Objective Inflammation plays a major role in the pathogenesis of coronary artery disease (CAD) and myocardial infarction (MI). CD14 is the receptor for bacterial lipopolysaccharide in monocytes and mediates the production of proinflammatory cytokines. The promoter of the CD14 gene has a polymorphic site in position - 159 (C-->T) and T-homozygotes have been shown to express higher amounts of CD14 by some investigators. We and others have found an association of the T-allele with past MI in former studies, but reports in the literature are contradictory. Methods and results We investigated a study group with an assumed high genetic risk by selecting 200 patients suffering from angiographically verified CAD or MI who were younger than 50 years or who had only one or no risk factor (hypertension, smoking, elevated body mass index, impaired glucose tolerance or elevated cholesterol levels). We used 252 healthy subjects as controls. Additionally, the levels of soluble (s) CD14 in plasma and amount of membranous (m) CD14 on the surface of monocytes were determined in different genotypes. We found no association of either genotype with CAD, extent of CAD, or a history of MI. No significant correlation was found after adjustment for vascular risk factors. In addition, no significant differences in the density of monocyte mCD14 or in plasma levels of sCD14 were detectable among the various genotypes. Conclusions The assumed weak association of the TT-genotype of the CD14 promoter polymorphism with MI could not be not established in a well-defined group of young patients with a high genetic risk. The association of the polymorphism with expression of sCD14 or mCD14 was not confirmed.

Research was conducted to examine the role of (Ca2+)(sub i) changes in endothelial cell preparations in which ATP reduces permeability. A comparison of the influence of ATP with those of ionomycin, a Ca2+ ionophore that causes a rise in (Ca2+)(sub i) in a receptor-independent manner, was also carried out. Results suggest that ATP reduces macromolecule permeability through a phospholipase C-mediated mechanism that is independent of the concomitant effects of ATP on cytosolic Ca2+, protein kinase C or cyclic nucleotides.

Objective. Matrix metalloproteinases, such as gelatinase B, are important in connective tissue remodelling processes associated with atherogenesis and plaque rupture. The T allele of the gelatinase B C (‐1562) T polymorphism has been reported to be associated with an almost 2‐fold increase in promoter activity and with the extent of coronary artery disease (CAD). The aim of this study was to analyse the relation of this gene variation to the risk and severity of CAD and the risk of myocardial infarction (MI). Material and methods. This case‐control study comprised 535 healthy controls and 2731 participants who had undergone coronary angiography. Results. In the total sample, the gelatinase B promoter polymorphism was not associated with the risk of CAD and MI or with the extent of CAD defined either by the number of diseased coronary arteries or – in patients with coronary angiography – by a score for coronary heart disease (CHD) according to the Gensini score. However, patients with TT genotype had higher CHD scores than the other genotypes in subgroups of individuals with high apolipoprotein B levels, high lipoprotein (a) plasma concentrations and high fibrinogen levels, or with combinations of increased levels of these coronary risk factors. These observations were made in the entire sample of individuals with coronary angiography and in the population of patients with documented CHD. Conclusions. Obviously, the gelatinase B C (‐1562) T gene polymorphism is not a risk indicator for CAD and MI. With respect to the extent of CHD, the impact of this gene variation may be restricted to individuals with high apolipoprotein B, lipoprotein (a) and/or fibrinogen levels. [ABSTRACT FROM AUTHOR]

Background There are conflicting results on the relationship of N5,N10-methylenetetrahydrofolate reductase C677T gene variation in coronary artery disease and myocardial infarction. Methods and Results We analysed this gene variation in 2453 male Caucasians whose coronary anatomy was defined by coronary angiography. In the total sample, the C677T gene polymorphism was not associated with the presence or the extent of coronary artery disease (defined by the degree of vessel disease or by the coronary heart disease score according to Gensini). However, after excluding individuals with low risk profiles, an association between the C677T TT genotype and the Gensini score was found. This observation applies only to individuals (i) with high glucose levels, (ii) with low apolipoprotein Al/apolipoprotein B ratios, (iii) with low apolipoprotein Al/apolipoprotein B ratios and high lipoprotein (a) levels and (iv) with low apolipoprotein Al/apolipoprotein B ratios and high glucose concentrations. In patients with high glucose levels, the paraoxonase 191 A/B gene variation presupposed whether differences in Gensini scores between C677T C allele carriers and TT homozygotes became apparent, since only in paraoxonase 191 AA homoxygotes, but not in paraoxonase 191 B allele carriers, did C677T TT homozygotes have clearly higher Gensini scores than C allele carriers (two-way interaction;P=0·013). The MTHFR C677T gene polymorphism was not associated with non-fatal myocardial infarction. Conclusion The present study extends previous observations by the finding that carriers of the N5,N10-methylenetetrahydrofolate reductase C677T TT genotype with various coronary high risk profiles had clearly higher coronary heart disease scores than individuals with at least one C677T C allele. - Abstract
Automatic mode switching (AMS) function in dual chamber pacemakers depends on adequate detection of atrial tachyarrhythmias. There are few data on showing how intra-operative atrial signal amplititude during sinus rhythm can predict atrial tachyarrhythmias after pacemaker implantation. In 43 patients undergoing DDDR pacemaker implantation and atrioventricular nodal ablation for the treatment of drug-refractory paroxysmal atrial fibrillation, atrial sensing thresholds during sinus rhythm and during induced atrial tachyarrhythmias (24–48 h after device implantation) were analysed. Five different DDDR pacemaker systems were implanted (Chorus 7034|Pt, Ela Medical n=13; Meta DDDR 1254|Pt, Telectronics Pacing Systems n=12; Vigor DR 1230|Pt, Guidant n=6; Trilogy DR 2364|Pt, Pacesetter, n=2; Kappa DR 401|Pt, Medtronic USA n=10). Every patient received a steroid-eluting, screw-in, bipolar atrial lead (Medtronic, Capsure-Fix 4068|Pt). The mean P wave amplitude during implantation was 3·91±1·14 mV. The mean atrial sensing threshold during sinus rhythm and during all modes of induced atrial tachy-arrhythmias was 3·35±1·0 mV, and 1·52±0·92 mV, respectively (P<0·001). Atrial fibrillation was induced in 36 patients. The mean sensing threshold during sinus rhythm in this patient group was 3·39±1·01 mV, the mean sensing threshold during atrial fibrillation was 1·27±0·56 mV, reflecting a 63% reduction of sensing threshold compared with sinus rhythm (P<0·001). Atrial flutter was induced in seven patients. The mean sensing threshold during sinus rhythm was 2·92±1·19 mV, the mean sensing threshold during atrial flutter was 2·79±1·26 mV, reflecting a reduction of 5% (ns) compared with sinus rhythm. Atrial sensing thresholds during sinus rhythm were significantly correlated with sensing thresholds during atrial tachyarrhythmias (r=0·44;P<0·002), but there were significant variations in intra-individual results. The reduction of atrial sensing thresholds between sinus rhythm and induced atrial tachyarrhythmias ranged from 30% to 82%.Conclusion Bipolar atrial sensing thresholds during sinus rhythm are correlated with sensing thresholds during atrial tachyarrhythmias, but there is a large degree of variance in individual patients. A 4:1 to 5:1 atrial sensing safety margin based on sensing threshold during sinus rhythm is a predictor for adequate postoperative detection of atrial tachyarrhythmias and the function of AMS devices. AimThere is evidence that interaction between angiotensin II type 1 receptor A1166C gene polymorphism and angio-tensin I-converting enzyme Insertion/Deletion gene variation might have an effect on the risk of myocardial infarction. The study was carried out in a population of 2244 male Caucasians, whose coronary anatomy was defined by means of coronary angiography. We analysed the relationship, on the risk of ischaemic heart disease, of angiotensin II type 1 receptor A1166C gene variation, not only to myocardial infarction but also to coronary artery disease, and its potential interaction with angio-tensin I-converting enzyme Insertion/Deletion gene polymorphism.Methods and ResultsNo association was detected between angiotensin II type 1 receptor A1166C gene polymorphism and coronary artery disease. Similarly, there was no link to myocardial infarction, either in the total population or in low risk groups. In addition, most importantly, we found no interaction between angiotensin II type 1 receptor A1166C gene variation and angiotensin I-converting Insertion/Deletion polymorphism, either in connection with the risk of coronary artery disease or myocardial infarction.ConclusionThis angiotensin II type 1 receptor A1166C gene variation is not associated with any detectable increase in risk of ischaemic heart disease. Apolipoprotein and lipid composition of differently sized chylomicrons from healthy volunteers was determined. During their intravasal catabolism the chylomicrons lose triacylglycerol and apolipoproteins. Decreasing particle size results in a loss of apolipoprotein C and apolipoprotein E peptides and an increase in apolipoproteins B and A-I, which constitutes more than 20% of the moiety of small chylomicrons. The C peptides do not seem to behave as a functional entity. Apolipoprotein C-III, the inhibitor of lipolytic activities, is catabolized independently of the other C peptides. Albumin constitutes about 15-25% of the protein moiety of all chylomicrons. The different chylomicron fractions were incubated with lipolytic activities of lipoprotein lipase and hepatic triacylglycerol lipase. At lower substrate concentrations the reactions were of first-order. Large chylomicrons were the favored substrate for both enzymes with Michaelis Menten constant Km = 1.1 mM for hepatic triacylglycerol lipase and 0.48 mM for lipoprotein lipase. After incubation with hepatic triacylglycerol lipase or lipoprotein lipase the shape of chylomicrons differs from that of control particles as demonstrated by electron microscopy. C peptides were completely dissociated and found in the infranatant. In the enzyme assay with triolein gum arabic substrate several apolipoproteins showed an influence on the activities of hepatic triacylglycerol lipase and lipoprotein lipase. Apolipoprotein C-III peptides were the most effective inhibitors of both enzymes. Also, apolipoprotein A-II, A-I and apolipoprotein C-I inhibited lipoprotein lipase activity, whereas only apolipoprotein A-II was able to decrease hepatic triacylglycerol activity.

Kinetic studies were performed incubating lipoprotein lipase and hepatic triacylglycerol lipase from human postheparin plasma with triacylglycerol-rich lipoproteins from two patients with apolipoprotein C-II deficiency. These lipoproteins differed in their lipid and apolipoprotein composition from normal very-low-density lipoproteins and chylomicrons. The addition of isolated apolipoprotein C-II and normal or apolipoprotein C-II-deficient high-density lipoproteins caused an increase of Vmax and a decrease of the Km for lipoprotein lipase-induced hydrolysis. Hepatic triacylglycerol lipase activity was not influenced by the presence of apolipoprotein C-II in the incubation medium, but was inhibited by increasing amounts of high-density lipoproteins. Binding studies were performed in order to analyze the interactions between lipolytic enzymes, apolipoprotein C-II, and triacylglycerol-rich lipoproteins. Apolipoprotein C-II was, as expected, rapidly taken up by apolipoprotein C-II-deficient very-low-density lipoproteins and chylomicrons when they were incubated with normal high-density lipoproteins or with the purified apolipoprotein. This uptake was inhibited by the addition of increasing amounts of lipoprotein lipase in conditions in which no lipolysis could occur. Binding of lipoprotein lipase to apolipoprotein C-II-deficient very-low-density lipoproteins or chylomicrons was not affected by the addition of apolipoprotein C-II when an excess of triacylglycerol-rich lipoprotein was present. The stability of lipoprotein lipase was also studied. Apolipoprotein C-II and high-density lipoproteins were unable to prolong the half-life of the enzyme activity, while triacylglycerol-rich particles effectively stabilized lipoprotein lipase. We conclude that binding of lipoprotein lipase to the substrate surface is not affected by apolipoprotein C-II. It is more likely that the peptide catalyzes the conversion of lipoprotein lipase from a less to a more active form.
