Abstract

IntroductionPeople who inject drugs (PWID) experience high HIV incidence and face significant barriers to engagement in HIV care and substance use treatment. Strategies for HIV treatment as prevention and substance use treatment present unique challenges in PWID that may vary regionally. Understanding differences in the risk structure for HIV transmission and disease progression among PWID is essential in developing and effectively targeting intervention strategies of HIV treatment as prevention.MethodsWe present a baseline analysis of HIV Prevention Trials Network (HPTN) 074, a two-arm, randomized controlled trial among PWID in Indonesia (n = 258), Ukraine (n = 457) and Vietnam (n = 439). HPTN 074 was designed to determine the feasibility, barriers and uptake of an integrated intervention combining health systems navigation and psychosocial counselling for the early engagement of antiretroviral therapy (ART) and substance use treatment for PWID living with HIV. Discordant PWID networks were enrolled, consisting of an HIV-positive index and their HIV-negative network injection partner(s). Among the enrolled cohort of 1154 participants (502 index participants and 652 network partners), we examine regional differences in the baseline risk structure, including sociodemographics, HIV and substance use treatment history, and injection and sexual risk behaviours.ResultsThe majority of participants were male (87%), with 82% of the enrolled females coming from Ukraine. The overall mean age was 34 (IQR: 30, 38). Most commonly injected substances included illegally manufactured methadone in Ukraine (84.2%), and heroin in Indonesia (81.8%) and Vietnam (99.5%). Injection network sizes varied by region: median number of people with whom participants self-reported injecting drugs was 3 (IQR: 2, 5) in Indonesia, 5 (IQR: 3, 10) in Ukraine and 3 (IQR: 2, 4) in Vietnam. Hazardous alcohol use, assessed using the Alcohol Use Disorders Identification Test - Alcohol Consumption Questions (AUDIT-C), was prominent in Ukraine (54.7%) and Vietnam (26.4%). Reported sexual risk behaviours in the past month, including having two or more sex partners and giving/receiving money or drugs in exchange for sex, were uncommon among all participants and regions.ConclusionsWhile regional differences in risk structure exist, PWID particularly in Ukraine need immediate attention for risk reduction strategies. Substantial regional differences in risk structure will require flexible, tailored treatment as prevention interventions for distinct PWID populations.

Abstract

Lithocarpus vuquangensis Ngoc & Hung is described from Vu Quang National Park, North Central Vietnam. The morphological comparison and phylogenetic analysis based on rbcL, matK and ITS provided evidence that the new species was not assignable to any of the previously known taxa in Vietnam and its surrounding countries. The description, photographs, preliminary conservation status and DNA barcode sequences are also provided for the new species.

37. Antioxidant and antimicrobial activities on uropathogenic bacteria of Asteraceae medicinal plants

Abstract

Antimicrobial and DPPH-scavenging assay were analyzed on nine Asteraceae species collected from Southest in Vietnam. Crude extracts from Taraxacum officinale, Chrysanthemum morifolium, Ageratum conyzoides, Tagetes erecta and essential oils from Ageratum conyzoides, Helianthus annuus, Artemisia vulgaris have antimicrobial activity on tested microorganisms. The essential oils of displayed effect on Candida spp. and Gram possitive bacteria, meanwhile, crude extracts expressed the inhibitory ability on bacteria. Ethyl acetate fraction from Ageratum conyzoides showed the most potent effect on urinary clinical strains ( E. coli and K. pneumoniae) with MIC at 1.25-10 mg/ml and 5-12.5 mg/ml, respectively. All ethanol crude extracts exhibited strong reducing power indicating on the TLC plate. The results of the DPPH scavenging assay showed that Tagetes erecta extract had the lowest IC 50 (17.280 μg/ml) and 2.5 times higher than vitamin C (7.321 μg/ml). This study revealed that A. conyzoides has good potential against uropathogenic E. coli and K. pneumoniae , and could therefore be applicable of prophylactic urinary tract infection.

Abstract

High-risk sharing practices among people who inject drugs (PWID) are associated with high rates of HIV transmission in several parts of the world. PWID in these regions face significant barriers to HIV testing and engagement in care, highlighting the need to develop and test interventions aimed at overcoming these barriers. Drug use and HIV are often highly stigmatized in these same settings, which raises safety concerns during the conduct of research. In preparing to address concerns about the safety and well-being of participants in an international research study, HPTN (HIV Prevention Trials Network) 074, we developed participant safety plans (PSPs) at each site to supplement local research ethics committee oversight, community engagement and usual clinical trial procedures. The PSPs were informed by systematic local legal and policy assessments as well as interviews with key stakeholders. Following PSP refinement and implementation, we assessed social impacts at each study visit to ensure continued safety. Throughout the study, five participants reported a negative social impact, with three resulting from study participation. Future research with stigmatized populations should consider using and assessing this approach to enhance participants’ safety and welfare.

Abstract

Asteraceae species were widely applied in traditional medicines in Asian countries as sources of natural antioxidants and antimicrobial agents. This study aimed to evaluate DPPH-scavenging capacities and antimicrobial activities of nine Asteraceae species collected from Southern Vietnam. Antioxidant and antimicrobial activities were determined by standard protocols. Essential oils from Ageratum conyzoides, Helianthus annuus, and Artemisia vulgaris indicated significant inhibitory effects on Staphylococcus aureus and Candida spp. Crude extracts and fractions from Taraxacum officinale, Chrysanthemum morifolium, A. conyzoides, and Tagetes erecta showed inhibitory ability on at least one testing bacterial strains including S. aureus, Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa. In a study on clinical isolates, ethyl acetate fraction from A. conyzoides flower displayed the most potent effect on uropathogenic E. coli and K. pneumoniae with MIC at 1.25–10 mg/ml and 5–12.5 mg/ml, respectively. DPPH-scavenging assay indicated that T. erecta extract had the lowest IC50 (17.280 μg/ml) and is 2.4 times higher than vitamin C (7.321 μg/ml). This study revealed that A. conyzoides has good potential against uropathogenic E. coli and K. pneumoniae, and therefore could be applied for prophylactic treatment of urinary infection.

Abstract

Mangroves form part of climate-change adaptation and mitigation measures but themselves are affected by climate change. Xuan Thuy National Park (XTNP) is the largest coastal wetland ecosystem in the north of Vietnam. Since the 1990s, several new plantings of mangroves have been established in the park; the dominant species used has been Kandelia obovata Sheue, Liu & Yong. In this study, the current state of an 18-22 year old plantation was examined. Kandelia obovata remains the dominant mangrove species. However from the sea landward, the density of trees decreases from 54,764 to 4,738 trees/ha. Loss of trees is linked to infestations by pest species of Cerambycidae, Coleoptera and Cossidae, Lepidoptera. A second mangrove species, Rhizophora stylosa Griff has the largest number of regenerating seedlings, though there is no advanced regeneration of any species. The potential for natural regeneration of K. obovata appears to be low. Loss of forest may be linked to climate-change phenomena. Cyclones, lightning, typhoons, and other extreme weather have led to stem and branch breakage and tree death, and indirectly to reduced resistance to pest and disease attack, leading to widespread degradation and loss of capacity for natural recovery to occur.

Abstract

Men who have sex with men (MSM) are highly vulnerable to sexual violence, a known driver of HIV infection. Homosexuality stigma may be a unique risk factor for sexual violence among MSM. In this study, we examine the relationship between homosexuality stigma measures and sexual violence in the last 12 months using a minority stress framework. MSM were recruited using convenience and snowball sampling. Participants completed an interviewer-administered survey and provided blood samples for HIV testing. Bivariable associations were tested between self-reported experience of sexual violence in the last 12 months and homosexuality stigma measures using odds ratios (ORs) produced by Cochran–Mantel–Haenszel Statistics. A logistic regression model for each type of minority stress was built to conduct the multivariable analyses with independent covariates. Of 202 MSM, 29 (14.4%) participants reported experiencing sexual violence in the last 12 months. About one fourth of participants reported experiencing high enacted (55/202; 27.2%), perceived (52/202; 25.7%), and internalized (60/202; 29.7%) homosexuality stigma. In bivariable and multivariable analyses, enacted homosexuality stigma was the only variable consistently associated with experience of sexual violence in the last 12 months (aOR: 3.5; 95% confidence interval [CI]: [1.5, 8.4]). Sexual violence and homosexuality stigma are highly prevalent among MSM in Hanoi, Vietnam. MSM-targeted HIV prevention interventions in Vietnam should incorporate violence prevention and homosexuality stigma reduction activities. Longitudinal studies are needed to understand how homosexuality stigma influences sexual violence and other HIV risk behaviors among MSM. [ABSTRACT FROM AUTHOR]

Abstract

BACKGROUND: Alcohol use, a highly normative behavior in Vietnam that is associated with high rates of HIV infection and lower antiretroviral treatment (ART) adherence, has been largely overlooked by HIV prevention efforts. OBJECTIVES: Using the risk environment framework, this qualitative study aims to explore the perceived microenvironmental (community-level) and endogenous (individual-level) barriers and facilitators to alcohol reduction among people living with HIV (PLHIV) with alcohol use disorders (AUDs) in Vietnam. METHODS: From June-July 2014, semi-structured interviews were conducted with thirty PLHIV (18 men; 12 women) recruited from an outpatient ART clinic in Thai Nguyen province, Vietnam. All participants had scores of >=8 on the Alcohol Use Disorders Identification Test and ten of the 30 participants were currently using injection drugs. Interviews were transcribed, translated, and analyzed to identify perceived barriers and facilitators to alcohol reduction. RESULTS: Most participants reported a spike in alcohol consumption at the time of HIV diagnosis. Most perceived barriers existed at the microenvironmental level, including perceived inability to refuse alcohol in the context of community-level social norms and lack of alcohol treatment programs. Two commonly mentioned endogenous barriers were compensatory behaviors when reducing injection drug use and using alcohol as a coping strategy for HIV-related sadness. Those who were able to successfully reduce alcohol use and adhere to ART reported having social support to buffer community-level social pressure and cope with sadness. CONCLUSIONS: It may be effective to introduce targeted alcohol reduction interventions in health care centers to address individual risk practices and microenvironmental social norms.
