2. Understanding Diameter and Length Effects in a Solution-Processable Tellurium-Poly(3,4-Ethylenedioxythiophene) Polystyrene Sulfonate Hybrid Thermoelectric Nanowire Mesh

Organic–inorganic hybrids offer great promise as solution-processable thermoelectric materials. However, they have struggled to surpass the performance of their rigid inorganic counterparts due, in part, to a lack of synthetic control and limited understanding of how inorganic nanostructure dimensions impact overall charge transport. While it has been hypothesized that length, diameter, and aspect ratio (AR) all impact electronic transport in hybrid nanowires, the field lacks clarity on the relative role of each. In this study, the experimental parameter of ligand molecular weight (MW) is investigated as a synthetic knob for modulating nanowire dimensions, as well as the deconvolution of nanowire length versus diameter impacts on electron transport. By increasing ligand MW, larger nanowire AR dispersions occur and an optimal power factor of ≈130 μWm−1 K−2 is achieved for a modest AR of 73. Power factors of this magnitude are thought to only be achievable in ultrahigh AR systems; representing a 183% increase in performance over literature reports with similar AR. Additionally, nanowire diameter is demonstrated to be a far more sensitive parameter for enhancing performance than modulating length. This study provides improved fundamental insight into rational synthetic design avenues for future enhancements in the performance of hybrid materials.

3. High-Performance, Wearable Thermoelectric Generator Based on a Highly Aligned Carbon Nanotube Sheet

A high-performance, wearable thermoelectric generator (TEG) was fabricated with a highly aligned carbon nanotube (CNT) sheet. The aligned CNT sheet exhibits extraordinary electrical conductivity compared to disordered CNT sheets and also can be directly fabricated as a continuous TEG without metal electrode interconnects. This provides a significant reduction in contact resistance between TE legs and electrodes compared to traditional TEGs, resulting in higher power output. In addition, the continuity of the module without any disconnected parts provides high degrees of mechanical stability and durability. This robust and scalable approach to flexible TEG fabrication paves the way for CNT applications in lightweight, flexible, and wearable electronics.

- Abstract
Background: A significant proportion of patients report persistent pain after surgical release for de Quervain disease (DQ). This study aimed to investigate the effectiveness of a surgical release for DQ and to identify the preoperative factors associated with pain after a surgical release for DQ. Methods: This prospective cohort study included 707 patients who underwent surgical release and completed a visual analogue scale questionnaire (VAS; range 0 to 100). We used a paired t test to analyze the effectiveness of the surgical release on pain at 3 months postoperatively compared with the preoperative measure. A hierarchical multivariable linear regression model was created to investigate the contribution of patient-related and disease-related characteristics to postoperative pain. Results: All VAS domains showed improvement after surgical release. On average, the mean VAS pain decreased by 44 points (95% CI, 42, 46). Smoking (B = 6.37; P < 0.01), younger age (B = -0.35; P < 0.01), longer duration of complaints (B = 0.13; P < 0.01), concomitant surgery (B = 14.40; P < 0.01), and higher VAS pain scores at intake (B = 0.15; P < 0.01) were associated with worse VAS pain scores postoperatively. Together, the variables explained 11% of the variance in mean VAS pain score at 3 months follow-up. Conclusions: This study confirms that surgical treatment for DQ significantly reduces patient-reported pain. Smoking, younger age, concomitant surgery, duration of complaints, and higher VAS pain scores at intake are associated with worse patient-reported pain 3 months after surgical release. However, the small effects suggest that these factors should not be considered the only important factors. CLINICAL QUESTION/LEVEL OF EVIDENCE: Risk, III.

- Abstract
Determining the scope of (structural) renovation of aging tunnels is a complicated task and modular renovation is becoming increasingly necessary. The COB Tunnel Programme aims at developing the knowledge necessary to renovate these tunnels in a modular fashion, to reduce hindrance as much as possible. Part of this program is the development of a Structural Health Analysis (SHA) for collecting data on the condition of the civil structure of tunnels, which will give tunnel owners insight in the need for research on structural parts of the tunnel, refurbishment and the risks for the tunnel performance in the future. As a lot of knowledge will be gathered and generated, structurally securing of this knowledge is also necessary. To meet this need, the tunnel program also started development of a knowledge management system. This paper describes the aims of the knowledge management system in relation to the overall tunnel program, as well as the tools and methods devised to gather all necessary information for tunnels constructed several decades ago, where not all required information is readily available in digital form, or even in paper archives., Geo-engineering

ObjectiveWe aimed to identify genes associated with genetic generalized epilepsy (GGE) by combining large cohorts enriched with individuals with a positive family history. Secondarily, we set out to compare the association of genes independently with familial and sporadic GGE.MethodsWe performed a case-control whole exome sequencing study in unrelated individuals of European descent diagnosed with GGE (previously recruited and sequenced through multiple international collaborations) and ancestry-matched controls. The association of ultra-rare variants (URVs; in 18834 protein-coding genes) with epilepsy was examined in 1928 individuals with GGE (vs. 8578 controls), then separately in 945 individuals with familial GGE (vs. 8626 controls), and finally in 1005 individuals with sporadic GGE (vs. 8621 controls). We additionally examined the association of URVs with familial and sporadic GGE in two gene sets important for inhibitory signaling (19genes encoding γ-aminobutyric acid type A [GABAA ] receptors, 113genes representing the GABAergic pathway).ResultsGABRG2 was associated with GGE (p=1.8×10-5 ), approaching study-wide significance in familial GGE (p=3.0×10-6 ), whereas no gene approached a significant association with sporadic GGE. Deleterious URVs in the most intolerant subgenic regions in genes encoding GABAA receptors were associated with familial GGE (odds ratio [OR]=3.9, 95% confidence interval [CI]=1.9-7.8, false discovery rate [FDR]-adjusted p=.0024), whereas their association with sporadic GGE had marginally lower odds (OR=3.1, 95% CI=1.3-6.7, FDR-adjusted p=.022). URVs in GABAergic pathway genes were associated with familial GGE (OR=1.8, 95% CI=1.3-2.5, FDR-adjusted p=.0024) but not with sporadic GGE (OR=1.3, 95% CI=.9-1.9, FDR-adjusted p=.19).SignificanceURVs in GABRG2 are likely an important risk factor for familial GGE. The association of gene sets of GABAergic signaling with familial GGE is more prominent than with sporadic GGE.

Determining the scope of (structural) renovation of aging tunnels is a complicated task and modular renovation is becoming increasingly necessary. The COB Tunnel Programme aims at developing the knowledge necessary to renovate these tunnels in a modular fashion, to reduce hindrance as much as possible. Part of this program is the development of a Structural Health Analysis (SHA) for collecting data on the condition of the civil structure of tunnels, which will give tunnel owners insight in the need for research on structural parts of the tunnel, refurbishment and the risks for the tunnel performance in the future. As a lot of knowledge will be gathered and generated, structurally securing of this knowledge is also necessary. To meet this need, the tunnel program also started development of a knowledge management system. This paper describes the aims of the knowledge management system in relation to the overall tunnel program, as well as the tools and methods devised to gather all necessary information for tunnels constructed several decades ago, where not all required information is readily available in digital form, or even in paper archives.

- Abstract
OBJECTIVE: We sought to identify novel genes and to establish the contribution of known genes in a large cohort of patients with nonsyndromic sporadic polymicrogyria and epilepsy. METHODS: We enrolled participants with polymicrogyria and their parents through the Epilepsy Phenome/Genome Project. We performed phenotyping and whole exome sequencing (WES), trio analysis, and gene-level collapsing analysis to identify de novo or inherited variants, including germline or mosaic (postzygotic) single nucleotide variants, small insertion-deletion (indel) variants, and copy number variants present in leukocyte-derived DNA. RESULTS: Across the cohort of 86 individuals with polymicrogyria and epilepsy, we identified seven with pathogenic or likely pathogenic variants in PIK3R2, including four germline and three mosaic variants. PIK3R2 was the only gene harboring more than expected de novo variants across the entire cohort, and likewise the only gene that passed the genome-wide threshold of significance in the gene-level rare variant collapsing analysis. Consistent with previous reports, the PIK3R2 phenotype consisted of bilateral polymicrogyria concentrated in the perisylvian region with macrocephaly. Beyond PIK3R2, we also identified one case each with likely causal de novo variants in CCND2 and DYNC1H1 and biallelic variants in WDR62, all genes previously associated with polymicrogyria. Candidate genetic explanations in this cohort included single nucleotide de novo variants in other epilepsy-associated and neurodevelopmental disease-associated genes (SCN2A in two individuals, GRIA3, CACNA1C) and a 597-kb deletion at 15q25, a neurodevelopmental disease susceptibility locus. SIGNIFICANCE: This study confirms germline and postzygotically acquired de novo variants in PIK3R2 as an important cause of bilateral perisylvian polymicrogyria, notably with macrocephaly. In total, trio-based WES identified a genetic diagnosis in 12% and a candidate diagnosis in 6% of our polymicrogyria

IMPORTANCEBrain metastases occur in 60% of patients with advanced melanoma and are a major cause of melanoma-related mortality and morbidity. Although our understanding of the molecular alterations associated with melanoma progression is improving, there are currently no validated biomarkers which might help identify those patients at highest risk of developing brain metastases.OBJECTIVETo examine the somatic mutational and copy-number landscape of brain metastases that develop as the isolated first visceral site of recurrence – “early brain-metastasis” compared to extracranial melanoma metastases.DESIGN, SETTING AND PARTICIPANTSWhole-exome sequencing of 50 tumors from patients undergoing surgical resection of one or more brain metastasis occurring as the first site of visceral relapse were identified from prospectively maintained databases in Sydney, Wellington, New York and Cambridge. Whole exome sequencing analyses allowed mutational profiles to be compared to cutaneous melanomas in The Cancer Genome Atlas (SKCM-TCGA; n=358) and the Memorial Sloan Kettering (SKCM-MSK-IMPACT; n=186) datasets. An external dataset comprising a further 18 patients with surgically resected early brain metastasis from two additional academic centers served as an independent validation cohort.MAIN OUTCOMES AND MEASURESTo assess the frequency of driver mutations in early brain metastasis and their influence on survival.RESULTSIn concordance with the landmark melanoma sequencing studies, we identified mutations in BRAF (21/50, 42%), NRAS (14/50, 28%) and NF1 (11/50, 22%) as the most frequently mutated melanoma driver genes. When compared to the mutational landscape of cutaneous melanomas in TCGA (SKCM-TCGA), KRAS was the most significantly enriched driver gene, with 5/50 (10%) of brain metastases harboring non-synonymous mutations, of which 4/5 (80%) were in the hotspot positions of codons 12 and 61. This was significantly higher than the corresponding frequency ofKRAS-mutations within the entire SKCM-TCGA (2% (7/358), p=0.009, Fisher’s Exact Test) as well as the SKCM-MSK-IMPACT cohort (1.6% (3/186), p=0.016). Variants in KRAS were mutually exclusive fromBRAFV600,NRASandHRASmutations and were associated with a significantly reduced overall survival from resection of brain metastasis (relative toKRAS-wild type brain metastases) in multivariate Cox proportional hazard models (HR 1.80, 95% CI 1.46-24.89, p=0.013). Mutations inKRASwere also clonal and concordant with extracranial disease, which suggests these mutations are present within the primary tumorCONCLUSIONS AND RELEVANCEOur analysis, the largest to date, suggests that early metastases to the brain (presenting as the first site of visceral relapse) are characterized by significant enrichment of hotspotKRASmutations, potentially implicating constitutive RAS-driven cellular programs in neurotropic metastatic behavior in these cases. Based on these data, we suggest that screening forKRASmutations might help identify those patients with primary melanoma at higher risk of brain metastases or poor survival, and could help inform future surveillance strategies.Key PointsQuestionWhat is the frequency of driver mutations in early melanoma brain metastases?FindingsIn this study of 50 patients with melanoma metastasizing first to the brain,KRASmutations were the most significantly enriched driver gene (n=5, 10% of patients) when compared to landmark cutaneous melanoma studies. © 2019 John Wiley & Sons Ltd Background: Behaviour and emotional problems are highly prevalent in children with autism spectrum disorder (ASD). In typically developing children, attachment quality acts as a risk/protective factor for behavioural outcomes and adjustment, warranting investigation in children with ASD. Method: We investigated the relationship between attachment and child behaviour and emotional problems in children with ASD and comorbid intellectual disability. Data were collected from parent–child dyads where children were diagnosed with ASD and ID (n = 28) or other developmental disabilities (n = 20). Results: Children with ASD had higher levels of behaviour and emotional problems and more attachment difficulties than children with other developmental disabilities. Poorer attachment quality contributed uniquely to the variance in child behaviour and emotional problems. Conclusions: Interventions targeting behaviour and emotional problems in children with ASD may benefit from an attachment model which addresses the child's difficulty in using caregivers as a coregulatory agent of emotions.
