In this article, we investigate the association between testicular cancer and hematological paraneoplastic syndrome, which is seen infrequently among all types of tumors in the literature. This case report provides a comprehensive evaluation of a 28-year-old patient admitted to the emergency department with symptoms of vomiting, nausea, and a medical history of epilepsy. Although the initial presentation did not exhibit specific symptoms or signs associated with anemia, subsequent blood tests were interpreted in favor of hemolytic anemia. Our study provides detailed radiological findings and laboratory tests that provide comprehensive information about the progression of the lesions and the patient’s response to our treatment. Objective: The aim of this study is to determine the importance of factors, such as mean platelet volume (MPV), fibrinogen, HbA1c, total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride, and fasting glucose, on cardiovascular complication processes for type 2 diabetes mellitus patients without coronary artery disease (CAD) and also to compare the relationship between these risk factors. Methods: For this study, data of 182 patients who with coronary hard disease and diabetes mellitus were retrospectively analyzed. The patients were followed up at the Outpatient Clinic of Diabetes of Istanbul Education and Research Hospital between 01.01.2009-31.12.201. The mean age of patients were 63.07±11.24 (40-90) years. 115 (63.2%) patients were male and 67 (36.8%) patients were female. The patients were divided into three groups: group 1: patients who had CAD but no DM (n=59), group 2: patients who had CAD and DM (n=60), and group 3: patients who had DM but no CAD (n=62). Results: 201 The mean age of patients was 63.07±11.24 years; 115 patients were male and 67 were female. The patients were divided into three groups: group 1: patients who had CAD but no DM (n=59), group 2: patients who had CAD and Diabetes Mellitus (DM) (n=60), and group 3: patients who had DM but no CAD (n=62). The mean age of patients in each group was as follows: 64±13, 62±9, and 57±11 years. According of post hoc analyses, the fibrinogen and MPV values of group 3 were lower than group 1 and group 2. There was a statistical difference between the groups in terms of mean values of total cholesterol, LDL-cholesterol, HbA1c, and the median values of fasting glucose and triglyceride. There was no difference between the 3 groups in terms of mean values of HDL-cholesterol. Conclusion: In conclusion, MPV and fibrinogen values may be independent risk factors of cardiovascular complications for type 2 DM patients and may be suggested as useful markers to follow up patients for progression to coronary artery disease. Background: Aging is significantly related to multiple comorbidities. Even with a good performance score, some elderly patients may have poor survival outcomes. We aimed to evaluate the prognostic value of the Charlson comorbidity index (CCI) for mortality and toxicity in elderly patients with locally advanced rectal cancer (LARC). Methods: Seventy-two elderly patients with LARC who were treated with neoadjuvant chemoradiotherapy (CRT) were included. Based on their CCI score, severity of the comorbidity was categorized into 2 groups: CCI<7 and CCI≥7. Results: The overall survival (OS) at 5 years was 54.4 percent in patients treated with neoadjuvant CRT. Median OS was not reached for all patients as well as patients with CCI score <7, but median OS was 25 (95% CI 1.0-62.1) months in patients with CCI≥7 (P = 0.002). The OS at 2 years was 79.1 percent in the patients with CCI <7 and 50.0 percent in the patients with CCI score ≥7 (P = 0.002). Moreover, there was a trend toward, patients with higher CCI score who had more treatment related to grade 3 or 4 toxicity compared to those with CCI score <7 (33.3% vs 13.3%, respectively, P = 0.09). Multivariable analysis indicated that the CCI score ≥7, presence of down-staging after therapy and clinical stage (III) independently predict mortality (HR 6.14, 95%CI 2.45-15.35, P < 0.001) in patients with LARC . Conclusion: Although CCI score was not significantly associated with both toxicity and disease-free survival (DFS), we suggest that baseline CCI score might be a valuable prognostic indicator for physicians to evaluate elderly patiens with LARC for optimal treatment.
