namely the development of theoretical models and the conducting of robust observational studies.

Withdrawal neuroleptic malignant syndrome is a not widely recognised variant of the rare adverse consequence of neuroleptic medication, neuroleptic malignant syndrome (NMS). We present a case to bring the disorder to the attention of clinicians responsible for treating serious mental illness and medical and judicial officers who may be involved in investigating serious untoward incidents occurring within a mental health care setting.

Sexual attacks on older women committed by young adult males are a cause of much concern. Despite this there is very little on the subject in the psychiatric literature. This article reviews the available literature and describes a small series of cases, highlighting not only common themes, but also the diversity of psychopathology evident in the perpetrator.

Examples of sexual offences perpetrated against older female victims are infrequently found in the medical literature. The case of a gerontophilic offender is reported. The criminal career of the subject bears a strong resemblance to more typical sex offenders with the exception of the very specific nature of victim selection. The current lack of knowledge and understanding of such offences and perpetrators is emphasized.
