2. Fast-growing pancreatic neuroendocrine carcinoma in a patient with multiple endocrine neoplasia type 1: a case report

Abstract

Abstract Introduction Predictive genetic screening and regular screening programs in patients with multiple endocrine neoplasia type 1 are intended to detect and treat malignant tumors at the earliest stage possible. Malignant neuroendocrine pancreatic tumors are the most frequent cause of death in these patients. However, the extent and intervals of screening in patients with multiple endocrine neoplasia type 1 are controversial as neuroendocrine tumors are usually slow growing. Here we report the case of a patient who developed a fast-growing neuroendocrine carcinoma within 15 months of a laparoscopic distal pancreatic resection. Case presentation We followed a group of 45 patients with multiple endocrine neoplasia type 1 by an annual screening program in the Department of Visceral, Thoracic, and Vascular Surgery at the University Hospital Marburg in cooperation with the Department of Radiology and the Division of Endocrinology. A man with multiple endocrine neoplasia type 1 who was diagnosed with a recurrent primary hyperparathyroidism underwent a distal pancreatic resection for a non-functional neuroendocrine tumor. In the context of our regular screening program, a large non-functional neuroendocrine tumor was diagnosed in the pancreatic head 15 months after the first pancreatic surgery. Therefore, we performed an enucleation and regional lymph node resection. At histology, the diagnosis of a neuroendocrine carcinoma with one lymph node metastasis was established. There was no evidence of recurrence 9 months after re-operation. Conclusion Fast-growing neuroendocrine tumors are rare in patients with multiple endocrine neoplasia type 1. The intervals, both postoperative and in newly diagnosed pancreatic lesions, in patients with multiple endocrine neoplasia type 1 should be reduced to 6 months to establish the early diagnosis of rapidly progressive disease in a small subset of patients.

Abstract

A 79 year old female patient was admitted to our emergency department with a fracture of the right medial femoral neck six days after a fall on her right side and a cemented hemiprosthesis was implanted. Five days later, she developed a hemorrhagic shock and was diagnosed with a delayed splenic rupture and the spleen was resected. Histopathological examination showed a delayed rupture of an otherwise normal spleen without signs of an underlying pathology. The outcome was fatal: In the postoperative course she developed pneumonia, three weeks later she succumbed due to multiple organ failure. Even careful reevaluation of the case did not provide any clues to expect an injury of the spleen according to trauma mechanism. This case shows that delayed splenic rupture of a normal spleen may occur even after a low energy trauma. Injury of the spleen should therefore always be considered, even with an uncharacteristic anamnesis. Physical examination after trauma should therefore always include a careful clinical evaluation. The clinical threshold for a FAST examination should be low. The coincidence of a femoral neck fracture and a splenic rupture after a low energy trauma has not been reported before.

Abstract

Background: Hypermethylation is an alternative mechanism for early gene inactivation in cancer cells. Reaquisition of p16/CDKN2A tumor suppressor gene expression by 5-Aza-2`deoxycytidine (5-Aza-CdR) results in concurrent growth inhibition of neuroendocrine pancreatic tumor cells. However, the growth suppressive effects of 5-Aza CdR is unlikely to be solely attributable to the restored p16/CDKN2A function but rather a consequence of reexpression of additional genes silenced by de novo methylation. In an effort to validate DNA methylation as a cancer-specific biomarker that could be integrated into prevention we attempted to isolate methylation-specific transcripts in neuroendocrine pancreatic tumor cells. Methods: Differentially expressed methylation-associated genes were identified by cDNA-representational difference analysis (cDNA-RDA). For the generation of representations RNA was extracted from QGP-1 cells cultured in the presence or absence of the methyltransferase-inhibitor 5-Aza-CdR and subjected to cDNA synthesis followed by two PCR-coupled rounds of substractive hybridisation and selective amplification (cDNA-RDA). The RNA of treated cells and untreated controls was used in separate experiments as tester or driver, respectively. The second difference products were cloned into Bluescript SK II for cDNA fragment sequencing. Sequence homology and chromosomal localization resulted from blast-algorithm. Differential expression was confirmed by semi-quantitative RT-PCR using insert specific primers. Results: We identified 48 differently expressed gene fragments and methylation-associated expression was confirmed by semi-quantitative RT-PCR. Sequence analysis of the isolated cDNA fragments revealed homology to 41 known genes. They were genes encoding tumor suppressors, sorting proteins, membrane proteins, proteases, enzymes, proteins involed in tumor invasion and metastases, apoptosis, ubiquitination, signal transduction, nuclear and ion transport, cytoskeleton, translational control and protein trafficking. The remaining 7 fragments showed homology to genes with unknown function. Conclusion: DNA methylation appears an important molecular mechanism in the process of neuroendocrine pancreatic tumorigenesis and the definition of DNA methylation patterns associated with neuroendocrine pancreatic tumors might open up the potential for a new sensitive and specific diagnostic and predictive tool., Hypermethylierung ist ein alternativer Mechanismus der frühen Gen-Inaktivierung in Tumorzellen. Es daher wurde angenommen, dass der demethylierungs-assoziierte Phänotyp neben der nachgewiesenen Restoration der p16/CDKN2A Expression eine Konsequenz der Reexpression mehrerer durch abberante Methylierung supprimierte Gene ist. Um die Bedeutung der DNA Methylierung zu validieren hatte die vorliegende Arbeit zum Ziel, methylierungs-spezifische Transkripte in QGP-1 Zellen zu isolieren. Differentiell exprimierte Gene wurden durch die cDNA-Representational Difference Analysis (cDNA-RDA) isoliert, sequenziert und die methylierungasabhängige Expression durch semi-quantitative RT-PCR bestätigt. Es konnten 48 methylierungsabhängig differentiell exprimierte Genfragmente in QGP-1 Zellen bestätigt werden. Davon waren nach 5-Aza-2´-Deoxycytidin-Behandlung 26 Genfragmente verstärkt bzw. neu exprimiert, und 22 weitere Genfragmente signifikant schwächer exprimiert oder supprimiert. Die Sequenzanalyse der isolierten cDNA Fragmente zeigte Homologie zu 41 bekannten Genen. Sieben der cDNA Fragmente zeigten Homologie zu Genen mit bisher unbekannter Funktion. Bei einigen der klonierten Genen besteht ein Zusammenhang mit der Onkogenese anderer Tumorentitäten, sodass diese auch in neuroendokrinen Pankreastumorzellen eine pathogenetische Rolle spielen könnten. Interessant ist die 5-Aza-2´-Deoxycytidin induzierte Neuexpression von Cofilin-1, das über Depolymerisation von Aktin-Filamenten die Bewegungsfähigkeit der Zellen und somit das Metastasierungspotential reduziert, sowie die Repression von Matriptase, einer Serinprotease, die durch Matrixdegradation Metastasierung und invasives Wachstum fördert. Die vorliegende Arbeit weist darauf hin, dass DNA Methylierung ein Mechanismus in der Genese neuroendokriner Pankreastumorzellen ist, sodass es möglich wäre, zukünftig den Methylierungsstatus diagnostisch im Sinne eines Methylierungsfingerprint einzusetzen.

Abstract

Purpose: Bacterial infections are a factor for morbidity in patients with acute appendicitis (AA). The spreading of multidrug-resistant (MDR) bacteria is a significant problem in surgery, and the most relevant MDR pathogens are summarized as Enterobacteriaceae, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Enterococci (ESKAPE) bacteria. Data regarding the species and distribution of bacteria in AA are available, but information about the resistances and their relevance is deficient. Methods: In this retrospective study, we analyzed microbiological swabs of patients with AA. The outcome parameters of patients after laparoscopic appendectomy were analyzed against microbiological results, including antibiotic resistance testing. Positive swabs were compared with bacteria cultivated after alternative abdominal emergency surgery (AES). Results: In total, 584 patients with AA were included and had a mean age of 35.5 years. In 216 patients (36.9%), a swab was taken, and in 128 (59.3%) swabs, bacteria could be cultivated. The most frequent organisms were Escherichia coli, Bacteroides species, and Pseudomonas. In 9.4% of the positive AA swabs, MDR germs were cultivated, and all of them were ESKAPE pathogens. Patients with MDR bacteria in AA suffered more infectious complications ( p = 0.006) and needed longer hospitalizations ( p < 0.009). In AES, aside from appendicitis, a different spectrum containing more MDR bacteria was cultivated (5.9 vs. 20.9%; p < 0.0001). Conclusions: Although they occur less frequently in appendectomy compared to emergency surgeries for other abdominal diseases, MDR bacteria are traceable in this common disease and contribute to additional morbidity. Abstract

Background: Colorectal foreign bodies are commonly seen in the emergency department. A delayed consultation may be associated with a worse outcome. Despite a low-lying position, the extraction can be a challenge and has to be done under general anesthesia. Sharp or potentially dangerous foreign bodies should be excluded before extraction manoeuvres and complications have to be ruled out after the retrieval., Method: Data were retrospectively collected from 2002 to 2017. Overall 33 patients were included. In addition, a systematic search, employing the PRISMA criteria, was performed. The search was carried out on PubMed (1623), Cochrane Library (17) and Google Scholar (300). Case reports, papers not written in English and publications before 1980 were excluded., Results: With an average delay of 33 h, patients admitted themselves to the emergency department. Attempts to remove foreign bodies in the emergency department were successful in 2 cases (6%). General anaesthesia was required in 31 (94%) patients. An emergency laparotomy was necessary in 3 cases (9%). No complications were noticed., Conclusion: A delayed consultation is not associated with a worse outcome. Complications should be ruled out by endoscopy.
