Abstract

The role of allogeneic hematopoietic stem cell transplantation (HSCT) for infants with acute lymphoblastic leukemia (ALL) and KMT2Agene rearrangement (KMT2A-r) is controversial in terms of both its efficacy and potential for acute and late toxicities. In Japanese Pediatric Leukemia/Lymphoma Study Group trial MLL-10, by introducing intensive chemotherapy, indication of HSCT was restricted to patients with high-risk (HR) features only (KMT2A-r and either age <180 days or presence of central nervous system leukemia). Of the 56 HR patients, 49 achieved complete remission. Forty-three patients received HSCT in first remission including 38 patients receiving protocol-specified HSCT with conditioning consisting of individualized targeted doses of busulfan, etoposide, and cyclophosphamide. Three-year event-free survival (EFS) of 56.8% (95% confidence interval [CI], 42.4% to 68.8%) and overall survival of 80.2% (95% CI, 67.1% to 88.5%) were accomplished. Univariable analysis showed that Interfant-HR criteria and flow cytometric minimal residual disease (MRD; ≥0.01%), both at the end of induction and at the end of consolidation (EOC), were significantly associated with poorer EFS. In the multivariable analysis, positive MRD at EOC was solely associated with poor EFS (P< .001). Rapid pretransplant MRD clearance and tailored HSCT strategy in the MLL-10 trial resulted in a favorable outcome for infants with HR KMT2A-r ALL. However, considering the high rate of potentially life-threatening toxicities and the risk of late effects, its indication should be further restricted or even eliminated in the future by introducing more effective therapeutic modalities with minimal toxicities. Abstract

1. We investigated the structure-activity relationship of 31 kinds of synthesized atorvastatin esters, thioesters, amides and lactone, selected as prodrug models, for metabolic activation by microsomes and hydrolases. 2. The susceptibility to human carboxylesterase 1 (hCES1) was influenced not only by the size of the acyl group and alkoxy group but also by the degree of steric crowding around the alkoxy group. 3. The susceptibility to human carboxylesterase 2 (hCES2) increased with a decrease in electron density around the alkoxy group of the substrate. 4. Abstract

Human carboxylesterase 1 (hCES1) is an enzyme that plays an important role in hydrolysis of pharmaceuticals in the human liver. In this study, elucidation of the chiral recognition ability of hCES1 was attempted using indomethacin esters in which various chiral alcohols were introduced. Indomethacin was condensed with various chiral alcohols to synthesize indomethacin esters. The synthesized esters were hydrolyzed with a human liver microsome (HLM) solution and a human intestine microsome (HIM) solution. High hydrolytic rate and high stereoselectivity were confirmed in the hydrolysis reaction in the HLM solution but not in the HIM solution, and these indomethacin esters were thought to be hydrolyzed by hCES1. Next, these indomethacin esters were hydrolyzed in recombinant hCES1 solution and the hydrolysis rates of the esters were calculated. The stereoselectivity confirmed in HLM solution was also confirmed in the hCES1 solution. In the hydrolysis reaction of esters in which a phenyl group is bonded next to the ester, the Vmax value of the (R) form was 10 times larger than that of the (S) form. [ABSTRACT FROM AUTHOR]

30. Uptake of Fractionated H-Heparin by Isolated Rat Kupffer Cells.

Abstract

Purpose and Methods. The uptake of fractionated H-heparin by isolated rat Kupffer cells was examined to determine the uptake mechanism. Results. The association of fractionated H-heparin was concentration-dependent with a dissociation constant of 3.4 nM and a maximum association capacity of 1.3 pmol/10 cells, suggesting the involvement of a specialized mechanism. Although 2,4-dinitrophenol inhibited neither the association nor internalization of fractionated H-heparin, lowering the temperature from 37°C to 4°C reduced the internalization of fractionated H-heparin by 70% without affecting the association. Conclusions. It is suggested that the uptake mechanism may differ from receptor-mediated endocytosis of polypeptides and be mediated by scavenger receptors, because organic anions, and several ligands of scavenger receptors, as well as several heparin analogs, inhibit the binding of fractionated H-heparin to Kupffer cells, while phenylarsine oxide, which is known to inhibit the receptor-mediated or absorptive endocytosis of polypeptides, inhibits neither the association nor internalization of fractionated H-heparin. 33. Internalization of Fractionated 3H-Heparin by the Scavenger-Like Receptor in Rat Liver Parenchymal Cells in Primary Culture

Abstract

The present study was aimed at clarifying the uptake mechanisms of fractionated 3H-heparin (FH) in rat liver parenchymal cells in an effort to explore further the clinical applications of mucopolysaccharides, including their utilization in drug delivery. The internalization and surface binding of FH were determined by removing surface-bound FH by the NaCl wash method in uptake experiments in rat liver parenchymal cells in primary culture. Initial and transient peaks were observed in the time course of the surface binding of FH, suggesting the involvement of receptor-mediated endocy tosis (RME) and downregulation of the receptors. Consistent with this suggestion, internalization of FH was reduced by lowering the temperature from 37 to 4°C, while total association was unchanged. Although the internalization of FH was slow and concentration independent, both total association and internalization were inhibited by ligands of the scavenger receptor and some anions, but not by inhibitors of the RME of polypeptides. All these results collectively suggest the involvement of the scavenger receptor or similar substance in terms of substrate specificity in the uptake of FH in rat liver parenchymal cells. This is the first suggestion of the existence of the scavenger-like receptor in liver parenchymal cells. Macromolecular compounds such as heparin have recently been increasingly investigated for their clinical applications, including utilization as carriers for drug delivery (Nishikawa et al. 1995). Although there have been a number of studies concerning the disposition of polypeptides in the body and the utilization of macromolecules as drug carriers, more extensive studies are required to exploit a variety of macromolecules for clinical applications.
