24. UKPDS 18: Estimated Dietary Intake in Type 2 Diabetic Patients Randomly Allocated to Diet, Sulphonylurea or Insulin Therapy

Abstract

Self‐reported dietary intake was estimated from 3‐day prospective food diaries completed by Type 2 diabetic patients in the UK Prospective Diabetes Study. All patients had received individual dietary advice and had been randomly allocated to diet, sulphonylurea or insulin therapy 3 months after diagnosis. A total of 132 patients (120 white Caucasian, 12 Asian) stratified for gender, obesity and allocated therapy with mean age 55 years (SD 8), body mass index 28 kg m−2(SD 4), and with a diabetes duration of 3 to 6 years were selected at random from 5 of 23 clinical centres. Patients reported a similar proportion of their energy intake as carbohydrate (43 %) to the general population and had not increased to the recommended 50–55 %. Their protein intake (21 %) was higher than the advised 10–15 %. Estimated energy intake from fat (37 %) was close to that recommended for diabetic patients (30–35%) and was lower than that reported for the UK population (40 %). The estimated polyunsaturated/saturated fat intake ratio (0.48) was higher than that reported for the UK population (0.35) compared with the recommended 1.0. Mean fibre intake at 22 g day−1was less than the recommended 30 g day−1. The 8 male Asian patients took a higher proportion of their dietary intake as fat (46 % vs 37 %) and lower as protein (14 % vs 21 %) than the male white Caucasian patients. No significant differences were seen in estimated nutrient constituents between patients allocated to diet, sulphonylurea or insulin therapy as part of the UK Prospective Diabetes Study and followed for mean 4.2 years (SD1.6). This suggests that dietary factors will not confound UK Prospective Diabetes Study treatment related analyses.

25. Antithrombin III Activity and Other Coagulation Changes in Proliferative Diabetic Retinopathy

Abstract

Antithrombin III, factor VIII, fibrinogen and fribrinolytic activity were measured in two groups of long-standing insulin-dependent diabetic subjects (24 with proliferative retinopathy, 24 without detectable retinopathy) and 24 non-diabetic controls. Mean antithrombin III ( ± 1 SD) was 115.9 ( ± 15.1), 109.8 ( ± 18.1) and 101.4% ( ± 12.5), respectively, in the retinopathy, non-retinopathy and control groups. Statistical significance was obtained when comparing the retinopathy and control groups (p < 0.001) and when comparing all 48 diabetics collectively with controls (p < 0.01). Mean factor VIII coagulant activity was 137.5 ( ± 37.0), 126.2 ( ± 58.2) and 97.0% ( ± 38.7), respectively, in the three groups. Again the differences between all diabetics and controls (p < 0.01) were statistically significant. Similar increases were observed for other modalities of factor VIII activity. Fibrinolytic activity was significantly increased in both diabetic groups but fibrinogen levels, although increased, were not statistically different from levels in controls. It is suggested that the observed changes are more likely to be secondary to the development of retinopathy and that the increase in antithrombin III activity is due to an increase in α2-macroglobulin.

Abstract

The circadian rhythms of preprandial and postprandial blood glucose, free fatty acids, and insulin levels were measured in groups of normal and of diabetic patients controlled by diet alone or by diet with tolbutamide, chlorpropamide, or metformin. All groups showed a greater insulin release after the first meal of the day than after subsequent meals. Diabetic patients controlled for more than two years on diguanide therapy showed the closest resemblance to non-diabetic control subjects. There was no significant difference between the values for diabetic patients on long-term control with diet alone and those on short- or long-acting sulphonylurea. The circadian rhythms disappeared in obese subjects on total starvation.

50. Insulin and C-peptide secretion in non-insulin-dependent diabetes mellitus and its response to dietary therapy

Abstract

We studied insulin and C-peptide levels in patients with non-insulin-dependent diabetes mellitus (NIDDM) during standard oral or intravenous glucose tolerance tests (GTT) at the time of diagnosis and after 3 months dietary therapy. On the second occasion they also had an ‘augmented’ GTT, in which slow intravenous infusion of glucose raised basal plasma glucose to a level similar to that at the time of diagnosis. Eight patients had oral tests, and seven patients intravenous tests. In both groups, dietary therapy significantly reduced fasting and peak plasma glucose (p<0.05 for oral; p<0.01 for intravenous GTT). Serum insulin levels during conventional oral GTT were not significantly different after dietary therapy compared to diagnosis, but were significantly higher during the ‘augmented’ oral GTT (p<0.05). In those patients who underwent intravenous GTT, there was a significant increase in both the total amount of insulin secreted (0–60 min) and in first-phase insulin secretion (0–10 min) during the ‘augmented’ test compared to diagnosis (p<0.01), but first-phase insulin secretion during the conventional intravenous GTT was unchanged. Serum C-peptide responses were also greater during ‘augmented’ tests (p<0.05), similar in pattern to serum insulin. There is a relative deficiency in insulin secretion in untreated NIDDM, which can be reversed by dietary therapy. It is essential to study insulin and C-peptide secretion in controlled ‘fasting’ glucose conditions.
